RESUMO
OBJECTIVES: To study the efficacy of hematopoietic stem cell transplantation (HCT) for ameliorating the clinical manifestations of alpha-mannosidosis. STUDY DESIGN: Four patients with alpha-mannosidosis underwent allogeneic HCT at the University of Minnesota. Diagnosis was established by assay of leukocyte alpha-mannosidase activity level. Physical features, donor engraftment, leukocyte alpha-mannosidase activity, neuropsychologic function, and hearing were monitored before and after transplantation, with follow-up ranging from 1 to 6 years. RESULTS: All 4 patients showed slowing of their neurocognitive development and sensorineural hearing loss before HCT. All patients are alive, with normalization of leukocyte enzyme activity after HCT. Intellectual function has stabilized, with improvement in adaptive skills and verbal memory function in 3 of 4 patients. Hearing has improved to normal or near normal for speech frequencies in 3 patients. No new skeletal abnormalities have developed. CONCLUSIONS: HCT can halt the progressive cognitive loss in patients with alpha-mannosidosis. Early diagnosis and treatment with HCT is critical for optimal results.

Monoclonal antibodies raised against a strain of Leishmania infantum isolated in Greece were produced and tested for their protective effect in an in vivo system (in BALB/C mice). A single monoclonal antibody, IgG2b isotype, can prevent the development of two Leishmania strains in vivo: one of L. major and one of L. mexicana amazonensis. This antibody-mediated protection may be dependent on complement.

Several economic factors justify investment in health, among them the increased productive capacity of a work force whose quantitative growth has been aided by improved health, and whose quality has been improved by health and education. There are wide variations in the health and economic status of the countries of Latin America, and even among various areas within the same country, and for that reason additional investment, both domestic and foreign, will be required
To ensure for maximum utilization of such investment it is proposed that the external financial agencies exert every effort to assure that the projects they finance make a greater contribution to social and economic development by improving the quality and supply of the human resources available for developmental purposes
By broadening the elegible fields of health lending, the strategy takes account of the key factors in the environment, such as nutrition, sanitation, and facilities which influence the health of the peoples of Latin America. By suggesting support for new approaches to training, research, and planning, it recognizes that the supply and quality of medical personnel must be increased and that the countries of Latin America must learn to make better use of their existing stock of material and human resources in the health field
