RESUMO

This is a case report of a 2 year/5 month old female patient with a venous catheter fragment lodged in her right heart and pulmonary artery. The patient was familiar to us because of her ventricular septum defect and pulmonary hypertension secondary to the heart defect. Routine X-rays showed a fragment of the catheter lodged in the heart. The physical examination showed nothing more in addition to that derived from the ventricular defect. Her records included a clavicular phlebotomy at 2 months when suffering from a severe case of dehydration. The cardiac catheterization done at 16 months confirmed the finding of the fragment, which was extracted transluminally. The intracardiac extraction of the venous catheter and the simultaneous closure of the septal defect comprised the treatment applied.

RESUMO

This case report deals with a nine year old male patient suffering from Holt-Oram Syndrome, an autosomic dominant disease which causes diverse skeletal malformations and several heart deformities. This is a case with no apparent family history of the disease. The outstanding clinical findings include: a defect of the auricular septum (ostium secundum type), a tri-phalanx thumb, hypoplasia of the first metacarpal and a shorter left arm. The skeletal abnormalities did not effect his manual capability nor his development. This case includes: series of skeletal X-rays, cardiovascular examinations, an examination of motor function. Surgical correction should be considered as part of a careful therapeutic plan. The patient's pre- and postsurgical evaluation are considered.

RESUMO

The following methods used to determine globular sedimentation rate (VSG) were reviewed: Westergren and Wintrobe techniques, Chattas micromethod, Sibora and Sediplast systems and z percentage sedimentation. Also included are the VSG phases and those factors which determine the aggregation of the red corpuscles whether they be plasmatic, globular or extrinsic.

RESUMO

Both the systemic and local reactions caused by the immunization with the vaccine against diphtheria, tetanus and whooping cough were studied. Side-effects, 48 hours after the vaccination were determined in 730 children between the ages of two months and five years and 11 months. Over 13% of the children were free from side-effects. Eighty-seven percent of the children immunized showed the following side-effects in percentages of frequency: fever, 66.0%; malaise, 37.8%; loss of appetite, 25.0%; sleep disorders, 20.4%; vomiting, 7.9%; and continuous crying, 7.6%. With respect to local disturbances: pain, 41.6%; reddening, 28.0% and subcutaneous nodules, 20.1%. None of the children had convulsions, hypotonic episodes or immediate neurological damage. Two types of DTP vaccines, one from the Connaught (Canadian) Laboratory and another from the National Institute of Hygiene (Venezuelan) were used. No significant differences were found in the appearance of the side-effects between the both, except for localized pain (P less than 0.01) with the national vaccine. The results obtained in relation to age and the number of doses were: there was a significant increase of localized pain as ages increased (P less than 0.01). There were significantly greater number of localized pain and subcutaneous nodules with greater numbers of doses (P less than 0.01).

The Westergren inclined technique can be used to determine the globular sedimentation rate (GSR) at 10 and 15 minutes which are equivalent to the first and second hours respectively of the classic method. The present study compares GSR correlations using both methods, on a sample of 64 healthy children, included according to predetermined study selection criteria. The sample included 38 preschoolers (59.37%) and 26 school children (48.63%); with an age range between 4.14 +/- 0.48 years and 8.68 +/- 0.62 years (mean +/- 2 EE) respectively. The hematocrit range was between 38.75 +/- 0.56 Vol%; no erythrocyte alterations were observed. A significant positive correlation was observed in the GSR determinations between 1 hour-10 min and 2 hours-15 min, which were r1 = 0.9049 (P less than 0.001) and r2 = 0.9404 (P less than 0.001). No significant difference (P greater than 0.05) was noted on the basis of sex (34 males, 30 females). The determinations taken with the Westergren inclined method proved superior to the classic method, but the technique requires the inclusion of normal values of the former. The conclusion is that the inclined method can be used and interpreted by the clinician in the same way as the classic Westergren method. It permits a time saving, which in large hospital practices implies an important economic factor.
