Cord lipomas are frequent findings in laparoscopic inguinal hernia surgeries in male patients. The symptoms of lipoma and the potential benefits of removing them are often overlooked because the focus is on the primary pathology of the hernia itself. Current recommendations are to reduce this fatty content, when present. When inguinal cord lipomas are left untreated in inguinal hernia surgery they can potentially cause symptoms and be detected in follow-up imaging exams. The objective of this study was to study incidence of cord lipomas in a cohort operated on by a single group specializing in abdominal wall surgery, as well as to analyze the possible relationship of this finding with the patient's symptoms, the characteristics of the operated hernia and postoperative outcome. This is a prospective study of male patients operated on for inguinal hernia laparoscopically or robotically in a single reference center. Of the total of 141 hernias, the distribution according to European Hernia Society classification showed that 45.4% were lateral, 19.1% medial and 35.5% mixed, highlighting a variety in the presentation of hernias. Analysis of the size of the hernias revealed that the majority (35.5%) were ≤ 1.5 cm. Inguinal cord lipoma was present in 64.5% of the samples, with no statistically significant association between the presence of the lipoma and an indirect hernia sac or obesity. The incidence of surgical site occurrences (SSO) was 9,2%, with seroma and hematoma. No recurrences were observed during follow-up, indicating a successful approach. There were no statistically significant relationships between SSO, the presence of lipoma and indirect hernial sac.

PURPOSE: Botulinum toxin type A (BTA) is an adjuvant tool used in the preoperative optimization of complex hernias before abdominal wall reconstruction (AWR). This study aims to investigate changes in the abdominal cavity and hernia sac dimensions after BTA application. METHOD: A prospective study with 27 patients with a hernia defect of ≥ 10 cm and loss of domain (LOD) ≥ 20% underwent AWR. Computed tomography (CT) measurements and volumetry before and after the application of BTA were performed. Intraoperative and postoperative outcomes were evaluated. RESULTS: Imaging post-BTA revealed hernia width reduction of 1.9 cm (p = 0.002), lateral abdominal wall muscle elongation of 3.1 cm (p < 0.001), hernia volume reduction (HV) from 2.9 ± 0.9L to 2.4 ± 0.8L (p < 0.001), increase in abdominal cavity volume (ACV) from 9.7 ± 2.5L to 10.3L ± 2.4L (p = 0.003), and a reduction in the HV/ACV ratio from 30.2 ± 5% to 23.4 ± 6% (p < 0.001). Fascial closure was achieved in 92.6% of cases and component separation was required in 78%. The average variation in pulmonary plateau pressure was 3.53 cmH2O, and there were no postoperative respiratory failure recorded. At the 90-day follow-up, the wound morbidity rate was 25%, unplanned readmissions were 11%, and hernia recurrence 7.4%. CONCLUSION: BTA produces measurable volumetric changes in abdominal wall and appears to facilitate fascial closure. Further studies are required to determine the role of BTA in the surgical armamentarium for complex hernia repair.

Abstract The antioxidant activity of Tetragonisca angustula honey (TAH) and its ethanolic extract (TAEE) were investigated. The total levels of phenolic (TPC) and flavonoids (TFC) were also evaluated. The results for TPC were 19.91 ± 0.38 and 29.37 ± 1.82 mg GAE g-1 and for TFC 0.20 ± 0.02 and 0.14 ± 0.01 mg QE g-1 of TAH and TAEE, respectively. Antioxidant activities were 73.29 ± 0.49% and 93.36 ± 0.27% in the DPPH assay and 71.73 ± 4.07% and 97.86 ± 0.35% in ABTS+ for TAH and TAEE, respectively. The total reducing activity was determined by the method of reducing power (PR) and iron ion (Fe III) and the results varied in PR from 151.7 ± 25.7 and 230.7 ± 25.2 mg GAE L-1, for TAH and TAEE respectively and for (Fe III) in EC50 0.284 in TAEE and 0.687 in TAH. Chemical analysis by HPLC-DAD of the ethanolic extract (TAEE) revealed the presence of ferulic acid as majority phenolic component in the extract. The 1H NMR analysis confirmed this structure and showed the also presence of glucose, citric acid, succinic acid, proline and hydrocarbon derivatives. In addition, the botanical origin was also investigated and showed a multifloral characteristic, having found 19 pollen types with a botanical predominance of the Anacardiaceae family, with Tapirira pollen occurring as predominant (42.6%) and Schinus as secondary (25.7%). The results showed that T. angustula honey is an interesting source of antioxidant phenolic compounds due to its floral origin and can act as a protector of human health when consumed.


Resumo A atividade antioxidante do mel de Tetragonisca angustula (TAH) e seu extrato etanólico (TAEE) foram investigados. Os níveis totais de fenólicos (TPC) e flavonóides (TFC) também foram avaliados. Os resultados para TPC foram 19,91 ± 0,38 e 29,37 ± 1,82 mg GAE g-1 e para TFC 0,20 ± 0,02 e 0,14 ± 0,01 mg QE g-1 de TAH e TAEE, respectivamente. As atividades antioxidantes foram 73,29 ± 0,49% e 93,36 ± 0,27% no ensaio DPPH e 71,73 ± 4,07% e 97,86 ± 0,35% no ABTS+ para TAH e TAEE, respectivamente. A atividade redutora total foi determinada pelo método de poder redutor (PR) e íon ferrico (Fe III) e os resultados variaram em PR de 151,7 ± 25,7 e 230,7 ± 25,2 mg GAE L-1, para TAH e TAEE respectivamente e para (Fe III) em EC50 0,284 em TAEE e 0,687 em TAH. A análise química por HPLC-DAD do extrato etanólico (TAEE) revelou a presença de ácido ferúlico como componente majoritário no extrato. A análise de RMN 1H confirmou esta estrutura e mostrou a presença de glicose, ácido cítrico, ácido succínico, prolina e derivados de hidrocarbonetos no TAEE. Além disso, a origem botânica também foi investigada e apresentou característica multifloral, tendo encontrado 19 tipos polínicos com predomínio botânico da família Anacardiaceae, sendo o pólen Tapirira predominante (42,6%) e o Schinus secundário (25,7%). Os resultados mostraram que o mel de T. angustula é uma interessante fonte de compostos fenólicos antioxidantes devido à sua origem floral e pode atuar como protetor da saúde humana quando consumido.

Two experiments were designed to evaluate the effects of altering body condition score (BCS) and the profile of a fatty acid (FA) supplement on the metabolism of Bos indicus Nellore females. In experiment 1, 16 and 24 B. indicus heifers and nonlactating cows, respectively, were assigned to (1) maintenance diet (MNT-MNT; n = 10), (2) maintenance diet and BCS loss (MNT-LSS; n = 10), (3) maintenance diet supplemented with calcium salts of soybean oil for 30 d and BCS loss for 40 d (MNT+CFA-LSS; n = 10), and (4) maintenance diet for 30 d and BCS loss for 40 d with a diet containing calcium salts of soybean oil (MNT-LSS+CFA; n = 10). Following the BCS loss period, MNT-LSS, MNT+CFA-LSS, and MNT-LSS+CFA were fed a diet to promote the gain of BCS. In experiment 2, 40 Bos indicus nulliparous heifers were assigned to (1) maintenance diet (MNT-MNT; n = 10), (2) BCS loss followed by a BCS gain (LSS-REM; n = 10), (3) BCS loss followed by a BCS gain diet with CFA of palm oil (LSS-REM+PLM; n = 10), and (4) BCS loss followed by a BCS gain diet with CFA of soybean oil (LSS-REM+SOY; n = 10). Blood samples were obtained for serum haptoglobin and fecal samples for pH (experiment 2 only). In experiment 1, a treatment × day interaction was observed for BCS during the 60-d BCS loss and gain period. Animals assigned to MNT-MNT had a greater BCS than the other treatment groups on d 40 and 60 of the experiment, but no other differences were observed. Moreover, a treatment × day interaction was observed for serum haptoglobin, as on d 60, MNT-LSS had a greater mean serum haptoglobin concentration. In experiment 2, a treatment × day interaction was also observed for BCS. From d -4 to 0, LSS-REM and LSS-REM+SOY had a reduced BCS versus MNT-MNT, but also lower for LSS-REM versus MNT-MNT on d 1, and LSS-REM+PLM versus MNT-MNT on d -1 and 0. For serum haptoglobin, no treatment or treatment × day interaction was observed. A treatment × day interaction was observed for fecal pH. From d -10 to 0, MNT often had a lower fecal pH, but during realimentation, LSS-REM heifers had a reduced fecal pH on d 1, 4, and 10. In summary, we failed to demonstrate an increase in serum haptoglobin due to a BCS loss. Still, supplementation with calcium salts of FA alleviated the increase in haptoglobin and maintained fecal pH at more stable values during realimentation, regardless of the FA profile of the supplement.

Weed coexistence with an agricultural crop can negatively affect its growth, development, and yield. From this perspective, this study aimed to evaluate weed management strategies and their effect on the agronomic parameters of cowpea cultivation in direct (SPD) and conventional (SPC) planting systems. The experiment was set up in a completely randomized block design with a split-plot arrangement with four replications. The plots received a source of variation referring to the planting systems (direct and conventional planting), and the subplots corresponded to ten weed management strategies (manual hoeing 18 days after planting (DAP); at 36 DAP; at 54 DAP; at 18 and 36 DAP; at 18 and 54 DAP; at 18 and 72 DAP; at 36 and 54 DAP; at 36-72 DAP; at 18, 36, and 54 DAP; and a control with no hoeing). Density and dry mass evaluations of the cowpea plants were performed at harvest (72 DAP) by determining the number of pods per plant, pod length, number of grains per pod, 1000-grain mass, and yield. A total of 28 species distributed in 12 botanical families were identified in the two cultivation systems. The family Poaceae showed the highest frequency, with 25% of the species identified. At the end of the assay, treatment 20 had the highest positive influence and provided significant quantitative gains to the complex of traits related to cowpea production (SPD and hoeing at 18, 36, and 54 DAP). UPGMA cluster analysis and canonical discriminant analysis were performed and allowed a better classification of the evaluated treatments. It was observed that the first two canonical variables explained 90.8% of the total variance contained in the original variables. The use of SPD with weeding at 18, 36, and 54 days after planting provides greater weed control and significant quantitative gains for the complex of characteristics related to cowpea production. The results underscore the importance of choosing the correct cropping system and implementing effective weeding practices to optimize weed control and improve crop performance.

PURPOSE: Surgical repair of large hernia defects requires detailed pre-operative planning, particularly in cases with loss of domain. This situation often hampers mid-line reconstruction, even after component separation, when the size of the hernia is disproportional to the volume of the abdominal area. In this case, other strategies may be needed to place the viscera back into the abdominal cavity after reducing the hernia sac. The administration of botulinum toxin prior to the surgical procedure has been indicated as an adjunct for more complex cases. This results in stretching of the lateral musculature of the abdomen, allowing midline approximation. In addition, the application of botulinum toxin alone has been investigated as a means of downstaging in the management of ventral hernias, thereby precluding component separation and enabling primary closure of the midline by placement of mesh within the retromuscular space using the Rives Stoppa technique. METHODS: Systematic review of the literature for observational studies involving patients undergoing pre-operative application of botulinum toxin for ventral hernia repair was conducted according to the PRISMA guidelines. RESULTS: Advance of the lateral musculature of the abdomen by an average of 4.11 cm with low heterogeneity, as well as low rates of surgical site infection (SSI), surgical site occurrences (SSO) and recurrence, was shown. CONCLUSION: Pre-operative application of botulinum toxin for ventral hernia repair promoted an increase in the length of the lateral musculature of the abdomen which can help improve the outcomes of morbidity and recurrence.

Inactivation of the cyclin-dependent kinase inhibitor 2A (CDKN2A) gene is considerably more frequent in squamous cell lung cancer (SqCLC) than in other subtypes of lung cancer and may be a promising target for this histology. Here, we present the course of diagnosis and treatment of a patient with advanced SqCLC, harboring not only CDKN2A mutation but also PIK3CA amplification, Tumor Mutational Burden-High (>10 mutations/megabase), and a Tumor Proportion Score of 80%. After disease progression on multiple lines of chemotherapy and immunotherapy, he responded favorably to treatment with the CDK4/6i Abemaciclib and later achieved a durable partial response to immunotherapy rechallenge with a combination of anti-PD-1 and anti-CTLA-4, nivolumab, and ipilimumab.

Objetivo: identificar as implicações do teletrabalho para o trabalhador no período pandêmico de covid-19.Métodos: Trata-se de uma revisão integrativa de literatura com buscas nas bases de dados Web of Science, Scopus, Medline via pubmed, Lilacs, Scielo e Cumulative Index to Nursing and Allied Health Literature, utilizando os descritores teleworking, pandemics, occupational health com operador booleano AND. A busca foi realizada em agosto de 2022 e contou com os seguintes critérios de inclusão: textos no formato artigo científico com texto na íntegra, publicados no período pandêmico (2020 - 2022), em inglês, português e espanhol. Excluíram-se os artigos duplicados, pesquisas de revisão e artigos reflexivos. Resultados: Foram encontrados 134 artigos e após a aplicação dos critérios e avaliação do corpus textual, obtivemos uma amostra de 19 estudos. A análise se desdobrou na criação de 03 categorias, a citar: modificações no estilo de vida durante o trabalho remoto; implicações relacionadas à saúde mental e ao isolamento e implicações relacionadas ao processo de trabalho. O teletrabalho foi uma ferramenta valiosa porque proporcionou a continuidade e funcionamento das empresas durante a pandemia de Covid-19 uma vez que a saúde física e mental do trabalhador foi imensamente afetada. Conclusão: O teletrabalho mostrou- se uma importante modalidade de trabalho frente à pandemia de Covid-19. Apesar de ter sido uma ferramenta imprescindível para manutenção dos empregos e continuidade do funcionamento das empresas durante a pandemia, a saúde física e mental do trabalhador foi bastante afetada neste período.


Objective: To identify the implications of teleworking for the worker in the pandemic period of covid-19.Methods: This is an integrative literature review with searches in the Web of Science, Scopus, Medline via pubmed, Lilacs, Scielo and Cumulative Index to Nursing and Allied Health Literature databases, using the descriptors teleworking, pandemics, occupational health with boolean operator AND. The search was carried out in August 2022 and had the following inclusion criteria: texts in the format of scientific article with full text, published in the pandemic period (2020 - 2022), in English, Portuguese and Spanish. Duplicate articles, review research, and reflective articles were excluded. Results: We found 134 articles and after applying the criteria and evaluation of the textual corpus, we obtained a sample of 19 studies. The analysis unfolded in the creation of 03 categories, to mention: changes in lifestyle during remote work; implications related to mental health and isolation and implications related to the work process. Teleworking was a valuable tool because it provided business continuity and operation during the COVID-19 pandemic as the worker's physical and mental health was greatly affected. Conclusion: Teleworking proved to be an important way of working against the COVID-19 pandemic. Although it was an indispensable tool for maintaining jobs and continuing the functioning of companies during the pandemic, the physical and mental health of the worker was greatly affected in this period.


Propósito: identificar las implicaciones del teletrabajo para los trabajadores en el periodo pandémico de covid-19.Métodos: Se trata de una revisión integrada de la literatura con búsquedas en la Web de Ciencia, Scopus, Medline via pubmed, Lilacs, Scielo y el Índice Acumulativo a las bases de datos de Enfermería y Literatura de Salud Aliada, utilizando los descriptores teletrabajo, las pandemias, la salud ocupacional con operador Y. La búsqueda se llevó a cabo en agosto de 2022 y pudo contar con los siguientes criterios de inclusión: textos en formato de artículo científico con texto completo, publicados en el período de la pandemia (2020-2022), en español, portugués e inglés. Se excluyeron artículos duplicados, encuestas de revisión y artículos reflexivos. Resultados: se encontraron 134 artículos, y tras la aplicación de los criterios de valoración del corpus textual, se obtuvo una muestra de 19 estudios. El análisis ha dado lugar a la creación de 30 categorías, por citar: cambios en el estilo de vida durante el trabajo a distancia; consecuencias relacionadas con la salud mental y el aislamiento y consecuencias relacionadas con el proceso de trabajo. El teletrabajo era un instrumento valioso porque ofrecía continuidad y funcionamiento de las operaciones durante la pandemia del Covid-19, ya que la salud física y mental del trabajador se veía inmensamente afectada. Conclusión: el teletrabajo ha demostrado ser una importante modalidad de trabajo frente a la pandemia del Covid-19. Aunque fue un instrumento indispensable para mantener el empleo y continuar el funcionamiento de las empresas durante la pandemia, la salud física y mental del trabajador se ha visto muy afectada en este período.

ABSTRACT: Antibody-drug conjugates (ADCs) deliver effective medications to tumor cells that express specific antigens, maximizing efficacy and reducing adverse effects. Because ado-trastuzumab emtansine was approved in 2013, 5 ADCs received US Food and Drug Administration approval for solid tumor treatment. Technical advancements in the development of each component of ADCs allowed novel monoclonal antibodies, linkers, and payloads to increase drug transport to malignant cells and drug activity even in cancers with heterogeneous antigen expression. In addition, several ADCs are in development using new molecular targets expressed across a broad range of histologies to allow the use of ADC biomarker-driven therapy irrespective of the primary tumor site. This suggests that the future efficacy of ADCs in multiple histologies may be similar to other classes of drugs that are considered histology-agnostic therapies nowadays. This review focuses on novel ADCs for the treatment of solid tumors, including topics such as their structure and mechanism of action, the latest indications of already US Food and Drug Administration-approved ADCs, and the outlook for new promising ADCs under development for the treatment of tumors of various histologies.
