Objetivo: Obtener un fitofármaco a base de extracto seco de limón con estándares de calidad, procesamiento amigable con el medio ambiente y con antecedentes de efectividad y seguridad, evaluando sus efectos en pacientes hipertensos con frecuentes crisis hipertensivas. Métodos y resultados: Se seleccionó la población de limónes de las Regiones Metropolitana y Coquimbo con las concentraciones más altas de ácido ascórbico y flavonoides totales, respectivamente. Se utilizó liofilización para obtener jugo de limón en polvo. Sólo durante el período de estudio, se informó un caso de aumento/ crisis hipertensiva. La formulación con la concentración más alta de ácido ascórbico disminuyó la presión arterial sistólica y diastólica en 16 mmHg desde 10 minutos hasta 60 minutos. Por otro lado, la formulación con mayor concentración de flavonoides disminuyó la presión arterial en 12 a 30 mmHg desde 5 a 60 minutos. Conclusión: Se obtuvo un producto innovador como complemento al manejo de los aumentos de presión arterial. Los principios activos con mayor contribución al mecanismo antihipertensivo del jugo de limón corresponden a compuestos fenólicos, específicamente, flavonoides.


Aim: To obtain a product based on a dry extract of standardized phytodrug lemon with high quality standards, using an environmentally friendly process; to evaluate its effects in hypertensive patients suffering frequent episodes of hypertensive crisis. Methods and Results: Lemons with high concentration of ascorbic acid (Coquimbo Region) or total flavonoids (Metropolitan Region) were selected. Lyophilization was used to obtain juice powder. During the study period only one case of hypertensive crisis was reported. The formula with the highest concentration of ascorbic acid decreased systolic and diastolic blood pressure by 16 mmHg from 10 minutes up to 60 minutes. On the other hand, the formula with the highest concentration of flavonoids decreased blood pressure from 12 to 30 mmHg between 5 and 60 minutes. Conclusion: An novel product to help decrease acutely elevated blood pressure was obtained that can be used as a complement to the management of acute blood pressure rises. The active principles with greater contribution to the antihypertensive mechanism of lemon juice corresponded to phenolic compounds, specifically flavonoids.

OBJECTIVE: to investigate the influence of the application of new methodologies on learning and the motivation of students of the Anatomy discipline. METHOD: randomized, longitudinal, prospective, intervention study. Sixty-two students were recruited to assess the impact of different methodologies. The sample was randomized to compare the results of teaching with a 3D atlas, ultrasound and the traditional method. The parameters were assessed through a satisfaction evaluation questionnaire and anatomical charts. Repeated measures ANOVA was used to determine statistical significance. RESULTS: in terms of the usefulness of the seminars, 98.1% of the students considered them to be very positive or positive, stating that they had stimulated their interest in anatomy. The students who learned with the 3D atlas improved their understanding of anatomy (p=0.040). In general, the students improved their grades by around 20%. CONCLUSION: the traditional method combined with new technologies increases the interest of students in human anatomy and enables them to acquire skills and competencies during the learning process.

Objetivo: investigar a influência da aplicação de novas metodologias na aprendizagem e a motivação de alunos da disciplina de Anatomia. Método: estudo de intervenção, prospectivo, longitudinal e randomizado. Um total de 62 alunos foram recrutados para se avaliar o impacto de diferentes metodologias. Eles foram distribuídos de forma aleatória para comparar os resultados de ensino usando o atlas 3D, o ultrassom e o método tradicional. Os parâmetros foram medidos por meio de um questionário de avaliação de satisfação do aluno e uma prova usando lâminas anatômicas. O teste ANOVA de medidas repetidas foi usado para determinar significância estatística. Resultados: relativo à satisfação dos alunos, 98,1% consideraram muito positivo ou positivo o uso dos seminários, além de afirmarem que o seminário havia estimulado seu interesse pela anatomia. Os alunos submetidos ao método com o atlas 3D apresentaram uma melhora em sua compreensão de anatomia (p=0,040). De forma geral, os alunos melhoraram suas pontuações em aproximadamente 20%. Conclusão: o método tradicional, junto com as novas tecnologias, contribui para aumentar o interesse dos alunos pela Anatomia Humana assim como na aquisição de habilidades e competências em seu processo de aprendizagem.

Método: estudio de intervención, prospectivo, longitudinal y aleatorizado. Fueron convocados 62 alumnos para evaluar el impacto de diferentes metodologías. Se los dividió aleatoriamente para comparar resultados entre enseñanza con atlas 3D, ultrasonido y metodología tradicional. Los parámetros fueron analizados mediante un cuestionario de evaluación de satisfacción y láminas anatómicas. Se usó ANOVA de medidas repetidas para determinar la significancia estadística. Resultados: el 98,1% de los alumnos consideraron muy positiva o positiva la utilización de los seminarios, afirmaron asimismo que el seminario había estimulado su interés por la anatomía. Los alumnos que realizaron el aprendizaje con atlas 3D mejoraron su compresión de la anatomía (p=0,040). En general, los alumnos mejoraron sus puntajes en alrededor del 20%. Conclusión: el método tradicional con el agregado de las nuevas tecnologías permite a los alumnos incrementar su interés por la Anatomía Humana, así como adquirir habilidades y competencias en su proceso de aprendizaje.

OBJECTIVE: The majority of patients who receive antihypertensive treatment do not achieve adequate blood pressure (BP) outcomes. Adherence to antihypertensive therapy contributes to adequate control of BP and is a substantial reason for therapeutic success. This study aims to evaluate the role of clinical pharmacists in improving patient adherence to pharmacological treatment and in improving BP outcomes. METHODS: A pre- and postpharmaceutical care interventional study was provided to hypertensive patients in the Cardiovascular Health Program of the Family Health Community Hospital in rural Coelemu, Chile. Patients received pharmaceutical care in three individualized sessions over 6 months. Written educational materials including basic information on high BP were provided to each patient in every session using easy-to-understand language. BP was measured at the beginning and end of each session using an electronic tensiometer, and patients reported their adherence to therapy using indirect, self-reported measures. FINDINGS: Over the study period, patients experienced average decreases in systolic and diastolic BP of 7.68 mmHg (P < 0.001) and 2.91 mmHg (P < 0.001), respectively. Patient adherence to medication, according to self-reported measures, increased from 22% to 60% over the study period. CONCLUSION: Pharmaceutical care, including education about hypertension, healthy lifestyle habits, and adherence, was associated with improved adherence to antihypertensive drug treatment and control of BP in hypertensive patients at a rural hospital in Chile.

INTRODUCTION: Pediculosis capitis is a public health problem with a high prevalence. The emergence of parasite resistance to conventional pediculicide is of great concern worldwide. OBJECTIVE: To develop alternatives pediculicide, effective and safe, based on the essential oil of Eucaliptus globulus. METHOD: Through bioassays active concentrations ranges of the essential oil were established, and proceeded to develop a standardized, stable, pharmaceutical form, evaluating its effects on our population. RESULTS: The results showed 100% effectiveness; short time of death, ovicidal action, activity on the adhesion of the egg, and low toxicity. DISCUSSION: In addition to great effect, the inability of the parasite to become resistant to the chemical composition of the essential oil makes this formulation an alternative to the problem of head lice solution.

OBJECTIVES: To assess and compare the prevalence of prescribing of potentially inappropriate medications (PIMs) identified using the Beers and STOPP criteria; and to determine the clinical variables related with the prescription of PIMs in older adults. METHODS: An observational study of 250 patients aged 65 years or older was conducted in a large teaching hospital. Beers (2012) and STOPP (2008) criteria were utilized to identify PIMs. Data on age, sex, admission and discharge dates, diagnoses, and medications prescribed were obtained from medical records. Multivariate logistic regression was used to determine patient variables related with the prescription of PIMs. KEY FINDINGS: Using Beers criteria, 375 PIMs were identified in 198 patients. 32% of these patients were prescribed one PIM, 20% were prescribed two PIMs, and 48% were prescribed more than two PIMs. Using STOPP criteria, 148 PIMs were identified in 120 patients. 41% of these patients were prescribed one PIM, 51% were prescribed two PIMs, and 8% were prescribed more than two PIMs. An association between PIM prescribing and polypharmacy was detected with both criteria. After adjusting for confounding variables, the prescription of Beers-identified PIMs was significantly associated with patients older than 80 (OR: 2.99; 95% CI: 1.13-7.89) and with length of hospital stay of more than 15days (OR: 2.78; 95% CI: 1.20-6.44). CONCLUSION: These two criteria showed that the prescription of PIMs is prevalent in hospitalized elderly patients. It may be beneficial to educate healthcare teams about these criteria to reduce the prescription of PIMs.

Introducción: La pediculosis capitis es un problema de salud pública con una alta prevalencia. La aparición de resistencia del parásito a los pediculicidas convencionales es de gran preocupación a nivel mundial. Objetivo: Desarrollar alternativas pediculicidas, efectivas y seguras, en base al aceite esencial de Eucaliptus globulus. Método: A través de bioensayos se establecieron rangos de concentraciones activas del aceite esencial, y se procedió al desarrollo de una forma farmacéutica, estandarizada, estable, evaluando sus efectos en nuestra población. Resultados: Los resultados mostraron 100% de efectividad; corto tiempo de muerte, acción ovicida, actividad sobre la adherencia del huevo, y baja toxicidad. Discusión: Además de la gran efectividad, la imposibilidad del parásito de adquirir resistencia a la composición química del aceite esencial hace de esta formulación una solución alternativa al problema de la pediculosis.

BACKGROUND: Non-adherence to prescribed treatment is a principle cause of treatment failure in patients with chronic diseases, such as type 2 diabetes mellitus (T2DM) and hypertension. Improved patient monitoring and education have been shown to improve adherence and clinical outcomes. OBJECTIVE: This study evaluates the impact of a pharmacotherapy optimization plan centered on patient education that was implemented at a hospital in Arauco, Chile. METHODS: A prospective study was conducted using 50 randomly selected patients diagnosed with both T2DM and hypertension. Each patient participated in three successive interview sessions over 6 months. At each session, the following variables were evaluated by a pharmacist: blood pressure, HbA1c, adherence to prescribed medication, and patient knowledge of disease. RESULTS: Significant decreases were observed for HbA1c (<0.001), and SBP (<0.001), with stronger and more significant decreases observed for females. Patient adherence to prescribed medication also improved, but not significantly. The number of patients with full knowledge of their diseases increased significantly (p < 0.001) from 10 to 66%. CONCLUSION: This study suggests that the incorporation of a pharmacotherapeutic optimization plan for patients with chronic diseases can have a positive impact on the control of chronic diseases, such as T2DM and hypertension.

The aim of this study was analyze the use of restricted antibiotics by patients hospitalized between 2004 and 2008 in Guillermo Grant Benavente Hospital in Concepcion. Also we attempted to identify possible correlations between antibiotic consumption and patterns of bacterial susceptibility. We performed a retrospective observational study that quantified the use of restricted antibiotics using DDD/100-bed-days, and cumulative susceptibility reports informed by the hospital's microbiology laboratory for bacterial susceptibility. The consumption of restricted antibiotics significantly increased between 2004 and 2008 (35%, p = 0.005). The groups with largest use were glycopeptides (37%) and carbapenems (30 %). These results can be explained by the emergence of endemic Methicillin-resistant Staphylococcus aureus (MRSA) and of Extended-spectrum beta-lactamase (ESBL) Gram negative bacilli. Results showed a decrease in susceptibility of P. aeruginosa to imipenem (p = 0.038) and K. pneumoniae to ciprofloxacin (p = 0.021). The total consumption of restricted antibiotic has significantly increased, especially among complex medical services. A significant decrease in bacterial susceptibility has been observed mainly in gram-negative bacilli. The monitoring of antimicrobial prescribing practices and local susceptibility patterns are essential to promote the rational use of antibiotics.


En Chile no existen estudios para cuantificar el consumo de antimicrobianos de uso restringido al interior de los hospitales. Objetivo: Analizar el consumo de antimicrobianos de uso restringido en pacientes hospitalizados durante los años 2004-2008 en el Hospital Guillermo Grant Benavente de Concepción. Además, se analizaron las correlaciones entre este consumo y el patrón de susceptibilidad in vitro. Material y Método: Se diseñó un estudio observacional retrospectivo empleando las DDD/100-días-cama para evaluar el consumo de antimicrobianos, y el informe acumulado de susceptibilidad in vitro entregado por el laboratorio local, para analizar la evolución de la susceptibilidad. Resultados: El consumo de antimicrobianos se incrementó en 35% (p = 0,005) durante los años 2004-2008, donde los más consumidos fueron glicopéptidos (37%) y carbapenémicos (30%). Estos resultados se pueden explicar por la aparición de cepas de Staphylococcus aureus resistente a meticilina y bacilos gramnegativos productores de Q-lactamasas de espectro extendido. Además, se observó una disminución de la susceptibilidad de Pseudomonas aeruginosa a imipenem (p = 0,038) y de Klebsiella pneumoniae a ciprofloxacina (p = 0,021). Conclusiones: El consumo total de antimicrobianos de uso restringido se incrementó significativamente en los servicios clínicos más complejos, observándose una disminución de la susceptibilidad de algunos bacilos gramnegativos. El monitoreo de la prescripción de antimicrobianos así como de la susceptibilidad in vitro local constituyen medidas esenciales para promover el uso racional de antimicrobianos.
