INTRODUCTION: Acne keloidalis nuchae (AKN) is a primary cicatricial alopecia with mixed infiltrate. It is more common in Africans or persons of African descent. OBJECTIVES: Our objective was to describe the epidemiology and clinical and trichoscopic presentations of AKN in a large series of Hispanic patients. METHODS: This was a retrospective study from 10 different dermatological centers in Argentina, Colombia, Mexico, and Peru. Patients with a clinical diagnosis of AKN treated by 12 dermatologists experienced in trichology from 2018 to 2022 were included. The Umar classification system was used to determine severity. RESULTS: We identified 142 patients with AKN: 98% were male (n=140) with a mean age of 32 years; 108 patients had a previous history of trauma to the nuchal area (76%, P < 0.001); and 48 were positive for a history of acne (33.8%, P = 0.021). Patients with >50 months of evolution were mainly classified in classes III and IV compared to patients with an evolution of <50 months (30%, n=9 vs. 14%, n=15; P = 0.019; respectively). CONCLUSION: AKN should be considered in the differential diagnosis in the Hispanic population. Advanced stages of the disease are correlated with chronic evolution.

RESUMO

La colistina o polimixina E es un antibiótico cuyo uso fue descontinuado por la toxicidad renal y neurológica relacionadas al uso de colistina sulfato. Estos efectos adversos han disminuido con el uso del profármaco colistimetato sódico. Actualmente el uso de colistina es más frecuente debido al incremento de infecciones ocasionadas por bacilos Gram negativos multirresistentes, sobre todo en las unidades de cuidados intensivos. Presentamos el caso de una mujer de 50 años de edad con antecedente de consumo de anti-inflamatorios no esteroideos y corticoides, pos- operada de perforación gástrica que evolucionó con colecciones abdominales por Acinetobacter sp. multirresistente. Recibió 34 días de colistina endovenosa y desarrolló hiperpigmentación cutánea, ataxia (neurotoxicidad) y falla renal (nefrotoxicidad) de forma secuencial secundaria a la administración de colistina, los efectos adversos desaparecieron con la suspensión del antibiótico. Colistin or polymycin E is an antibiotic that was discontinued due to its renal and neurologic toxicity related to its colistin sulfate content. These adverse effects have been reduced with the use of sodium colistemathe. There is currently a more frequent use of colistin due to an increase of multi-resistant Gram negative infections, particularly in intensive care units. We present the case of a 50-year-old woman with history of surgery due to gastric perforation, use of steroids and non-steroidal anti-inflammatory drugs, that developed intra-abdominal abscesses due to multidrug resistant Acinetobacter spp. She received 34 days of intravenous colistin and developed skin hyperpigmentation, ataxia and renal failure. These adverse effects disappeared with discontinuation of the drug. (AU)

BACKGROUND: Lichen planopilaris (LPP) and frontal fibrosing alopecia (FFA) represent 2 entities that cause primary cicatricial alopecia. These entities are clinically different; nevertheless, the literature suggests that FFA represents a form of LPP. The main argument in support of this hypothesis is that previous studies comparing the histologic findings have not found obvious differences between these diseases. METHODS: Our objective was to more critically compare and contrast 20 histologic findings of these diseases in a large number of patients in order to determine any significant histologic differences between LPP and FFA. RESULTS: We found 3 parameters that were statistically different, namely the presence of terminal catagen-telogen hairs (50% FFA vs 23.5% LPP; P = .020); a severe perifollicular inflammatory infiltrate (29.4% LPP vs 4.6% FFA; P = .010) and a zone of concentric lamellar fibroplasia (85.3% LPP vs 63.6% FFA; P = .041). CONCLUSIONS: Although a few histologic features differ between FFA and LPP, we believe that these differences are too subtle or non-specific to distinguish between them with confidence. Therefore, clinical correlation is essential to establish the diagnosis.

Se presenta el caso de un paciente varón de 56 años quien es evaluado por presentar a nivel del dorso de ambas manos cicatrices hiperpigmentadas e hipopigmentadas, asociadas a quistes de milia. Se le realizó estudios del metabolismo de las porfirinas y biopsia cutánea de las lesiones los cuales resultaron compatibles con porfiria cutánea tarda. En el laboratorio inicial se encontró elevación de los valores de transaminasas, identificándose posteriormente infección crónica por virus de hepatitis C. Con la finalidad de tratar la infección viral y resolver el compromiso dérmico, considerado como manifestación extrahepática del virus hepatitis C, se inició tratamiento con interferón pegilado y ribavirina evolucionando favorablemente con respuesta viral rápida, carga viral no detectable hasta la actualidad (36 semanas de tratamiento), disminución del nivel de transaminasas séricas y mejoría de las lesiones dérmicas.

It was studied the metabolism of porphyrins and skin biopsy of the lesions which were compatible with porphyria cutanea tarda. In the initial laboratory, elevated transaminases values were found and subsequently identified chronic infection of hepatitis C virus. In order to treat viral infection and resolve the dermal commitment; considered extrahepatic manifestation of hepatitis C virus, treatment was started with pegylated interferon and ribavirin, with favorably development and rapid viral response, with undetectable viral load until now (24 weeks of treatment), decreased level of serum transaminases and improvement of skin lesions.
