In late October 2021, one of the veterinarians and the occupational physician of a bovine and swine abattoir from Entre Ríos Province, Argentina were alerted about workers with atypical pneumonia symptoms, raising suspicious of a possible Q fever outbreak. An outbreak epidemiological investigation was carried out. Analysis was based on the description of the study population, according to gender, age, symptoms, and position within the abattoir, as well as on outbreak epidemic curve and its probable origin. Cases of Q fever in the workers were confirmed by serology. Measurements of the association between the evaluated variables and the risk of exposure were investigated and calculated as attack rates. The outbreak occurred between October and November 2021, symptomatically affecting 11 workers, out of a total exposed population of 49 individuals. The index case was a 33-year-old male who started with symptoms on 27 October 2021, and the outbreak extended for at least 17 days. Workers in the clean zone of the slaughter floor had a 4.68 times higher risk of contracting Q fever than people located in other areas. Importantly, two pregnant cows were slaughtered a few days before the outbreak began, which could have been the origin of the outbreak. The present study demonstrates the urgent need to consider Q fever when diagnosing abortive diseases of ruminants in Argentina, as well as in zoonotic disease epidemiological surveillance to inform all actors of the health system.

Prognostic scales may help to optimize the use of hospital resources, which may be of prime interest in the context of a fast spreading pandemics. Nonetheless, such tools are underdeveloped in the context of COVID-19. In the present article we asked whether accurate prognostic scales could be developed to optimize the use of hospital resources. We retrospectively studied 467 files of hospitalized patients after COVID-19. The odds ratios for 16 different biomarkers were calculated, those that were significantly associated were screened by a Pearson's correlation, and such index was used to establish the mathematical function for each marker. The scales to predict the need for hospitalization, intensive-care requirement and mortality had enhanced sensitivities (0.91 CI 0.87-0.94; 0.96 CI 0.94-0.98; 0.96 CI 0.94-0.98; all with p < 0.0001) and specificities (0.74 CI 0.62-0.83; 0.92 CI 0.87-0.96 and 0.91 CI 0.86-0.94; all with p < 0.0001). Interestingly, when a different population was assayed, these parameters did not change considerably. These results show a novel approach to establish the mathematical function of a marker in the development of highly sensitive prognostic tools, which in this case, may aid in the optimization of hospital resources. The biomagnification of Hg and Se was studied using nitrogen stable isotope analysis during four seasons in a coastal lagoon of the eastern central Gulf of California. This lagoon receives agricultural, municipal, and shrimp aquaculture effluents. The species were categorized into organism groups and presented a significant accumulation of Hg and Se with respect to the sources, while the concentration of both elements in sediment and suspended particulate matter (SPM) was low. Our data confirms the positive transfers (biomagnification factors >1) of Hg and Se in the entire studied food web, and it was structured in five trophic levels across all seasons. Additionally, there were no linear correlations between the molar Se:Hg ratios and the trophic levels of the organism groups. However, the Se:Hg ratios among organism groups were >1, which indicates that there is an excess of Se and that it is not a limiting factor for the detoxification of Hg.

The perception that food affects our health has increased over the past fifty years in industrialised Western countries. The notion of "healthy food" has become ubiquitous in medical, political, and media discourse as well as in the discourses of the lay population. This study seeks to understand the social representations of "healthy food" of Brazilian, Spanish and French dietitians and young laywomen. A qualitative and comparative methodology based on 131 individual semi-structured interviews was set up. According to the analysis of the discourses, the notion of "healthy food" is ambivalent and polysemic. Scientific-nutritional rationality is not the only way to think about "healthy food". Two main ways of categorising "healthy foods" are observed in the three countries. They are based on eating analysis criteria that referred to diverse value systems and different scientific, symbolic and moral rationalities: on the one hand, a physiological, nutritional and functional conception, and on the other, an "eco-ideological" conception that took into account production, culture and distribution methods. Though dietitians have a greater technical knowledge of nutrients and metabolic processes than laywomen, professionals and young laywomen, mainly within the same nationality, shared similar discourses concerning the notion of "healthy food", revealing patterns that differentiate each nationality. Taking together, the results reveal that although medical-nutritional discourses are disseminated internationally, discourses on "healthy food" are constructed taking into account social, cultural, symbolic and moral dimensions. These discourses are linked to a historical and sociocultural context.

In Central and South American traditional medicine, people use Cecropia obtusifolia Bertol (Cecropiaceae) for the treatment of diabetes mellitus. However, its hypoglycemic action mechanism at pancreatic and liver level has been poorly explored. The present research aimed to establish the influence of the aqueous extract of C. obtusifolia, standardized in its content of chlorogenic acid, on insulin secretion in RINm5F cells and over the liver carbohydrates and lipids metabolism, and to determine concomitantly its hepatoprotective effect on mice with streptozotocin-induced diabetes. In RINm5F cells, concentrations 5, 50, 100, and 200 µg/mL of aqueous extract of C. obtusifolia were used to determine [Ca2+]i and insulin secretion. In an acute study, the extract was administered at doses of 500 mg/kg. In another test (subacute), the extract was daily administrated to diabetic mice (200 mg/kg/day) for 30 days. Blood glucose levels and other biochemical parameters were determined, and a liver histological analysis was performed. In RINm5F cells, C. obtusifolia increased [Ca2+]i and insulin secretion, whereas in diabetic mice exhibited acute and subacute hypoglycemic effects. Daily administration of C. obtusifolia to diabetic mice also increased liver glycogen storage and glycogen synthase levels, without apparent changes in gluconeogenesis. Besides, it increased peroxisome proliferator-activated receptor-α (PPAR-α) and long-chain-fatty-acid-CoA ligase 1 (ACSL-1) expression and reduced triglycerides, transaminases (alanine aminotransferase and aspartate aminotransferase), and collagen fibers, modifying anti-inflammatory (adiponectin and interleukin-10) and inflammatory (tumor necrosis factor-α) cytokines in serum. Therefore, the hypoglycemic effect of C. obtusifolia implicates a dual action, promoting insulin secretion, liver glycogen accumulation, and hepatoprotection by decreasing collagen fibers and inflammatory markers, whereas it improves lipid metabolism, due in part to PPAR-α.

ABSTRACT Objective: To investigate the influence of patient age on the diagnosis and management of appendicitis, as well as to evaluate the rate of complications according to the age group. Methods: We undertook a retrospective analysis of 1,736 children who underwent laparoscopic appendectomy in our center between January 2000 and December 2013. Patients were divided in groups taken into account their age: group A were infants, group B were preschoolers, group C were those ones older than five years old, and group D were those ones younger than five years old. A p value of 0.05 was considered statistically significant. Results: We found higher incidence of misdiagnosis and atypical symptoms in the youngest patients. The rate of perforation was similar between group A and B (p=0.17). However, it was higher in group D than in group C (p<0.0001). The incidence of postoperative complications was higher in the youngest patients too (p=0.0002). Conclusions: The age does make a difference in acute appendicitis. Because of its unusual presentation in children younger than five years old, it is often misdiagnosed, which leads to an increased morbidity. Although clinical presentation varies between infants and preschoolers, no statistically significant differences were observed in the rate of perforated appendix or postoperative complications.


RESUMO Objetivo: Investigar a influência da idade do paciente no diagnóstico e tratamento de apendicite, bem como avaliar a frequência de complicações dependendo da faixa etária. Métodos: Análise retrospectiva dos 1.736 pacientes pediátricos que foram submetidos à apendicectomia laparoscópica em nosso hospital de janeiro de 2000 a dezembro de 2013. Os pacientes foram divididos em grupos de acordo com sua idade: grupo A eram crianças, grupo B eram pré-escolares, grupo C eram maiores de cinco anos de idade e grupo D eram menores de cinco anos de idade. Considerou-se estatisticamente significante p-valor <0,05. Resultados: Encontramos maior incidência de diagnóstico incorreto e sintomas atípicos em pacientes mais novos. A taxa de perfuração foi semelhante entre os grupos A e B (p=0.17); foi maior, porém, no grupo D que no grupo C (p<0.0001). A incidência de complicações no pós-operatório também foi maior em pacientes mais novos (p=0.0002). Conclusões: A idade faz diferença em casos de apendicite aguda. Por causa da sua apresentação rara em crianças menores de cinco anos, é frequentemente diagnosticada incorretamente, o que aumenta a morbidade. Apesar de sua apresentação clínica variar entre lactentes e pré-escolares, não foram observadas diferenças estatisticamente significativas na proporção de apêndices perfurados nem na de complicações pós-operatórias.

The natural killer group 2 (NKG2) family of receptors, encoded within the NK complex gene region (NKC), modulate the cytotoxic activity of NK cells. Two haplotype blocks throughout the NKC, hb-1 and hb-2 have been associated with different levels of overall natural cytotoxicity. Here, we evaluated allelic and genotype frequencies at rs1049174, rs2617160, rs2617170, rs2617171, rs1983526 (hb-1 haplotype), and rs2255336 and rs2246809 (hb-2 haplotype) in 928 subjects examined from Mexico City. The most frequent alleles and genotypes were as follows: C, CG to rs1049174; G, GG to rs2255336; T, AT to rs2617160; G, GG to rs2246809; C, CT to rs2617170; G, CG to rs2617171; and G, CG to rs1983526. Linkage disequilibrium analysis revealed that rs1049174, rs2617160, rs2617170, and rs2617171 constituted the haplotype block-1 variant (hb-1v) (r2 ≥ 0.89). Two predominant haplotypes of hb-1v were identified based on the allele content and included CTCG and GATC. This study is the first to evaluate the allelic and genotype frequency distribution of rs1049174, rs2255336, rs2617160, rs2246809, rs2617170, rs2617171, and rs1983526 in the population of Mexico City.

RESUMEN En este artículo se describen y analizan las percepciones sociales sobre el riesgo alimentario en Cataluña (España), que inciden en qué alimentos son percibidos como peligrosos, pero también en cómo, cuándo, dónde, por qué y de qué manera se concibe este riesgo a partir de las narrativas de los informantes. A través de un estudio cualitativo, se ha explorado cómo se construyen y gestionan los discursos legos, que conforman imaginarios diversos sobre el riesgo alimentario que no siempre coinciden con la mirada biomédica. Se señala que el riesgo sobre los alimentos no siempre se asocia a los peligros del progreso o la industrialización, ni se centra necesariamente en el debate dicotómico “alimento industrializado” versus “alimento natural”, sino en un conjunto de posibilidades que tienen que ver también con el contexto de su producción, distribución, preparación y/o consumo.


ABSTRACT This article describes and analyzes social perceptions regarding food risks in Catalonia (Spain). In particular it uses the narratives of informants to determine which foods are perceived as dangerous and how, when, where and why this perception of risk develops. Through a qualitative research study, we explored how lay discourses are constructed and managed, creating diverse imaginaries regarding food risk that do not always coincide with the biomedical view. It is highlighted that food risk is not always associated with the dangers of progress or industrialization, nor is it necessarily focused on the dichotomous debate of “industrially produced food” versus “natural food”; rather food risk perceptions revolve around a series of possibilities that are also related to the production, distribution, preparation and/or consumption of food.

BACKGROUND: Whether low serum magnesium is an epiphenomenon related with obesity or, whether obesity per se is cause of hypomagnesemia, remains to be clarified. OBJECTIVE: To examine the relationship between body weight status and hypomagnesemia in apparently healthy subjects. METHODS: A total of 681 healthy individuals aged 30 to 65years were enrolled in A cross-sectional study. Extreme exercise, chronic diarrhea, alcohol intake, use of diuretics, smoking, oral magnesium supplementation, diabetes, malnutrition, hypertension, liver disease, thyroid disorders, and renal damage were exclusion criteria. Based in the Body Mass Index (BMI), body weight status was defined as follows: normal weight (BMI <25kg/m2); overweight (BMI ≥25<30 BMIkg/m2); and obesity (BMI ≥30kg/m2). Hypomagnesemia was defined by serum magnesium concentration ≤0.74mmol/L. A multiple logistic regression analysis was used to compute the odds ratio (OR) between body weight status (independent variables) and hypomagnesemia (dependent variable). RESULTS: The multivariate logistic regression analysis showed that dietary magnesium intake (OR 2.11; 95%CI 1.4-5.7) but no obesity (OR 1.53; 95%CI 0.9-2.5), overweight (OR 1.40; 95%CI 0.8-2.4), and normal weight (OR 0.78; 95%CI 0.6-2.09) were associated with hypomagnesemia. A subsequent logistic regression analysis adjusted by body mass index, waist circumference, total body fat, systolic and diastolic blood pressure, and triglycerides levels showed that hyperglycemia (2.19; 95%CI 1.1-7.0) and dietary magnesium intake (2.21; 95%CI 1.1-8.9) remained associated with hypomagnesemia. CONCLUSIONS: Our results show that body weight status is not associated with hypomagnesemia and that, irrespective of obesity, hyperglycemia is cause of hypomagnesemia in non-diabetic individuals.
