RESUMO

Los gemelos toracópagos están unidos de hecho como se dice cara a cara por el pecho y suelen compartir uno o varios órganos y en este caso y en particular el corazón. Existen casos reportados hace años atrás en las que algunos suelen compartir el hígado el diafragma e incluso intestino, y obviamente la piel o puente tisular. En este reporte de caso se trata de una paciente multigesta, y producto de su tercera gestación que fue diagnosticada como embarazo gemelar tora copago o (siamés toraco pago). La cual fue captada inicialmente en un primer nivel de atención proveniente del área rural y derivada a segundo nivel en nuestra ciudad para su reevaluación por especialidad de gineco-obstetricia utilizando como herramienta diagnóstica un equipo ultrasonográfico de alta gama.

MAX phases are an advanced class of ceramics based on ternary carbides or nitrides that combine some of the ceramic and metallic properties, which make them potential candidate materials for many engineering applications under severe conditions. The present work reports the successful synthesis of nearly single bulk Ti2AlN MAX phase (>98% purity) through solid-state reaction and from a Ti and AlN powder mixture in a molar ratio of 2:1 as starting materials. The mixture of Ti and AlN powders was subjected to reactive spark plasma sintering (SPS) under 30 MPa at 1200 °C and 1300 °C for 10 min in a vacuum atmosphere. It was found that the massive formation of Al2O3 particles at the grain boundaries during sintering inhibits the development of the Ti2AlN MAX phase in the outer zone of the samples. The effect of sintering temperature on the microstructure and mechanical properties of the Ti2AlN MAX phase was investigated and discussed.

RESUMEN Con el objetivo de determinar la utilidad de la citometría de flujo para la detección de Pseudomonas aeruginosa productoras de metalobetalactamasas (MBL), se estudiaron aislamientos de P. aeruginosa genotípicamente caracterizados del cepario del laboratorio de Epidemiología Molecular y Genética de la Universidad Nacional Mayor de San Marcos. Se analizaron 29 aislamientos (17 productoras de MBL y 12 no productoras de MBL) con el kit de viabilidad celular FACSCalibur (Becton Dickinson). Se utilizaron dos tratamientos, uno con meropenem y el otro con meropenem-EDTA. Usando la razón de aumento de fluorescencia en las células no vivas, se demostró una diferencia significativa entre las productoras de MBL y las no MBL, considerando como punto de corte una razón >1,6. Se determinó una sensibilidad de 94,1% y una especificidad del 100%. La citometría de flujo constituye una alternativa para la detección de P. aeruginosa productora de MBL.

INTRODUCTION: Kidney chronic disease patients are being increasingly identified. The disability generated by this disease must consider physical and social effects given the lack of attention and the socioeconomic conditions that generate it. Therefore, access to services to treat kidney chronic disease is determined by social and biological factors. OBJECTIVE: To analyze the effect of the social components on kidney chronic disease in a sector of the Mexican population that suffers from the disease, particularly in the case of women. METHODS: The Poisson generalized linear model was applied, selecting the variables related to equity in the administration of health services. Statistical data reported by the National Institute of Statistics and Geography of Mexico in the period 2009-2015 in women was taken. The variables considered were the level of schooling, occupation, access to health, geographical region and habitable zone, as well as stage of life. RESULTS: The highest incidence rate for kidney chronic disease is attributed to the intermediate adult woman, who works in informal services legally excluded from institutional health coverage, has low schooling and lives in a rural area of the Center zone, while the young adult woman that lives in an urban metropolis in the North zone presents lowest incidence profile. CONCLUSIONS: The economic determinants derived from people’s activities, as well as their age, the educational level and the environment in which they live influence both the acquisition of the disease and the possibilities of managing it successfully.


INTRODUCCIÓN: La insuficiencia renal crónica es una enfermedad que se encuentra en un estado de constante crecimiento. La discapacidad que genera esta enfermedad debe considerar efectos físicos y sociales, dada la falta de atención y a las condiciones socioeconómicas que la generan. Por lo tanto, el acceso a los servicios para tratar la insuficiencia renal crónica está condicionado a factores de tipo social y de tipo biológico. OBJETIVO: Analizar el efecto de los componentes sociales en la insuficiencia renal crónica en un sector de la población de México que padece la enfermedad, en particular para el caso de la mujer. MÉTODOS: Se aplicó el modelo lineal generalizado de Poisson, seleccionando las variables relacionadas con la equidad en la aplicación de los servicios de salud. Se tomaron datos estadísticos reportados en mujeres por el Instituto Nacional de Estadística y Geografía de México en el periodo 2009-2015. Las variables consideradas fueron grado de escolaridad, ocupación, acceso a la salud, región geográfica y zona habitable, así como le etapa de vida. RESULTADOS: La mayor tasa de incidencia para la insuficiencia renal crónica corresponde a la mujer adulta intermedia, que trabaja en servicios informales excluidos legalmente de la cobertura institucional de salud, tiene baja escolaridad y vive en un área rural de la zona centro; mientras que la mujer adulta joven que vive en una metrópoli urbana de la zona norte presenta el perfil de menor incidencia. CONCLUSIONES: Los determinantes económicos derivados de la actividad de las personas, así como la edad, el nivel educativo y el entorno en el que habitan, influyen tanto en el padecimiento de la enfermedad como en las posibilidades de enfrentarla con éxito.

BACKGROUND: Aim: To compare the concentration of secretory immunoglobulin-A (sIgA) in young adults with average or excellent aerobic capacity before and after a cardiopulmonary graded exercise test. METHODS: Participants were nine apparently healthy physically active males (Mean age = 21.3 ± 2.1 yr.), randomly allocated in two groups based on their VO2max: a) average aerobic capacity (AEC, n = 5) or b) excellent aerobic capacity (EAC, n = 4). Participants performed the Bruce protocol to determine their aerobic capacity. The sIgA was measured before the test, immediately after the test and 60-, 120-, 240-, and 1440-min after the test. RESULTS: Mixed factorial 2 x 6 ANOVA indicated no significant interactions between groups and measurements (p = 0.956), and main effect groups on sIgA (AEC = 85.4 ± 19.3 µg/mL vs. EAC = 79.2 ± 21.5 µg/mL, p = 0.836). Tukey's post hoc analysis revealed significant differences measurement obtained immediately after the test and between the initial measurement (p = 0.020), 60-min (p = 0.030), 240-min (p = 0.016), and 1440-min (p = 0.028) following the test. CONCLUSION: There is no change in sIgA kinetics depending on the aerobic capacity of the participants following an aerobic capacity cardiopulmonary graded exercise test.

BACKGROUND: Glutamine is the most abundant free amino acid in the body. It modulates immune cell function and is an important energy substrate for cells in critically ill patients. Reduction of injury cardiac markers had been observed in patients receiving intravenous glutamine and in a pilot study with oral glutamine. The aim of this study was to analyze the effect of preoperative oral supplementation of glutamine on postoperative serum levels of cardiac injury markers. METHODS: A randomized clinical trial was performed in 28 Mexican patients with ischemic heart disease who underwent cardiopulmonary bypass with extracorporeal circulation. Patients were randomly assigned to receive oral glutamine (0.5 g/kg/day) or maltodextrin 3 days before surgery. Cardiac injury markers as troponin-I, creatine phosphokinase, and creatine phosphokinase-Mb were measured at 1, 12, and 24 hours postoperatively. RESULTS: At 12 and 24 hours serum markers levels were significantly lower in the glutamine group compared with controls (p = 0.01 and p = 0.001, respectively) (p = 0.004 and p < 0.001, respectively). Overall, complications were significantly lower in the glutamine group (p = 0.01, RR = 0.54, 95% CI 0.31-0.93). Mortality was observed with 2 cases of multiple organ failure in control group and 1 case of pulmonary embolism in glutamine group (p = 0.50). CONCLUSION: Preoperative oral glutamine standardized at a dose of 0.5 g/kg/day in our study group showed a significant reduction in postoperative myocardial damage. Lower cardiac injury markers levels, morbidity and mortality were observed in patients receiving glutamine.

Introducción. Objetivo: Comparar la cinética en la concentración de inmunoglobulina A salival (IgAs) en adultos jóvenes con capacidad aeróbica promedio (n: 5) o excelente (n: 4) antes y después de una prueba de esfuerzo. Método: 9 adultos jóvenes (edad 21,3 ± 2,1), divididos de acuerdo su VO2máx, realizaron una prueba de esfuerzo mediante el protocolo de Bruce. La concentración de IgAs fue determinada mediante el Salimetrics IgA Kit®, evaluando inicial, inmediatamente finalizada la prueba, +60, +120, +240, +1.440 min. Resultados: La prueba ANOVA 2x6 mixta indicó que no existieron interacciones significativas entre grupos y mediciones (p = 0,956). Tampoco se encontró una diferencia significativa en la media de IgAs en los grupos (Promedio = 85,4 ± 19.3 μg/mL vs Excelente=79,2 ± 21.5 μg/mL, p = 0,836). Independientemente de las mediciones, el análisis post hoc de Tukey indicó que las diferencias se encontraron en la medición obtenida inmediatamente después de la prueba y entre la medición inicial (p: 0,020), la medición obtenida 60 min (p: 0,030), 240 min (p: 0,016) y 1.440 min (p = 0,028) posteriores a la prueba. Conclusión: Los datos encontrados sugieren que no hay un cambio en la concentración de IgAs a través del tiempo en función de la capacidad aeróbica de los participantes.

La profilaxis antibiótica debe considerarse en personas con contacto con algún caso de enfermedad meningocócica o en poblaciones con altos porcentajes de portadores de N. meningitidis. El uso de antibióticos produce una reducción significativa del riesgo de enfermedad entre los contactos; así, la rifampicina, la ciprofloxacina y la ceftriaxona se consideran como las mejores opciones para la quimioprofilaxis, sin embargo su empleo está asociado con el aumento de resistencia antibiótica. Actualmente, la tendencia a la aparición de meningococos resistentes a la rifampicina después de la profilaxis es un aspecto reconocido, aunque parece que no es un fenómeno ampliamente extendido. La aparición de resistencia de alto nivel a la rifampicina está provocada por mutaciones en el gen rpoB, aunque se puede asociar a mutaciones del locus mtr implicadas en mecanismos de expulsión y bombeo. Sin embargo, los cambios en el gen rpoB dan lugar a cepas poco adaptadas a la supervivencia y este costo biológico podría explicar la ausencia de diseminación clonal de los aislamientos con resistencia adquirida a la rifampicina. La resistencia o sensibilidad disminuida a la ciprofloxacina se relaciona con mutaciones en la región determinante de resistencia a quinolonas (QRDR) del gen gyrA, además existen datos que apoyan la existencia de mecanismos de expulsión. Hasta el momento, la resistencia o la sensibilidad reducida a las quinolonas ha avanzado lentamente. No se han comunicado problemas de resistencia a la ceftriaxona, siendo la opción más segura para su uso en quimioprofilaxis.La espiramicina no es una opción adecuada aunque sigue siendo recomendada por la OMS.

BACKGROUND: Outbreaks of microepidemics of tuberculosis indicate the existence of Mycobacterium tuberculosis transmission. We describe a microepidemic in a school in which an index case with low infective capacity gave rise to a high percentage of tuberculosis infection and disease in the center. METHODS: Contact investigation was performed in 423 pupils of a school after a recently arrived teacher with few symptoms was diagnosed with tuberculosis. A cross-sectional study was carried out to calculate the prevalence of tuberculosis infection and disease in the school center. RESULTS: Statistically significant differences were found in the distribution of positive results in the first tuberculosis skin test for primary and secondary students, with more positive results in secondary school pupils (18.4%) than in primary school pupils (6.1%). The incidence of skin tuberculosis test converters was 5.7%. The prevalence of infection by educational level was 10% in primary education and 23.4% in secondary education. The risk of infection in secondary school pupils was more than twice that in primary school pupils (OR = 2.4; 95% CI, 1.5-3.5). There were 6 new cases of tuberculosis in pupils. CONCLUSION: The high percentage of conversions in the second tuberculosis skin test indicated the existence of transmission in the school. The results of this study support contact investigation in schools where there is exposure to a case of tuberculosis, whether highly infective or nort.
