RESUMO

The use and knowledge of native and naturalized medicinal plants were evaluated in four communities of Villa Corzo, Chiapas, Mexico. The information was obtained through semi - structured surveys conducted with 1 18 people of different age and gender categories. To determine the most important plant species, indexes of value and frequency of use were generated. Seventy - nine species of 49 families were recorded, as well as their use in the treatment of various disea ses according to local knowledge. The species with the highest value of use were Aloe vera (L.) Burm. f., Verbena officinalis L. (verbena), Ocimum basilicum L. (basil) and Psidium guajava L. (guava). (guava). The most important families by number of specie s used were, in order of importance: Asteraceae and Rutaceae with five species Lamiaceae, Fabaceae and Rosaceae with four species each.


Se evaluaron el uso y conocimiento de las plantas medicinales nativas y naturalizadas en cuatro comunidades de Villa Corzo, Chiapas, México. La información se obtuvo a través de encuestas semiestructuradas realizadas a 118 personas de distintas categorías de edad y género. Para determinar las especies de plantas más importantes se generaron índices de valor y frecuencia de uso. Se registraron 79 especies de 49 familias, así como su uso en el tratamiento de diversas enfermedades de acuerdo al conocimiento local. Las especies con mayor valor de uso fueron Aloe vera (L.) Burm. f. (sábila), Verbena officinalis L. (verbena), Ocimum basilicum L. (albahaca) y Psidium guajava L. (guayaba). Las familias más importantes por el número de especies utilizadas fueron, en orden de importancia: Asteraceae y Rutaceae con cinco especies, Lamiaceae, Fabaceae y Rosaceae con cuatro especies cada una.

Protected areas are of paramount relevance to conserving wildlife and ecosystem contributions to people. Yet, their conservation success is increasingly threatened by human activities including habitat loss, climate change, pollution, and species overexploitation. Thus, understanding the underlying and proximate drivers of anthropogenic threats is urgently needed to improve protected areas' effectiveness, especially in the biodiversity-rich tropics. We addressed this issue by analyzing expert-provided data on long-term biodiversity change (last three decades) over 14 biosphere reserves from the Mesoamerican Biodiversity Hotspot. Using multivariate analyses and structural equation modeling, we tested the influence of major socioeconomic drivers (demographic, economic, and political factors), spatial indicators of human activities (agriculture expansion and road extension), and forest landscape modifications (forest loss and isolation) as drivers of biodiversity change. We uncovered a significant proliferation of disturbance-tolerant guilds and the loss or decline of disturbance-sensitive guilds within reserves causing a "winner and loser" species replacement over time. Guild change was directly related to forest spatial changes promoted by the expansion of agriculture and roads within reserves. High human population density and low nonfarming occupation were identified as the main underlying drivers of biodiversity change. Our findings suggest that to mitigate anthropogenic threats to biodiversity within biosphere reserves, fostering human population well-being via sustainable, nonfarming livelihood opportunities around reserves is imperative.

BACKGROUND: Perfluorocarbon (PFC)possesses unique chemical properties that favor the pigment epithelium's adhesion and allows the drainage of subretinal fluid through retinal holes present in retinal detachment cases. However, PFC as a temporary tamponade agent has been limited due to its high potential for toxicity. MAIN BODY: We conducted a scoping review regarding the use of PFC in vitreoretinal surgery as a temporary tamponade in subjects with severe ocular trauma or severe retinal detachment who received a therapeutic intervention (vitrectomy via posterior approach with the use of PFC as a temporary tamponade), compared to vitrectomy without the use of PFC as a temporary tamponade. Outcomes of interest were retinal reattachment, visual acuity (VA), postoperative complications and retinal toxicity. The search was performed in Medline, Medline In-Process & Other Non-Indexed Citations, Medline Daily Update, Embase databases. Reference lists from relevant review articles were also included. Two hundred thirty-eight studies were found, with no duplicate entries. In the first selection, 230 articles were eliminated; in the second selection, 6 additional articles were discarded. In total, 8 articles were obtained in this review. Two selected articles corresponded to animal studies and 6 to studies in humans. Regarding study design, 5 were case series, and 1 was a cohort study. CONCLUSION: PFC as a short-term tamponade had high rates of reapplication, improved VA, and the most frequent adverse effects were reversible after PFC withdrawal. Nonetheless, the quality of the studies was poor. Studies with more rigorous methodologies are needed to determine visual and structural outcomes and potential risks of PFC use as a temporary tamponade in vitreoretinal surgery.

Congenital heart disease (CHD) is a common structural anomaly, affecting ~ 1% of live births worldwide. Advancements in medical and surgical management have significantly improved survival for children with CHD, however, extracardiac malformations (ECM) continue to be a significant cause of morbidity and mortality. Despite clinical significance, there is limited literature available on ECM in neonates with CHD, especially from Latin America. A cross-sectional study of neonates with severe CHD evaluated by the medical-surgical board team at Fundación Cardiovascular de Colombia from 2014 to 2019 was completed to characterize morbidity, mortality, surgical outcomes, and ECM. Demographics and surgical outcomes were compared between neonates with and without ECM. Medical record data were abstracted and descriptive statistical analysis was performed. Of 378 neonates with CHD, 262 had isolated CHD (69.3%) and 116 had ECM (30.7%). The most common ECM was gastrointestinal (n = 18, 15.5%) followed by central nervous system (n = 14, 12%). Most neonates required a biventricular surgical approach (n = 220, 58.2%). Genetic testing was performed more often for neonates with ECM (n = 65, 56%) than neonates with isolated CHD (n = 14, 5.3%). Neonates with ECM had lower birth weight, longer hospital stays, and higher postsurgical complications rates. There was no difference in survival between groups. Overall, Screening for ECM in neonates with CHD is important and identification of ECM can guide clinical decision-making. These findings have important implications for pediatric healthcare providers, especially in low- and middle-income countries, where the burden of CHD is high and resources for managing CHD and extracardiac malformations may be limited.

Introducción: Los bosques de niebla se caracterizan por su distribución insular en sistemas montañosos tropicales. Florísticamente son complejos y heterogéneos, y con vacíos de información que afectan la comprensión de cómo varía su diversidad y estructura. Objetivo: Analizar los patrones de diversidad, estructura y composición del bosque de niebla en México. Métodos: Contamos y medimos plantas leñosas en 40 parcelas de 0.1 ha de cinco sitios en aproximadamente 200 km a lo largo de la Sierra Madre de Chiapas, México (1 700 - 2 100 m.s.n.m.). Usamos tres métodos estadísticos: análisis de varianza (diversidad verdadera y estructura entre sitios), regresión simple y métodos multivariados (clima y elevación). Resultados: Registramos 4 021 individuos (220 especies, 60 familias). En la diversidad verdadera, solo encontramos diferencias en riqueza de especies, con un ligero incremento hacia la parte central del área. La diversidad fue constante para especies comunes y dominantes. No encontramos diferencias en la densidad de árboles o área basal entre los sitios. Sin embargo, hubo menor densidad y mayor área basal a mayores elevaciones. La diversidad beta y la diferenciación en la composición florística es alta e importante entre las parcelas de un mismo sitio y se incrementa con la distancia entre los sitios. Conclusiones: La diversidad beta y la diferenciación florística contribuyen significativamente en la variación del bosque de niebla. Las variables climáticas y la altitud tienen efectos distintos sobre la diversidad y estructura.


Introduction: Cloud forests are noted for their narrow distribution in tropical mountain systems. Floristically, they are complex and heterogeneous, with gaps in information that hinder the understanding of how their diversity and structure vary. Objective: To analyze patterns of diversity, structure, and composition of the cloud forest in Mexico. Methods: We counted and measured woody plants in forty 0.1 ha plots from five sites in approximately 200 km along the Sierra Madre of Chiapas, Mexico (1 700 - 2 100 m.a.s.l.). We used three statistical methods: analysis of variance (diversity true and structure among sites), simple regression and multivariate statistics (climate and elevation). Results: We recorded 4 021 individuals (220 species, 60 families). In true diversity, we only found differences for species richness, with a slight increase toward the central part of the area. Diversity was constant for common and dominant species. We found no differences in tree density or basal area between sites. However, there was less density and more basal area at higher elevations. Beta diversity and differentiation in floristic composition are high and important between plots of the same site, and they increase with distance between sites. Conclusions: Beta diversity and floristic differentiation significantly contribute to variation in the cloud forest. Climatic variables and altitude have different effects on diversity and structure.

RESUMEN Introducción: la preeclampsia (PE) es la principal causa de morbimortalidad materno-fetal en nuestro país. Alteraciones hemodinámicas precoces durante el embarazo podrían predecir la evolución a PE. El machine learning (ML) permite el hallazgo de patrones ocultos que podrían detectar precozmente el desarrollo de PE. Objetivos: desarrollar un árbol de clasificación con variables de hemodinamia no invasiva para predecir precozmente desarrollo de PE. Material y métodos: estudio observacional prospectivo con embarazadas de alto riesgo (n=1155) derivadas del servicio de Obstetricia desde enero 2016 a octubre 2022 para el muestreo de entrenamiento por ML con árbol de clasificación j48. Se seleccionaron 112 embarazadas entre semanas 10 a 16, sin tratamiento farmacológico y que completaron el seguimiento con el término de su embarazo con evento final combinado (PE): preeclampsia, eclampsia y síndrome HELLP. Se evaluaron simultáneamente con cardiografía de impedancia y velocidad de onda del pulso y con monitoreo ambulatorio de presión arterial de 24 hs (MAPA). Resultados: presentaron PE 17 pacientes (15,18%). Se generó un árbol de clasificación predictivo con las siguientes variables: índice de complacencia arterial (ICA), índice cardíaco (IC), índice de trabajo sistólico (ITS), cociente de tiempos eyectivos (CTE), índice de Heather (IH). Se clasificaron correctamente el 93,75%; coeficiente Kappa 0,70, valor predictivo positivo (VPP) 0,94 y negativo (VPN) 0,35. Precisión 0,94, área bajo la curva ROC 0,93. Conclusión: las variables ICA, IC, ITS, CTE e IH predijeron en nuestra muestra el desarrollo de PE con excelente discriminación y precisión, de forma precoz, no invasiva, segura y con bajo costo.


ABSTRACT Background: Preeclampsia (PE) is the main cause of maternal-fetal morbidity and mortality in our country. Early hemodynamic changes during pregnancy could predict progression to PE. Machine learning (ML) enables the discovery of hidden patterns that could early detect PE development. Objectives: The aim of this study was to build a classification tree with non-invasive hemodynamic variables for the early prediction of PE occurrence. Results: Seventeen patients (15.18%) presented PE. A predictive classification tree was generated with arterial compliance index (ACI), cardiac index (CI), cardiac work index (CWI), ejective time ratio (ETR), and Heather index (HI). A total of 93.75% patients were correctly classified (Kappa 0.70, positive predictive value 0.94 and negative predictive value 0.35; accuracy 0.94, and area under the ROC curve 0.93). Conclusion: ACI, CI, CWI, ETR and HI variables predicted the early development of PE in our sample with excellent discrimination and accuracy, non-invasively, safely and at low cost.

AIMS: To evaluate arterial stiffness indicators in people with prediabetes (PreD) and its possible pathogenesis. MATERIALS AND METHODS: Pulse wave velocity (PWV) was measured in 208 people with FINDRISC ≥ 13 (57 ± 8 years old, 68.7% women) and thereafter divided into those having either normal glucose tolerance (NGT) or PreD. In each subgroup we also identified those with/out insulin resistance (IR) measured by the triglyceride/HDL-c ratio (normal cut off values previously established in our population). Clinical and metabolic data were collected for all participants. PWV was compared between subgroups using independent t test. RESULTS: Women and men had comparable clinical and metabolic characteristics with obesity (BMI ≥ 30) and antihypertensive-statin treatment, almost half with either NGT or PreD. Whereas 48% of NGT people presented IR (abnormally high TG/HDL-c ratio), 52% had PreD. PWV was significantly higher only in those with a complete picture of metabolic syndrome (MS). CONCLUSIONS: Since PWV was significantly impaired in people with a complete picture of MS, clinicians must carefully search for early diagnosis of this condition and prescribe a healthy life-style to prevent development/progression of CVD. This proactive attitude would provide a cost-effective preventive strategy to avoid CVD's negative impact on patients' quality of life and on health systems due to their higher care costs.

Introducción: La vasculatura fetal persistente bilateral es una malformación ocular poco frecuente, generalmente unilateral, que suele presentarse como leucocoria. Objetivo: Presentar el caso de una paciente de 6 meses con leucocoria bilateral y diagnóstico de vasculatura fetal persistente bilateral. Resumen del caso: Reporte de caso por medio de recopilación de datos de la historia clínica, fotografías del segmento anterior de la paciente, ecografía ocular y resonancia magnética de cerebro. Conclusiones: La vasculatura fetal persistente bilateral es poco frecuente, pero hay que tenerla en cuenta en el momento del abordaje de un paciente con leucocoria bilateral, cuyo pronóstico visual depende de su reconocimiento y tratamiento tempranos.


Introduction: Bilateral persistent fetal vasculature is a rare developmental disorder of the eye, usually seen as leukocoria affecting only one eye. Objective: Report on a case of a 6 month old patient with bilateral leukocoria and diagnosis of bilateral persistent fetal vasculature. Case summary: Descriptive case report study with data from de medical history, anterior segment photographs, ocular ultrasonography and cerebral magnetic resonance imaging. Conclusions: Bilateral persistent fetal vasculature is an infrequent disease that has to be taken into account when approaching a patient with bilateral leukocoria, whose visual prognosis depends on the early detection and treatment

Introducción El posicionamiento adecuado de la copa acetabular en el reemplazo total de cadera (RTC) puede mejorar la función, reducir el desgaste, el pinzamiento y la luxación. La posición de la copa se describe como la relación espacial entre el centro de rotación de la cadera y la pelvis. El objetivo de este estudio es describir el posicionamiento de una copa monobloque no cementada en el posoperatorio inmediato de los pacientes a quienes se les realizo un RTC primario. Materiales y Métodos Estudio observacional descriptivo, los pacientes fueron operados por 4 cirujanos especialistas en cirugía de reemplazo articular de cadera en 8 clínicas, los procedimientos se realizaron entre octubre de 2015 a agosto de 2018, las medidas radiológicas fueron la inclinación, anteversión, lateralización, porcentaje de descubrimiento y espacio detrás de la copa acetabular. Resultados Los mayores porcentajes son para las copas acetabulares con inclinación de 36 a 48 grados, anteversión de 5 a 15 grados, lateralización de -2 a +2mm, porcentaje de descubrimiento del 0% y espacio detrás de la copa de 0 a 3mm. Discusión Diferentes autores han identificado zonas seguras de implantación las cuales concuerdan con los resultados de nuestro estudio. El posicionamiento adecuado de la copa acetabular monobloque no cementada se pudo lograr con una correcta indicación del implante, un abordaje quirúrgico que permitió una ubicación tridimensional idónea y un acetábulo sin alteraciones anatómicas a nivel periférico.


Background Appropriate positioning of the acetabular cup in total hip arthroplasty (THA) can improve hip function, reduce wear, impingement and dislocation. The position of the cup is described as the spatial relationship between the centre of rotation of the hip and the pelvis. The aim of this study is to describe the positioning of the uncemented monoblock cup in the immediate postoperative period in patients who underwent a primary THA. Methods Descriptive observational study. The patients were operated by 4 joints arthroplasty surgeons in 8 different hospitals, from October 2015 to August 2018. The radiological measurements were taken from the AP Pelvis Xrays of the immediate postoperative. The inclination, anteversion, lateralization, percentage of cup uncovered and the space behind the acetabular cup. Results 82.05% of the cups were in the range of inclination from 36 to 48 degrees; 85.71% had an anteversion between 5 to 15 degrees, 76.56% presented a lateralization of -2 to +2mm, 94.51% of the patients didnt have a uncover cup, and 92.67% of the cups had a space behind the cup from 0 to 3mm. Discussion In our study the highest percentage of cups were implanted with parameters agree to the safety areas reported by various authors. Adequate positioning of the uncemented monoblock acetabular cup can be achieved with a proper indication, a surgical approach that allows an ideal three-dimensional location and an acetabulum with a good peripheral rim that provides the required press-fit.
