INTRODUCTION: The multifactorial etiology of gastroenteritis emphasizes the need for different laboratory methods to identify or exclude infectious agents and evaluate the severity of diarrheal disease. OBJECTIVE: To diagnose the infectious etiology in diarrheic children and to evaluate some fecal markers associated with intestinal integrity. MATERIALS AND METHODS: The study group comprised 45 children with diarrheal disease, tested for enteropathogens and malabsorption markers, and 76 children whose feces were used for fat evaluation by the traditional and acid steatocrit tests. RESULTS: We observed acute diarrhea in 80% of the children and persistent diarrhea in 20%. Of the diarrheic individuals analyzed, 40% were positive for enteropathogens, with rotavirus (13.3%) and Giardia duodenalis (11.1%) the most frequently diagnosed. Among the infected patients, occult blood was more evident in those carrying pathogenic bacteria (40%) and enteroviruses (40%), while steatorrhea was observed in infections by the protozoa G. duodenalis (35.7%). Children with diarrhea excreted significantly more lipids in feces than non-diarrheic children, as determined by the traditional (p<0.0003) and acid steatocrit (p<0.0001) methods. Moreover, the acid steatocrit method detected 16.7% more fecal fat than the traditional method. CONCLUSIONS: Childhood diarrhea can lead to increasingly severe nutrient deficiencies. Steatorrhea is the hallmark of malabsorption, and a stool test, such as the acid steatocrit, can be routinely used as a laboratory tool for the semi-quantitative evaluation of fat malabsorption in diarrheic children.


Introducción. La etiología multifactorial de la gastroenteritis enfatiza la necesidad de usar diferentes métodos de laboratorio para identificar o excluir agentes infecciosos y evaluar la gravedad de la enfermedad diarreica. Objetivo. Diagnosticar la etiología infecciosa de la diarrea en niños y evaluar algunos marcadores fecales asociados con la integridad intestinal. Materiales y métodos. Se estudiaron 45 niños con enfermedad diarreica, en los cuales se evaluaron la presencia de enteropatógenos y los marcadores de malabsorción. Se analizaron las muestras fecales de 76 niños, mediante las pruebas de esteatocrito tradicional y esteatocrito ácido, para la cuantificación de la grasa. Resultados. Se observó diarrea aguda en el 80 % de los niños y diarrea persistente en el 20 %. De los individuos con diarrea, el 40 % fue positivo para enteropatógenos; los más diagnosticados fueron rotavirus (13,3 %) y Giardia duodenalis (11,1 %). Entre los pacientes infectados, la sangre oculta fue más evidente en aquellos portadores de bacterias patógenas (40 %) o enterovirus (40%), mientras que la esteatorrea se observó en infecciones por el protozoo G. duodenalis (35,7 %). Los niños con diarrea excretaron significativamente más lípidos en las heces que aquellos sin diarrea, según lo determinado por los métodos de esteatocrito tradicional (p<0,0003) y esteatocrito ácido (p<0,0001). Conclusiones. La diarrea infantil puede provocar deficiencias graves de nutrientes. La esteatorrea es distintiva de la malabsorción intestinal y puede detectarse mediante la estimación del esteatocrito ácido. Esta prueba podría utilizarse de forma rutinaria como una herramienta de laboratorio para la evaluación semicuantitativa de la malabsorción de grasas en niños con diarrea.

ABSTRACT Objective: Demonstrate the feasibility of endoscopic assisted L5S1 intraforaminal lumbar interbody fusion (iLIF) through a transiliac approach. Methods: Ten transiliac iLIF and ten supra iliac iLIF were performed bilaterally at L5S1 in five randomly selected fresh-frozen human cadavers. The following measurements were recorded: distances from the transiliac track to the iliac crest, posterior superior iliac spine, and superior gluteal neurovascular bundle; pelvic parameters; approach angles; cage's lateral and AP center point ratio (CPR); percentage of the cage crossing the midline in the AP and lateral views. Endplate integrity was assessed through endoscopic visualization. In addition, the facet joint, sacroiliac joint, iliolumbar ligament, and exiting and traversing nerve roots were checked for integrity through anatomic dissection. Results: In the transiliac technique, the axial and coronal approach angles were significantly decreased by 13.5º (95% CI -15.5; -11.5; p value<0.001) and 13.2º (95% CI -15.3; -11.1; p value<0.001), respectively, the sagittal approach angle was significantly increased by 5.4º (95% CI 1.8,8.9; p-value = 0.008), and the AP CPR was significantly higher (MD 0.16; 95% CI 0.12,0.20; p value<0.001). The percentage of the cage crossing the AP view's midline was increased by 31.6% (95% CI 19.8,43.4; p value<0.001). The integrity of endplates, facet joints, sacroiliac joints, iliolumbar ligament, and exiting and traversing nerve roots was maintained. Conclusion: L5S1 transiliac iLIF is a feasible surgical technique. It allows a more centrally placed interbody cage in the coronal plane without compromising the anterior position in the lateral plane. The integrity of the major anatomic structures at risk was preserved. Evidence Level III: A case-control study.


Resumo: Objetivo: Demonstrar a viabilidade da fusão intersomática lombar assistida por endoscopia (iLIF) em L5S1 através de abordagem transilíaca. Métodos: Dez iLIF por via transilíaca e dez iLIF por via suprailíaca foram realizados bilateralmente em L5S1 em cinco cadáveres selecionados aleatoriamente. Foram registadas as seguintes medidas: Distâncias da via transilíaca até a crista ilíaca, crista ilíaca póstero-superior e feixe neurovascular do glúteo superior; parâmetros pélvicos; ângulos da abordagem; relação do ponto central lateral e AP do cage (CPR); percentagem do cage cruzando a linha média nas incidências AP e perfil. A integridade das placas vertebrais foi avaliada através de visualização endoscópica. Foi verificada através de dissecção anatómica a integridade das articulações facetárias, sacroilíacas, ligamento iliolombar e raízes de L5 e S1. Resultados: Na técnica transilíaca, os ângulos de abordagem axial e coronal foram significativamente menores em 13,5º (CI 95% -15,5;-11,5; p<0,001) e 13,2º (CI 95% -15,3;-11,1; p<0,001 ), respectivamente, o ângulo de abordagem sagital aumentou significativamente em 5,4º (CI 95% 1,8,8,9; p = 0,008), e o AP CPR foi significativamente maior (MD 0,16; CI 95% 0,12,0,20; p <0,001). A percentagem do cage cruzando a linha média em AP foi superior em 31,6% (CI 95% 19,8,43,4; valor p<0,001). A integridade das placas vertebrais, articulações facetadas, articulações sacroilíacas, ligamento iliolombar e raízes de L5 e S1 foi mantida. Conclusão: A realização de iLIF L5S1 por via transilíaca é uma técnica cirúrgica viável. Permite que o cage seja colocado mais centrado no plano coronal sem comprometer a posição anterior no plano sagital. A integridade das principais estruturas anatómicas em risco foi preservada. Nível de Evidencia III: Estudo caso-controle.

Métodos: Se registraron las siguientes mediciones: distancias del abordaje transilíaco a la cresta ilíaca, la cresta ilíaca posterosuperior y el haz neurovascular glúteo superior; parámetros pélvicos; ángulos de abordaje; relación del punto medio lateral y AP del cage (CPR); porcentaje del cage que cruza la línea media en las incidencias AP y perfil. Se evaluó la integridad de las placas vertebrales por visualización endoscópica. Se comprobó la integridad de las articulaciones facetarias, las articulaciones sacroilíacas, el ligamento iliolumbar y las raíces de L5 y S1 mediante disección anatómica. Resultados: En la técnica transilíaca, los ángulos de abordaje axial y coronal fueron significativamente menores en 13,5° (IC 95% -15,5;-11,5; p<0,001) y 13,2º (IC 95% -15,3;-11,1); p<0,001 ), respectivamente, el ángulo de aproximación sagital aumentó significativamente en 5,4º (IC 95% 1,8,8,9; p = 0,008), y el AP CPR fue significativamente mayor (MD 0,16; IC 95% 0,12,0,20; p <0,001). El porcentaje del cage que cruzaba la línea media en AP era mayor en un 31,6% (IC 95% 19,8,43,4; valor p <0,001). Se mantuvo la integridad de las placas vertebrales, las articulaciones facetarias, las articulaciones sacroilíacas, el ligamento iliolumbar y las raíces de L5 y S1. Conclusión: La iLIF transilíaca L5S1 es una técnica quirúrgica viable. Permite colocar el cage más centrado en el plano coronal sin comprometer la posición anterior en el plano sagital. Se preservó la integridad de las principales estructuras anatómicas en riesgo. Nivel de evidencia III: Estudio de casos y controles.

Objective This study aimed to evaluate the nociceptive profile and the intake of analgesic drugs of patients submitted to rotator cuff repair surgery. Also, to evaluate the nociceptive thresholds and the integrity of the descending inhibitory system, pain catastrophism and prevalence of nociceptive or neuropathic pain. Methods Approved by the Ethics Committee of La Salle University (1.325.433/2015). 40 patients (>18 years old) who underwent rotator cuff repair surgery (divided in small and large injuries) were recruited. The used instruments were: Sociodemographic Questionnaire, Functional Pain Scale, Visual Analogue Scale (VAS), Quantitative Sensory Test (QST) and Conditioned Pain Modulation Task (CPM). Results Patients had a significant difference in pain thresholds QST heat (independent samples t test) and quality of sleep, mood and anxiety (paired t test) in groups preoperative. There was a significant correlation between preoperative CPM and postoperative VAS (Pearson Correlation). It was observed that, in preoperative, 38 patients used analgesics continuously. Besides that, in postoperative, use of opioid drugs was higher in patients with small injury (13 patients) than in those with large injury (9 patients). Conclusion Therefore, patients with rotator cuff injuries did not present alterations in the descending inhibitory system, but showed alterations in pain thresholds, which may interfere in the postoperative period and still be related to the consumption of analgesics.

Resumo Objetivo O objetivo deste estudo foi avaliar o perfil nociceptivo e o uso de analgésicos em pacientes submetidos à cirurgia de reparo do manguito rotador. Além disso, os limiares nociceptivos e a integridade do sistema inibidor descendente, o catastrofismo da dor e a prevalência de dor nociceptiva ou neuropática também foram analisados. Métodos Este estudo foi aprovado pelo Comitê de Ética da Universidade La Salle (1.325.433/2015). Quarenta pacientes (maiores de 18 anos) submetidos à cirurgia de reparo do manguito rotador (divididos entre aqueles com lesões pequenas e grandes) participaram do estudo. Os instrumentos utilizados foram o Questionário Sociodemográfico, a Escala Funcional de Dor, a Escala Visual Análoga (EVA), o Teste Sensorial Quantitativo (QST) e a Tarefa de Modulação Condicionada da Dor (CPM). Resultados Os pacientes apresentaram diferenças significativas nos limiares de dor e QST de calor (teste t de amostras independentes) e qualidade do sono, humor e ansiedade (teste t pareado) nos grupos pré-operatórios. Houve uma correlação significativa entre CPM pré-operatória e EVA pós-operatória (correlação de Pearson). Observou-se que, no período pré-operatório, 38 pacientes utilizavam analgésico de forma contínua. Além disso, no período pós-operatório, o uso de opioides foi maior nos pacientes com lesões pequenas (13 pacientes) em comparação àqueles com lesões grandes (nove pacientes). Conclusão Os pacientes com lesão do manguito rotador não apresentaram alterações no sistema inibidor descendente, mas sim alterações nos limiares de dor, o que pode interferir no período pós-operatório e estar relacionado ao consumo de analgésicos.

OBJECTIVES: To describe the differences in clinical presentation and relative disease burden of congenital Zika syndrome (CZS)-associated microcephaly at 2 large hospitals in Salvador, Brazil that serve patients of different socioeconomic status (SES). METHODS: Clinical and serologic data were collected prospectively from pregnant women and their infants, who delivered at 2 study centers during the 2015-2016 Zika virus (ZIKV) epidemic in Salvador, Brazil. RESULTS: Pregnant women from Salvador, Brazil delivering in a low SES hospital had 3 times higher ZIKV exposure rate than women at a high SES hospital. However, different SES hospitals had similar prevalence of infants with CZS-associated microcephaly (10% vs 6%, p = 0.16) after controlling for ZIKV exposure in their mothers. CONCLUSIONS: Our study supports the positive association between low SES, high maternal ZIKV exposure, and high rates of CZS-associated microcephaly.

The novel virus severe acute respiratory syndrome coronavirus 2 (SARS-COV-2) is highly virulent and causes coronavirus disease 2019 (COVID-19), resulting in high morbidity and mortality mainly associated with pulmonary complications. Because this virus is highly transmissible, it was quickly spread globally, resulting in COVID-19 being declared as a pandemic. This study aimed to analyze the prevalence of mortality and the factors related to mortality due to COVID-19 in patients with severe acute respiratory syndrome (SARS) at a university hospital in the Central­West region of Brazil. This retrospective cross-sectional study was based on an analysis of the medical records of patients with SARS aged >18 years and admitted to an intensive care unit due to COVID-19 with the requirement of invasive mechanical ventilation. Hospital death was considered as an outcome variable in this study. Moreover, demographic and lifestyle-related variables as well as the therapeutic measures used during the hospital stay were recorded and correlated with the death outcome. After excluding 188 medical records, 397 were analyzed. Most of the participants were men (59.7%), and the mortality rate in patients with SARS due to COVID-19 was 46.1%. Multiple regression analysis indicated that the independent factors associated with mortality in patients with SARS due to COVID-19 were the age of >60 years (p < 0.001) and the use of azithromycin (p = 0.012). Protective factors for mortality were considered as not having the following diseases: hyperthyroidism, asthma, hepatic inheritance, and not being a smoker. The mortality rate in patients with SARS due to COVID-19 was associated with older age and the use of azithromycin.

RESUMO

Abstract Myocardial bypass (MB) is known to have scientific relevance and is present in several studies with great statistical significance regarding its clinical manifestations and complications. There are still questions about MB in its relationship with heart disease and repercussion in life-threatening conditions. We present a case report of a MB in the left anterior descending coronary artery, whose objective is to identify this rare congenital anomaly and to highlight the patient's clinical outcome in order to elicit greater contributions about the presence of this variant in the emergency room, its diagnosis by angiography and therapeutic management.

BACKGROUND: Giardia duodenalis is conventionally diagnosed in fecal samples using parasitological methods. However, sensitivity is poor when only a single sample is analyzed, due to intermittent excretion of cysts in feces. Alternatively, the serum antibodies to G. duodenalis can be used for parasite diagnosis and epidemiological studies to determine previous exposure. We compared the rate of G. duodenalis infection between serum anti-Giardia IgG and IgA antibodies and fecal examination in Brazilian children. METHODS: Fecal and serum samples were tested from 287 children at a clinical laboratory and from 187 children at daycare centers. Fecal samples were processed using conventional parasitological methods and coproantigen detection for Giardia diagnosis. Serum samples were tested using an in-house ELISA for detection of anti-Giardia IgG and IgA. RESULTS: G. duodenalis was found in 8.2% (N=39) of the 474 children analyzed. The sensitivity and specificity of ELISA were 80.0% and 90.0% for IgG and 80.0% and 83.3% for IgA, respectively. The total positivity rate of anti-Giardia IgG and IgA in the sera was 13.9% (N=66) and 23.6% (N=112). The agreement between the positivity of specific antibodies and the detection of G. duodenalis in feces was moderate for ELISA-IgG, kappa index (95% CI)=0.543 (0.422-0.664), and mild for ELISA-IgA, kappa index (95% CI)=0.283 (0.162-0.404). Among the children infected with other enteroparasites, 11.6% (N=10) and 24.4% (N=21) showed reactivity to anti-Giardia IgG and to IgA, respectively. This cross-reactivity was more frequent in samples from children infected with Endolimax nana and Entamoeba coli. CONCLUSIONS: The higher frequency of specific antibody reactivity compared with G. duodenalis diagnosis in feces could reflect continuous exposure of children to G. duodenalis infection, resulting in long-lasting immunological memory and/or cross-reactivity with other intestinal amoebas.

The aim of the present study was to evaluate the accumulation of carbohydrates, protein, and proline as well as the activity of the antioxidant enzymes superoxide dismutase and catalase in the Ambiance cultivar of cut rose plants grown with the application of physiological effect products in the Sub-Middle São Francisco River Valley in Brazil. The experiment was performed under a mesh screen with 50% shading. The experimental design used randomized blocks with four repetitions and six treatments: T1) control (water); T2) boscalid; T3) pyraclostrobin; T4) boscalid + pyraclostrobin (T2 + T3); T5) fluxapyroxad + pyraclostrobin; T6) plant growth regulators 4-(indol-3-yl) butyric acid (IBA) + gibberellic acid (GA3) + kinetin; these treatments were applied every 15 days throughout the crop cycle. To determine the accumulation of solutes and enzymatic activity, 8 leaves was collected every 48 h. Leaves were immediately immersed in liquid nitrogen and frozen until further analysis in the laboratory. Results showed that the product combinations boscalid + pyraclostrobin and fluxapyroxad + pyraclostrobin as well as the plant growth regulators were the treatments with the most consistent responses throughout the evaluated cycle, providing a greater accumulation of solutes in rose leaves, as an osmotic adjustment strategy against stress from high temperatures, particularly when proline accumulation is observed. With regard to enzymatic activity, plant regulators showed more consistent results when compared with other treatments, increasing both superoxide dismutase and catalase activity. The marked accumulation of organic solutes and the high enzymatic activity, particularly of catalase, indicated that rose plants use such mechanisms as a defense against the regions high temperatures.(AU)


Objetivou-se, com este estudo, avaliar o acúmulo de carboidratos, proteína e prolina, bem como a atividade de enzimas antioxidantes superóxido dismutase e catalases em plantas de rosa de corte ‘Ambiance cultivadas sob aplicação de produtos de efeitos fisiológicos no Vale do Submédio São Francisco. O experimento foi realizado em telado com 50% de sombreamento. O delineamento experimental foi em blocos casualizados com quatro repetições e seis tratamentos: T1) testemunha (água); T2) boscalida; T3) piraclostrobina; T4) boscalida + piraclostrobina (T2 + T3); T5) fluxapiroxade + piraclostrobina; T6) regulador vegetal IBA + GA3 + cinetina, aplicados a cada 15 dias durante todo o ciclo da cultura. Para a determinação do acúmulo de solutos e atividade enzimática foram realizadas 8 coletas de folhas a cada 48 horas. As mesmas foram imediatamente imersas em nitrogênio líquido e congeladas para análise posterior em laboratório. Os resultados demonstram que os produtos conjugados boscalida + piraclostrobina, fluxapiroxade + piraclostrobina e os reguladores vegetais foram aqueles com respostas mais consistentes ao longo do ciclo avaliado, propiciando maior acúmulo de solutos nas folhas das roseiras, como estratégia de ajustamento osmótico diante do estresse por temperatura elevada, especialmente quando se observa o acúmulo de prolina. Para a atividade enzimática, os reguladores vegetais apresentam resultados mais regulares quando comparados aos demais tratamentos, aumentando a atividade da SOD e da CAT. O acúmulo acentuado de solutos orgânicos junto com a elevada atividade enzimática, especialmente da CAT, indicam que as roseiras utilizam tais mecanismos como defesa diante das temperaturas elevadas da região.(AU)

The aim of the present study was to evaluate the accumulation of carbohydrates, protein, and proline as well as the activity of the antioxidant enzymes superoxide dismutase and catalase in the Ambiance cultivar of cut rose plants grown with the application of physiological effect products in the Sub-Middle São Francisco River Valley in Brazil. The experiment was performed under a mesh screen with 50% shading. The experimental design used randomized blocks with four repetitions and six treatments: T1) control (water); T2) boscalid; T3) pyraclostrobin; T4) boscalid + pyraclostrobin (T2 + T3); T5) fluxapyroxad + pyraclostrobin; T6) plant growth regulators 4-(indol-3-yl) butyric acid (IBA) + gibberellic acid (GA3) + kinetin; these treatments were applied every 15 days throughout the crop cycle. To determine the accumulation of solutes and enzymatic activity, 8 leaves was collected every 48 h. Leaves were immediately immersed in liquid nitrogen and frozen until further analysis in the laboratory. Results showed that the product combinations boscalid + pyraclostrobin and fluxapyroxad + pyraclostrobin as well as the plant growth regulators were the treatments with the most consistent responses throughout the evaluated cycle, providing a greater accumulation of solutes in rose leaves, as an osmotic adjustment strategy against stress from high temperatures, particularly when proline accumulation is observed. With regard to enzymatic activity, plant regulators showed more consistent results when compared with other treatments, increasing both superoxide dismutase and catalase activity. The marked accumulation of organic solutes and the high enzymatic activity, particularly of catalase, indicated that rose plants use such mechanisms as a defense against the region’s high temperatures.


Objetivou-se, com este estudo, avaliar o acúmulo de carboidratos, proteína e prolina, bem como a atividade de enzimas antioxidantes superóxido dismutase e catalases em plantas de rosa de corte ‘Ambiance’ cultivadas sob aplicação de produtos de efeitos fisiológicos no Vale do Submédio São Francisco. O experimento foi realizado em telado com 50% de sombreamento. O delineamento experimental foi em blocos casualizados com quatro repetições e seis tratamentos: T1) testemunha (água); T2) boscalida; T3) piraclostrobina; T4) boscalida + piraclostrobina (T2 + T3); T5) fluxapiroxade + piraclostrobina; T6) regulador vegetal IBA + GA3 + cinetina, aplicados a cada 15 dias durante todo o ciclo da cultura. Para a determinação do acúmulo de solutos e atividade enzimática foram realizadas 8 coletas de folhas a cada 48 horas. As mesmas foram imediatamente imersas em nitrogênio líquido e congeladas para análise posterior em laboratório. Os resultados demonstram que os produtos conjugados boscalida + piraclostrobina, fluxapiroxade + piraclostrobina e os reguladores vegetais foram aqueles com respostas mais consistentes ao longo do ciclo avaliado, propiciando maior acúmulo de solutos nas folhas das roseiras, como estratégia de ajustamento osmótico diante do estresse por temperatura elevada, especialmente quando se observa o acúmulo de prolina. Para a atividade enzimática, os reguladores vegetais apresentam resultados mais regulares quando comparados aos demais tratamentos, aumentando a atividade da SOD e da CAT. O acúmulo acentuado de solutos orgânicos junto com a elevada atividade enzimática, especialmente da CAT, indicam que as roseiras utilizam tais mecanismos como defesa diante das temperaturas elevadas da região.
