Dermal substitutes have become fundamental tools for covering skin defects, most recently with biological subtypes such as glycerolized acellular dermal matrix (GADM). However, literature regarding this matter is scarce in Latin America and Colombia. In this descriptive observational study, we compared the use of partial skin autografts (PSA) combined with GADM and autografts without GADM. Patients were selected from the burn unit of a hospital in northeastern Colombia between 2021 and 2022. Two study groups were defined: one receiving GADM plus PSA and the other control receiving only a partial split-thickness autograft. A total of 29 patients with 68 body areas were included, with an average age of 20 years. Most cases involved third-degree burns caused by flame. Hospitalization time was the same for both groups (41 days). The percentage of grafts taken was similar in both groups; in the GADM with autografts group, it was 94.7% compared with 96% in the control group. The presence of complications was similar in both groups. GADM produced in local tissue banks is a cost-effective alternative. It can be used in a single surgical procedure without increasing complications, providing a postsurgical course similar to autografts alone. Granting the potential long-term benefits that dermal matrices give for healing in these patients, which should be evaluated in subsequent studies.

BACKGROUND: Spirometry tests with a bronchodilator response (BDR) in FEV1, a methacholine concentration that produces a 20% drop in FEV1 (PC20) ≤ 2 mg/mL, and a positive exercise test have high specificity for the diagnosis of asthma in children. However, the value of forced expiratory flow during the middle half of the FVC maneuver (FEF25-75) in spirometry has been questioned. The objective of this study was to relate the BDR in FEF25-75 of spirometry tests with normal FEV1 and FEV1/FVC to airway hyper-responsiveness (AHR) to methacholine or exercise in children age 5-15 y with clinical suspicion of asthma. METHODS: This was a cross-sectional study of spirometry tests performed between January 2017-December 2019 in children age 5-15 y with diagnostic suspicion of asthma who had a methacholine and/or exercise testing within a period not exceeding 60 d between exams. RESULTS: The mean (± SD) age of the children was 9.04 ± 2.67 y, with a range of 5-15 y, and 56.17% were male. Of the 324 spirometry tests with normal FEV1 and FEV1/FVC, 66 (20.4%) tests showed BDR in FEF25-75. A total of 46.9% and 33.3% of the children with and without BDR in FEF25-75, respectively, had a PC20 value ≤ 2 mg/mL and/or a positive exercise testing (P = .039). CONCLUSIONS: Children with suspected asthma and normal spirometry, other than BDR in FEF25-75, had greater AHR than those without BDR in FEF25-75. BDR in FEF25-75 was not always accompanied by AHR to confirm the diagnosis of asthma, so this study suggests that assessment of FEF25-75 alone is not always reliable for ruling in or ruling out AHR in the setting of otherwise normal spirometry results in children with suspected asthma.

El tumor fibroso solitario (TFS) es una neoplasia mesenquimatosa de tipo fibroblástico que, a pesar de ser localizado principalmente en pleura, se ha observado en otros órganos como la próstata. Por su parte, el tumor fibroso solitario de la próstata es una neoplasia de baja incidencia, crecimiento lento y potencial maligno incierto, que generalmente se compone de células fusiformes de apariencia citológicamente benignas, dispuestas en una arquitectura desorganizada, mezcladas con colágeno y pequeños vasos sanguíneos. Establecer su diagnóstico se ha vuelto más reproducible desde la identificación de la fusión de los genes NAB2-STAT6 por biología molecular, que lleva a la sobreexpresión de STAT6 por inmunohistoquímica, el cual es un marcador muy sensible y específico para TFS. Presentamos el caso clínico de un paciente que debutó con síntomas de compresión vesical, en quien se identificó una masa con epicentro en la próstata que infiltraba la vejiga y llegaba a la pared rectal, y que luego de estudios de patología, inmunohistoquímica y pruebas moleculares se clasificó como un TFS de la próstata, finalmente tratado con cistoprostatectomía radical más derivación urinaria


Solitary fibrous tumor (SFT) is a mesenchymal neoplasm of fibroblastic type, which despite being located mainly in the pleura, has been observed in other organs such as the prostate. On the other hand, solitary fibrous tumor of the prostate is a rare neoplasm, slow growing, and of uncertain malignant potential, which is generally composed of spindle cells of cytologically benign appearance, arranged in a disorganized architecture, mixed with collagen and small blood vessels. Establishing its diagnosis has become more reproducible since the identification of the NAB2-STAT6 gene fusion by molecular biology, leading to the overexpression of STAT6 by immunohistochemistry, a very sensitive and specific marker for SFT. We present a clinical report of a patient who consulted with symptoms of bladder compression, in whom a mass was identified with the epicenter in the prostate infiltrating into the bladder and reaching the rectal wall. Following histopathology study, immunohistochemistry and molecular tests it was classified as a SFT of the prostate, finally treated with radical cystoprostatectomy plus urinary shunt

El pasado mes de diciembre surgió en Wuhan, China, el SARS-CoV-2, un nuevo coronavirus productor de la coronavirus disease (COVID-19), que al 15 de abril de 2020 ha infectado a más de 2 000 000 de personas y ha causado 128 833 muertes en el mundo. Por la forma de transmisión del SARS-CoV-2, las salas de en-doscopia y los procedimientos endoscópicos son una fuente de fácil diseminación, debido a que cumple con las características de un ambiente contaminado para los pacientes y el personal asistencial que participa en los procedimientos endoscópicos. Por esta razón, es de gran importancia establecer medidas dentro de las unidades de endoscopia para prevenir y disminuir el riesgo de propagación de esta infección, por medio de la creación de un entorno seguro que proteja a los pacientes y al personal de salud. Se realizó una búsqueda en Pubmed con los siguientes términos: ("COVID-19" OR "coronavirus" OR "SARS-Cov-2") y ("gastrointesti-nal" OR "transmission" OR "intestinal" OR "digestive" OR "endoscopy" OR "esophagogastroduodenoscopy" OR "colonoscopy"). El grupo de gastroenterología de nuestra institución considera que todos los pacientes llevados a procedimientos endoscópicos deben ser catalogados de alto riesgo, por lo cual presenta las re-comendaciones que todo el personal asistencial de las unidades de endoscopia debe tener en cuenta para disminuir el riesgo de contagio.(AU)


Last December, SARS-CoV-2 emerged in Wuhan, China, like a new coronavirus, producing the coronavirus disease (COVID-19), that as of April 15, 2020, it has infected more than 2,000,000 people worldwide and caused 128,833 deaths. Due to the way SARS-CoV-2 is transmitted, endoscopy rooms and endoscopic pro-cedures are an easy source of dissemination, as they fulfill the different characteristics of a contaminated envi-ronment for patients and healthcare personnel involved in endoscopic procedures. It is of great importance to establish measures within endoscopy units to prevent and reduce the risk of spreading this infection, creating a safe environment that protects patients and health personnel. A Pubmed search was performed with the following terms: ("COVID-19" OR "coronavirus" OR "SARS-Cov-2") and ("gastrointestinal" OR "transmission" OR "intestinal" OR "digestive" OR "endoscopy "OR" esophagogastroduodenoscopy "OR" colonoscopy "). The gastroenterology team of our institution, considers that all patients brought to endoscopic procedures should be classified as high risk and presents the recommendations that all healthcare staff in endoscopy units, should take into account to reduce the risk of infection.(AU)

Objective: This study aimed to determine the association between venous-arterial CO2 difference (Pv-aCO2) and clinical outcomes of interest in children with severe sepsis and septic shock. Design: An analytical observational study of a prospective cohort was conducted. Setting: The study was carried out from January 2015 to January 2018 in the pediatric intensive care unit of a referral hospital. Materials and methods: Of a total of 1159 patients who were admitted to pediatric critical care, 375 had severe sepsis and septic shock, of which 67 fulfilled the inclusion criteria. Arterial and venous gases were drawn simultaneously with a transthoracic echocardiogram, Pv-aCO2, and other measures of tissue perfusion such as arterial lactate, venous, and evolution to multiple organ failure. Measurements and main results: Half (53.7%) of the patients were under 24 months old, with a slight predominance of male patients. The main site of infection was the lungs in 56% of the cases, with a 91.2% survival rate. Patients who died had a higher venous lactate level (interquartile range 16.2-33.6, p = 0.02). However, there was no correlation between myocardial dysfunction seen on echocardiogram and a Pv-aCO2 greater than 6 mm Hg in children with severe sepsis and septic shock (r = 0.13). Pv-aCO2 and central venous saturation had low sensitivity to detect multiple organ failure and poor correlation with the number of compromised systems (r = 0.8). Conclusion: Pv-aCO2 was not associated with myocardial dysfunction, measured by echocardiogram, in children with severe sepsis and septic shock. It also did not correlate with the number of organs involved or mortality.

El aneurisma de la aorta abdominal es muy frecuente en nuestro medio y a nivel mundial, por lo que debe conocerse el manejo anestésico y sus implicaciones a largo plazo. Objetivo: comparar las técnicas anestésicas en pacientes con aneurisma de aorta abdominal llevados a reparación con prótesis endovascular entre enero 2009 a enero 2015 en el Hospital de San José de Bogotá. Métodos: estudio descriptivo retrospectivo de pacientes mayores de 18 años con aneurisma de aorta abdominal corregidos (EVAR) con prótesis endovascular excluyendo los rotos, los practicados de urgencia o con corrección de manera abierta. Resultados: de 79 reparos endovasculares (EVARs) 58 (73,4%) fueron en hombres, los antecedentes más observados fueron hipertensión arterial (68,3%), enfermedad pulmonar obstructiva crónica (20,2%) y consumo de tabaco (15,1%). La técnica más utilizada fue anestesia local más sedación (50,6%), seguida de anestesia general (39,2%) y regional (10,1%); esta última mostró mayor estabilidad hemodinámica, menor requerimiento de vasopresores e inotrópicos. Hubo 33 (41,7%) complicaciones anestésicas. Conclusiones: la anestesia local más sedación fue la técnica más utilizada, con una mayor frecuencia en casos con comorbilidades. El tiempo anestésico y la estadía hospitalaria fue similar a la anestesia general, aunque con menores complicaciones; la regional requirió menor uso de vasopresores e inotrópicos.


Abdominal aortic aneurysm is very common in our country andworldwide, thus, an understanding of anesthetic management and anesthesia long-term implications is essential. Objective: To compare the anesthetic techniques used in patients who underwent prosthetic [stent-graft] endovascular abdominal aortic aneurysm repair between January 2009 and January 2015 at Hospital de San José in Bogotá. Methods: A descriptive retrospective study including patients aged 18 years and older who had an EVAR. Those with ruptured aneurysms, who underwent emergency operations or open procedures, were excluded. Results: Out of 79 EVARs, 58 (73.4%) were performed in men, the most common antecedents were, hypertension (68.3%), chronic obstructive pulmonary disease (20.2%) and tobacco smoking (15.1%). Local anesthesia associated with sedation was the most frequently used technique (50.6%), followed by general anesthesia (39.2%) and regional anesthesia (10.1%); the latter provided greater hemodynamic stability and reduced need for vasopressor agents and inotropes. Thirty-three (41.7%) anesthesia complications were observed. Conclusions: Local anesthesia with sedation was the most frequently used technique predominantly in cases with concomitant comorbidities. Operating time and length of stay in hospital was similar to that of general anesthesia, with fewer complications; regional anesthesia required less use of vasopressor and inotropic support.

Operamos con técnica de desplazamiento de la conjuntiva 50 pterigiones con un éxito del 98% y un seguimiento de 7 años. Debido a los buenos resultados consideramos que es una técnica quirúrgica recomendable como primera cirugía
