Objective: To ascertain how screening for preterm birth is performed among obstetricians working in public and private practice in a middle-income country. Methods: Cross-sectional study of 265 obstetrician-gynecologists employed at public and private facilities. An online questionnaire was administered, with items designed to collect data on prematurity screening and prevention practices. Results: The mean age of respondents was 44.5 years; 78.5% were female, and 97.7% had completed a medical residency program. Universal screening (i.e., by ultrasound measurement of cervical length) was carried out by only 11.3% of respondents in public practice; 43% request transvaginal ultrasound if the manual exam is abnormal, and 74.6% request it in pregnant women with risk factors for preterm birth. Conversely, 60.7% of respondents in private practice performed universal screening. This difference in screening practices between public and private practice was highly significant (p < 0.001). Nearly all respondents (90.6%) reported prescribing vaginal progesterone for short cervix. Conclusion: In the setting of this study, universal ultrasound screening to prevent preterm birth was used by just over half of doctors in private practice. In public facilities, screening was even less common. Use of vaginal progesterone in cervical shortening was highly prevalent. There is an unmet need for formal protocols for screening and prevention of preterm birth in middle-income settings.

Cardiovascular disease (CVD) is the leading cause of death in rheumatoid arthritis (RA). Resistin is an adipokine that induces adipose tissue inflammation and activation of monocytes/macrophages via adenylate cyclase-associated protein-1 (CAP1). Resistin levels are increased in RA and might cause perivascular adipose tissue (PVAT) dysfunction, leading to vascular damage and CVD. This study aimed to investigate the role of resistin in promoting PVAT dysfunction by increasing local macrophage and inflammatory cytokines content in antigen-induced arthritis (AIA). Resistin pharmacological effects were assessed by using C57Bl/6J wild-type (WT) mice, humanized resistin mice expressing human resistin in monocytes-macrophages (hRTN+/-/-), and resistin knockout mice (RTN-/-) with AIA and respective controls. We investigated AIA disease activity and functional, cellular, and molecular parameters of the PVAT. Resistin did not contribute to AIA disease activity and its concentrations were augmented in the PVAT and plasma of WT AIA and hRTN+/-/- AIA animals. In vitro exposure of murine arteries to resistin impaired vascular function by decreasing the anti-contractile effect of PVAT. WT AIA mice and hRTN+/-/- AIA mice exhibited PVAT dysfunction and knockdown of resistin prevented it. Macrophage-derived cytokines, markers of types 1 and 2 macrophages, and CAP1 expression were increased in the PVAT of resistin humanized mice with AIA, but not in knockout mice for resistin. This study reveals that macrophage-derived resistin promotes PVAT inflammation and dysfunction regardless of AIA disease activity. Resistin might represent a translational target to reduce RA-driven vascular dysfunction and CVD.

BACKGROUND: Brazil has one of the highest prevalence of cesarean sections in the world. The private health system is responsible for carrying out most of these surgical procedures. A quality improvement project called Adequate Childbirth Project ("Projeto Parto Adequado"- PPA) was developed to identify models of care for labor and childbirth, which place value on vaginal birth and reduce the frequency of cesarean sections without a clinical indication. This research aims to evaluate the implementation of PPA in private hospitals in Brazil. METHOD: Evaluative hospital-based survey, carried out in 2017, in 12 private hospitals, including 4,322 women. We used a Bayesian network strategy to develop a theoretical model for implementation analysis. We estimated and compared the degree of implementation of two major driving components of PPA-"Participation of women" and "Reorganization of care" - among the 12 hospitals and according to type of hospital (belonging to a health insurance company or not). To assess whether the degree of implementation was correlated with the rate of vaginal birth data we used the Bayesian Network and compared the difference between the group "Exposed to the PPA model of care" and the group "Standard of care model". RESULTS: PPA had a low degree of implementation in both components "Reorganization of Care" (0.17 - 0.32) and "Participation of Women" (0.21 - 0.34). The combined implementation score was 0.39-0.64 and was higher in hospitals that belonged to a health insurance company. The vaginal birth rate was higher in hospitals with a higher degree of implementation of PPA. CONCLUSION: The degree of implementation of PPA was low, which reflects the difficulties in changing childbirth care practices. Nevertheless, PPA increased vaginal birth rates in private hospitals with higher implementation scores. PPA is an ongoing quality improvement project and these results demonstrate the need for changes in the involvement of women and the care offered by the provider.

Objetivo: explorar o impacto da doença celíaca no bem-estar e no desenvolvimento de crianças, conforme percebido por suas mães, participantes do grupo "Criança Celíaca" no Facebook. Método: Estudo qualitativo, utilizando o método de história oral. A pesquisa centraliza a questão: Como a doença celíaca influencia a qualidade de vida e a saúde durante o crescimento e desenvolvimento infantil? Seis mães foram escolhidas com base em critérios de inclusão específicos e concordaram em participar do estudo. Os dados foram coletados através de entrevistas gravadas no Zoom e analisados por meio de transcrição, agrupamento e categorização dos depoimentos. Resultados:Os achados são divididos em sete categorias: experiências de crianças com doença celíaca; impacto na qualidade de vida, crescimento e desenvolvimento; interações em eventos sociais; experiências escolares; emoções diante da doença; e necessidades das famílias afetadas. Conclusão: O estudo revela a necessidade de maior envolvimento dos profissionais de enfermagem neste campo, enfatizando a importância do papel ativo dos pais na garantia da qualidade de vida de crianças celíacas


Objective: to explore the impact of celiac disease on the well-being and development of children as perceived by their mothers, members of the "CeliacChild" group on Facebook. Method:This qualitative study, employing oral history methodology. The research focuses on the question: How does celiac disease affect the quality of life and health during childhood growth and development? Six mothers were selected based on specific inclusion criteria and agreed to participate in the study. Data were collected through interviews recorded on Zoom and analyzed through transcription, grouping, and categorization of the testimonies. Results:The findings are divided into seven categories: experiences of children with celiac disease; impact on quality of life, growth, and development; interactions at social events; school experiences; emotions in the face of the disease; and the needs of affected families. Conclusion:The study highlights the need for greater involvement of nursing professionals in this field, emphasizing the importance of parents' active role in ensuring the quality of life of children with celiac disease


Objetivo: explorar el impacto de la enfermedad celíaca en el bienestar y desarrollo de los niños, según lo percibido por sus madres, integrantes del grupo "Niño Celíaco" en Facebook. Método:Este estudio cualitativo, utilizando el método de historia oral. La investigación se centra en la pregunta: ¿Cómo afecta la enfermedad celíaca a la calidad de vida y la salud durante el crecimiento y desarrollo infantil? Se seleccionaron seis madres basadas en criterios específicos de inclusión y acordaron participar en el estudio. Los datos se recopilaron a través de entrevistas grabadas en Zoom y se analizaron mediante transcripción, agrupación y categorización de los testimonios. Resultados:Los hallazgos se dividen en siete categorías: experiencias de niños con enfermedad celíaca; impacto en la calidad de vida, crecimiento y desarrollo; interacciones en eventos sociales; experiencias escolares; emociones frente a la enfermedad; y las necesidades de las familias afectadas. Conclusión:El estudio resalta la necesidad de una mayorimplicación de los profesionales de enfermería en este campo, enfatizando la importancia del rol activo de los padres en garantizar la calidad de vida de los niños celíacos.

Objetivo: descrever as dificuldades de adaptação dos familiares cuidadores de pacientes portadores de Esclerose Lateral Amiotrófica (ELA), cadastrados na central de Medicamentos de Alto Custo da Regional de Saúde Pirineus, na cidade de Anápolis -GO. Método: Trata-se de um estudo de abordagem qualitativa com o método descritivo. Os participantes foram previamente selecionados mediante adequação aos critérios de inclusão e concordância em fazer parte da pesquisa. A coleta de dados deu-se em local escolhido pelos mesmos, por meio de entrevista gravada em smartphone. A análise dos dados deu-se concomitante e após o termino da coleta. Resultados:Para a obtenção dos resultados e discussão foram entrevistados 4 (quatro) estão apresentados em 6 categorias, sendo elas: dificuldades para o cuidado; sobrecarga do cuidador; aceitação familiar; arendizado e dúvidas dos cuidadores; sobre atendimento; adaptação para os cuidados no núcleo familiar. Diante disto evidenciou-se as implicações ligadas a equipe de saúde, que são: a necessidade da equipe de saúde se manter informada não só sobre as mudanças relacionadas a doença, como também as dificuldades enfrentadas pela família, para que seja auxiliada em relação às suas dúvidas e durante a adaptação e a progressão da doença. Conclusão:estudo procura trazer um novo olhar que vai além do paciente, evidenciando as necessidades da família, tratando não somente o paciente, mas também cada família dentro de suas particularidades


Objective:describe the adaptation difficulties of family caregivers of patients with Amyotrophic Lateral Sclerosis (ALS), registered at the High-Cost Medication center of the Pirineus Regional Health in the city of Anápolis -GO. Method:This is a qualitative approach study using a descriptive method. Participants were previously selected based on inclusion criteria and agreement to participate in the research. Data collection took place at a location chosen by them, through interviews recorded on a smartphone. Dataanalysis was conducted concurrently with and after data collection. Results:For the results and discussion, 4 (four) were interviewed and are presented in 6 categories: difficulties in care; caregiver burden; family acceptance; learning and doubts of caregivers; about care; adaptation to care in the family nucleus. This evidenced the implications linked to the health team, which are: the need for the health team to stay informed not only about changes related to the disease but also about the difficultiesfaced by the family, so that they can be assisted regarding their doubts and during the adaptation and progression of the disease. Conclusion:the study seeks to bring a new perspective that goes beyond the patient, highlighting the needs of the family, treating not only the patient but also each family within its particularities.


Objetivo: Describir las dificultades de adaptación de los cuidadores familiares de pacientes con Esclerosis Lateral Amiotrófica (ELA), registrados en el Centro de Medicamentos de Alto Costo de la Región Sanitaria Pirineus, en la ciudad de Anápolis -GO. Método:Estudio cualitativo con método descriptivo. Los participantes fueron previamente seleccionados de acuerdo con el cumplimiento de los criterios de inclusión y el acuerdo para participar en la investigación. La recolección de datos se llevó a cabo en un lugar elegido por ellos, a través de entrevistas grabadas en un teléfono inteligente. El análisis de los datos se realizó de forma concomitante y una vez finalizada la recolección de datos. Resultados:Para obtener los resultados y la discusión, 4 (cuatro) encuestados son presentados en 6 categorías, a saber: dificultades para el cuidado; carga del cuidador; aceptación familiar; y dudas de los cuidadores; sobre el servicio al cliente; Adaptación al cuidado en el núcleo familiar. Frente a esto, se evidenciaron las implicaciones relacionadas con el equipo de salud, las cuales son: la necesidad de que el equipo de salud esté informado no solo sobre los cambios relacionados a la enfermedad, sino también sobre las dificultades enfrentadas por la familia, para que puedan ser ayudados en relación a sus dudas y durante la adaptación y progresión de la enfermedad. Conclusión:este estudio busca aportar una nueva mirada que vaya más allá del paciente, resaltando las necesidades de la familia, tratando no solo al paciente, sinotambién a cada familia dentro de sus particularidades.

Objetivo:descrever a o impacto da histerectomia e a participação dos profissionais de saúde durante o processo cirúrgico. O problema de pesquisa parte do seguinte questionamento: que impacto tem a histerectomia na vida da mulher em idade reprodutiva? Que participação tem os profissionais de saúde no processo cirúrgico? Método: Estudo de natureza descritiva com abordagem qualitativa conforme os pressupostos de Minayo (2009) com método de pesquisa exploratória. Resultados:Foram entrevistadas 7 mulheres que foram submetidas a histerectomia, com idade acima de 18 anos, que responderam perguntas referentes ao impacto do diagnóstico, conhecimento sobre a cirugia e experiências sobre o acolhimento dos profissionais da saúde no processo cirúrgico. A entrevista foi realizada em ambiente virtual na plataforma Google Forms. Conclusão: As entrevistadas revelaram sentimentos de angústia e medo ao se depararem com o diagnóstico cirúrgico por ausência de conhecimento inicialmente, evidenciando insatisfação e ausência dos profissionais da saúde quanto à cirurgia. O estudo reforça a importância das equipes multidisciplinares em hospitais e/ou locais de cuidados de saúde, uma vez que o foco principal no trabalho com pessoas acometidas por uma enfermidade não é a doença em si, mas elas mesmas.


Objective: to describe the impact of hysterectomy and the involvement of healthcare professionals during the surgical process. The research problem originates from the following questions: What impact does hysterectomy have on the life of a woman of reproductive age?What role do healthcare professionals play in the surgical process? Method:A descriptive study with a qualitative approach according to Minayo's (2009) assumptions using an exploratory research method. Results:Seven women who underwent hysterectomy, aged over 18 years, were interviewed. They answered questions regarding the impact of the diagnosis, knowledge about the surgery, and experiences of healthcare professionals' support during the surgical process. The interviews were conducted in a virtual environment using the Google Forms platform. Conclusion:The interviewees revealed feelings of anguish and fear when faced with the surgical diagnosis due to initial lack of knowledge, showing dissatisfaction and absence of healthcare professionals regarding the surgery. The study reinforces the importance of multidisciplinary teams in hospitals and/or healthcare facilities, as the main focus in working with people affected by an illness is not the disease itself, but the individuals themselves


Objetivo: describir el impacto de la histerectomía y la participación de los profesionales de la salud durante el proceso quirúrgico. El problema de investigación se basa en la siguiente pregunta: ¿qué impacto tiene la histerectomía en la vida de las mujeres en edadreproductiva? ¿Qué papel juegan los profesionales de la salud en el proceso quirúrgico? Método:Se trata de un estudio descriptivo con enfoque cualitativo según los supuestos de Minayo (2009) con un método de investigación exploratorio. Resultados:Se entrevistó a 7 mujeres mayores de 18 años sometidas a histerectomía, quienes respondieron preguntassobre el impacto del diagnóstico, conocimientos sobre cirugía y experiencias sobre la recepción de profesionales de la salud en el proceso quirúrgico. La entrevista se realizó en un entorno virtual en la plataforma Google Forms. Conclusión:Los entrevistados revelaron sentimientos de angustia y miedo frente al diagnóstico quirúrgico debido a la falta de conocimiento inicial, evidenciando insatisfacción y ausencia de profesionales de la salud con respecto a la cirugía. El estudio refuerza la importancia de los equipos multidisciplinarios en los hospitales y/o en los entornos sanitarios, ya que el foco principal en el trabajo con personas afectadas por una enfermedad no es la enfermedad en sí, sino ellos mismos.

This study aimed to evaluate the use of equine amniotic membrane (EAM), frozen indirectly using liquid nitrogen and stored between -10° and -24°C, in the treatment of equine skin lesions. Six healthy female horses, aged 3-10 years, were included in this study. EAM was collected from previously evaluated healthy parturient mares. Wounds were surgically created at the distal ends of the forelimbs. One limb was chosen for treatment, and the contralateral limb was chosen as the control. Pain sensitivity, presence of granulation tissue, secretions, and bleeding after debridement during cleaning were evaluated. Microscopically, the following were evaluated: the integrity of the epithelium, the organization of the connective tissue, the presence of hemorrhage, fibroplasia, epithelial hyperplasia, hyperkeratosis, neovascularization, and the types of cells present. Assessments were performed on days 0, 3, 7, 14, 21, 28, and 63, and the time to complete the lesion closure. Treatment with EAM promoted faster recovery, greater neovascularization, better quality fibroplasia, and less sensitivity to pain than the control group. We concluded that the use of EAM was advantageous compared to the control group.


O objetivo deste estudo foi avaliar a utilização da membrana amniótica equina (MAE) congelada indiretamente por meio do nitrogênio líquido e conservada entre -10° e -24°C no tratamento de lesões cutâneas de equinos. Foram empregados seis equinos, fêmeas, hígidas, com idades de 3 a 10 anos. A MAE foi coletada de éguas parturientes hígidas, previamente avaliadas. As feridas foram produzidas cirurgicamente nas extremidades distais dos membros anteriores. Um dos membros foi escolhido como tratado, permanecendo o contralateral como controle. Foram avaliados a sensibilidade dolorosa; presença de tecido de granulação; secreções; e hemorragia após desbridamento durante a limpeza. Microscopicamente foram avaliados: a integridade do epitélio, a organização do tecido conjuntivo, a presença de hemorragia, fibroplasia, hiperplasia epitelial, hiperqueratose, neovascularização e os tipos celulares presentes. As avaliações foram realizadas nos dias 0, 3, 7, 14, 21, 28, e 63, e o tempo até o fechamento completo da lesão. O tratamento com MAE promoveu uma recuperação mais rápida que o grupo controle, maior neovascularização, melhor qualidade fibroplasia e menor sensibilidade à dor. Conclui-se que o emprego da MAE foi vantajoso em relação ao grupo controle.

BACKGROUND: Although the literature shows that an increase in both the number and suppressive function of CD4+forkhead box P3 (FOXP3)+ T-regulatory cells (Tregs) during sepsis contributes to an immunosuppressed state, little is known about the identity of these cells. METHODS: Using the sepsis mouse model of cecal ligation and puncture (CLP), we analyzed the frequency and molecular signature of the T-cell immunoglobulin and ITIM domain (TIGIT)+ and TIGIT- Treg subsets, using flow cytometry and quantitative polymerase chain reaction. In addition, ST2-/- and signal transducer and activator of transcription 6 (STAT6)-/- mice were submitted to CLP or recombinant interleukin 33 (IL-33) treatment to investigate the mechanism whereby TIGIT+ Tregs differentiate during sepsis. RESULTS: Sepsis was marked by the sustained expansion of the highly suppressive TIGIT+ Treg subset, which expresses Helios, neuropilin 1, and high levels of Tnfrsf18 and Pdcd1 at 15 days after CLP. The increase in TIGIT+ Tregs was accompanied by higher susceptibility to nosocomial bacteria challenge, suggesting their association with post sepsis immunosuppression. Mechanistically, we found that the ST2 deletion abrogated the expansion of the TIGIT+ Treg subset during sepsis. Furthermore, treatment with recombinant IL-33 resulted in the expansion of TIGIT+ Tregs depending on the STAT6 and M2 macrophages. CONCLUSIONS: These findings demonstrated that only the TIGIT+ Tregs remain stably expanded at the late phase of sepsis. Moreover, the expansion of TIGIT+ Tregs is dependent on the IL-33/ST2/STAT6/M2 macrophage axis.

Although deforestation remains widespread in the tropics, many places are now experiencing significant forest recovery (i.e., forest transition), offering an optimistic outlook for natural ecosystem recovery and carbon sequestration. Naturally regenerated forests, however, may not persist, so a more nuanced understanding of the drivers of forest change in the tropics is critical to ensure the success of reforestation efforts and carbon sequestration targets. Here we use 35 years of detailed land cover data to investigate forest trajectories in 3014 municipalities in the Brazilian Atlantic Forest (AF), a biodiversity and conservation hotspot. Although deforestation was evident in some regions, deforestation reversals, the typical forest transition trajectory, were the prevalent trend in the AF, accounting for 38% of municipalities. However, simultaneous reforestation reversals in the region (13% of municipalities) suggest that these short-term increases in native forest cover do not necessarily translate into persistent trends. In the absence of reversals in reforestation, forests in the region could have sequestered 1.75 Pg C, over three times the actual estimated carbon sequestration (0.52 Pg C). We also showed that failure to distinguish native and planted forests would have masked native forest cover loss in the region and overestimated reforestation by 3.2 Mha and carbon sequestration from natural forest regeneration by 0.37 Pg C. Deforestation reversals were prevalent in urbanized municipalities with limited forest cover and high agricultural productivity, highlighting the importance of favorable socioeconomic conditions in promoting reforestation. Successful forest restoration efforts will require development and enforcement of environmental policies that promote forest regeneration and ensure the permanence of regrowing forests. This is crucial not only for the fate and conservation of the AF, but also for other tropical nations to achieve their restoration and carbon sequestration commitments.
