BACKGROUND: Low flow nasal cannula (LFNC) are frequently used in preterm infants. However, the delivered inspired oxygen concentration and airway pressures are not well established. OBJECTIVE: To determine the fraction of inspired oxygen (FiO2 ) and hypopharyngeal pressures generated by LFNC at different gas flows, gas mixture concentrations and infant's weight. DESIGN/METHODS: Serial samples of hypopharyngeal gas were obtained in 33 very low birth weight infants who were receiving oxygen with LFNC. Measurements were obtained with different gas flows and oxygen concentrations. FiO2 was measured using an electrochemical cell analyzer and hypopharyngeal pressures with a pressure transducer. RESULTS: 33 infants with a mean BW of 910 ± 284 g and 27 ± 1.7 weeks gestational age were studied at 36 ± 22 days after birth. FiO2 increased proportionally to gas flow, but with large variability: median (range) FiO 2 were 0.33 (0.23-0.54), 0.44 (0.29-0.67), 0.57 (0.33-0.81), and 0.69 (0.51-0.92) at 0.1, 0.3, 0.5, and 1 L/minute, respectively. Significantly higher mean FiO 2 were observed despite low flows in infants ≤ 1000 g compared to those > 1000 g (0.5 ± 0.1 vs 0.4 ± 0.07 at 0.3 L/minute; 0.66 ± 0.09 vs 0.5 ± 0.08 with 0.5 L/minute, respectively, P < .05). Hypopharyngeal pressures increased proportionally to gas flow with high variability: mean ± standard deviation pressures were 1.5 ± 0.8; 2.8 ± 1.2; 4.6 ± 1.3; 6.1 ± 1.6 cm H 2 O at 0.5, 1, 2, and 3 L/minute of gas flow. Peak pressures > 15 cm H 2 O were frequently observed with gas flows ≥ 2 L/min. CONCLUSIONS: Large variability in FiO2 and hypopharyngeal pressures were observed with oxygen administration through LFNC. Very high FiO 2 were observed despite low flows in infants < 1000 g. Excessive peak pressures can be generated with flows ≥ 2 L/minute especially among infants < 1000 g.

OBJECTIVES: Lyme disease is a tick-borne disease caused by infections with Borrelia. Persons infected with Borrelia can be asymptomatic or can develop disseminated disease. Diagnosis and recognition of groups at risk of infection with Borrelia burgdorferi is of great interest to contemporary rheumatology. There are a few reports about Borrelia infection in Mexico, including lymphocytoma cases positive to B. burgdorferi sensu stricto by PCR and a patient with acrodermatitis chronica atrophicans. Veterinarians have an occupational risk due to high rates of tick contact. The aim of this work was to investigate antibodies to Borrelia in students at the Faculty of Veterinary Medicine and Zootechnics, at Nuevo León, Mexico, and determine the antibody profile to B. burgdorferi antigens. MATERIAL AND METHODS: Sera were screened using a C6 ELISA, IgG and IgM ELISA using recombinant proteins from B. burgdorferi, B. garinii and B. afzelii. Sera with positive or grey-zone values were tested by IgG Western blot to B. burgdorferi sensu stricto. RESULTS: All volunteers reported tick exposures and 72.5% remembered tick bites. Only nine persons described mild Lyme disease related symptoms, including headaches, paresthesias, myalgias and arthralgias. None of the volunteers reported erythema migrans. Nine samples were confirmed by IgG Western blot. The profile showed 89% reactivity to OspA, 67% to p83, and 45% to BmpA. CONCLUSIONS: Positive sera samples shared antibody reactivity to the markers of late immune response p83 and BmpA, even if individuals did not present symptoms of Lyme arthritis or post-Lyme disease. The best criterion to diagnose Lyme disease in our country remains to be established, because it is probable that different strains coexist in Mexico. This is the first report of antibodies to B. burgdorferi in Latin American veterinarians. Veterinarians and high-risk people should be alert to take precautionary measures to prevent tick-borne diseases.

RESUMO

La sífilis es una infección potencialmente devastadora en el recién nacido, cuya incidencia ha ido aumentando. Se presenta el caso de un prematuro que evoluciona con falla respiratoria y compromiso multisistémico precoz secundario a sífilis congénita. Se comentan hallazgos en el examen físico, laboratorio e imágenes. Se discute sobre las formas de presentación en el niño, pruebas diagnósticas y diagnóstico diferencial.


Syphilitic infection in newborn could be a devastating disease with up ward trend in incidence. We present a case of newborn with congenital syphilis with early respiratory distress and multisystemic failure. Clinical, laboratory and imaging findings are commented. Clinical presentation, diagnostic test and differential diagnosis are discussed.
