RESUMO

Due to the anthropogenic pressures of expansion areas for livestock and agricultural production in the Brazilian Cerrado, it is of paramount importance to understand the dynamics of land use/land cover (LULC) changes in this region. Thus, we investigated LULC changes in two sub-basins of the Tocantins-Araguaia River basin from 1997 to 2015 and consequently projected future changes for the timespan between 2030 and 2050. The Formoso sub-basin experienced significant expansion of agricultural and pasture areas, whereas the Sono sub-basin limited farmland expansion (more stable native vegetation) due to substantial protected areas, trends that were also observed for future projections (2030 and 2050). Pastureland in the Formoso sub-basin increased by 5.8%, while the Sono sub-basin saw significant gains in cultivated land, according to change detection analyses during the 1997-2015 period. High stability probabilities of no change (> 70%) for grassland areas in the Sono River sub-basin and pasturelands in the Formoso River sub-basin were computed. The CA-Markov model demonstrated a high consistency level with actual LULC classes for both sub-basins, as indicated by an overall Kappa coefficient above 0.8. Future projections for 2030 and 2050 show a substantial expansion of agriculture and pasture in both sub-basins, driven by specific factors such as soil organic carbon stocks, distance from rural settlements, and proximity to rivers. Short- and mid-term simulations indicate substantial expansion of agriculture and pasture in both basins, with potential adverse impacts on water erosion. Consequently, developing policies for soil management and sustainable land use planning is essential for agroecosystem sustainability, promoting a balanced approach to economic development while addressing climate change and anthropogenic challenges.

Introduction: promoting scientifi c research is one of the main functions of universities. Medical schools should not be an exception; deans should also have appropriate research experience that allows them to have a different perspective on the importance of research in undergraduate medicine. Aim: to determine the medical school dean's scientific production of the main universities by continent. Methods: an observational, analytical, and transversal study. We identify the medical school dean's scientific production of the 20 universities with the best position from South America, Central America, North America, Europe, Asia, Africa, and Oceania, according to the Ranking Webometrics 2022, in the Scopus database. Results: 140 directors or deans of medicine were evaluated, of which 136 (97.1%) had published an article at least once in their life, 128 (91.4%) in the last fi ve years, 103 (73.6%) in the previous year, and 93 (66.4%) in the current year. The total number of published articles was 24.5 (Me=98), receiving a total of 1,251,766 citations (range 0 to 101,868), an Hindex 24 (range 0 to 140), and in collaboration with 154,711 coauthors. Conclusions: the medical school dean's scientific production from the main universities by continent was high, with notable differences between those who came from universities in Asia, North America, and Europe compared to Oceania, Africa, South America, and Central America


Introdução: promover a pesquisa científica é uma das principais funções das universidades. As escolas médicas não devem ser uma exceção. Os reitores também devem ter uma experiência de pesquisa adequada que lhes permita ter uma perspectiva diferente sobre a importância da pesquisa na graduação em medicina. Objetivo: determinar a produção científica dos reitores de medicina das principais universidades por continente. Métodos: estudo observacional, analítico e transversal. Identificamos a produção científica dos pró-reitores de medicina das 20 universidades com melhor posição da América do Sul, América Central, América do Norte, Europa, Ásia, África e Oceania, segundo o Ranking Webometrics 2022, na base Scopus. Resultados: foram avaliados 140 diretores ou reitores de medicina, dos quais 136 (97.1%) publicaram artigo pelo menos uma vez na vida, 128 (91.4%) nos últimos cinco anos, 103 (73.5%) no ano anterior, e 93 (66.4%) no ano corrente. O número total de artigos publicados foi de 24.5 (Me=98), recebendo um total de 1,251,766 citações (intervalo de 0 a 101.868), índice H = 24 (intervalo de 0 a 140) e em colaboração com 154,711 coautores. Conclusões: a produção científica dos reitores de medicina das principais universidades por continente foi alta, com diferenças notáveis entre aqueles que vieram de universidades da Ásia, América do Norte e Europa em comparação com Oceania, África, América do Sul e América Central

Introducción: El Perú posee un gran número revistas de áreas biomédicas, pero solo algunas han logrado visibilizar e indizar sus publicaciones en diferentes medios de indización de alcance global. Objetivo: Analizar las características de publicación de la Revista del Cuerpo Médico Hospital Nacional Almanzor Aguinaga Asenjo (Chiclayo) a partir de indicadores bibliométricos. Material y métodos: Estudio descriptivo que analiza 587 registros bibliográficos de los artículos publicados en la revista durante 2011-2020, a partir de indicadores de impacto científico y análisis de términos. Resultados: La mayor producción de artículos corresponde a originales (60%; X̅=66). Los artículos publicados sobre medicina general obtuvieron más impacto en años anteriores. Los temas más frecuentes fueron estudios sobre pacientes de hospitales de Chiclayo, Lambayeque con casos de covid-19. Conclusiones: La Revista del Cuerpo Médico HNAAA se perfila como una publicación seriada de importancia para visibilizar la producción nacional y local por la cobertura temática en especialidades médicas de relevancia regional que publica.


Background: There are several journals about biomedical areas in Peru, however only a few have managed to make their publications avisible and indexed in different global indexing media. Objective: To analyze the main publication characteristics from Revista del Cuerpo Médico Hospital Nacional Almanzor Aguinaga Asenjo (Chiclayo) based on bibliometric methods. Material and methods: Descriptive study that analyzes 587 articles published by the journal from 2011 to 2020, based on bibliometric methods and indicators: citations, scientific impact and co-word analysis. Results: Original articles were the highest types of document published (60%; X̅=66). Published research articles on general medicine received more impact in previous years than the recents. Most frequent topic analyzed were studies about patients from hospitals in Chiclayo city (Lambayeque) with covid-19 clinical cases. Conclusion: Revista del Cuerpo Médico HNAAA is emerging as an important serial publication to make local, national, and international production visible due to thematic coverage in relevant medical specialties.

Objetivo:Ofrecer un alcance de la relevancia del diagnóstico oportuno del cáncer testicular, a partir de un caso infrecuente de rabdomiosarcoma paratesticular en un adulto. Se trata de un paciente varón Reporte de Caso:de 39 años, que ingresa al servicio de cirugía - urología por un aumento de volumen de contenido escrotal, de forma progresiva desde hace 2 años, con crecimiento paulatino de la masa. En la ecografía testicular se evidencia un notorio aumento de volumen de bolsas escrotales por aparente tumoración o masa de consistencia dura, de estructura heterogénea y aspecto sólido de bordes mal definidos. Considerando los hallazgos clínicos, marcadores testiculareseimagenológicossediagnosticóuntumorparatesticular derecho. Se realizó una orquiectomía derecha más exéresis de tumor paratesticular derecho. Patología concluyó en rabdomiosarcoma embrionario paratesticular con cápsula rota. Paciente fue dado de alta con seguimiento por consulta externa y referido a hospital oncológico de cuarto nivel.

Considering the clinical findings, testicular markers and imaging, a right paratesticular tumor was diagnosed. Aright orchiectomy plus right paratesticular tumor exeresis was performed. Pathology concluded in paratesticular embryonic rhabdomyosarcoma with a broken capsule. Patient was discharged with outpatient follow-up and referred to the fourth level cancer hospital

Vector-borne pathogens are responsible for serious emerging diseases and Rangelia vitalii, the etiologic agent of canine rangeliosis, is one of the most pathogenic tick-borne pathogens for dogs in South America. This protozoan is transmitted by the Amblyomma aureolatum tick bite and the clinical features associated to the disease are fever, hemolytic anemia, jaundice, hepatosplenomegaly and bleeding from natural orifices, mainly from the ear egde. The reports of canine rangeliosis in Argentina are scarce. In the present study we report the detection of Rangelia vitalii in a naturally infected dog from Gualeguay, Entre Ríos, Argentina with history of tick infestation and clinical signs compatible with rangeliosis. An initial blood sample was positive to piroplasmids by blood smear examination and the molecular amplification of a fragment of the 18SrRNA gene. Sequencing of the fragment confirmed the pathogen identity. After treatment with imidocarb dipropionate, the clinical signs remitted and the blood smear tested negative.

Obesity is a chronic inflammatory disease associated with adipose tissue macrophage (ATM) activation. ATMs from lean mice contribute to tissue homeostasis by their M2-oriented polarization, whereas obesity leads to an increase of M1 inflammatory ATMs that underlies obesity-related metabolic disorders. In humans, studies characterizing ATMs and their functional status are limited. Here we investigated ATM phenotype in visceral (VAT) and subcutaneous (SAT) adipose tissue from healthy lean and obese individuals using two molecules previously identified as markers of M1-like and M2-like/tissue-resident macrophages, the C-type lectin CLEC5A and the scavenger receptor CD163L1, respectively. CD163L1 was expressed by the majority of ATMs, and CD163L1+ ATM density was greater with respect to cells expressing the pan-macrophage markers CD68 or CD11b. ATM counts in SAT, but not in VAT, increased in obese compared to lean individuals, measured with the three markers. Accordingly, CD163L1, CD68 and ITGAM gene expression was significantly enhanced in obese with respect to control individuals only in SAT. CLEC5A+ ATMs had a proinflammatory profile and were abundant in the lean VAT, but their density diminished in obesity. The only ATM subset that increased its counts in the obese VAT had a mixed M1-like (CD11c+ CD163- CD209- ) and M2-like (CLEC5A- CD206+ ) phenotype. ATM expansion was dominated by a subset of M2-like macrophages (CD11c- CLEC5A- CD163+ CD206+ CD209+ ) in the obese SAT, with a minor contribution of a CD11c+ CLEC5A- ATM subpopulation. Thus, both SAT and VAT seems to limit inflammation during obesity by differentially altering their ATM subset composition.

BACKGROUND: Chagas disease (CD) continues to be a neglected infectious disease with one of the largest burdens globally. Despite the modest cure rates in adult chronic patients and its safety profile, benznidazole (BNZ) is still the drug of choice. Its current recommended dose is based on nonrandomized studies, and efficacy and safety of the optimal dose of BNZ have been scarcely analyzed in clinical trials. METHODS/DESIGN: MULTIBENZ is a phase II, randomized, noninferiority, double-blind, multicenter international clinical trial. A total of 240 patients with Trypanosoma CD in the chronic phase will be recruited in four different countries (Argentina, Brazil, Colombia, and Spain). Patients will be randomized to receive BNZ 150 mg/day for 60 days, 400 mg/day for 15 days, or 300 mg/day for 60 days (comparator arm). The primary outcome is the efficacy of three different BNZ therapeutic schemes in terms of dose and duration. Efficacy will be assessed according to the proportion of patients with sustained parasitic load suppression in peripheral blood measured by polymerase chain reaction. The secondary outcomes are related to pharmacokinetics and drug tolerability. The follow-up will be 12 months from randomization to end of study participation. Recruitment was started in April 2018. CONCLUSION: This is a clinical trial conducted for the assessment of different dose schemes of BNZ compared with the standard treatment regimen for the treatment of CD in the chronic phase. MULTIBENZ may help to clarify which is the most adequate BNZ regimen in terms of efficacy and safety, predicated on sustained parasitic load suppression in peripheral blood. Concentrados com alta proteína bruta e minerais e com menor concentração de energia poderiam ser utilizados em menores quantidades no balanceamento de dietas com volumosos tropicais. Assim, objetivou-se avaliar o consumo de matéria seca, a resposta produtiva e econômica de vacas da raça Girolando mantidas em pastagem de Tifton 85 durante o período de seca, as quais receberam diferentes concentrações de suplementação proteica. Foram utilizadas 12 vacas da raça Girolando em lactação, com produção de leite média inicial de 18,00±1,615kg/dia. Os animais foram distribuídos em três quadrados latinos (4x4), constituídos de quatro tratamentos e quatro períodos experimentais, com duração de 15 dias cada. Os tratamentos consistiram de uma dieta controle, apenas sal mineral e três níveis de suplementação proteica com 40; 50 e 60% de proteína bruta (PB) sendo fornecidas nas quantidades de 2,5kg/animal/dia. Observou-se que os fornecimentos de suplementos proteicos proporcionaram um efeito substitutivo da forragem em relação à suplementação mineral. O uso de suplementação concentrada com teor de 40% de proteína bruta foi mais eficiente em termos produtivos, econômicos e de produção de proteína bruta no leite.(AU)

The objective of this study was to evaluate the dry matter intake, the productive and economical response of Girolando cows kept in Tifton 85 pasture during dry season receiving different concentrations of protein supplementation. Twelve lactating Girolando cows with initial mean milk yield of 18.00±1.615kg/day were used. The animals were distributed in three Latin squares (4x4), consisting of four treatments and four experimental periods, each lasting 15 days. The treatments were composed of control diet with only mineral salt, and three levels of protein supplementation with 40, 50, and 60% crude protein (CP) being supplied in the amounts of 2.5kg/animal/day. It was found that the protein supplement supplies provided a substitutive effect of forage relative to mineral supplementation. The use of concentrated supplementation with 40% crude protein content was more efficient in terms of production, production of crude protein in milk and economically.(AU)
