INTRODUCTION: The benefits and safety of using orthodontic aligners have been reported more by clinical experience and expert opinion than by scientific evidence. Another important aspect is that aligners are constantly evolving. It is important to obtain evidence that allows for new updates in manufacturing technology, in the development of new movement planning protocols, in the incorporation and design of attachments, and in the aid of skeletal anchorage. METHODS: Evidence retrieved from six electronic databases (CINAHL, MEDLINE, EMBASE, Psych Info, the Cochrane Library and the Joanna Briggs Library) is presented by means of questions and answers. CONCLUSIONS: There is evidence that the aligners presented different levels of difficulty in performing each type of movement, with rotational and vertical movements being the most difficult to perform. Regarding perception of pain due to tooth movement, it seems to have less impact at the beginning of treatment; but dealing with more phonoarticulatory changes seems to require more treatment time in more complex cases. Aligners do not prevent the occurrence of root resorption, although the incidence and severity of resorption may be reduced, making oral hygiene easier and accepting the risk of white spots, caries and periodontal disease. Given the conflicting evidence, the release of bisphenol-A from the aligner cannot be denied. Solutions must be found to reduce the environmental impact of aligners disposal. There is an urgent need for well-designed randomized controlled trials.

RESUMO Introdução: As vantagens e desvantagens do uso de alinhadores ortodônticos têm sido reportadas com base mais na experiência clínica e opinião de experts do que em evidências científicas. Outro aspecto importante é que os alinhadores estão em processo de evolução constante. Assim, torna-se importante obter evidências recentes, que abranjam as novas atualizações na tecnologia de confecção, no desenvolvimento de novos protocolos de planejamento para as movimentações, a incorporação e desenho dos attachments e o auxílio da ancoragem esquelética. Métodos: As evidências encontradas em seis bases de dados eletrônicas (CINAHL, MEDLINE, EMBASE, Psych Info, Biblioteca Cochrane e Biblioteca Joanna Briggs) serão apresentadas por meio de perguntas e respostas. Conclusões: Há evidências de que os alinhadores apresentam diferentes níveis de dificuldade para realizar cada tipo de movimento, sendo os movimentos rotacionais e verticais os mais difíceis de serem executados. Quanto à percepção da dor causada pela movimentação dentária, parece haver menos impacto no início do tratamento, mas os alinhadores produzem mais alterações fonoarticulatórias e parecem exigir mais tempo de tratamento em casos mais complexos. Não há evidência de diferença na estabilidade pós-tratamento, e os alinhadores não impedem a ocorrência de reabsorção radicular, apesar da incidência e a gravidade da reabsorção poderem ser menores, facilitam a higiene bucal, reduzindo o risco de manchas brancas, cáries e doenças periodontais. Dada a evidência conflitante, a liberação de bisfenol-A pelo alinhador não pode ser negada. É preciso encontrar soluções que reduzam o impacto ambiental do descarte dos alinhadores. Há uma necessidade urgente de ensaios clínicos randomizados bem desenhados.

INTRODUCTION: This paper aims to evaluate the effect of tube height on uprighting a mandibular second molar with a continuous arch, specifically the main effects on the molar tube and the side effects on the second premolar bracket. METHODS: Slot brackets (0.18-in) and tubes were passively bonded from the left second molar to the right second premolar of the patient's cast and fixed to an orthodontic force tester. A mesially tipped second molar along with its tube was positioned at 5 different heights in relation to the bracket slots of the remaining appliance: 2 mm below (group -2), 1 mm below (group -1), leveled (group 0), 1 mm above (group 1) and 2 mm above (group 2). Each group had 10 different 0.016 × 0.022-in superelastic nickel-titanium wires tied one at a time to the appliance to register the vertical forces (Fz) and tipping moments (Mx) produced at the molar tube and the premolar bracket, statistically. Analysis of variance was used to identify differences of load among the groups for wire response to the position of the tube in relation to the other brackets' slots. RESULTS: As the molar tube was positioned more occlusally, Fz and Mx decreased at the molar tube from 1.31 N to 0.75 N and 13.98 N.mm to 11.84 N.mm, respectively, whereas at the premolar bracket, Fz and Mx decreased from 1.52 N to 0.88 N and 8.77 N.mm to 1.05 N.mm, respectively. CONCLUSIONS: In all groups, the load system at the molar tube (the main effects) registered a distal tipping moment and an extrusive force, whereas at the premolar bracket, the load system (side effects) included a distal tipping moment and an intrusive force. As the tube moved occlusally, the extrusive force and the uprighting moment decreased, along with the side effects at the premolar bracket. Attempting to bond the molar tube more occlusally (within reasonable intraoral dimensions) when trying to upright a molar does not appear to be sufficient to keep a molar from extruding.

RESUMO

The aim of this study was to evaluate the usability of the free software available that allow visualization and measurement of orthodontic digital models. 80 graduate students of orthodontics were asked to perform pre-defined tasks in a digital model through 3D Viewer ® and 3D-Tool ® software. The success in accomplishing the tasks and the time spent were recorded. To end, each participant answered a questionnaire to express their satisfaction regarding the software. There were no statistically significant difference between the software when compared to the accomplishment rates of tasks and the time spent by participants on each one. The software were evaluated as "slightly satisfactory" in several criteria. There is scope for optimization of orthodontic software by manufacturers since lack of their interface usability can discourage orthodontists' adherence to new resources that could provide benefits to their daily routine, even if they are freely available.

The aim of this study was to evaluate and compare changes in linear distance and inclination of lower incisors and canines and intercanine distance after a 30 months orthodontic treatment with self-ligating appliances. Seven patients were treated orthodontically with a Roth prescription passive self-ligating bracket. To perform the measurements and comparisons, CBCT scans were taken before the start of the orthodontic treatment (T1) and after a period of 30 months treatment (T2). The following measurements were performed: (1) the lower incisors and canines inclination in relation to the mandibular plane, (2) intercanine linear distance in millimeters and (3) linear distance in millimeters of the incisal and apical part of lower anterior teeth to a plane (POGM) passing through pogonion point and perpendicular to the mandibular plane. No significant difference were observed between T1 and T2 for canine inclination (p = 0.835), incisors inclination (p = 0.149), canine incisal distance to POGM (p = 0.423) and incisors incisal distance to POGM (p = 0.966), however canine apical distance (p = 0.049) and incisors apical distance (p = 0.002) to POGM was lower at T1 than at T2. The intercanine distance was significantly lower (p = 0.022) at T1 when compared to T2. The use of passive self-ligating brackets in orthodontic treatment to solve 4 mm tooth crowding were able to produce dental arch expansion by bodily tooth movement.

RESUMO

The first Edgewise brackets were utilized with gold wire and the slot designed with 0.022" high to present sufficient rigidity. When gold was replaced by stainless steel, cheaper and stiffer, it was proposed to reduce the slot size to 0.018". The two different sizes of brackets are used in contemporary orthodontics and many clinical orthodontists are unaware of the advantages they may have. The aim of this paper is to present, based on the orthodontic literature, the main characteristics influenced by the brackets slot size and compile data to drive orthodontists in choosing the bracket that best meets their clinical needs.


Os primeiros braquetes Edgewise eram utilizados com fios de ouro e concebidos com o slot de 0,022" de altura, para que apresentassem rigidez suficiente. Com a substituição do ouro pelo aço inoxidável, mais barato e mais rígido, foi proposta a redução do slot para 0,018". As duas diferentes dimensões de braquetes são utilizadas na Ortodontia contemporânea e muitos ortodontistas clínicos desconhecem suas vantagens. O objetivo deste artigo foi apresentar, baseando-se na literatura ortodôntica, as principais características influenciadas pelo tamanho do slot dos braquetes e compilar os dados para direcionar o ortodontista na escolha do braquete que melhor atenda às suas necessidades clínicas.

Atualmente, a busca pela estética tem aumentado a procura por tratamento ortodôntico na população adulta. A agenesia dentária tem uma grande influência na estética do sorriso. Consequentemente, na harmonia da face, a ausência dos incisivos laterais contribui de maneira significativa. Dentre as várias formas de tratamento, pode-se optar pelo fechamento de espaço, com substituição do incisivo lateral pelo canino ou abertura de espaço para uma posterior reabilitação implantoprotética. Neste artigo, foi descrito o tratamento da agenesia dos incisivos laterais superiores e revisadas na literatura as suas possibilidades de tratamento, com enfoque na abertura do espaço associado ao planejamento 3D e reabilitação estética através da reabilitação implantoprotética


Nowadays, the seek for aesthetics has been the subject of increased demand for orthodontic treatment in adults. The tooth agenesis has a great influence on the smile esthetics and consequently the harmony of the face, which is significantly affected by the absence of lateral incisors. Among the various treatment forms, the clinician can decide for space closure with replacement of the lateral incisor or canine space opening for later prosthetic implant rehabilitation. In this article we described the treatment of agenesis of the maxillary lateral incisors and made a literature review about the treatment options, focusing on space opening associated with the planning and 3-D aesthetic management through the prosthetic implant rehabilitatio

O desejo de ter um tratamento ortodôntico mais rápido é comum a pacientes e profissionais, e muitos meios para acelerar o movimento dentário têm sido pesquisado em todo o mundo. Um dos recursos mais publicitados, se fundamenta na aplicação de estímulos vibratórios nos dentes para induzir uma resposta biológica mais rápida. Desta forma, este artigo objetivou discutir, através de revisão crítica da literatura atual se há segurança na aplicação deste estímulo na clínica diária.


The desire to have a short-term orthodontic treatment is common to both patients and orthodontists and many ways to accelerate tooth movement has been researched around the world. One of the most publicized ways is based on the application of vibratory stimuli to the teeth to induce faster biological response. The authors conducted a critical review of the current literature aiming to clarify whether is safety in the application of this stimulus in everyday practice.

A má-oclusão de classe II foi descrita por Angle e é caracterizada por alterações dentárias e/ou esqueléticas. Seja qual for a combinação dos fatores que acometem o paciente de classe II, o sulco vestibular do molar inferior encontra-se distalmente à cúspide mesiovestibular do molar superior. Estratégias biomecânicas para estabelecer a correta relação entre os arcos envolvem elásticos intermaxilares e dispositivos removíveis que necessitam da colaboração do paciente, ou ainda de aparelhos funcionais fixos, os quais dispensam a colaboração e prometem maior eficiência clínica. O objetivo deste artigo foi revisar as características gerais destes tipos de aparelho, sua forma de funcionamento e aplicabilidades clínicas.


The class II malocclusion has been described by Angle and characterized by dental and/or skeletal abnormalities. Whatever the combination of factors that affect the class II patients, the buccal groove of the lower molar is distal to the mesiobuccal cusp of the upper molar. Biomechanical strategies to establish the correct relationship between the archs involve intermaxillary elastics and removable devices that require the patient's cooperation or fixed functional appliances, which do not require collaboration and promise greater clinical efficiency. The aim of this article was to review the general characteristics of these types of appliance, how they work, and their and clinical applications.
