Resumen Introducción: la Reumatología es una especialidad clínica con elevada demanda y gran desigualdad en su ejercicio en Latinoamérica y el país. Objetivos: describir el estado del ejercicio de la Reumatología en nuestro país. Materiales y métodos: estudio observacional de corte transversal que recabó información a través de una encuesta online difundida por e-mail y de un código QR en el 54º Congreso Argentino de Reumatología. Resultados: se recibieron 127 respuestas; 86 (67,7%) fueron del sexo femenino, con una mediana de edad de 45 años. Ciento trece (113; 89%) encuestados eran especialistas en Reumatología, con 20 años (RIC 14-28) de ejercicio de la medicina y 13 años (RIC 6,5-20) de la especialidad. El 55,5% ejercía tanto en el ámbito público como privado, el 38,6% solo en el privado y el 7,6% únicamente en el público. Del total, el 84,3% trabajaba en dos o más lugares y el 21,2% en cuatro o más. Sesenta de los encuestados (60; 59,8%) recibía una remuneración mensual de entre 100.000 a 249.000 pesos (U$S: 501,68-1249,18) y 27 (23,1%) de menos de 100.000 pesos (U$S 501,68). Conclusiones: nuestro estudio muestra un alto porcentaje de profesionales que se desempeña en múltiples lugares de trabajo. Una quinta parte de ellos tenía remuneración baja.


Abstract Introduction: Rheumatology is a clinical specialty with high demand and great inequality in its practice throughout Latin America and our country. Objectives: to describe the state of the practice of Rheumatology in our country. Materials and methods: cross-sectional observational study that collected information through an online survey distributed by e-mail and through a QR code in the 54th Argentine Congress of Rheumatology. Results: 127 responses were received. Eighty-six (67.7%) were women, with a median age of 45 years. One hundred thirteen (89%) were specialists in Rheumatology, with 20 years (IQR 14-28) in the practice of medicine and 13 years (IQR 6.5-20) in the specialty. The 55.5% worked in both the public and private spheres, 38.6% only in the private sphere and 7.6% only in the public sphere. Of the total, 84.3% worked in 2 or more locations, with 21.2% working in 4 or more. In 60 (59.8%), the monthly remuneration was between 100,000 to 249,000 pesos (U$S: 501.68-1249.18) and in 27 (23.1%) it was less than 100,000 pesos (US$ 501.68). Conclusions: Our study shows that a high percentage of professionals who work in multiple places. One-fifth of respondents had low pay.

La encefalitis autoinmune es un trastorno inmunomediado que compromete distintos territorios del parénquima cerebral, involucrando frecuentemente la materia gris profunda o la corteza, con o sin compromiso de la materia blanca, meninges o médula espinal. Se asocia frecuentemente con enfermedades autoinmunes o paraneoplásicas, y constituye un reto diagnóstico. Reportamos el caso de una mujer de 55 años con antecedente de síndrome de Sjögren que consultó a Emergencias por cefalea y confusión. El líquido cefalorraquídeo (LCR) presentaba leucocitosis con neutrofilia. En la resonancia magnética nuclear (RMN) cerebral se evidenciaron múltiples imágenes de comportamiento restrictivo, de señal hiperintensa en T2 y FLAIR, a predominio córtico-subcortical a nivel occipital bilateral, hemisferio cerebeloso derecho y parietal derecho. Se descartaron infecciones y neoplasias. El panel de anticuerpos para encefalitis autoinmune aquaporina-4 y anti-MOG en LCR fue negativo. Recibió metilprednisolona endovenosa con mejoría progresiva de los síntomas.


Autoimmune encephalitis is an immune-mediated disorder that affects different areas of the brain parenchyma, often involving deep gray matter or the cortex, with or without involvement of white matter, meninges, or spinal cord. It is frequently associated with autoimmune or paraneoplastic diseases and is a diagnostic challenge. We report the case of a 55-year-old woman with history of Sjögren's syndrome who presented to the emergency department with headache and episodes of confusion. Cerebrospinal fluid (CSF) analysis showed leukocytosis with neutrophilia. Brain MRI revealed multiple restricted diffusion lesions with hyperintense signal on T2 and FLAIR sequences, predominantly in the bilateral occipital region, right cerebellar hemisphere, and right parietal region. Infections and neoplasms were ruled out. The panel of antibodies for autoimmune encephalitis, including Aquaporin-4 and anti-MOG in CSF, was negative. She received intravenous methylprednisolone, leading to symptom improvement.

Resumen Objetivo analizar el desarrollo histórico, la naturaleza y el volumen de la literatura científica de la Investigación sobre Educación Interprofesional en terapia ocupacional, así como los tipos de intervenciones que se desarrollan. Método Se utilizó un scoping review con un marco metodológico de cuatro etapas para capturar la evidencia que describe cómo las Investigación sobre Educación Interprofesional y la identidad profesional ha sido conceptualizada e integradas en la terapia ocupacional. Las bases de datos incluidas fueron MEDLINE, EMBASE, PubMed, Pubmed Central, ERIC, PsychInfo, Cinahl, Ciberindex, Dialnet, Scopus, Web of Science, Cochrane y Scielo. Como criterios de inclusión los artículos debían describir conceptos relacionados con la terapia ocupacional y otras disciplinas, identidad profesional y la educación interprofesional en estudiantes y/o profesionales. Del listado final se analizaron las variables: información demográfica, objetivo/propósito del estudio, detalles metodológicos, tipos de intervenciones realizadas y resultados/hallazgos principales. Resultados 18 artículos cumplieron los criterios y 16 se tuvieron en cuenta para la síntesis cualitativa. Predomina la investigación universitaria (92,3%). El 75% de la muestra de las investigaciones se concentra en estudiantes. La investigación mixta representa el 50% del estudio. Las intervenciones más numerosas en Investigación sobre Educación Interprofesional son prácticas en el aula (62,5%) frente a prácticas de campo (37,5%). Conclusión este estudio muestra la fortaleza de las prácticas ligadas a la educación interprofesional y su vínculo para desarrollar la identidad profesional. La formación de colaboración interprofesional en comparación con la educación clínica tradicional, aumenta la identidad profesional a través de la autoeficacia percibida de los estudiantes.

Resultados 18 artigos atenderam aos critérios e 16 foram considerados para a síntese qualitativa. Predomina a pesquisa universitária (92,3%). 75% da amostra da pesquisa se concentra nos alunos. A pesquisa mista representa 50% do estudo. As intervenções mais numerosas na Pesquisa em Educação Interprofissional são as práticas de sala de aula (62,5%) em oposição às práticas de campo (37,5%). Conclusão este estudo mostra a força das práticas vinculadas à educação interprofissional e seu vínculo com o desenvolvimento da identidade profissional. O treinamento interprofissional colaborativo, em comparação com a educação clínica tradicional, aumenta a identidade profissional por meio da autoeficácia percebida pelos alunos.

From the final list, the following variables were analyzed: demographic information, study objective/purpose, methodological details, types of interventions carried out, and main results/findings. Results 18 articles met the criteria and 16 were considered for qualitative synthesis. University research predominated (92.3%). 75% of the research sample focuses on students. Mixed research represents 50% of the study. The most numerous interventions in Interprofessional Education Research are in-class practices (62.5%) versus field practices (37.5%). Conclusion This study shows the strength of practices linked to interprofessional education and their link to developing professional identity. Interprofessional collaboration training compared to traditional clinical education increases professional identity through perceived self-efficacy of students.

La pandemia por Covid-19 ha marcado cambio en paradigma de la atención médica, generando la necesidad del uso de equipo de protección individual (EPI), para salvaguardar la salud. Lo que ha originado manifestaciones cutáneas secundarias asociadas al uso de los mismos. Se realizó una investigación descriptiva de corte transversal con el objetivo de caracterizar las lesiones cutáneas asociadas al uso de EPI e infecciones por Staphylococcusaureus en 283 enfermeros, el diagnóstico dermatológico fue realizado por especialistas; el cultivo bacteriológico se realizó a partir de hisopados de algodón estériles o frotis de aposición sobre la lesión de piel. Para el aislamiento del género Staphylococcus, las muestras se sembraron en agar salino manitol rojo de fenol, incubándose de 24 a 48 horas a 37°C. La identificación de S. aureus se efectúo por la fermentación del manitol en el agar selectivo, la reacción positiva de la prueba de la coagulasa. El S. aureus se diferenció del Staphylococcus coagulasa negativo con el empleo de la prueba de la Dnasa. Se precisó LC-EPI en 75,618% de los enfermeros, de los cuales 92,523% presentaron síntomas sugestivos a infección bacteriana secundaria, ratificada en su totalidad por cultivo microbiológico, identificando a S. aureus. Se estimó que más del 60% de las LC-EPI eran de presentación única catalogada grado I, afectando mayormente las regiones: orbitaria, temporal, nasal, infraorbitaria y frontal, asociándose con el uso de lentes/protectores faciales, gorros y mascarillas como causantes de las lesiones. Se recomienda la definición de protocolos de prevención de LC-EPI(AU)


The Covid-19 pandemic has marked a paradigm shift in medical care, generating the need for the use of personal protective equipment (PPE) to safeguard health. This has caused secondary skin manifestations associated with their use. A descriptive cross-sectional investigation was carried out with the objective of characterizing the skin lesions associated with the use of PPE and Staphylococcus aureus infections in 283 nurses. The dermatological diagnosis was made by specialists; the bacteriological culture was performed using sterile cotton swabs or apposition smears on the skin lesion. For the isolation of the genus Staphylococcus, the samples were seeded in phenol red mannitol saline agar, incubating for 24 to 48 hours at 37°C. The identification of S. aureus was carried out by the fermentation of mannitol in the selective agar, the positive reaction of the coagulase test. S. aureus was differentiated from coagulase-negative Staphylococcus using the DNase test. LC-EPI was required in 75.618% of the nurses, of which 92.523% presented symptoms suggestive of secondary bacterial infection, fully confirmed by microbiological culture, identifying S. aureus. It was estimated that more than 60% of the LC-EPI were of a single presentation classified as grade I, mainly affecting the regions: orbital, temporal, nasal, infraorbital, and frontal, associated with the use of glasses/face protectors, hats, and masks as causes of the injuries. The definition of LC-EPI prevention protocols is recommended(AU)

Las leishmaniasis son causadas por aproximadamente 15 especies de protozoos del género Leishmania. Prevalecen en áreas tropicales y subtropicales del Viejo y Nuevo Mundo, la movilidad humana también los convierte en un problema médico en áreas no endémicas. Las manifestaciones clínicas pueden comprender formas cutáneas que pueden ser localizadas. El diagnóstico y el tratamiento varían según las manifestaciones clínicas, el área geográfica y la especie involucrada. Este artículo destaca aspectos epidemiológicos y clínicos en la provincia de Santo Domingo de los Tsachilas, Ecuador. Se utilizaron fichas clínicas y epidemiológicas de pacientes que presentaron lesiones ulceradas o no, con un tiempo de evolución igual o mayor a 2 semanas. El mayor número de casos están entre los grupos sucesivos de adolescentes de 11 a 19 años y los adultos jóvenes entre 20 y 39 años. El 81,79% de los positivos procedían de áreas rurales o periurbanas. Dos terceras partes de los pacientes eran masculinos. Hubo predominio de lesiones únicas (87,79%); ubicadas en cuello y región cervical, y extremidades inferiores (24,84 y 24,20%, respectivamente). Cerca del 88% de los pacientes no presentaron ningún tipo de sintomatología asociada a la infección parasitaria. El 97,86% de los diagnósticos de laboratorio fueron por observación microscópica directa. 81,80% obtuvieron cura total de las lesiones y el 22% de los pacientes abandonaron la terapia antiparasitaria. Se requieren pruebas rápidas y accesibles que puedan definir las especies de Leishmania, ya que la discriminación tiene una importancia significativa para el pronóstico y tratamientos específicos de la especie(AU)


Leishmaniases are caused by approximately 15 species of protozoa of the genus Leishmania. They prevail in tropical and subtropical areas of the Old and New World human mobility also makes them a medical problem in nonendemic areas. Clinical manifestations may comprise cutaneous forms that may be localized. Diagnosis and treatment vary according to the clinical manifestations, geographic area, and species involved. This article highlights epidemiological and clinical aspects in the province of Santo Domingo de los Tsachilas, Ecuador. Clinical and epidemiological records of patients who presented ulcerated lesions or not, with an evolution time equal to or greater than 2 weeks were used. The largest numbers of cases are between the successive groups of adolescents from 11 to 19 years old and young adults between 20 and 39 years old. 81.79% of the positives came from rural or peri-urban areas. Two thirds of the patients were male. There was a predominance of single lesions (87.79%); located in the neck and cervical region, and lower extremities (24.84 and 24.20%, respectively). About 88% of the patients did not present any type of symptomatology associated with the parasitic infection. 97.86% of laboratory diagnoses were by direct microscopic observation. 81.80% obtained complete cure of the lesions and 22% of the patients abandoned antiparasitic therapy. Rapid and accessible tests that can define Leishmania species are required, as discrimination is of significant importance for species-specific prognosis and treatment(AU)

Objetivo: valorar el uso de terapias alternativas no farmacológicas en migraña y variables relacionadas. La migraña afecta a una de cada diez personas en el mundo, con aumento creciente y mayor prevalencia entre mujeres de zonas urbanas. Aunque los tratamientos habituales son farmacológicos, se ha incrementado el uso de terapias alternativas no farmacológicas en migraña y variables relacionadas. Materiales y métodos: estudio observacional transversal mediante encuesta anónima vía web a 3.342 pacientes de países europeos sobre el uso de terapias alternativas para tratamiento de migraña. Variables de estudio: edad, género, país, tipo de localidad, nivel de estudios, ámbito rural o urbano y respuesta al tratamiento. Resultados: la utilización de terapias alternativas en migraña es un fenómeno creciente, con mayor uso en pacientes entre 41-60 años y estudios superiores. Conclusiones: en migraña se observan diferencias por edad, género, nivel cultural y país de procedencia. El uso de la medicina complementaria como alternativa terapéutica en la migraña es poco habitual y se debería indagar con mayor profundidad estableciendo relaciones que permitan apoyar al paciente afectado y priorizar las de mayor evidencia científica..(Au)


Objective: to evaluate the use of alternative non-pharmacological therapies in migraine and related variables. Migraine affects one in ten people worldwide, with a growing increase and a higher prevalence among women in urban areas. Although the usual treatments are pharmacological, the use of alternative techniques is increasing. Materials and methods: cross-sectional observational study by anonymous web survey of 3,342 patients from European countries on the use of alternative therapies. Study variables: age, gender, country, type of locality, educational level, rural or urban setting and treatment response. Results: the use of alternative therapies in migraine treatment is a growing phenomenon, with greater use in patients between 41-60 years of age and higher education. Conclusions: in Migraine, differences are observed by age, gender, cultural level and country of origin. The use of complementary medicine as an therapeutic alternative in migraine is uncommon and should be investigated in greater depth, establishing relationships that allow supporting the affected patient and prioritizing, if possible, some over others with scientific evidence..(Au)

de enfermería del Hospital General Docente Ambato. Estudio epidemiológico, prospectivo, observacional con la combinación de un enfoque cuali-cuantitativo. Con una población de 64 licenciadas de enfermería, tomándose como muestra a 54 licenciadas que laboran en los servicios de Emergencia, Cirugía y Sala de Operaciones quienes han sido expuestas a riesgo infecciosos. Como técnica de recolección de los datos se empleó la observación científica, la encuesta y la lista de chequeo. Los métodos de análisis del nivel teórico fueron el Inductivo-deductivo, utilizándose la estadística descriptiva, para mostrar los resultados a través de figuras y tablas. Entre los resultados obtenidos se evidencian que en cuanto a la clasificación del tipo de agentes según los riesgos, se obtuvieron los siguientes; enfermeras contagiadas con VHC 52%, VHB 26%, VIH 18% y otros virus o bacterias 4%; el 59,26% del personal de enfermería tiene un déficit de conocimientos de los riesgos infecciosos propios del personal de salud; el 77,77% si ha tenido un evento asociado con fluidos corporales con riesgo infeccioso asociado.; que aunque el 77,78% del personal casi siempre lo realiza el lavado adecuado de manos antes y después de atención al paciente, hay un 5,56% nunca lo realiza; el dispositivo que colocó a la enfermera en riesgo infeccioso con un 46,30% fue con una aguja hueca; el 12,96% al dejar el material usado en un lugar no adecuado. Se recomienda la evaluación, diseño y aplicación de una guía de medidas de bioseguridad con énfasis en riesgos infecciosos propiciados por contacto con los fluidos corporales(AU)


To evaluate the infectious risk of HIV and hepatitis C in nursing personnel of the Ambato General Teaching Hospital. An epidemiological, prospective, observational study with the combination of a qualitative-quantitative approach. With a population of 64 nursing graduates, taking as a sample 54 graduates who work in the Emergency, Surgery and Operating Room services who have been exposed to infectious risk. Scientific observation, survey and checklist were used as the data collection technique. The theoretical level analysis methods were inductive-deductive, using descriptive statistics to show the results through figures and tables. Among the results obtained, it is evident that regarding the classification of the type of agents according to the risks, the following were obtained; nurses infected with HCV 52%, HIB 26%, HIV 18% and other viruses or bacteria 4%; 59.26% of the nursing personnel have a deficit of knowledge of the infectious risks of the health personnel; 77.77% if they have had an event associated with bodily fluids with associated infectious risk; Although 77.78% of the personnel almost always perform adequate hand washing before and after patient care, 5.56% never do it; The device that placed the nurse at infectious risk with 46.30% was with a hollow needle; 12.96% when leaving the used material in an inappropriate place. The evaluation, design and application of a guide to biosafety measures with emphasis on infectious risks caused by contact with body fluids is recommended(AU)

INTRODUCTION: Assessment of risk both for pregnancy morbidity and thrombosis in the presence of anti-phospholipid antibodies (aPL) is still a challenge in Systemic Lupus Erythematosus (SLE) patients. The Global Antiphospholipid Syndrome Score (GAPSS) takes into account the aPL profile (criteria and non-criteria aPL), the conventional cardiovascular risk factors and the autoimmune antibody profile. An adjusted model of the score (aGAPSS) excluding anti-phosphatidylserine/Prothrombin (aPS/PT), suggests that the score is able to stratify patients for their rate of events making it widely applicable in daily clinical practice. OBJECTIVE: To validate the aGAPSS in a multicentric cohort of SLE patients in Argentina. PATIENTS AND METHODS: consecutive SLE patients with and with andwithout thrombotic events from seven Rheumatologist centers were included. Traditional cardiovascular risk factors, aPL antibodies and medications received (aspirin, hydroxychloroquine and anticoagulation) were collected. The score aGAPSS was calculated for each patient at the last visit by adding together the points corresponding to the risk factors: 1 for hypertension, 3 for dyslipidemia, 4 for LA and B2GPI (IgM or IgG) antibodies and 5 for aCL (IgM or IgG) antibodies. The discriminative ability of the aGAPSS was calculated by measuring the area under the receiver operating characteristic curve (AUC). Multivariate logistic regression analysis was performed to examine the impact of multiple cardiovascular risk factors and laboratory parameters on the occurrence of thrombosis. RESULTS: Two hundred and ninety-six SLE patients were included. One-hundred and twenty-one patients (40.9%) presented thrombotic and/or pregnancy complications. Median aGAPSS was significantly higher in patients who experienced an event (thrombosis and/or pregnancy morbidity) compared with those without [4 (IQR 1-9) versus 1 (IQR 0-5); p < 0.001]. The best cut off point for the diagnosis of thrombosis and/or pregnancy complications was aGAPSS ≥4. Multivariate logistic regression analysis showed that aCL antibodies [OR 2.1 (95% CI 1.16-3.90); p = 0.015] were an independent risk factors for thrombotic events. CONCLUSIONS: This score is a simple tool, easy to apply to SLE patients in daily practice. The use of the aGAPSS could change the non-pharmacologic and pharmacologic treatment in higher risk patients to improve their survival.

OBJECTIVES: Methotrexate (MTX) has been studied with conflicting results in patients with polymyalgia rheumatica (PMR). Our objective was to evaluate the effectiveness of MTX to reduce relapses and recurrences in patients with PMR. METHODS: This observational longitudinal cohort study included 94 consecutive patients with PMR. Patients were assigned to 3 groups according to the prescribed treatment: group 1, treated with glucocorticoids (GCs) alone; group 2, treated with GCs initially plus MTX after a relapse or recurrence; and group 3, treated with GCs plus MTX since diagnosis.Factors associated with a first relapse were studied in the population. To evaluate MTX effect, patients from group 2 were evaluated comparing results from the first treatment period (GCs alone) to the second treatment period with GCs plus MTX. RESULTS: Ninety-four patients were included. The median follow-up time was 21.3 months (interquartile range [IQR], 11.7-56.2). Fifty-three patients (56.4%) were in group 1, 33 (35.1%) in group 2, and 8 (8.5%) in group 3. We found that female sex had a tendency to be associated with a first relapse (p = 0.07).In group 2, 35 relapses were identified during the first treatment period and only 8 relapses during the combined treatment period (p < 0.001). In this group, after the addition of MTX, the GCs dose at relapse was lower (5.1 vs 3 mg/d, p = 0.02) and the time to accomplish remission was shorter (22.9 vs 8.7 months, p = 0.01). There were no differences in the number of recurrences. CONCLUSIONS: The use of MTX in PMR patients who already had a relapse reduced the number of future relapses and decreased the time to achieve remission. Adding MTX allowed a reduction of GCs dose at relapse.

Introduction: Giant cell arteritis (GCA) is the most frequent systemic vasculitis in patients older than 50 years. The diagnosis is based on the clinical history, laboratory findings and imaging studies associated with a temporal artery biopsy (TAB). However, the biopsy result could be inconclusive in up to 40% of the cases. The aim of this study was to review the current management of the patients with clinical suspect of GCA in the university hospital CEMIC in Buenos Aires, Argentina, and correlate the disease behavior with the TAB result. Methods: Retrospective study that reviewed consecutive patients to whom a TAB was made in an 11-year period (2005-2016). Clinical and pathology reports were reviewed. Descriptive statistics were performed. Quantitative variables were described as mean (S.D.) or median [range or inter- quartile range (IQR)] and qualitative variables as number (%). To compare the characteristics of the groups, bivariate analyzes were performed using contingency tables and logistic regression models if necessary. Results: Sixty three patients were included, 68% women. The mean age was 72 years old (SD 8.4). Seventeen biopsies (26.9%) were positive for GCA. The average post fixation length was 1.68 cm (SD 1.2). Patients were divided into 3 groups taking into account the result of the TAB, the ACR criteria and the imaging studies. We could not identify predictors of biopsy positivity. The group of patients with GCA and negative TAB showed a higher percentage of patients with abnormal temporal artery at physical examination. Conclusion: The TAB positive percentage (26.9%) was similar to the reported in other series as well as the post fixation length. We could not identify predictors of biopsy positivity.


Introducción: La arteritis de células gigantes (ACG) es la vasculitis sistémica primaria más frecuente en pacientes mayores de 50 años. El diagnóstico de ACG se basa en la evaluación clínica, de laboratorio y estudios por imágenes, asociados a una biopsia. Sin embargo, el resultado de la biopsia puede no ser concluyente en más del 40% de los casos. El objetivo de este estudio fue revisar el manejo de los pacientes con sospecha de ACG en el hospital universitario CEMIC en Buenos Aires, Argentina, y correlacionar el comportamiento de la enfermedad con el resultado de la biopsia de arteria temporal (BAT). Métodos: Estudio retrospectivo que analizó pacientes consecutivos a los cuales se les realizó BAT en un período de 11 años (2005-2016). La información recolectada se obtuvo a partir de las historias clínicas y de los informes de anatomía patológica. Se realizó estadística descriptiva. Para las variables cuantitativas se estimaron medias y sus respectivos desvíos estándar o medianas y percentil 25-75, y para las variables cualitativas, la cantidad y el porcentaje. Para comparar las características de los grupos se realizaron análisis bivariados mediante tablas de contingencia y modelos de regresión logística de ser necesario. Resultados: Sesenta y tres pacientes fueron incluidos, 68% mujeres, con una edad media de 72 años (DS 8,4). Diecisiete biopsias (26,9%) fueron positivas. La longitud media posfijación fue de 1,68 cm (DS 1,2). La población se dividió en 3 grupos según la BAT, los criterios ACR y los estudios por imágenes. No se identificaron factores predictores de positividad de la BAT. El grupo con ACG y BAT negativa presentó mayor porcentaje de pacientes con arteria temporal anormal al examen físico. Conclusión: El porcentaje de positividad de las biopsias (26,9%) fue similar al reportado por otras series, así como la longitud de la biopsia luego de la fijación (1,68 cm). No identificamos factores predictores de positividad de la BAT.
