INTRODUCTION: Fifty percent of small bowel bleeding is caused by angioectasia and the rebleeding rate due to small bowel angioectasia (SBA) is 80%. Its endoscopic treatment is difficult. Beneficial effects of octreotide on gastrointestinal angioectasia have been described, but no studies have reported its efficacy in SBA. AIM: Our aim was to investigate the effectiveness of octreotide in the prevention of rebleeding due to SBA. MATERIAL AND METHODS: Sixteen patients with bleeding caused by SBA were assigned to treatment with octreotide 100 µg/24 h SC, for at least 6 months, and compared with a non-treatment group of 36 patients. The primary outcome was the rebleeding rate, and the secondary outcomes were the number of hospital readmissions, bleeding-related death, and adverse effects. RESULTS: Octreotide was administered for 10.5 ± 8.4 months. Follow-up was 12.9 ± 17.3 months and 15.3 ± 17.7 months, in the treatment and non-treatment groups, respectively (p = 0.09). At the end of follow-up, 4 (25%) treatment group patients and 26 (72.2%) non-treatment group patients presented with rebleeding (p = 0.002). In the treatment group and non-treatment group, the cumulative probability of remaining rebleeding-free at one year was 79% vs 44.2%, and 79% vs 34.6% at 2 years, respectively (p = 0.05). Through the multiple logistic regression analysis, treatment was the protective variable. Six patients presented with adverse events. One of those patients (6.25%) had a major adverse event. CONCLUSIONS: Our results suggest that treatment with octreotide could be efficacious in the prevention of rebleeding due to SBA.

Abstract Introduction: The prevalence of iron deficiency anemia in developed countries has been estimated between 2-5 %, associated with high morbidity and mortality. Etiological identification is sometimes difficult and requires diagnostic methods, such as video capsule endoscopy (VCE). Aim: This study intends to characterize the findings of this technique in patients with unexplained iron deficiency anemia. Materials and methods: Descriptive observational retrospective study. We describe the VCE findings (PillCamSB2-SB3) in all patients seen between 2011 and 2019. The findings were interpreted according to the Saurin classification: normal (P0), uncertain potential (P1), and high bleeding potential (P2). Results: Of the 490 VCEs performed during the study period, 155 indicated iron deficiency anemia; 106 were women (68.4 %), and the mean age was 57.1 ± 16.6 years. The main comorbidities were cardiovascular in 23 (18.3 %) and arterial hypertension in 16 (12.6 %). Antiplatelets were present in 18 (15.4 %) and anticoagulants in six (5.1 %). Small bowel lesions were vascular in 44 studies (28.4 %), inflammatory in 33 (21.2 %), and neoplastic in seven (4.5 %). Angiectasias were the most frequent lesions in 33 cases (21.3 %). P2 lesions were present in 53 VCEs (34.2 %). Conclusions: VCE is helpful in the study of iron deficiency anemia and helps detect positive findings in the midgut in three out of four patients for which it is indicated. The most frequent significant P2 lesions were vascular. These findings allow providing adequate treatment.


Resumen Introducción: la prevalencia de la anemia ferropénica en países desarrollados se ha estimado entre 2 %-5 %, asociada con una alta morbimortalidad. La identificación etiológica a veces es difícil, y requiere de métodos diagnósticos, como la videocápsula endoscópica (VCE). Objetivo: el objeto del presente estudio fue caracterizar los hallazgos de esta técnica en pacientes con anemia ferropénica inexplicada. Materiales y métodos: estudio descriptivo, observacional y retrospectivo. Se describen los hallazgos de VCE (PillCamSB2-SB3) en todos los pacientes atendidos entre 2011 y 2019. Los hallazgos se interpretaron según la clasificación de Saurin: normal (P0), potencial incierto (P1) y alto potencial de sangrado (P2). Resultados: del total de 490 VCE realizadas durante el período del estudio, 155 se efectuaron con indicación de anemia ferropénica; 106 fueron mujeres (68,4 %) y la edad media fue de 57,1 ± 16,6 años. Las comorbilidades principales fueron cardiovasculares en 23 (18,3 %) e hipertensión arterial en 16 (12,6 %). La ingesta de antiplaquetarios se presentó en 18 (15,4 %) y anticoagulantes en 6 (5,1 %). Las lesiones en el intestino delgado fueron vasculares en 44 estudios (28,4 %), inflamatorias en 33 (21,2 %) y neoplásicas en 7 (4,5 %). Las angiectasias fueron las lesiones más frecuentes en 33 casos (21,3 %). En 53 VCE se presentaron lesiones P2 (34,2 %). Conclusiones: la VCE es útil en el estudio de la anemia ferropénica, y ayuda a detectar hallazgos positivos en el intestino medio en 3 de cada 4 pacientes en los cuales se indica su uso. Las lesiones P2 significativas más frecuentes fueron las vasculares. Estos hallazgos permiten enfocar un tratamiento adecuado.

A síndrome de Rendu-Osler-Weber, ou telangiectasia hemorrágica hereditária (THH), é uma doença autossômica dominante de penetrância variável que altera a camada muscular e elástica da parede dos vasos sanguíneos, causando neles grande fragilidade e dilatação e tornando-os suscetíveis a sangramentos secundários, a traumatismos e malformações arteriovenosas (MAV) ­ sendo elas as mais comuns no cérebro, nos pulmões e no trato gastrointestinal e hepático. Este relato de caso tem como objetivo realizar uma revisão sobre essa síndrome, bem como a respeito da melhor terapia e do melhor manejo para sangramentos gastrointestinais por angiectasias. Trata-se de uma paciente de 62 anos de idade, com episódio de hematêmese e melena recorrentes, que foi admitida instável hemodinamicamente e com queda importante dos níveis de hemoglobina, necessitando de múltiplas transfusões sanguíneas. Após entrevista clínica, que chamou a atenção para o histórico familiar de quadros similares, epistaxes recorrentes associados ao exame físico e angiectasias gástricas à endoscopia digestiva alta, foi atribuído o diagnóstico de síndrome de Rendu-Osler-Weber. Angiectasias foram erradicadas com terapia endoscópica usando plasma de argônio. A THH é uma doença subdiagnosticada que merece atenção especial por conta do seu risco de causar hemorragias graves com expressão fenotípica mais aflorada com o avançar da idade. Por seu cunho hereditário, faz-se necessário um acompanhamento médico linear com a família para evitar complicações secundárias da doença, bem como para o manejo adequado de suas apresentações.


Osler-Weber-Rendu syndrome or hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant disease that changes the muscular and elastic layer of the blood vessel wall, making them fragile and more susceptible to bleeding secondary to trauma and arteriovenous malformations (AVM) ­ a condition that commonly affects the brain, lungs, gastrointestinal tract, and liver. This study aims to perform a literature review on this syndrome, as well as on the best treatment and management for gastrointestinal bleeding from angioctasia. To this end, it reports the case of a 62-year-old female patient with recurrent episodes of hematemesis and melena, who was admitted to hospital with hemodynamic instability and low hemoglobin levels, requiring multiple blood transfusions. After clinical interview, which indicated family history of recurrent epistaxis, physical examination, and upper digestive endoscopy showing gastric angiectasias, the patient was diagnosed with Osler-Weber-Rendu syndrome. Angiectasias were treated with endoscopic argon plasma. Considering the risk of severe hemorrhages and its more pronounced phenotypic expression with advancing age, HHT is an underdiagnosed disease that deserves special attention. Due to its hereditary nature, a linear medical follow-up with the family is necessary to avoid secondary complications of the disease, as well as the proper management of its presentations.


El síndrome de Rendu-Osler-Weber o telangiectasia hemorrágica hereditaria (THH) es una enfermedad autosómica dominante de penetrancia variable, que modifica la capa muscular y elástica de la pared de los vasos sanguíneos, provocando una gran fragilidad y dilatación de estas, haciéndolas susceptibles a hemorragias secundarias a trauma y malformaciones arteriovenosas (MAV), que son las más comunes en el cerebro, pulmones, tracto gastrointestinal e hígado. Este estudio tiene como objetivo realizar una revisión de este síndrome, así como la mejor terapia y manejo del sangrado gastrointestinal por angioctasia en esta patología. Se trata de un paciente de 62 años, con episodio de hematemesis recurrente y melena, que ingresa hemodinámicamente inestable y con descenso significativo de los niveles de hemoglobina, requiriendo múltiples transfusiones sanguíneas. Tras entrevista clínica, destacando los antecedentes familiares de patologías similares, epistaxis recurrente, exploración física y angiectasias gástricas a endoscopia digestiva alta, se asignó el diagnóstico de síndrome de Rendu-Osler-Weber. Las angiectasias se erradicaron con terapia endoscópica con plasma de argón. La HHT es una enfermedad infradiagnosticada que merece una atención especial por su riesgo de hemorragias severas, con una expresión fenotípica más pronunciada con el avance de la edad. Además de su carácter hereditario, es necesario un seguimiento médico lineal con la familia para evitar complicaciones secundarias de la enfermedad, así como el adecuado manejo de sus presentaciones.

INTRODUCTION: Video capsule endoscopy and balloon-assisted enteroscopy are complementary diagnostic methods in the study of small bowel bleeding, and different factors can affect their diagnostic yield. AIMS: To define the level of agreement between video capsule endoscopy and enteroscopy in small bowel bleeding, according to the type of lesion, in a cohort of patients at a tertiary care referral center. MATERIALS AND METHODS: A retrospective study was conducted that included 428 capsule endoscopies performed within the time frame of 2011 and 2019 at our healthcare institution. Seventy-four video capsule endoscopies, followed by enteroscopy, in 71 patients suspected of presenting with small bowel bleeding, were analyzed. RESULTS: Mean patient age was 63.9 ± 13.5 years and 42 patients were women. The two diagnostic procedures were performed. Overall diagnostic yield of positive findings between video capsule endoscopy and enteroscopy was 86.5% vs. 58.1%, respectively (p = 0.0527). Agreement between video capsule endoscopy and enteroscopy for positive pathologic findings was weak (Ik = 0.17, 95% CI: -0.0097-0.3543), but according to lesion type, it was good for inflammatory lesions (Ik = 0.71, 95% CI: 0.5182-0.9119) and moderate for angiectasias (Ik = 0.45, 95% CI: 0.2469-0.6538) and tumors (Ik = 0.40, 95% CI: 0.1217-0.6794). The results between the two methods differed in 38 patients (51.3%). There was complete intestinal capsule retention in one patient (1.4%) and active bleeding in 13 (17.6%). CONCLUSIONS: The present study showed that the two techniques had a similar overall detection rate for small bowel lesions, but the type of lesion was the main factor that could modify diagnostic agreement.

Objetivos: La cápsula endoscópica (CE) ha revolucionado la evaluación diagnóstica de pacientes con sangradogastrointestinal de origen oscuro (SGO). El objetivo de nuestro estudio fue evaluar el rendimientodiagnóstico de CE en pacientes con SGO en nuestro centro. Métodos: Se realizó un estudio descriptivoy retrospectivo, revisando historias clínicas en 60 pacientes consecutivos llevados a CE con indicación deSGO, entre septiembre de 2009 y septiembre de 2011, en el Hospital Pablo Tobón Uribe. Los hallazgos enCE se interpretaron según el grado de relevancia clínica para el diagnóstico definitivo: normales (P0), poco(P1) y altamente relevante (P2). Resultados: El rendimiento diagnóstico de CE en pacientes con SGO fue de57%. No se encontró diferencia significativa en la presencia de lesiones relevantes (P2), entre pacientes conSGO evidente y oculto (63% vs. 52%, p: 0,49); 26% de lesiones significativas (P2) en CE, se encontrabanal alcance de endoscopia alta (17%) o colonoscopia total (9%). En cuanto al tipo de lesión P2 encontrada,47% fueron vasculares, 44% inflamatorias y 9% neoplásicas. Los pacientes con lesiones P2 eran de mayoredad, comparados con aquellos con lesiones P1 y P0 (p: 0,05). Conclusión: El rendimiento diagnóstico deCE en SGO, en nuestra serie, es muy similar al reportado en publicaciones internacionales. Objectives: Capsule endoscopy (CE) has revolutionized diagnostic evaluation of patients with obscure gastrointestinal bleeding (OGB). The aim of our study was to evaluate diagnostic performance of CE in patients with OGB at our center. Methods: This retrospective study reviewed the medical records of 60 consecutive patients who had undergone capsule endoscopy because of OGB at the Hospital Pablo Tobon Uribe between September 2009 and September 2011. CE findings were interpreted according to degree of clinical relevance for definitive diagnosis: normal (P0), not very relevant (P1) and highly relevant (P2). Results: The diagnostic performance of CE for patients with OGB was 57%. No significant difference was found among patients with obvious and hidden relevant lesions (P2) OGB (63% vs. 52%, p: 0.49). 26% of relevant lesions (P2) found by EC could have been found with upper endoscopy (17%) or total colonoscopy (9%). Of the P2 lesions found, 47% were vascular, 44% were neoplastic and 9% were inflammatory. Patients with P2 lesions were older tan those with P1 and P0 lesions (p: 0.05). Conclusion: The diagnostic performance of CE for OGB in our seriesis similar to that reported in international publications. The most frequently found P2 lesions were vascular lesions

La videocápsula endoscópica (VCE), se introdujo en Colombia en junio de 2003 como método diagnóstico de las enfermedades del intestino delgado; realizamos un estudio observacional, retrospectivo, descriptivo, con 100 pacientes referidos para la práctica de VCE. Objetivos. Presentar la experiencia clínica de la utilización de la VCE en nuestra población, determinar las indicaciones más frecuentes para el estudio del intestino delgado con VCE en nuestro país, definir las patologías encontradas en el grupo de pacientes estudiados, describir los hallazgos de videocápsula en relación a los síntomas de los pacientes. Metodología. Se realizó un análisis descriptivo de las variables identificadas y estudiadas, se analizó el comportamiento de los datos, se calcularon las medidas de tendencia central y de dispersión de las variables en mención. Resultados. En el 97% de los exámenes la indicación fue el estudio del sangrado digestivo oscuro oculto o manifiesto, la patología diagnosticada con mayor frecuencia fue la patología vascular (angiectasias) del intestino delgado, seguida por las patologías inflamatoria (úlceras), parasitaria y tumoral. Conclusiones. El examen de VCE es de gran ayuda en el estudio del sangrado digestivo oscuro ya sea oculto o manifiesto y debe considerarse en nuestro medio de primera elección luego de la realización de endoscopia digestiva alta con visualización de la segunda porción duodenal (2) y de colonoscopia con ileoscopia (2), si el paciente no presenta síntomas obstructivos.


The VCE was introduced in Colombia in June 2003 as a method to diagnose diseases in the small intestine. We ran comprehensive observations with a series of 100 cases and created a retrospective and descriptive study of patients referred to us for the practice of VCE. Objectives. To present the clinical experience in the use of the VCE in our population. To determine the most frequent findings for the study of the small intestine with VCE in our country. To define the pathologies found in the group of patients studied, describe the video capsule findings in relation to the symptoms. Methodology. We conducted a descriptive analysis of the identified and studied variables, looking at the behaviour of the data the central trend and dispersion of the variables was calculated. Results. In 97% of the cases, the indication was for the study of occult or overt obscure digestive bleeding. The most frequent diagnosed pathology was the vascular pathology (angiectasias) of the small intestine; followed by the inflammatory pathology (ulcers) with tumours and parasite. Conclusion. The video capsule endoscopy exam is of great help in the study of obscure digestive bleeding whether it is occult or overt. It should be considered in our country as the primary choice after the upper digestive endoscopy with visualisation of the second duodenal portion (2) and the colonoscopy with ileoscopy (2) if the patient doesn’t present obstructive symptoms.
