INTRODUCTION: This micro-computed tomographic study investigated the original anatomic diameters of the apical portion of mandibular molar mesial canals and matched them to the dimensions of instruments that are currently available and commonly used for apical preparation. METHODS: One hundred eight mandibular molar mesial roots with Vertucci type IV configuration were scanned by micro-computed tomographic imaging, and the largest anatomic diameter of both mesial canals at 1, 2, 3, and 4 mm short of the apical foramen was measured. Canal diameters were compared with the dimensions of 10 endodontic instruments for simulation of preparation 1 mm short of the apical foramen. Accordingly, the instruments with a diameter larger than the largest canal diameter were regarded as having the potential to prepare 100% of the canal walls at each specific point or over the 4-mm apical segment. RESULTS: At 1 and 2 mm short of the apical foramen, a 45/.02 instrument had a larger diameter than the mesiobuccal canal diameter in 73% and 55% of the teeth, respectively. Corresponding figures for the mesiolingual canal were 65% and 55%. When the entire 4-mm apical segment was considered, a 40/.06 instrument had better results, being larger than the apical canal in about 20% of the specimens. Overall, 78% of the apical canals would not be completely prepared by any instrument. Based on the mean (median) anatomic diameters, the adequate instrument dimensions would be 40/.10 (40/.08) for mesiobuccal canals and 45/.08 (40/.09) for mesiolingual canals. CONCLUSIONS: The dimensions of the available instruments are not compatible with complete apical preparation in the majority of cases. The ideal instrument size/taper to include all the canal walls in apical preparation is too large and may not be safe for clinical use.

El conocimiento preciso de la morfología radicular dental son claves para el éxito en las terapias endodónticas. El objetivo de este estudio fue evaluar las distintas variaciones de la morfología radicular y apical en primeros molares superiores e inferiores de una población maya moderna. Se observaron microfotografías provenientes de 80 primeros molares superiores e inferiores, presentadas en cortes transversales a 1 y 2,5 mm del vértice apical, así como de las porciones 1/3, 1⁄2, 2/3 del largo radicular y en la unión amelocementaria. Se determinó el número de raíces, conductos y configuración de Vertucci, así como forma y medidas de diámetro mayor y menor a 1 mm del vértice apical. El 90 % de primeros molares superiores reportó la presencia de 3 raíces y el 10 % mostró 2. En tanto que el total de primeros molares inferiores (100 %) reportó 2 raíces. En general, en las raíces mesiales prevaleció el Tipo II de Vertucci mientras que en las distales y palatinas la Tipo I. En el corte a 1 mm del vértice apical la forma redonda fue la de mayor prevalencia en los conductos de primeros molares superiores (44,5 %), seguida de la irregular (34,1 %) y la oval (21,4 %); mientras que en los primeros molares inferiores la más prevalente fue la irregular (54, 5 %), seguida de la redonda (23,9 %) y oval (21,6 %). Los diámetros mayor y menor de los conductos mesiales a 1 mm apical midieron 0,46 y 0,23 mm y 0,64 y 0,25 mm en molares superiores e inferiores respectivamente. La frecuencia de MB2 en primeros molares superiores fue del 77,8 %. Los conductos con mayor variabilidad fueron los mesiales. Se observó baja frecuencia de conductos ovales. Los diámetros mayores de los conductos mesiales en general, fueron superiores a 0,45 mm.


Accurate knowledge of dental root morphology is a key to success in endodontic therapies. The aim of this study was to evaluate the different variations of the root and apical morphology in upper and lower first molars of a modern Mayan population. Photomicrographs were taken from 80 upper and lower first molars, presented in cross sections at 1 and 2.5 mm from the apical vertex, as well as from the 1/3, 1⁄2, 2/3 portions of the root length and at the cementoenamel junction. The number of roots, canal and Vertucci`s configuration was determined, as well as the shape and measurements of the major and minor diameters at 1 mm from the apex. In this study 90 % of upper first molars reported the presence of 3 roots and 10 % showed 2, while the total lower first molars (100 %) reported 2 roots. In general, Vertucci`s Type II prevailed in the mesial roots while Type I was prevalent in the distal and palatal ones. In the 1 mm cut of the apical vertex, the round shape was the most prevalent in the canal of the first upper molars (44.5 %), followed by irregular (34.1 %) and oval (21.4 %); while in the first lower molars the most prevalent one was irregular (54.5 %), followed by round (23.9 %) and oval (21.6 %). The major and minor diameters of the mesial canals at 1 mm apical recorded 0.46 and 0.23 mm and 0.64 and 0.25 mm in upper and lower molars respectively. The frequency of MB2 in upper first molars was 77.8 %. The canals with greater variability were the mesial ones. Low frequency of oval canals was observed. The largest diameters of the mesial canal in general were greater than 0.45 mm.

SUMMARY: The purpose of this study was to determine the shape and diameter of the physiological foramen in maxillary premolars of a Chilean population. The sample consisted of 125 extracted maxillary premolars, in which the apical three to five millimeters were cut and observed under magnification of 40x, photographed (Motic Cam), and analyzed using Motic Images Plus 2.0 ML in order to obtain measurements of the minor and major diameter of each physiological foramen. The average minor and major diameter in maxillary premolars was 0.270 and 0.413 mm, respectively. In terms of shape, 72.19 % had oval shaped physiological foramen; 18.93 % were irregularly shaped and 8.88 % were round. Oval and irregular canals are significantly more difficult to shape and require special attention in order to obtain a thoroughly disinfected root canal system.


RESUMEN: El objetivo del estudio fue determinar la forma y el diámetro del foramen fisiológico de premolares maxilares en una población Chilena. La muestra consistió en 125 premolares maxilares extraídos, a los que se cortó de 3 a 5 milímetros apicales, fueron observados bajo magnificación de 40x, fotografiado (Motic Cam) y analizados utilizando Motic Images Plus 2.0 ML para obtener las mediciones de el diámetro menor y mayor de cada foramen fisiológico. El promedio del diámetro menor y mayor en premolares maxilares fue de 0,270 y 0,413 mm respectivamente. En cuanto a la forma 72,19 % tenía el foramen fisiológico de forma oval; 18,93 % tenía forma irregular y 8,88 % fue redondo. Canales ovales e irregulares son significativamente más difíciles de conformar y requieren especial atención para obtener una adecuada desinfección del sistema de canales radiculares.

INTRODUCTION: A thorough knowledge of root canal anatomy is critical for successful root canal treatments. This study evaluated the internal anatomy of the palatal roots of maxillary first molars with micro-computed tomography (microCT). METHODS: The palatal roots of extracted maxillary first molars (n = 169) were scanned with microCT to determine several anatomic parameters, including main canal classification, lateral canal occurrence and location, degree of curvature, main foramen position, apical constriction presence, diameters 1 and 2 mm from the apex and 1 mm from the foramen, minor dentin thickness in those regions, canal volume, surface area, and convexity. RESULTS: All canals were classified as Vertucci type I. The cross sections were oval in 61% of the canals. Lateral canals were found in 25% of the samples. The main foramen did not coincide with the root apex in 95% of the cases. Only 8% of the canals were classified as straight. Apical constriction was identified in 38% of the roots. The minor and major canal diameters and minor dentin thickness were decreased near the apex. The minor dentin thickness 1 mm from the foramen was 0.82 mm. The palatal canals exhibited a volume of 6.91 mm(3) and surface area of 55.31 mm(2) and were rod-shaped. CONCLUSIONS: The root canals of the palatal roots were classified as type I. However, some factors need to be considered during the treatment of these roots, including the frequent ocurrence of moderate/severe curvatures, oval-shaped cross-sections, and lateral canals, noncoincidence of the apical foramen with the root apex, and absence of apical constriction in most cases.

Information regarding the anatomy of the physiological apical foramen is limited. Knowing its diameter and shapes contributes to clinical work, specifically to the cleaning and shaping of the apical third. The aim of this ex vivo study was to determine the minimum and maximum diameters and shape of the physiological apical foramen in the roots of maxillary and mandibular first molars. A descriptive study was conducted on 89 recently extracted first molars. Roots 3-5 mm from the apex were sectioned and prepared for analysis at 40x magnification. The minimum and maximum diameters of each physiological foramen were measured using the program Motic Images plus 2.0 ML. The shape of the foramina, classified as round, oval or irregular, was determined by the difference between the maximum and minimum diameters. A total of 174 physiological foramina were analyzed. The average of the minimum and maximum diameters was between 0.24-0.33 mm in maxillary first molars and between 0.25-0.33 mm in mandibular first molars. In maxillary molars, the most common shape of the foramen was oval (50%), then irregular (32%), then round (18%). In mandibular molars, the oval shape was also the most frequent (59%), followed by irregular (23%) and round (18%). The findings of this study regarding the morphology of physiological apical foramina in first molars make it easier for the operator to choose the appropriately-sized instruments to perform endodontic therapy successfully.


La información relacionada a la anatomía del foramen fisiológico apical es limitada. Conocer su diámetro y forma contribuye al trabajo clínico, específicamente en los procedimientos de limpieza y conformación del tercio apical. El objetivo de este estudio ex vivo fue determinar los diámetros menor, mayor y la forma del foramen fisiológico apical en las raíces de primeros molares maxilares y mandibulares. Se realizó un estudio descriptivo sobre 89 primeros molares recientemente extraídos. Se seccionaron las raíces a 3­5 mm del ápice y fueron preparadas para su análisis bajo magnificación de 40X. Se midieron los diámetros mayor y menor de cada foramen fisiológico mediante el programa Motic Images plus 2.0 ML. La forma de los forámenes fue determinada de acuerdo a la diferencia entre diámetro mayor y menor, clasificándose como redondo, oval o irregular. Un total de 174 forámenes fisiológicos fueron analizados. El promedio del diámetro menor y mayor fue entre 0,24­0,33 mm en primeros molares maxilares y entre 0,25­0,33 mm en primeros molares mandibulares. En molares maxilares, la forma del foramen más común fue la oval (50%), luego irregular (32%) y redonda (18%). En molares mandibulares, la forma oval también fue la más frecuente (59%), seguida por la irregular (23%) y redonda (18%). Los hallazgos de este estudio en relación a morfología de los forámenes fisiológicos apicales en primeros molares, permite al operador facilitar la elección de instrumentos del calibre adecuado para realizar con éxito la terapia endodóntica.
