Introduction: The aim was to conduct a comprehensive scoping review of available researches on the impact of blepharoplasty on quality of life and its scope within the Unified Health System. Methods: A literature search was performed in the Virtual Health Library, Capes Journals, SciELO, LILACS, Medline, and Cochrane Library databases for studies published within the past five years. Results: Out of the 323 studies initially selected, nineteen studies were included in the final sample. Seven studies (36.8%) examined the impact of blepharoplasty on quality of life, while others assessed functional criteria (31.5%) or subjective factors related to quality of life (26.3%). The majority of the studies demonstrated positive effects of blepharoplasty on quality of life, self-esteem, personal satisfaction, well-being, and visual improvement. However, limited data were found regarding the coverage of blepharoplasty in the public healthcare system, with no specific studies on this topic. Conclusion: Blepharoplasty has been shown to positively impact quality of life, highlighting the need to ensure the access to this procedure within the public healthcare when appropriate. The lack of data on blepharoplasty within the Unified Health System indicates a research gap and emphasizes the importance of further studies to explore its impact on quality of life, as well as its coverage and expansion.

RESUMO

ABSTRACT Purpose: This study aimed to investigate the effect of upper eyelid blepharoplasty with the removal of the skin and a strip of orbicularis oculi muscle on the ocular surface, tear film, and dry eye-related symptoms. Methods: Twenty-two eyes of 22 consecutive patients operated by a single surgeon (21 females; mean age, 61 years; age range, 41-75 years) were included. All subjects completed the Ocular Surface Disease Index questionnaire, underwent in vivo confocal microscopy, tear film breakup time measurements, the Schirmer test with anesthesia, and fluorescein and lissamine green staining measurements before, 1 month, and 6 months after upper blepharoplasty alone with preseptal orbicularis excision. Results: A significant increase in Ocular Surface Disease Index, and corneal fluorescein and lissamine green staining and a significant decrease in tear film breakup time were observed after 1 month (p=0.003, p=0.004, p=0.029, and p=0.024 respectively) and 6 months (p=0.001 for all findings). No significant difference in the Schirmer test score was observed during the follow-up. None of the in vivo confocal microscopy parameters showed significant changes during the study. Conclusions: An increase in dry eye symptoms and a decrease in tear film stability along with ocular surface staining were observed in patients undergoing upper eyelid blepharoplasty.

RESUMO

BACKGROUND: Eyebrow tail ptosis is usually associated with dermatochalasis of the upper eyelid, and it is necessary to treat them together. For these associated procedures to be incorporated as a routine in upper eyelid surgeries, they should preferably be less invasive with consequent fewer complications. OBJECTIVE: We describe a minimally invasive technique for the correction of mild-to-moderate eyebrow tail ptosis corrected together with superior dermatochalasis through the blepharoplasty incision. METHODS: A phase 2 clinical trial was conducted from February 2020 to December 2021. A total of 50 patients underwent conventional upper blepharoplasty surgery associated with the proposed eyebrow lift technique: internal pexia in the periosteum, dissection, and posterior fixation of the orbicularis muscle to the arcus marginalis, removal of the lateral part of the orbicularis muscle, and loosening of the orbital retention ligament. Evaluation of the height of the eyebrow tail was performed with digital photography in the initial consultation and at 30-90 days postoperatively and analyzed with ImageJ software. RESULTS: Mean difference in brow height at 1-month and 3-month postoperative evaluations compared to the preoperative period was 3.45-3.33 mm, respectively. CONCLUSION: Our study demonstrated a minimally invasive surgical technique for the treatment of mild-to-moderate eyebrow ptosis with significant eyebrow tail lift results that remained stable during the study period.

Lower eyelid deformities are frequently associated with facial aging. Plastic surgery of the eyelids became popular in the second half of the 20th century. Furnas identified the redundancy of the inferior orbicularis muscle, focused on resection of excess flaccid tissue to restore a more youthful appearance. The case of a 40-years-old female patient is presented, who attended the Plastic Surgery consultation requesting correction of her upper and lower eyelids, and with malar bags nonconformity. The defects were corrected with an inverted inferior orbicularis muscle flap, which was fixed to the fascia of the temporalis muscle through a transcutaneous blepharoplasty. The aesthetic results achieved in this patient show that transcutaneous blepharoplasty and the use of an inverted flap of the orbicularis oculi are effective alternatives to achieve eyelid rejuvenation and correction of the malar bags, by conferring a greater harmony of these structures in relation to face.

Introduction: Eyelid xanthelasma is the most common form of cutaneous xanthoma, characterized by yellowish patches on the eyelid's skin. Despite being a benign condition and not presenting with functional limitations, it is an important aesthetic complaint that impacts the patient's social and emotional life. There are clinical therapeutic options, but the most widespread approach is the surgical approach with excision of the lesions, a simple procedure with few complications and lower local recurrence rates. This study aims to describe the surgical treatment of palpebral xanthelasma, to assess postoperative patient satisfaction and post-surgical recurrence rates. Methods: This is a retrospective study with a sample of 25 patients undergoing surgical treatment of eyelid xanthelasmas. Postoperative follow-up was performed at intervals of 7 days, 30 days, 90 days and 12 months with an interview, physical examination and application of a questionnaire that included the identification of local recurrences, postoperative complications and satisfaction with the aesthetic result. Results: Four patients evolved with local recurrence, and only two expressed dissatisfaction with the aesthetic result after the outcome. No patient who underwent surgical resection of lesions associated with autograft recurrence or dissatisfaction with the aesthetic result was observed. Conclusions: Surgical treatment as the first option in the therapeutic approach of eyelid xanthelasmas should be considered, given the aesthetic and psychological impact of such a condition. It is a simple technique, easy to apply and reproducible, effective, and safe, with relevant satisfaction rates and low recurrences.
