RESUMO

Introduction: Psychosocial stress during pregnancy has long-lasting and important consequences in the following generations, as it can affect intrauterine development. The impact on the developing immune system is notoriously important due to the associated morbidity and mortality in the first years of life. Little attention has been given to the role of violence during pregnancy (VDP), especially its impact on infant infectious morbidity. Methods: We analyzed data from two Brazilian birth cohorts (n = 2,847) in two distinct cities (Ribeirão Preto and São Luís), collected during pregnancy and at the beginning of the second year of life. The association between VDP and infection in infancy was analyzed with structural equation modeling, using the WHO-VAW questionnaire as exposure and a latent variable for infection as the outcome. Results: VDP was reported by 2.48% (sexual), 11.56% (physical), and 45.90% (psychological) of the mothers. The models presented an adequate fit. In the city of São Luís, VDP was significantly associated with the latent construct for infection (standardized beta = 0.182; p = 0.022), while that was not the case for the Ribeirão Preto sample (standardized beta = 0.113; p = 0.113). Further analyses showed a gradient effect for the different dimensions of the exposure, from psychological to physical and sexual violence. Conclusion: Our results suggest an association of VDP with infant morbidity in a poorer socioeconomic setting, and highlight the importance of considering the different dimensions of intimate partner violence. These findings may have important implications for the comprehension of global health inequalities and of the effects of gender-based violence.

RESUMO

Objective: Intimate partner violence (IPV) is a major public health problem in Latin America. The present study investigates the protective factors that contribute to minimizing the risk of exposure to IPV analyzing different variables in a sample of Chilean women victims of IPV. Methods: We used data from the Cicatrices Foundation, a nonprofit Chilean organization providing psychological support to IPV victims. Relevant variables for IPV prevention were identified analyzing a database containing all the information reported by victims during a structured interview. A final sample of 444 women suffering IPV was used in the present study. Results: Logistic regression analysis was calculated in order to make predictions related to IPV protective factors, showing that having a support network (OR = 2.85), treatment compliance (OR = 2.05) and being younger (OR = 0.95) increased the probability of not living with the aggressor. Another logistic regression analysis was calculated in order to predict IPV victims´ health taking medication intake as an indicator. A significant association was observed between this variable and working outside (p = 0.002) and between mediation intake and age (p < 0.001), with an OR of 1.987 and 0.93, respectively. Working outside and being younger were identified as protective factors against consuming medication. Conclusion: To the best of our knowledge, this is one of the first studies conducted in Chile on the prevention of IPV in a sample of victims seeking for help. Our results will contribute to guide policy makers, researchers and other women in the prevention of potential risks for IPV.

Abstract Introduction: Violence against the elderly is a growing phenomenon, causing damage to health, with different outcomes and consequences to the victims. The possibility of elderly women suffering it in the family context surpasses that of men, with gender being a considerable risk factor. Objective: To analyze the understanding of violence against the elderly according to elderly women. Method: Descriptive research with a qualitative approach developed with 22 elderly women from a community in the state of Paraíba, Brazil, chosen for convenience. The data collection was based on semi-structured interviews, processed by the Iramuteq software, with subsequent Content Analysis. Results: Five classes of violence against the elderly were evidenced: cycle of violence; support network for the elderly victims of violence; experience of violent situations; financial violence; and symbolism of violence in society, which denote an understanding of violence involving the different types. They are based on the factors of family experience, culture, and others, placing the health professional as a fundamental element for care and support. Gender influenced the perspective on physical and psychological violence, as well as the relevance given to health teams for the identification of occurrences and the prevention of possible damage. Conclusion: The various types of violence against the elderly have been recognized, including individual, community, and social factors in the violent cycle. In addition, they associated aging with greater susceptibility to suffering violence, regardless of the typology. It highlights the potential of the health service in assisting the elderly victim of violence, elucidating cases, and acting early to interrupt the cycles perpetrated, requiring the constant need for professional updating to deal with detected situations.

RESUMO

The purpose of this research is to identify predominant problematizations in Colombian feminist Instagram accounts regarding gender-based violence during the first year of the COVID-19 pandemic. Employing a qualitative approach, a digital ethnography based on the principles of netnography was conducted as an alternative to detail the social worlds constructed from online groups. Fifty pieces of content from @lainsumisa and twenty pieces of content from @feministasenconstrucción, published between March 2020 and March 2021, were selected based on observations made within the online fieldwork framework. These were analyzed using discourse analysis techniques. The findings discuss the following emerging categories: unpaid household work, romanticized harassment, fatphobia, and the violation of Black and racialized women's rights. In this context, cyberactivism is presented as an opportunity for the emergence of collectives and support networks for women advocating for gender equality and their rights, towards questioning patriarchal ideas that jeopardize their well-being.

INTRODUCTION: In recent decades, there has been an increasing need to rethink healthcare models, as well as the creation of effective public policies for significant portions of the Brazilian population in conditions of greater vulnerability, thus aiming to improve the supply of products and services that promote improvements in quality of life, especially in the area of health. This demand is reinforced mainly when crossed by the structural racism that surrounds access to health care for black women. OBJECTIVE: In this way, the present study intended to understand the psychological experience of pregnant women in relation to the care they received during prenatal care, as well as to understand possible factors that generate psychological distress. METHOD: To this end, qualitative research was carried out using a questionnaire with semi-open questions and a socio-demographic questionnaire. The sample consisted of six women. FINAL CONSIDERATIONS: The data were understood in light of psychoanalytic theory associated with theorists who address cross-cutting health issues.

RESUMO

The COVID-19 pandemic brought on a marked increase in intimate partner violence (IPV) worldwide, Mexico being no exception. Factors that exacerbated gender-based violence (GBV) in the household during the pandemic include gendered loss of income, regression in access to social and legal justice resources, reversal to more traditional gender norms and roles, and increased alcoholism. While there are studies about the prevalence and determinants of IPV in rural and urban Mexico, there appears to be a lack of information regarding how these realities differed as they interacted with the compounding pressures of the COVID-19 pandemic. Stemming from 10 ethnographic interviews with women across rural and urban localities of Oaxaca, Mexico City, and Mexico State, who were recruited from NGOs providing psychological and legal services against GBV, we analyze some factors associated with the prevalence of IPV during confinement. We conclude that all women in our study experienced IPV both before and during the pandemic, with variations in IPV patterns influenced by their rural or urban residence, socio-economic status, ethnic-racial identity, and proximity to the abuser's network. We also found that not all impacts were negative, rather COVID-19 measures had a paradoxical effect for some women where restrictions on geographical mobility and decrease in access to alcohol became pivotal protective factors. We recommend that public policymakers and civil society organizations alike pay attention to these differential challenges and benefits in their crisis responses.

RESUMO

Violence Against Women is a global problem that affects millions of women around the world. The main objective of this study was to evaluate the Mental Health and Quality of Life of Ecuadorian women exposed or not exposed to Gender Violence mediated by sociodemographic factors. A total of 433 Ecuadorian women, 18 to 64 years of age, most of whom (69%) reported being exposed to gender violence. Most of the women lived in the province of Pichincha and were selected through a type of nonprobabilistic sampling based on a survey about gender violence. Women exposed to gender violence showed a greater impact on personality profiles, clinical tests, and quality of life in the physical, psychological, social, and environmental domains compared to women not exposed to violence. In addition, these effects were mediated by age, economic level, education, marital status, and exposure to physical violence and psychological or sexual abuse within the socio-family or work contexts. The results of this study could contribute to improving public health systems, showing improvements in care programs for victims of violence.

RESUMO

BACKGROUND: Transgender women are disproportionately affected by both HIV and gender-based violence (GBV), defined as physical, sexual, or emotional violence perpetrated against an individual based on their gender identity/expression. While a growing body of evidence demonstrates that GBV leads to poor HIV care and treatment outcomes among cisgender women, less research has examined this association among transgender women. We assessed the impact of lifetime experiences of GBV on subsequent retention in HIV care and laboratory confirmed viral suppression among a sample of transgender women living with HIV (TWH) in Brazil. METHODS: A pilot trial of a peer navigation intervention to improve HIV care and treatment among TWH was conducted in São Paulo, Brazil between 2018 and 2019. TWH were recruited and randomized into the intervention or control arm and participated in a baseline and 9-month follow-up survey and ongoing extraction of clinical visit, prescribing, and laboratory data. Generalized linear model regressions with a Poisson distribution estimated the relative risk (RR) for the association of lifetime physical and sexual violence reported at baseline with treatment outcomes (retention in HIV care and viral suppression) at follow-up, adjusting for baseline sociodemographic characteristics. RESULTS: A total of 113 TWH participated in the study. At baseline, median age was 30 years, and the prevalence of lifetime physical and sexual violence was 62% and 45%, respectively. At follow-up, 58% (n = 66/113) were retained in care and 35% (n = 40/113) had evidence of viral suppression. In adjusted models, lifetime physical violence was non-significantly associated with a 10% reduction in retention in care (aRR: 0.90, 95% CI: 0.67, 1.22) and a 31% reduction in viral suppression (aRR: 0.69; 95% CI: 0.43, 1.11). Lifetime sexual violence was non-significantly associated with a 28% reduction in retention in HIV care (aRR: 0.72, 95% CI: 0.52, 1.00) and significantly associated with a 56% reduction in viral suppression (aRR: 0.44; 95% CI: 0.24, 0.79). CONCLUSION: Our findings are among the first to demonstrate that lifetime experiences with physical and sexual violence are associated with poor HIV outcomes over time among transgender women. Interventions seeking to improve HIV treatment outcomes should assess and address experiences of GBV among this population. TRIAL REGISTRATION: ClinicalTrials.gov Identifier: NCT03525340.

RESUMO

Background: Intimate Partner Violence (IPV) carries significant global burden, with approximately 27% of women who have ever had a partner experiencing IPV. Additionally, substance use (alcohol and drugs) is often associated with aggressive attitudes and serves as a risk factor for IPV. Objective: Determine the association between substance use and the recurrence of IPV in the Peruvian population in 2022. Methods: A cross-sectional study was conducted using public data from the Ministry of Women, employing regression based on generalized linear models to calculate crude and adjusted Odds Ratios. Results: A total of 65,290 cases of IPV were analyzed, the results revealed that 93.70% of the reports were cases of recidivism. A relationship was identified between substance use and IPV, with an Odds Ratio of 2.24 for the perpetrator's alcohol consumption and an Odds Ratio of 2.33 for drug use. Conclusion: Based on these findings, it can be concluded that a relationship exists between substance use and IPV, and national strategies should incorporate proper monitoring after the initial report of violence, as well as effective control of substance use among perpetrators.

Feminicide results from the sexist patriarchal heritage and social structures historically permeated by unequal gender relations. This ecological study analyzes the epidemiological aspects of feminicides recorded in Pernambuco between 2016 and 2019. Records of homicides and undetermined causes of death of women available in the Mortality Information System were the proxy for feminicide events and accepted as reference. Search steps were established with probabilistic linkage in health databases, as well as manual reviews on the Pernambuco Court of Justice (TJPE) platform and online news media as a complement. Data were described by the distribution of frequencies, measures of central tendency and dispersion. Local empirical Bayesian method and Moran's autocorrelation index were used in the spatial pattern analysis of femicide rates. Hierarchical logistic regression was applied to verify the association between femicide and interpersonal violence and to estimate the Odds Ratio with 95% confidence intervals (95%CI). Search returned a total of 490 feminicides, corresponding to a rate of 2.5 per 100,000 women. The highest proportion of feminicides was identified by manual search in online news media and in the TJPE database (n = 247; 50.41%). Most victims were aged between 20 and 39 years (n = 286; 58.37%), black (n = 400; 81.63%), had no partner (n = 407; 83.06%) and with over eight years of schooling (n = 303; 61.84%). Spatial autocorrelation of the event was confirmed by the Global Moran Index of smoothed rates (I = 0.3; p = 0.001). Local autocorrelation reinforces the interiorization of the phenomenon by pointing to a critical area (Q1) formed by municipalities located in the Vale do São Francisco and Araripe macroregion. At the distal level, the following variables increase the chances of feminicide: living in a small city (OR = 2.10); unavailability of specialized police stations (OR = 1.11); and absence of referrals to the care and protection network in the event of a previous aggression (OR = 1.32). Regarding intermediate determinants, the greater the violence inflicted the greater the chance of a fatal outcome, emphasizing the use of sharp objects (OR = 3.93) and firearms (OR = 11.14). Regarding proximal determinants, victims in the age group 10-19 years old were less likely to be a victim of femicide (OR = 0.51). Characterizing the study population can further knowledge about structural violence against women, and aspects of the notification and the care and protection network. Spatial analysis pointed to the interiorization of the phenomenon and transition areas in Pernambuco, requiring prioritization of interventions. The modeling showed that women are exposed to multiple risk factors for feminicide and further discussions at all levels of determination are necessary.
