Introduction The measurement of extended high-frequency (EHF) audiometry has become more popular recently, mainly in connection with ototoxicity and noiseinduced hearing loss. New-onset tinnitus evaluation includes a standard hearing test that shows no pathology. Objective The aim of the present study was to evaluate the possibility that acute tinnitus is essentially connected to sudden sensory neural hearing loss (SSNHL), by utilizing EHF audiometry in cases in which standard audiometry for frequencies between 250 Hz to 8 kHz is within normal limits. Methods A retrospective study was conducted between January 2009 and May 2014 that included all patients presenting with acute tinnitus and normal standard audiometry. All patients underwent EHF audiometry and were treated accordingly. Results Thirty-two patients with acute tinnitus and asymmetric sensorineural hearing loss on EHF audiometry were identified. The average deltas between the ears were between 9.2 and 33dB (worse in the affected ear). Conclusion Extended high-frequency audiometry up to 20,000 Hz should be performed in all patients with acute tinnitus and standard audiometry within normal limits.

Introducción: La audiometría de altas frecuencias (9000-20 000 Hz) puede ser de gran utilidad en el diagnóstico precoz de hipoacusia inducida por ruidos en contraste con la audiometría convencional (125-8000 Hz). Objetivo: Evaluar la utilidad de la audiometría de altas frecuencias en el diagnóstico precoz de la hipoacusia inducida por ruidos en adolescentes. Métodos: Estudio descriptivo, observacional y transversal en 85 adolescentes expuestos a ruidos; 45 cursaban el décimo grado en un preuniversitario de la provincia La Habana, en el curso escolar 2013-2014 y 40, el primer año de la carrera de medicina, en una facultad de La Habana en el curso escolar 2015- 2016. A todos se le realizó una encuesta, examen físico de otorrinolaringología, audiometría convencional y audiometría de alta frecuencia. Resultados: La audiometría convencional fue normal en todos los casos. En la audiometría de altas frecuencias en todos los adolescentes estudiados comenzó a disminuir la audición a partir de los 13 000 Hz y se apreció una hipoacusia neurosensorial en aumento hasta 85 decibeles en oído derecho y 78 decibeles en oído izquierdo en 20 000 Hz de frecuencia. Conclusiones: Los adolescentes evaluados tienen hábitos auditivos que pueden perjudicar su audición, todos ya poseen signos de daño que no se manifiestan a través de la audiometría convencional pero sí a través de la audiometría de alta frecuencia en las diferentes exposiciones a ruidos de altos decibeles. Lo anterior demuestra la utilidad de esta audiometría, no solo dentro del campo de la audiología, sino también en la medicina preventiva(AU)


Introduction: High frequency audiometry (9000-20 000 Hz) can be really useful in the early diagnosis of deafness induced by noises, in contrast with conventional tonal audiometry (125-8000 Hz). Objective: Assess the usefulness of high frequency audiometry in the early diagnosis of deafness induced by noises in adolescents. Methods: Descriptive, observational and cross-sectional study in 85 adolescents exposed to noises; 45 of them were studying the 10th grade in a high school of Havana province, and 40 of them were in the first year of Medicine in a faculty of Havana province during school year 2015-2016. All of them took a survey, a physical examination of otolaryngology, liminar total audiometry and high frequency audiometry. Results: Tonal audiometry was normal in all the cases. In high frequency audiometry of all the studied adolescents started to decrease audition from 13000 HZ and it was noticed an increasing neurosensorial deafness up to 85 decibels in the right ear and 78 decibels in the left ear in 20 000 Hz of frequency. Conclusions: The adolescents assessed have auditive habits that can jeopardize their audition; all of them already have signs of damage that are not noticed in the tonal audiometry but in the high frequency audiometry in the different exposures to noises of high decibels. This proves the usefulness of this kind of audiometry not only in the field of audiology, but also in preventive medicine(AU)

OBJECTIVE: Worldwide research trends on the prevalence of hearing loss within young rural populations are scarce. Current evidence suggests extended high-frequency audiometry could be a sensitive predictor to detect subclinical hearing loss. This study aimed to determine the prevalence of hearing loss through conventional pure tone (0.25-8 kHz) and extended high-frequency (9-20 kHz) audiometry tests in a group of adolescents aged 13 to 19 years old from two rural areas of Colombia. A sociodemographic and associated factor for hearing loss questionnaire was applied. DESIGN: Cross-sectional study. STUDY SAMPLE: Three hundred sixty-six adolescents aged 13 to 19 years old from two rural municipalities in Colombia: Soracá, and Palmas del Socorro. RESULTS: Hearing loss prevalence assessed through pure tone audiometry was 3.28% in right ears and 3.83% in left ears while through extended high-frequency audiometry was 14.21% and 13.11%, respectively. Associated factors found by the logistic regression were age (OR: 1.36; CI 95% 1.13-1.64) and obesity (OR: 3.74; CI 95% 1.07-13.11). CONCLUSIONS: Using 15 dB as the normal hearing threshold parameter for audiometric frequencies, our results suggest that extended high-frequency hearing loss is already widespread among rural adolescents. This highlights the need for research on extended high-frequency audiometry in rural school-aged children.

INTRODUCCIÓN: Recientes investigaciones mencionan que, debido a los altos niveles de ruidos, el 75% de los habitantes en las ciudades industrializadas padecen algún tipo de deficiencia auditiva. La audiometría de alta frecuencia es un examen complementario importante para detectar tempranamente la pérdida de audición. OBJETIVO: Determinar la utilidad diagnóstica de la audiometría de alta frecuencia en la detección temprana de la hipoacusia en sujetos expuestos a ruido recreacional. MATERIAL Y MÉTODO: Estudio analítico de corte transversal. Se evaluaron 87 estudiantes de fonoaudiología. Se aplicó una audiometría convencional y de alta frecuencia mediante audiómetro clínico. RESULTADOS: La audiometría de alta frecuencia presenta una sensibilidad del 100%, especificidad del 64,60%. El valor predictivo (+) es de 14,70%, mientras que el valor predictivo (-) es del 100%. La razón de verosimilitud (+) es de 2,28, y para la razón de verosimilitud (-) es de 0,0. CONCLUSIÓN: La audiometría de alta frecuencia puede ser utilizada para monitorizar la audición de los sujetos, comprobando que efectivamente los umbrales auditivos de alta frecuencia se encuentren dentro de rangos normales. Lo anterior, dado por sus valores de sensibilidad, valor predictivo negativo, razón de verosimilitud negativa y por el aumento entre la probabilidad preprueba y posprueba.


INTRODUCTION: Recent research mentions that, due to the high noise levels, 75% of the habitants in industrialized cities suffer from some type of hearing impairment. High frequency audiometry is an important complementary test to detect early hearing loss. AIM: To determine the diagnostic utility of high frequency audiometry in the early detection of hearing loss in subjects exposed to recreational noise. MATERIAL AND METHODS: Study analytical type and cross section. 87 speech therapy students were evaluated. A conventional and high frequency audiometry was applied, using a clinical audiometer. RESULTS: High frequency audiometry has a sensitivity of 100%, specificity of 64,60%. The predictive value (+) is 14,70%, while the predictive value (-) is 100%. The likelihood ratio (+) is 2,28, and for the likelihood ratio (-) it is 0,0. CONCLUSION: High frequency audiometry can be used to monitor the hearing of the subjects, checking that effectively the high frequency hearing thresholds are within normal ranges. The above, given by their values of sensitivity, negative predictive value, negative likelihood ratio and the increase between the pre-test probability and the posttest probability.

The aim of this study is to evaluate the hearing behavior of rheumatoid arthritis (RA) and primary Sjögren syndrome (PSS) patients and compare them with a healthy control group and with each other. A comparative cross-sectional study was performed with a group of 117 female RA patients, a group of 60 female PSS patients, and a 251 female healthy control group. Every subject underwent a series of studies including high-frequency audiometry, speech audiometry, and tympanometry. The high-frequency audiometry measured 250 to 16,000 Hz. The 117 patients with RA and the 60 with PSS were diagnosed according to American College of Rheumatology criteria / ACR 2010, and the validated classification of the American-European Consensus Group. Hearing loss was present in 36.8% of the RA group in 500-3000 Hz, 68.4% in 4000-8000 Hz, and 94.9% in 10,000-16,000 Hz. Hearing loss was present in 60% of the PSS group in 500-3000 Hz, 70% in 4000-8000 Hz, and 100% in 10,000-16,000 Hz. The hearing impairment prevalence of both groups was significantly different (p < 0.05) when compared with the healthy control group. We also compared the hearing thresholds between RA and PSS patients, finding a significant hearing threshold increase in 500-3000 Hz of the PSS group. This study consolidates the association between RA and PSS with hearing impairment. A deeper hearing loss was reported in PSS than in RA patients, demonstrating a greater auditory and speech recognition repercussion.

The aim of the study is to evaluate hearing function by extended high-frequency audiometry in primary Sjögren Syndrome (pSS) and to correlate it with disease activity. All patients diagnosed with pSS were recruited within 1 year. Patients underwent otologic evaluation, extended high-frequency audiometry, tympanogram, and the ESSPRI scale evaluation. A total of 63 (60 female and 3 male) patients with mean age 49.5 ± 9.9 years with pSS were included. We detected a hearing loss prevalence of 95.2% (60 patients) in both ears in very high frequencies (10000-16,000 Hz). The hearing loss of all patients was sensorineural. There was no significant difference between the degree of disease activity and the hearing thresholds. We enrolled 188 subjects with a mean age of 47.20 ± 9.7 years as a healthy control group. We compared the hearing thresholds of both ears of patients with pSS versus the healthy control group and observed a significant difference in frequencies above 500 Hz. Sensorineural hearing loss was found in pSS patients in frequencies above 4000 Hz. Sensorineural hearing loss was highly prevalent in pSS patients, predominantly in very high frequencies. We claim that sensorineural hearing loss could be an associated symptom of primary Sjögren syndrome and suggest further research to support this conclusion.

BACKGROUND: The rheumatoid arthritis is a clinical entity capable to cause hearing impairment that can be diagnosed promptly with high frequencies audiometry. OBJECTIVE: To detect subclinical sensorineural hearing loss in patients with rheumatoid arthritis. MATERIAL AND METHODS: Cross-sectional study on patients with rheumatoid arthritis performing high frequency audiometry 125Hz to 16,000Hz and tympanometry. The results were correlated with markers of disease activity and response to therapy. RESULTS: High frequency audiometry was performed in 117 female patients aged from 19 to 65 years. Sensorineural hearing loss was observed at a sensitivity of pure tones from 125 to 8,000 Hz in 43.59%, a tone threshold of 10,000 to 16,000Hz in 94.02% patients in the right ear and in 95.73% in the left ear. Hearing was normal in 8 (6.84%) patients. Hearing loss was observed in 109 (93.16%), and was asymmetric in 36 (30.77%), symmetric in 73 (62.37%), bilateral in 107 (91.45%), unilateral in 2 (1.71%), and no conduction and/or mixed hearing loss was encountered. Eight (6.83%) patients presented vertigo, 24 (20.51%) tinnitus. Tympanogram type A presented in 88.90% in the right ear and 91.46% in the left ear, with 5.98 to 10.25% type As. Stapedius reflex was present in 75.3 to 85.2%. Speech discrimination in the left ear was significantly different (p = 0.02)in the group older than 50 years. No association was found regarding markers of disease activity, but there was an association with the onset of rheumatoid arthritis disease. CONCLUSIONS: Patients with rheumatoid arthritis had a high prevalence of sensorineural hearing loss for high and very high frequencies.

A audiometria de alta freqüência é capaz de detectar precocemente alterações em sensibilidade advindas de processos como o envelhecimento. Seu uso é limitado, o que recomenda estudos para esclarecer seu desempenho, especialmente entre adultos de mais idade. OBJETIVO: Comparar os limiares para as freqüências de 250Hz a 16kHz, entre adultos jovens e mais velhos normoacúsicos, com e sem queixa audiológica. CASUíSTICA E MÉTODO: A sensibilidade a tons puros de 250Hz a 16kHz foi avaliada com audiômetro AC-40, em 64 adultos, igualmente distribuídos: jovens (25 a 35 anos) e mais velhos (45 a 55 anos) de ambos os gêneros, com forma de estudo de coorte transversal. RESULTADOS: Os adultos mais velhos apresentaram limiares mais elevados em todas as freqüências, mais significativamente nas mais altas (8 a 16kHz), quando comparados com os adultos jovens. Homens apresentaram limiares mais elevados do que mulheres entre 3 e 10kHz. CONCLUSÃO: O processo de envelhecimento auditivo, envolvendo perda de sensibilidade auditiva para altas freqüências, pode ser detectado em idades anteriores às tipicamente pesquisadas, uma vez que a audiometria de alta freqüência demonstrou ser instrumento importante para distinguir a sensibilidade auditiva entre adultos jovens e mais velhos, quando audiologicamente normais.


High-frequency audiometry can detect early changes in auditory sensitivity resulting from processes such as aging. Nonetheless its use is still limited, and additional studies are required to establish its use, particularly among older adults. AIM: To compare pure tone thresholds for frequencies from 250 Hz to 16 kHz in young and older adults, with or without audiologic complaints. METHOD: Pure tone sensitivity to 250 Hz to 16 kHz was assessed with an AC-40 audiometer in 64 adults, evenly distributed in young (25 to 35 years-old) and older (45 to 55 years-old) adults of both sexes. This is a cross-sectional study. RESULTS: Although all participants presented normal audiometry in frequencies from 250 Hz to 8 kHz, according to clinical parameters, older adults had significantly higher thresholds compared to young adults, according to statistical parameters, with greater significance in higher frequencies (8 to 16 kHz). Presence or absence of clinical complaints did not distinguish thresholds. CONCLUSIONS: The process of auditory aging, including loss of sensitivity to higher frequencies, can be detected at earlier ages than those usually investigated. High frequency audiometry is an important instrument to distinguish auditory sensitivity in young and older adults, even for those considered as audiologically normal.
