Objetivo: Avaliar o impacto da implantação do inventário rotativo diário na acurácia do estoque de medicamentos em um hospital público de médio porte. Metodologia: Trata-se de um estudo descritivo exploratório transversal que avaliou a implantação de um inventário rotativo de janeiro a julho de 2022. Foram selecionados 143 medicamentos que posteriormente foram inventariados, uma vez por mês, ao longo de sete meses. Esses medicamentos foram classificados segundo a curva ABC, XYZ e o grau de divergência em: baixo, médio e alto. Foi calculada a acurácia para cada grupo, ao longo dos meses analisados, e o Percentual de Mudança Relativa (PMR) para comparação entre o período inicial e final. Resultado: Houveum aumento de 64% na acurácia do estoque de medicamentos, sendo observada diferença entre os grupos ABC e XYZ. Em relação a curva ABC, o grupo A teve um aumento de 170%, o B de 358% e o C de 23% no mesmo período. Em relação a criticidade, o grupo Z foi o que teve maior acurácia, seguido dos grupos Y e X. Os medicamentos classificados como de alta taxa de divergência tiveram redução de 80% e o de baixa 25% após implantação do inventário. Conclusão: A implantação do inventário rotativo aumentou a acurácia do estoque de medicamentos ao longo dos meses avaliados. Essa ferramenta pode ser uma estratégia utilizada na melhoria do gerenciamento de estoque de medicamentos. (AU)


Objective: To evaluate the impact of implementing a daily rotating inventory on the accuracy of medicines stocks in a medium-sized public hospital. Methodology: This is a cross-sectional exploratory descriptive study that evaluated the implementation of a rotating inventory from January to July 2022. 143 medicines were selected and subsequently inventoried once a month over seven months. These medicines were classified according to the ABC, XYZ curve and the degree of divergence in low, medium and high. Accuracy was calculated for each group, over the analyzed months, and the Relative Percentage Change (RPC) for comparison between the initial and final period. Result: There was a 64% increase in the accuracy of medicines inventory, with a difference being observed between the ABC and XYZ groups. Regarding the ABC curve, group A had an increase of 170%, group B 358% and group C 23% in the same period. Regarding criticality, group Z had the highest accuracy, followed by groups Y and X. Medicines classified as having a high rate of divergence had a reduction of 80% and the low rate of 25% after implementation of the inventory. Conclusion: The implementation of the rotating inventory increased the accuracy of the medicines stock over the evaluated months. This tool can be a strategy used to improve medicine inventory management. (AU)

Construction and demolition waste (CDW) is an environmental problem that affects all regions of the world. Particularly in the Brazilian Amazon Forest region, the volume of CDW generated almost doubled between 2007 and 2019. Indeed, despite Brazil having environmental regulations for waste management, these have been insufficient to solve the environmental problem because there is no CDW reverse supply chain (RSC) properly developed in the Amazon region. Previous studies have proposed a conceptual model of a CDW RSC but have hitherto failed to apply them against real world practice. This paper, therefore, attempts to test existing conceptual models that describe a CDW RSC against real industry practice prior to developing an applied model of a CDW RSC for the Brazilian Amazon. To modify the conceptual model for CDW RSC, qualitative data through 15 semi-structured interviews with five different types of stakeholders of the Amazonian CDW RSC were collected and analyzed using qualitative content analysis methods using NVivo software. The proposed applied model includes present and future reverse logistics (RL) practices, and strategies and tasks necessary for the implementation of a CDW RSC in the city of Belém of Pará, in the Brazilian Amazon. Findings reveal that several overlooked problems, particularly the limitations of the existing legal framework in Brazil, are not enough to promote a robust CDW RSC. This is perhaps the first study to examine CDW RSC in the Amazonian rainforest. Arguments provided in this study highlight the necessity for an Amazonian CDW RSC that must be promoted and regulated by the government. This can be addressed by the utilizing public-private partnership (PPP) for developing a CDW RSC.

In January 2021, oxygen supplies in the Amazon region's largest city were allowed to run out at the peak of the second wave of the COVID-19 epidemic, shocking the world as hospital patients expired for lack of this basic medical resource in Manaus, which during the first COVID-19 wave had become the world's first city to bury its dead in mass graves. Brazil's authorities used this tragedy to further a political agenda that implies enormous environmental and human-rights consequences. Transport of oxygen was used to promote building a road that, together with its planned side roads, would give deforesters access to much of what remains of Brazil's Amazon Forest. Here, we demonstrate that the logistical strategy adopted by the Jair Bolsonaro administration's Ministries of Health and Infrastructure to bring oxygen to Manaus was the worst possible choice, and the foreseeable delay in the arrival of oxygen cost hundreds of lives. Rather than sending trucks to carry oxygen on the nearly impassible Highway BR-319 during the rainy season, the most appropriate transport option was barges on the Madeira River. As oxygen supplies dwindled in Manaus, the families of wealthier COVID-19 victims scrambled to buy the few remaining cylinders at prices out of reach for those in poorer (and often ethnically distinct) economic strata. Ethnic health disparities are aggravated by both the direct consequences of the oxygen crisis and, on the longer term, by the consequences of the highway project that political use of the crisis materially advanced.

Background: The coronavirus disease 2019 pandemic has prompted the exploration of new response strategies for such health contingencies in the near future. Over the last 15 years, several pharmacy-based immunization (PBI) strategies have emerged seeking to exploit the potential of pharmacies as immunization, medication sale, and rapid test centers. However, the participation of pharmacies during the last pandemic was very uneven from one country to another, suggesting a lack of consensus on the definition of their roles and gaps between the literature and practice. Purpose: This study aimed to consolidate the current state of the literature on PBI, document its progress over time, and identify the gaps not yet addressed. Moreover, this study seeks to (i) provide new researchers with an overview of the studies on PBI and (ii) to inform both public health and private organization managers on the range of possible immunization models and strategies. Methodology: A systematic review of scientific qualitative and quantitative studies on the most important scientific databases was conducted. The Preferred Reporting Items for Systematic Reviews and Meta-analyzes guidelines were followed. Finally, this study discusses the trends, challenges, and limitations on the existing literature on PBI. Findings: Must studies concluded that PBI is a beneficial strategy for the population, particularly in terms of accessibility and territorial equity. However, the effectiveness of PBI is affected by the economic, political, and/or social context of the region. The collaboration between the public (government and health departments) and private (various pharmacy chains) sectors contributes to PBI's success. Originality: Unlike previous literature reviews on PBI that compiled qualitative and statistical studies, this study reviewed studies proposing mathematical optimization methods to approach PBI.

The COVID-19 pandemic has changed lifestyles, with consequent impacts on urban freight movements. This paper analyzes the impacts of COVID-19 on urban deliveries in the Belo Horizonte Metropolitan Region, Brazil. The Lee index and the Local Indicator of Spatial Association were calculated using data on urban deliveries (retail and home deliveries) and COVID-19 cases. The results confirmed the negative impacts on retail deliveries and the positive impacts on home deliveries. The spatial analysis demonstrated that the most interconnected cities presented more similar patterns. At the beginning of the pandemic, consumers were considerably concerned about the virus spread, and the changes in consumption behavior were slow. The findings suggest the importance of alternative strategies to traditional retail. In addition, the local infrastructure should adapt to the increased demand for home deliveries during pandemics.

RESUMO

The logistics sector was one of the most impacted during the COVID-19 pandemic. This article aims to analyze the emerging trends on this subject using articles referring to studies related to the impact of the pandemic in engineering and related areas of knowledge, also looking for new research opportunities. A descriptive methodology was used through a systematic review of the literature. The publications in the "Engineering" area were filtered and the results were analyzed with the VOSviewer tool. Results show the relevance of the studies in the last mile. Likewise, opportunities were observed to carry out studies related to the transformation of retail.

Resumo O objetivo do estudo é identificar na literatura a relação entre a logística humanitária e a evolução da pandemia de COVID-19. Trata-se de uma revisão integrativa de literatura (RIL), com busca nas bases de dados SCOPUS, MEDLINE e ENEGEP, de abril e maio de 2022. Um total de 61 artigos foram avaliados conforme os critérios estabelecidos: artigo original ou de revisão de literatura publicado em revista científica; resumo e texto completo disponível; artigo com objeto de investigação sobre a logística humanitária em relação à pandemia de COVID-19. Foram selecionadas 11 publicações, organizadas e analisadas por meio de matriz de síntese. Das publicações, 72% foram em periódicos internacionais, e majoritariamente no ano de 2021 (56%). A presença da cadeia de suprimentos tem determinado a dinâmica de setores econômicos e sociais, estabelecendo, por meio de um olhar interdisciplinar, as operações humanitárias frente à pandemia de COVID-19. Constatou-se escassez de estudos que analisam a logística humanitária com o intuito de amenizar os impactos causados por esses desastres, tanto no contexto da atual pandemia como em eventos futuros da mesma natureza. Contudo, o caráter mundialmente emergencial sugere a necessidade de fortalecer o conhecimento científico acerca da temática da logística humanitária relacionada a eventos de desastres.


Abstract An integrative literature review (ILR) was conducted to identify the relationship between humanitarian logistics and the development of the COVID-19 pandemic based on research in the SCOPUS, MEDLINE and ENEGEP databases in April and May 2022. In all, 61 articles were evaluated according to the following criteria: original article or review of literature published in a scientific journal; abstract and full text available; article on humanitarian logistics in relation to the COVID-19 pandemic. The resulting sample comprised eleven publications organized and analyzed through a synthesis matrix, where 72% were published in international journals and mostly in 2021 (56%). The presence of the supply chain defines the course of action of economic and social sectors, which in turn determine, by means of an interdisciplinary approach, humanitarian operations in the face of the COVID-19 pandemic. The lack of studies narrows down humanitarian logistics to mitigate the impacts caused by these disasters, both in the context of the current pandemic and in future events of the same nature. However, as a global emergency, it suggests the need to increase scientific knowledge on the subject of humanitarian logistics related to disaster events.

The management of healthcare waste and end-of-life medication coming from different sources are primary challenges faced by public health leaders. Several factors may be considered critical and inhibitive to reverse logistics within the context of waste management processes. If those factors are not addressed, they may become obstacles to reverse logistics implementation. The purpose of this study is to evaluate the effect that critical factors play in the adoption of end-of-life management practices for medication and its influence on logistics performance. Literature provided some critical factors: management factor, collaboration factor, information technology factor, infrastructure factor, politics factor, financial and economic factor, end-of-life management practices, and logistics performance factor. A sample of 67 professionals from the public pharmaceutical care process answered a structured questionnaire. The collected data was analyzed using partial least square-structural equation modeling. The theoretical structural test confirmed eleven out of the fifteen hypotheses considered. The results have indicated that end-of-life management practices exert a direct influence on logistics performance. The analysis confirmed a direct effect of the information technology factor on end-of-life management practices, but not a moderation effect. Findings have contributed to the literature by providing deeper insights into the relationship between end-of-life management practices for medicines and logistics performance. Moreover, it supports health managers' decision-making in the pharmaceutical care process improvement and engagement with solid waste management policies.

Abstract The incorrect disposal of medicines and their environmental impact has been related to the health medicalization and the improper use of medication by society. In this sense, it is very important to know the profile of drug disposal for foster health policies. The aim was to identify the profile of disposal of medicines by the population, including the cost perspective. This is an inquiry descriptive study that began in September 2019. Medicine disposal health education program was carried out over six months in two University pharmacies. A questionnaire for sociodemographic and discarded medicines data collection was applied. Logistic regression analysis for variables association of correct disposal and the chi-square and t-student analysis for comparison between disposal programs were performed for a level of 5% and test power of 80%. Medicines weighed 23.3 kg and 28.5 kg, with the cost variation from US$ 13.5 to US$ 16.1 until the final treatment. The correct disposal was strongly associated with the disposal reason (p=0.013), source of information (p=0.006), prescription (p=0.03), form of use (p=0.01), acquisition source (p=0.001), cost with medication (p=0.0001), education (p=0.028) and age (p=0.05). The correct medicine disposal was associated with important features of the community related to education health.

A temática do descarte inadequado de medicamentos e sua relevância no desenvolvimento dos mecanismos de resistência aos antibióticos são os menos debatidos na literatura. Portanto, este trabalho tem como objetivo apresentar e discutir os dados obtidos sobre o recebimento de antimicrobianos vencidos e/ou sem uso descartados no ponto de coleta de medicamentos localizado no Departamento de Ciências Farmacêuticas, da Universidade Federal de Pernambuco, Campus Recife. Ao todo foram recolhidos 50,03 kg de medicamentos, dos quais 4,12 kg eram de antimicrobianos, sendo a terceira classe farmacológica mais frequente; destes, 93,07% (n=1.948 unidades) estavam vencidos e a forma farmacêutica mais encontrada foram os comprimidos (36,32%). Entre as classes de antibióticos predominaram, em relação ao mecanismo de ação, os que inibem a síntese proteica (29,33%), e pela estrutura química, as quinolonas (17,45%). A realização do descarte adequado desses medicamentos possibilita uma redução dos impactos que esse quantitativo recolhido causaria no meio ambiente. Portanto, deve-se investir em campanhas de conscientização sobre o uso correto dos medicamentos e seguimento da prescrição médica para evitar consumo indiscriminado destes fármacos. Com base o que estabelece o Decreto n° 10.388/2020, o descarte correto de medicamentos precisa ser divulgado, para que os estabelecimentos realizem a logística reversa dessas substâncias.


The issue of inappropriate disposal of medicines and its relevance in development of mechanisms of resistance to antibiotics is the least discussed in the literature. Therefore, this work aims to present and discuss the data obtained on the receipt of expired and/or unused antimicrobials discarded at the medication collection point located in the Department of Pharmaceutical Sciences, Federal University of Pernambuco, Campus Recife. In all, 50.03 kg of drugs were collected, of which 4.12 kg were antimicrobials, being the third most frequent pharmacological class; of these, 93.07% (n=1,948 units) were expired and the most common pharmaceutical form was pills (36.32%). Among the classes of antibiotics, in terms of mechanism of action, those that inhibit protein synthesis (29.33%) and chemical structure, quinolones (17.45%) predominated. Carrying out the proper disposal of these medicines makes it possible to reduce the impacts that this collected quantity would cause on the environment. Therefore, one should invest in awareness campaigns about the correct use of medicines and follow-up of medical prescriptions to avoid indiscriminate consumption of these drugs. Based on the provisions of Decree No. 10.388/2020, the correct disposal of medicines needs to be disclosed, so that establishments carry out the reverse logistics of these substances.


La cuestión de la eliminación inadecuada de los medicamentos y su relevancia en el desarrollo de mecanismos de resistencia a los antibióticos son los menos discutidos en la literatura. Por lo tanto, este trabajo tiene el objetivo de presentar y discutir los datos obtenidos sobre la recepción de antimicrobianos perdidos y/o no utilizados desechados en el punto de recolección de medicamentos ubicado en el Departamento de Ciencias Farmacéuticas de la Universidad Federal de Pernambuco, Campus Recife. En total, se tomaron 50,03 kg de medicamentos, de los cuales 4,12 kg fueron antimicrobianos, siendo la tercera clase farmacológica más común el 93,07% (n=1.948 unidades) retrasada y la forma farmacéutica más encontrada fueron los comprimidos (36,32%). Entre las clases de antibióticos predominaron, en relación con el mecanismo de acción, las que inhiben la síntesis de proteínas (29,33%), y por la estructura química, las quinolonas (17,45%). La eliminación adecuada de estos medicamentos permite reducir el impacto que esta cantidad recolectada podría causar en el medio ambiente. Por lo tanto, se deberían invertir en campañas de sensibilización sobre el uso correcto de medicamentos y el seguimiento de las recetas médicas para evitar el consumo indiscriminado de estos medicamentos. Sobre la base del Decreto no 10.388/2020, es necesario hacer pública la eliminación correcta de los medicamentos, para que los establecimientos realicen la logística inversa de estas sustancias.
