RESUMO

Background: Perineal hernia is a serious disease characterized by weakening or atrophy and separation of the muscles andfasciae of the perineal musculature, followed by the caudal displacement of pelvic and abdominal organs to the perineumregion. Treatment is invariably surgical and several approaches have been proposed, but complication and recurrence ratesremain high. This study aimed to evaluate 120 cases of perineal hernia treated at the Veterinary Hospital of Uberaba (HVU)from 2005 to 2020, addressing the clinical and surgical aspects and the postoperative period, seeking to identify the mostrelevant factors to improve care and treatment of future patients affected by this disease.Materials, Methods & Results: The medical records of dogs with a definitive diagnosis of perineal hernia were collected.The prevalence of perineal hernia was calculated. Data were obtained regarding sex, being castrated or not, age, bodymass, race, clinical signs, affected side, possibility of hernia reduction, treatments used, associated pathologies. Of the 120cases, only 69 underwent surgery at the HVU and from these cases data were obtained regarding hernia content, surgicaltechniques, surgical wires used, recurrences and postoperative complications. Fishers exact test was applied to assess theinfluence of the type of surgical treatment and surgical thread on the occurrence of complications and recurrences. Perineal hernia was the second most frequently observed hernia. Mixed breed, male, non-neutered and geriatric dogs were themost affected. The most frequently observed clinical signs were related to the digestive and urinary tract, consistent withthe most commonly found hernia contents (bladder, prostate and rectum). ANTECEDENTES: El sarcoma fibromixoide de bajo grado es una neoplasia rara de histología someramente benigna, pero con un comportamiento agresivo metastásico tardío. Las hernias perineales son de presentación inusitada y no existen más de 100 casos reportados en la literatura mundial. DISCUSIÓN: La presentación de esta neoplasia fibroblástica es poco frecuente, y aún más como contenido de una hernia perineal primaria. CONCLUSIÓN: Es una neoplasia infrecuente que se presenta en aproximadamente el 0.7% de los casos, con comportamiento agresivo metastásico y con necesidad de tratamiento adyuvante. BACKGROUND: Low-grade fibromyxoid sarcoma is a rare neoplasm with apparently benign histology but with late aggressive metastatic behavior. The perineal hernias are unusual, there are no more than 100 cases reported in the world literature. DISCUSSION: The presentation of this fibroblastic neoplasm is rare and makes it even more rare if it is as the content of a primary perineal hernia. CONCLUSION: It is an infrequent neoplasm that occurs in approximately 0.7% of all cases, with aggressive metastatic behavior and in need of adjuvant treatment.

Se presenta el caso clínico de una anciana de 86 años de edad con evisceración del intestino delgado a través del periné. Se efectuó la exploración quirúrgica urgente del abdomen y se observó la perforación del fondo del saco de Douglas y la salida de 50 cm de intestino delgado estrangulado, de manera que se realizó la resección intestinal de todo el segmento afectado y anastomosis termino-terminal. Luego se reparó el defecto del fondo del saco de Douglas con una plastia perineal, para lo cual se utilizó una malla de polipropileno. Este proceder es una buena alternativa para el tratamiento quirúrgico en quienes presentan hernias perineales, pues permite un cierre mejor, disecar el saco herniario y reducirlo adecuadamente.


The case report of a 86 years elderly is presented with evisceration of the small bowel through the perineum. The urgent surgical exploration of the abdomen was carried out and it was observed the perforation of the Douglas pouch and the 50 cm prominence of impacted small bowel, so that the bowel resection of the whole affected segment and end to end anastomosis was carried out. Then the Douglas pouch defect was repaired with a perineal plasty, for which a polypropylene mesh was used. This procedure is a good alternative for the surgical treatment in those who present perineal hernias, because it allows a better closing, to dissect the hernial sack and to reduce it appropriately.

BACKGROUND: Primary posterior perineal hernias in men are rare. We report a case of this type of hernia associated with dolichocolon, a condition which, to our knowledge, has not been reported previously. CLINICAL CASE: A 71-year old male presenting with a perineal tumour of 40 years evolution. He had no history of perineal surgery or trauma. On physical examination, a lump of 4×3cm was observed in the right para-anal region, which increased in volume during the Valsalva manoeuvre. Computed tomography showed a defect in the pelvic floor, which was reconstructed using a roll of polypropylene mesh in the hernia defect. DISCUSSION: The case described is of interest, not only because a perineal hernia is a rare clinical entity, but also because repair using a roll of mesh has not been reported associated with a dolichocolon, which can be considered a factor risk for development. CONCLUSIONS: The surgical approach and repair technique of the pelvic floor for perineal hernias should be individualised. The use of mesh for reconstruction should always be considered. The presence of dolichocolon can contribute to the gradual development of a perineal hernia.

BACKGROUND: Perineal hernias may occur after the completion of surgery in this region, or may occur as a primary lesion. Among the less frequent are the perineal hernias. CLINICAL CASE: Female patient, 42 years old, with perineal tumor mass on the right of the anus, spontaneously occurring, five years of evolution, with dimensions of 6 × 10 cm, soft and reducible behind transverse perineal muscle. With dignosis of primary posterior perineal hernia, surgery was performed by the perineum with Mersilene mesh placement. There was no recurrence after 18 months follow-up. CONCLUSION: The perineal plasty can be a good alternative for the surgical treatment of primary perineal hernias, as it allows for a better seal, dissect the hernia sac and reduce it properly.


Antecedentes: las hernias perineales pueden aparecer luego de una intervención quirúrgica en esta región o como una lesión primaria. Entre las hernias menos frecuentes están las perineales. Caso clínico: paciente femenina de 42 años de edad, con un tumor perianal a la derecha del orificio anal, de aparición espontánea y de cinco años de evolución, con dimensiones de 6 × 10 cm, blando y reductible por detrás del músculo transverso del periné. Con diagnóstico de hernia perineal posterior primaria se intervino quirúrgicamente por vía perineal y se le colocó una malla de mersilene. Después de 18 meses de seguimiento no hubo recidiva. Conclusiones: la plastia perineal puede ser una buena alternativa para el tratamiento quirúrgico de las hernias perineales primarias porque permite efectuar un cierre mejor, disecar el saco herniario y reducirlo adecuadamente.

The coexistence of rectal diseases that go undiagnosed or are not correctly treated can cause recurrence and postoperative complications. The objective of this report is to describe a surgical technique for treatment of rectal sacculation through lateral resection in dogs with perineal hernia, whereby restoring the rectal integrity.(AU)


A ocorrência de hérnias perineais em cães na rotina clínica cirúrgica é frequente. A coexistência de doenças do reto não diagnosticadas ou não tratadas corretamente pode causar recidiva e complicações pós-operatórias. Este relato tem como objetivo descrever uma técnica cirúrgica para o tratamento de saculação retal por meio de ressecção lateral em cães com hérnia perineal, com restabelecimento da integridade retal.(AU)

A utilização de membrana biológica constitui-se método alternativo na reparação de hérnia perineal, e avaliar suaeficiência como reforço após a herniorrafia pela técnica de elevação do músculo obturador interno consiste no objetivodeste trabalho. Para tanto, utilizaram-se dez cães atendidos no Serviço de Cirurgia de Pequenos Animais do HospitalVeterinário da FMVZ/USP. Os animais foram distribuídos em dois grupos, GI e GII, com cinco representantes em cadaum. A hérnia perineal foi corrigida pela técnica de elevação do músculo obturador interno em ambos os grupos, sendoque o grupo GII ganhou reforço no diafragma pélvico com enxerto de retalho de pericárdio equino Puro Sangue Inglês,conservado em glicerina a 98%. Os animais foram avaliados por até 90 dias de pós-operatório, mediante análises clínica,ultrassonográfica e radiográfica da região perineal. Os resultados obtidos demonstraram que houve reforço no local daherniorrafia ao se fixar o pericárdio de equino conservado em glicerina a 98%, diminuindo a ocorrência de deiscênciade pontos e ruptura do diafragma operado.(AU)


The use of biological membrane constitutes an alternative method to repair perineal hernia, and evaluate its performanceas a reinforcement after herniorrhaphy by of raising internal obturator muscle was the goal of this study. Ten dogsadmitted at the Department of Small Animal Surgery Veterinary Hospital of FMVZ / USP were included. Animalswere equally divided into two groups, GI and GII. Perineal hernia was corrected by the elevating the internal obturatormuscle in both groups, but in group GII won the strengthening of the pelvic diaphragm flap of equine pericardiumpure English Thoroughbred, preserved in glycerol to 98%. The animals were evaluated until 90 days post-operativelyby clinical examination, ultrasound and radiographic images of the perineal region. The results showed that therewere enhanced at the site of the hernia with equine pericardium preserved in glycerol 98%, reducing the occurrence ofdehiscence points and rupture of the diaphragm operated.(AU)
