Objective: To investigate if individual and contextual socioeconomic factors are associated with contraceptive use in Brazilian women from 18 to 49 years old, stratified by parity. Methods: Cross-sectional, population-based study that analyzed data from 16,879 women from 18 to 49 years old, respondents of the 2013 National Health Survey. Individual factors such as reproductive history, access to health services, and sociodemographic characteristics were considered; and as contextual factors, Human Development Index (HDI), Sociodemographic Index (SDI) Primary Health Care Coverage (PHC coverage) and Average Monthly Income were included. Multilevel logistic regression models were estimated, stratified by parity, with women being level 1 and States and Federal District of level 2 units. Results: Nulliparous women had lower prevalence of contraceptive use (77.9 %) when compared with primiparous and multiparous (88.7 %), as well as greater variability in the chance of using contraception (ICC = 2.1 vs. ICC = 1.1, respectively). Women who lived in States with higher levels of HDI, average monthly income and SDI were more likely to use contraception. The greater PHC coverage was positively associated with the use of contraceptives for primiparous/multiparous women and negatively for the nulliparous. Furthermore, higher education increased the chances of using contraception, both for nulliparous and primiparous/multiparous women. Conclusions: The high contraceptive coverage in Brazil hides important inequities in access, highlighting contextual characteristics associated with the use of contraceptives, in addition to individual factors. The lower prevalence and chance of using contraceptives for nulliparous women with greater social vulnerability reveal inequity and priority in public policies. Implications for practice: The need to improve access to contraception is highlighted, considering both the individual and contextual vulnerabilities of women, which implies ensuring timely and qualified access to contraceptive methods, especially for young and nulliparous women who are more socially vulnerable.

Objetivo: Relatar o impacto da capacitação teórica e prática dos enfermeiros para realização da consulta de enfermagem com ênfase no Planejamento Reprodutivo e Sexual e na inserção de Dispositivo Intrauterino de cobre. Métodos: Trata-se de um estudo descritivo do tipo relato de experiencia que foi desenvolvido a partir do projeto de Consulta de Enfermagem Ginecológica com ênfase na inserção do dispositivo intrauterino por enfermeiros, idealizado pelo Conselho Regional de Enfermagem de Alagoas (Coren-AL). Resultados: Foi possível analisar o aumento dos números de inserções do Dispositivo Intrauterino de cobre na Atenção Básica de Saúde. Ao todo foram 1016 inserções em todo o estado de Alagoas, sendo os municípios de Penedo e Arapiraca responsáveis por 660 inserções, somando-se os dados do período de 2016 ao início de 2021. Observou-se o impacto que a capacitação dos enfermeiros ocasionou. Conclusão: O projeto ampliou a disponibilidade do Dispositivo Intrauterino no Sistema Único de Saúde, garantindo os direitos das mulheres ao acesso dos métodos contraceptivos. Possibilitou acessibilidade e ampliou o escopo de ações do enfermeiro na atenção primária em saúde, aspecto importante da resolutividade. (AU)


Objective: To report the impact of theoretical and practical training for nurses to carry out nursing consultations with an emphasis on Reproductive and Sexual Planning and the insertion of an Intrauterine Device of copper. Methods: This is a descriptive study of the experience report type that was developed from the Gynecological Nursing Consultation project with emphasis on the insertion of copper intrauterine device by nurses, conceived by the Regional Nursing Council of Alagoas (Coren-AL). Results: It was possible to analyze the increase in the number of insertions of the copper intrauterine device in primary health care. In all, there were 1016 insertions throughout the State of Alagoas, with the municipalities of Penedo and Arapiraca being responsible for 660 insertions, adding data from the period from 2016 to the beginning of 2021. The impact caused by the training of nurses was observed. Conclusion: The project expanded the availability of the IUD in the Unified Health System, guaranteeing women's rights to access contraceptive methods. Enabled accessibility; and expanded the scope of nurses' actions in primary health care; important aspect of resoluteness. (AU)


Objetivo: Informar el impacto de la formación teórica y práctica de enfermeras para la realización de consultas de enfermería con énfasis en Planificación Sexual y Reproductiva y la inserción de un Dispositivo Intrauterino de cobre. OBJECTIVE: Limited Health Literacy implies an insufficient understanding of relevant health information, being associated with various variables. The objective of the study was to measure the prevalence of the level of Limited Sexual and Reproductive Health Literacy (AS-SR), its associated variables and the differences in scores between levels of AS-SR, universities and science of study in Chilean university students. METHODS: A multicenter and cross-sectional study, which applied a validated scale to measure levels of AS-SR, was carried out in a sample of 2,186 Chilean university students, categorizing it as high, medium high, medium low and low. The level of Limited AS-SR was obtained by adding the medium-low and low categories. Tests were carried out: descriptive, psychometric and reliability; association, logistic regression and differences between variables of interest. RESULTS: The prevalence of Limited AS-SR was 52.7%. The variables most associated with the level of Limited AS-SR were: low interest in information about health care (OR=2.819; 95% CI: 2.132-3.726), prevention (OR=2.564; 95% CI: 1.941-3.388), sexuality (OR=2.497; 95% CI: 1.807-3.452) and health promotion (OR=1.515; 95% CI: 1.239-1.853); certain sources of Information (OR=1.915; 95% CI:1.614-2.272); low economic income (OR=1.661; 95% CI: 1.361-2.026), among others. There were statistically significant differences in scores between categories of AS-SR levels, universities and study science. The scale had a reliability of 0.940. CONCLUSIONS: More than half of the students have Limited AS-SR, mainly associated with low interest in health information. The scale presents excellent psychometric indicators, being recommended for diagnoses of health situations.


OBJECTIVE: La Alfabetización en Salud Limitada implica una comprensión insuficiente de la información relevante en salud, asociándose con diversas variables. El objetivo del estudio fue medir la prevalencia del nivel de Alfabetización en Salud Sexual y Reproductiva (AS-SR) Limitada, sus variables asociadas y las diferencias de puntajes entre niveles de AS-SR, universidades y ciencia de estudio en universitarios chilenos. METHODS: Se realizó un estudio multicéntrico y transversal, que aplicó una escala validada para medir niveles de AS-SR, en una muestra de 2.186 estudiantes universitarios chilenos, categorizándola en alta, media-alta, media-baja y baja. El nivel de AS-SR Limitada se obtuvo mediante el sumatorio de categorías media-baja y baja. Se realizaron pruebas: descriptivas, psicométricas y fiabilidad; asociación, regresión logística y de diferencias entre variables de interés. RESULTS: La prevalencia de AS-SR Limitada fue del 52,7%. Las variables mayormente asociadas al nivel de AS-SR Limitada fueron: bajo interés en información sobre atención en salud (OR=2,819; IC 95%:2,132-3,726), prevención (OR=2,564; IC 95%: 1,941-3,388), sexualidad (OR=2,497; IC 95%: 1,807-3,452) y promoción de la salud (OR=1,515; IC 95%: 1,239-1,853); ciertas fuentes de Información (OR=1,915; IC 95%:1,614-2,272); bajo ingreso económico (OR=1,661; IC 95%: 1,361-2,026), entre otras. Existieron diferencias estadísticamente significativas de puntajes entre categorías de niveles de AS-SR, universidades y ciencia de estudio. La escala presentó fiabilidad de 0,940. CONCLUSIONS: Más de la mitad de los estudiantes poseen AS-SR Limitada, asociada principalmente al bajo interés en información en salud. La escala presenta excelentes indicadores psicométricos, siendo recomendable para diagnósticos de situación de salud.

Loreto, in the Peruvian Amazon, has one of the highest adolescent pregnancy rates in the country. However, underlying causes of adolescent pregnancy are not fully understood as data are limited in Indigenous and remote Amazonian communities. This study investigated adolescent reproductive health within Loreto using an ecological systems framework. Forty-one semi-structured interviews were conducted in June 2022: community leaders (n = 12) and adolescent participants between 15 and 17 years of age (pregnant girls, n = 11; never pregnant girls, n = 9; and boys, n = 9). We also conducted focus group discussions with community health workers and educators in October 2022 (three focus groups, n = 15). Adolescent reproductive health is complex with multi-layered factors that put girls at higher risk of pregnancy. We found a paradoxical relationship between expected social and gender norms and individual desires. This research provides a contextual understanding of the lived experience of adolescents and young people in the Amazon region of Peru. Our findings suggest the need for greater exploration of the contradictory ideas surrounding adolescent pregnancy and female sexuality.

Background: In Colombia, the average age of first sexual intercourse is between 14 and 15, and it has one of the highest fertility rates among adolescents in Latin America, which reflects poor access to health services and a lack of knowledge about contraception. Some laws support comprehensive sex education as a learning process that families and schools should provide. The objective of this work was to assess the frequency of adequate knowledge and attitudes of acceptance towards contraception in students, their associated factors, sexual behaviours in sexually active students, and experiences with sex education. Methods: Mixed methods study consisting of an analytical cross-sectional study and a qualitative hermeneutic phenomenological study. The former included students who completed a questionnaire with sociodemographic variables, surveys on knowledge and attitudes toward contraception, and questions about sexual behaviour. Focus groups were used in the qualitative study to evaluate students' and parents' sex education experiences. Findings: 827 students were surveyed, of whom 52.3% had adequate knowledge about contraceptives and 80.1% had attitudes of acceptance. Talking with parents about sex education, receiving sex education, and being in the eleventh grade increased the likelihood of having adequate knowledge. Each year of age, being male, being a victim of forced displacement, and studying in a public school were associated with lower possibilities of attitudes of acceptance. Using condoms, compared to other contraceptives, was associated with less adequate knowledge and attitudes of acceptance (p < 0.05). Experiences with sex education, contraception, and sexual and reproductive rights were three emerging categories in the focus groups. Interpretation: In a group of Colombian high school students, half of them had adequate knowledge, eight out of 10 had attitudes of acceptance about contraception, and both were associated with having received sex education. Nevertheless, different types of barriers to sexual education were identified at home and in schools. The results will allow the generation of educational policies that modify the educational model as well as new strategies by health professionals to raise awareness about responsible sexuality. In September 2021, the Mexican Supreme Court issued a decision disallowing any federal or local judicial authority to indict someone for the offense of voluntary or consensual abortion. This decision also declared unconstitutional penalties imposed on medical personnel who facilitate or assist such procedures. Furthermore, the Court decided that limiting access to abortion in cases of rape to a specific time frame was disproportionate. Later on, in September 2023, the Supreme Court confirmed that absolute criminalization of abortion was unconstitutional and declared that the rule supporting criminalization in the Federal Penal Code was without effects. Consequently, healthcare providers who work in public federal health institutions cannot be criminalized for guaranteeing the right to abortion. This article reviews the reasons advanced by the Supreme Court to guarantee the right of reproductive self-determination, as well as the effects of both decisions beyond the decriminalization of abortion by Mexican federal and state legislatures. The paper also examines the scope and limitations of these rulings and identifies the remaining challenges regarding voluntary abortion procedures in Mexico.

Introducción: en la ejecución de políticas públicas de salud sexual, resultan cruciales las estrategias utiliza-das en la construcción de la relación agente de salud-usuario. En la literatura son pocos los artículos que exploran las estrategias que utilizan los agentes de salud para interactuar con pobladores rurales al abor-dar estas problemáticas. Este artículo se propone describir y comprender las estrategias que utilizan los agentes de salud en la atención de la salud sexual y reproductiva de pobladores rurales de bajos ingresos. Para ello, se conceptualiza la relación médico-paciente como una interfaz social, es decir, como un espacio de articulación entre los mundos de sentido de los actores involucrados. Materiales y métodos: se desarrolló un estudio de carácter exploratorio-descriptivo de tipo transversal. Se realizaron 21 entrevistas semiestruc-turadas a agentes del sistema de salud, y su análisis se hizo desde un enfoque cualitativo. Resultados: los agentes de salud utilizan un amplio repertorio de estrategias para abordar la salud sexual de los pobladores rurales. Se identificaron y caracterizaron dos tipos de estrategias (dialógicas y monológicas), con sus respectivos subtipos. Conclusión: el estudio visibiliza las estrategias dialógicas como modo alternativo de relación médico-paciente, en contraste con estrategias tradicionales, de tipo monológico. Además, contribuye a la formación de los agentes de salud, y en la conformación de los equipos que abordan la salud sexual y reproductiva en contextos rurales


Introduction: The strategies used for constructing health agent-user relationship are crucial for exe-cuting public policies on sexual health. In the literature, few articles explore the strategies used by health agents to interact with rural residents when addressing these problems. We aimed to describe and understand these strategies used by health agents for sexual health care in low-income rural resi-dents. To achieve this, the doctor­patient relationship is assumed to be a social interface and a space of articulation between the worlds of meaning of the actors involved. Materials and methods: An explor-atory, descriptive, and cross-sectional study was developed. Overall, 21 semistructured interviews were conducted with health agents, and the results were qualitatively analyzed. Results: Health agents use a wide repertoire of strategies to address the sexual health of rural residents. Two types of strategies (dia-logical and monological) and their respective subtypes were identified and characterized. Conclusion:Dialogic strategies are an alternative to the doctor­patient relationship and are contradictory to the traditional monological strategies. They impact the training of health agents and the formation of teams that address sexual and reproductive health in rural settings


Introdução: na execução das políticas públicas de saúde sexual, as estratégias utilizadas na construção da relação agente de saúde-usuário são cruciais. Na literatura são escassos os artigos que exploram as estratégias utilizadas pelos agentes de saúde para interagir com os moradores rurais na abordagem desses problemas. Este estudo se propõe a descrever e compreender as estratégias utilizadas pelos agen-tes de saúde na atenção à saúde sexual e reprodutiva de moradores rurais de baixa renda. Para isso, a relação médico-paciente é conceituada como interface social, ou seja, como espaço de articulação entre os mundos de sentido dos atores envolvidos. Materiais e métodos: foi desenvolvido um estudo transver-sal exploratório-descritivo. Foram realizadas 21 entrevistas semiestruturadas com agentes do sistema de saúde e sua análise foi feita a partir de uma abordagem qualitativa. Resultados: os agentes de saúde utilizam um amplo repertório de estratégias para abordar a saúde sexual dos residentes rurais. Dois tipos de estratégias (dialógicas e monológicas) foram identificadas e caracterizadas, com seus respecti-vos subtipos. Conclusão: este estudo torna visíveis as estratégias dialógicas como modo alternativo de relação médico-paciente, em contraste com as estratégias tradicionais de tipo monológico. Além disso, contribui para a formação de agentes de saúde e na formação de equipes que abordem saúde sexual e reprodutiva em contextos rurais.

OBJECTIVE: The aim of our study was to assess the covariates of contraceptive switching and abandonment among Brazilian women stratified by oral pills, condoms and injectables. MATERIALS AND METHODS: Women attending primary health care services in three Brazilian mid- to large-sized cities were interviewed face-to-face about their contraceptive practices (n = 2,051). Data were collected using a contraceptive calendar. Analysis included estimates using Kaplan-Meier multiple-decrement life-table probabilities and discrete-time hazards modelling of switching from a method to another or to no method. RESULTS: Among 3,280 segments of contraceptive use, we observed that five-year contraceptive switching rates ranged from 34.9% among injectable users to 56.1% among pill users. Of particular concern were the high discontinuation rates of abandonment, which ranged from 50.9% among injectable users to 77.4% among pill users. Covariates of method switching and abandonment varied by type of method, but age, race/ethnicity, religion and relationship status must be highlighted as key elements of discontinuation. CONCLUSION: Contraceptive method switching and abandoning are frequent outcomes of contraceptive use. Understanding the factors that shape women's decisions to continue or discontinue the use of a contraceptive method can help tailoring comprehensive contraceptive counselling that meet their expectations and reproductive needs when starting using a method.


Contraceptive discontinuation among women in need of contraception may difficult the achievement of their fertility desires, which can lead to unintended pregnancies.

Resumo Objetivou-se compreender as experiências de dupla maternidade na atenção à saúde durante o pré-natal, parto e puerpério a partir de um método qualitativo, utilizando entrevistas individuais on-line e grupo focal on-line assíncrono com mulheres cisgêneros, a maioria em relacionamentos homoafetivos. Os resultados revelaram a marginalização das vivências parentais dessas mulheres, destacando a violência institucional presente nos serviços de saúde brasileiros, sendo apresentados em dois eixos temáticos: 1) Cisheteronormatividade e seu impacto nas experiências de dupla maternidade e 2) Violência institucional nos serviços de saúde: da curiosidade à LGBTQIA+fobia. Conclui-se que a cisheteronormatividade prejudica a atenção à saúde para essas experiências, especialmente ao invisibilizar a mãe não gestante, destacando a urgência de capacitar profissionais de saúde, repensar e desafiar as normas cisgênero e heterossexuais e promover políticas inclusivas para garantir cuidados equitativos e combater a violência institucional.


Abstract The objective was to understand experiences of double motherhood during antenatal, childbirth and postpartum healthcare, using a qualitative method involving individual online interviews and asynchronous, online focus groups of cisgender women, mostly in same-sex relationships. The results revealed how these women's experiences of parenting were marginalised, highlighting institutional violence in Brazilian healthcare services, which are presented here in two thematic dimensions: 1) Cisheteronormativity and its impact on experiences of double motherhood; and 2) Institutional violence in healthcare services: from curiosity to LGBTQIA+phobia. It was concluded that cisheteronormativity hinders healthcare for these experiences, especially by rendering the non-gestational mother invisible. This underscores the urgent need to train healthcare personnel, rethink and challenge cisgender and heterosexual norms and promote inclusive policies to ensure equitable care and combat institutional violence.

Introducción: la justicia reproductiva es la capacidad de las personas y las sociedades de poder concretar los derechos sexuales y reproductivos. Por el contrario, la injusticia reproductiva (IR) expone la presencia de riesgos para el proceso de desarrollo durante el embarazo y la primera infancia. Objetivo: describir la evolución de un conjunto de indicadores vinculados a la justicia reproductiva en el Sistema Nacional Integrado de Salud (SNIS) de Uruguay en los últimos 12 años y comparar las tendencias entre el subsector público y el subsector privado. Metodología: estudio descriptivo, retrospectivo, de un conjunto de indicadores incluidos en los objetivos de desarrollo sostenibles (ODS) y de los objetivos sanitarios nacionales. Se analizaron razón de mortalidad materna (MM), incidencia de parto pretérmino (PPT), bajo peso al nacer (BPN) y sífilis congénita (SC), en el subsector público y privado del SNIS, durante los últimos 12 años. Resultados: la razón de MM en el período de tiempo analizado ha sido siempre superior en el subsector público, salvo en el año 2015. La incidencia de PPT en el período de tiempo ha oscilado entre 8,6% y 10%. Ésta es superior en el subsector público, salvo en algunos períodos donde es mayor en el subsector privado. La incidencia de BPN es superior siempre en el subsector público, con su mayor incidencia en 2022, de 9,3. La SC siempre fue superior en el subsector público desde 1,3 a 7,1, mientras que en el subsector privado los valores van de 0,2 a 0,6. Conclusiones: la diferencia en estos indicadores de salud perinatal entre los dos subsectores de atención de nuestro país refleja que a pesar de contar con un SNIS, existe una disparidad que impacta sobre los resultados de indicadores finales e intermedios, determinando así la existencia de una IR.


Introduction: Reproductive justice is the ability of individuals and societies to realize sexual and reproductive rights. On the contrary, reproductive injustice (RI) exposes the presence of risks to the developmental process during pregnancy and early childhood. Objective: To describe the evolution of a set of indicators related to reproductive justice in the Integrated National Health System (SNIS) of Uruguay over the last 12 years and compare trends between the public and private subsectors. Methodology: A descriptive, retrospective study of a set of indicators included in the Sustainable Development Goals (SDGs) and national health objectives was conducted. Maternal mortality ratio (MMR), incidence of preterm birth (PTB), low birth weight (LBW), and congenital syphilis (CS) were analyzed in the public and private subsectors of the SNIS over the past 12 years. Results: During the analyzed period the maternal mortality ratio has always been higher in the public subsector, except in the year 2015. The incidence of preterm birth during the period has ranged between 8.6% and 10%. It is higher in the public subsector, except in some periods where it is higher in the private subsector. The incidence of low birth weight is always higher in the public subsector, with its highest incidence in 2022 at 9.3. Congenital syphilis has always been higher in the public subsector, ranging from 1.3 to 7.1, while in the private subsector, the values range from 0.2 to 0.6. Conclusions: The difference in these perinatal health indicators between the two healthcare subsectors in our country reflects that despite having an Integrated National Health System, there is a disparity that impacts the results of final and intermediate indicators, thus determining the existence of reproductive injustice.


Introdução: A justiça reprodutiva é a capacidade dos indivíduos e das sociedades de poderem realizar seus direitos sexuais e reprodutivos. A injustiça reprodutiva (IR), por outro lado, expõe a presença de riscos ao processo de desenvolvimento durante a gravidez e a primeira infância. Objetivo: descrever a evolução de um conjunto de indicadores relacionados à justiça reprodutiva no Sistema Nacional Integrado de Saúde (SNIS) do Uruguai nos últimos 12 anos e comparar as tendências entre os subsetores público e privado. Metodologia: estudo descritivo e retrospectivo de um conjunto de indicadores incluídos nos Objetivos de Desenvolvimento Sustentável (SDGs) e nas metas nacionais de saúde. A taxa de mortalidade materna (MM), a incidência de parto prematuro (PTB), baixo peso ao nascer (BPN) e sífilis congênita (SC) foram analisadas no subsetor público e privado do SNIS nos últimos 12 anos. Resultados: A taxa de mortalidade materna foi maior no subsetor público durante o período analisado, exceto em 2015. A incidência de nascimento pré-termo no período variou entre 8,6 e 10%. Ela é maior no subsetor público, exceto em alguns períodos em que é maior no subsetor privado. A incidência de baixo peso ao nascer é sempre maior no subsetor público, com sua maior incidência em 2022, com 9,3. A sífilis congênita sempre foi mais alta no subsetor público, de 1,3 a 7,1, enquanto no subsetor privado os valores variam de 0,2 a 0,6. Conclusões: A diferença nesses indicadores de saúde perinatal entre os dois subsetores de atendimento em nosso país reflete que, apesar da existência de um Sistema Nacional de Saúde Integrado, há uma disparidade que impacta nos resultados dos indicadores finais e intermediários, determinando assim a existência de uma injustiça reprodutiva.
