RESUMO

Objective: Nurses inevitably encounter patients who require care aimed at limiting therapeutic effort (LTE), even though many of them are not prepared to provide support to individuals with terminal illnesses and their families. One of the contexts in which the LTE is considered is the intensive care unit (ICU). This review is to describe the competencies for the execution of a nursing professional role in the LTE in the ICU. Method: An integrative review of the literature published between the years 2010 and 2023. The search was carried out in five databases: Medline, Wiley Online Library, SciELO, ScienceDirect, and Web of Science. The Critical Appraisal Skills Programme in Spanish was used as the template for study evaluation. The methodology of the Oxford Center for Evidence-Based Medicine (CEBM) was used to assess the level of evidence and the degree of recommendation. Result: A total of 25 articles in a wide range of studies were included. The findings suggest that the competencies for LTE in the ICU are direct patient care, family-centered care, and the role of the nurse within the team. However, more high-quality studies are needed to confirm these conclusions. Three categories were identified: (a) competencies as defender agent between the patient, his family, and the interdisciplinary team; (b) competencies for decision-making in limiting the therapeutic effort; and (c) competencies for nursing therapeutic interventions at the end of life. Conclusion: The competencies of the nursing professionals who work in the adult ICU in the LTE are essential to the patient's quality of life, dignity of their death, and comprehensive family support for coping with grief.

RESUMO

Purpose: To analyze and compare metabolic, lifestyle and mental health parameters in relatives and people-with-T2DM (PDM) with and without support. Patients and Methods: We included 160 patients with <5 years of diagnosis of T2DM, without disabling complications, and non-smokers, attending a multidisciplinary program for diabetes control, and their accompanying relatives. If the patients or relatives abandoned the program, we contacted them and asked to take laboratory tests and answer surveys regarding anxiety, depression, and perception of their family support. Variables distribution was assessed with the Kolmogorov-Smirnov test. We used ANOVA or Kruskal Wallis Tests, according to variable distribution. Frequencies and percentages are used for categorical values and analyzed with a chi-square test. We separated the participants in four groups: relatives with and without support and PDM with and without support. Results: We included 160 participants, age 51±10, and 54.3% women. Total cholesterol (188±36 vs 204±43 vs 170±34 vs 181±35 mg/dL, p=0.001), LDL-cholesterol (113±35 vs 125±27 vs 101 ±30 vs 109±29, p=0.008), and non-HDL cholesterol (143±32 vs 154 ±30 vs 129±33 vs 135±35 mg/dL, p=0.010) were higher in the group without support. Although patients without family support had lower values, they did not achieve metabolic goals. Weight (75±17 vs 77±19 vs 74.2±10.5 vs 90.2±17.3 kg) and body mass index (28.9±4.8 vs 30.1±4.7 vs 27.4±3.3 vs 33±4.3 kg/m2) were higher in PDM without family support (p<0.001 for both). Conclusion: Support in PDM and their families is important in metabolic control. However, raising awareness among family members to screen for diabetes and changes in lifestyle are points to improve. Including the evaluation of social and family support will allow a more complete assessment to identify barriers to achieving goals.

RESUMO

BACKGROUND AND OBJECTIVES: To examine the role of probable dementia on changes in living arrangements and mortality among very old Mexicans and Mexican Americans in 2 different nations. RESEARCH DESIGN AND METHODS: We employ the Hispanic Established Population for the Epidemiologic Study of the Elderly and the Mexican Health and Aging Study, 2 comparable longitudinal data sets, to identify predictors of changes in living arrangements using multinomial logistic regression, controlling for cognitive status, demographic characteristics, and resources. RESULTS: In Mexico, women with dementia who lived alone at baseline were more likely to become part of an extended family household than men with similar levels of cognitive impairment. A similar pattern emerges for the oldest Mexican-American women. Spousal loss increases the likelihood of living alone for women in the United States regardless of dementia. Although dementia elevates the risk of mortality for men living alone in the United States, in both countries, women in their 90s who lived alone with dementia had a lower risk of mortality relative to men. DISCUSSION AND IMPLICATIONS: Longer life spans increase the risk of living alone with dementia in both countries, especially for women. Older individuals in both countries face financial hardships. Mexicans have limited formal options in dementia care. Mexican Americans with dementia continue to live alone despite low income although, unlike the Mexicans, they have access to Medicaid long-term care. For Mexico and the United States, the growing number of older individuals with dementia represents a growing public health concern.

RESUMO

Objective Emotional dysregulation, family and social support are associated with depression. The present study aims to explore, by path analysis, relationships between these variables. Method The path analysis method was applied, and, additionally, comparisons of the means were made between participants with and without a diagnosis of depression and suicide attempts. The sample consisted of 712 Brazilians (M = 23.8; SD = 8.13), 66.7% of whom were women. Results The results show a correlation between both types of support for emotional dysregulation and depression. Emotional dysregulation was found to be an associated variable in depression. Positive correlations were observed between emotional dysregulation and depression. Both were negatively correlated with family and social support. Conclusion People with a diagnosis and previous suicide attempts had higher averages in emotional dysregulation and depression, and lower averages in family and social support.

Objetivo Desregulação emocional, suportes familiar e social estão associadas a depressão.

Abstract Background The perception of family support in chronic disease can be relevant. Objective To assess the perception of family support in adult patients with epilepsy (PWEs) and relate it to quality of life (QoL) and clinical aspects. Methods Data from the Perceived Family Support Inventory (IPFS) of 130 PWEs were related to the clinical variables, QOLIE-31 scores, and the Neurological Disorders Depression Inventory for Epilepsy (NDDI-E) with statistical tests, with p < 0.05. Results The mean age was 49.9 ± 17.2 years, and the duration of epilepsy was 20.8 ± 15.4 years. The presence of depression (scores ≥ 15) was associated with lower family support. Being married and non-occurrence of depression were the variables associated with a higher IPFS score (R = 0.2112), in the multiple linear regression. Conclusion The perception of greater family support was associated with demographic aspects, the absence of depression, and better QoL. Family relationships may play an essential role in health adjustment behaviors and QoL in epilepsy.

Abstract The family plays a fundamental role both in socialization and in the configuration of values in minors and adolescents; including those related to environmental care. In keeping with this premise, Unicef (2018) announced that the sustainable development of the planet requires counting on families; it pointed them out as essential collaborators of governments for the quality of life of future generations. Considered a natural and elemental unit of all modern societies and an educational agent of the first order for the development and psychosocial adjustment of its members, the family is also a primary socialization agent (Fontana-Abad, Gil y Reyero, 2013). It plays a key role in the internalization of prosocial values, such as self-transcendence (care and universalism) and conservation (conformity, safety, and tradition); values closely related to pro-environmental behaviors (Barrera-Hernández, Sotelo, Echeverría y Tapia, 2020). Following this line, this research studies the perceptions that adolescents have about family values, parental support for autonomy, and environmental care practices, according to the 3R rule, in a triple perspective: reduce, reuse, and recycle. It uses a quantitative, descriptive, and relational methodology. The target population was high school adolescents from the city of Cuenca (Ecuador). The sample was made up of 122 adolescents which attended three schools: one private educational unit (36.1 %) and two public (27.9 % and 36.1 %). Of the respondents, 38.8 % were female and 61.2 % were male, and they all were between 15 and 19 years old (M = 16.4; SD = 1.2); 27 % were in their first year of high school (15-17 years), 44.3 % were in their second year of high school (16-18 years) and the 28.7 % were in their third year (17-19 years). Data analysis includes comparison of means, Pearson's r correlation coefficient and simple linear regression. The results indicate that a high percentage of the adolescents in the study carry out at least one of the following environmental care actions (these are organized from highest to lowest frequency): saving water and electricity consumption, using garbage cans, reusing bottles, separating garbage, and participating in actions in favor of environment. Likewise, it is detected that they perceive family as a driving agent for their development as autonomous subjects, and a promising factor for the process of integration of values and the development of competencies for caring for the environment; however, they still recognize the school as the place where their commitment to a sustainable future is most intensely forged. Additionally, the adolescents in the study perceive a high parental support for their autonomy and, likewise, that mothers attach greater importance to the value of helping others, a central axis in the ethics of care that characterizes the paradigm of sustainability. This research corroborated, once again, the ONU's (2015) position when it indicates that the family is a main agent of change for sustainability, with a decisive influence on the preservation of life and ecosystems. It endorses the relevance of promoting educational strategies that promote family-school relations in order to enhance the role of the former in the acquisition of pro-environmental values and, ultimately, its collaboration in the quality of formal education aimed at sustainability. With these results, a path is opened to improve the knowledge on the parental role in relation to the promotion of behaviors of environmental care (pro-sustainability), within the framework of adolescents' and young people's formal education.

RESUMO

Purpose: The study aimed to determine the association between family support and coping strategies of people diagnosed with COVID-19. Methods: The study was analytical and cross-sectional. The sample consisted of 500 participants who were selected by non-probabilistic and snowball sampling and included residents of both sexes who belonged to the city of Lima, with a diagnosis of COVID-19, who lived with relatives, and who accepted to participate in the research. For data collection, the scales "family support" and "Coping and Adaptation Process-Coping Adaptation Processing Scale (CAPS)" were used. The technique used was the survey through the home visit and the questionnaire instrument. To measure the relationship of the study variables, binary logistic regression was chosen, considering coping strategies as the dependent variable and socio-demographic data and family support as independent variables. Results: Of the 500 participants, 50.4% were women, and 49.6% were men. The results revealed that most participants presented a high capacity for coping strategies and high perceived family support (97.2% and 81%, respectively). In the bivariate analysis, socio-demographic aspects and family support and their dimensions were related to high or low capacity for coping strategies. Significant differences were verified between marital status (p=0.026), having children (p=0.037), family support (p=0.000), and its dimensions with coping strategies. Finally, the multivariate analysis found that people with COVID-19 who perceived high family support were 33.74 times (95% CI: 7266-156,739) more likely to have a high capacity for coping strategies. Conclusion: Therefore, it is necessary to promote the development of parental and family support skills in the face of the health emergency caused by COVID-19.

RESUMO

The relationship between family involvement in students' academic activities, their learning, and academic achievement has been widely studied. Two different types of family involvement are recognized: parental participation, which is linked to activities implemented by the school, and parental support, which occurs at home and has to do with the family's educational practice. This study analyzed the influence of parental participation in school activities, parental support at home, and family socioeconomic status on student learning in mathematics. The database of 104,973 third-year secondary education students (50.1% female and 49.9% male) from the 2018 assessment of academic achievement in mathematical thinking was considered. Path analysis was employed in structural equation modeling, where a general model of parental support was proposed to compare the learning of students from the lowest quartile and those from the highest socioeconomic level; the model presented a good fit for each group. The models used showed acceptable adjustments in some criteria; in these models, there were positive and significant associations between parental schooling and socioeconomic level and achievement in mathematics. The results obtained are consistent with the findings of other studies in which parental support, mediated by socioeconomic status, significantly influences student learning.

Self-injurious behaviors in adolescence have become a public health problem worldwide. Often these behaviors are symptoms of psychiatric diseases, conceptualized as dysfunctional strategies of emotional regulation. Therefore, this study investigates the relation between self-injurious behaviors, family support perception and anxiety and depressive symptoms within adolescents from the state education network in Recife. It also estimates the prevalence of the phenomenon (n=501). Another goal was to estimate the prevalence of the phenomenon and its relation with sociodemographic variables. The instruments used were the Health Behavior in School-Aged Children Protocol, the Spence Children's Anxiety Scale and the Children's Depression Inventory, all adapted to Brazilian Portuguese. Internal consistency, chi-square, t-test, correlation and descriptive analyzes were performed. The results showed a prevalence of 31.3% for self-injurious behaviors, with female adolescents being 2.26 times more likely to engage in self-injurious behaviors. There were significant differences between participants who engaged in self-injurious behaviors and those who did not due to their perceptions of depressive symptoms, anxious symptoms and family support. Our findings emphasize the importance of family bonds in combating self-injurious behaviors. Thus, we hope to contribute to the creation of preventive strategies against risk behaviors, anxiety and depression.
