RESUMO

Herpes simplex virus (HSV) pseudotumor is a rare presentation of HSV and has not been previously reported in the stomach. A 51-year-old man with a medical history of HIV presented with new-onset dysphagia. Endoscopy revealed an HSV-positive mass at the gastroesophageal junction. After antiviral treatment, the patient returned with a 100-pound unintentional weight loss. Computed tomography showed an infiltrative mass with enlarged lymph nodes. The mass had progressed despite HSV treatment, and a repeat set of biopsies were negative for HSV with cells concerning for B-cell lymphoma. The patient was taken to the operating room for a full-thickness biopsy because of increasing concern for malignancy. The procedure was complicated by gastric perforation, leading to a total gastrectomy. Final pathology demonstrated an HSV-positive pseudotumor, negative for malignancy. It is important to diagnose gastric masses, especially in HIV-positive patients at high risk of infection and malignancy. However, immunocompromised patients with an HSV-positive mass should be treated for HSV pseudotumor with a longer than standard duration of antiviral therapy.

RESUMO

A 13-year-old female with polyarteritis nodosa underwent a partial gastrectomy for ischemic necrosis and gastric perforation following left gastric artery thrombosis. She later presented with vomiting, early satiety, weight loss, and severe malnutrition, when she was diagnosed with an occlusive gastric stricture. She successfully underwent repeated therapeutic endoscopic balloon dilations until the endpoint of 15-18 mm lumen was achieved without any complications, and her symptoms resolved.

INTRODUCTION AND IMPORTANCE: The Potter sequence is defined as a series of congenital defects related to severe oligohydramnios, associated with polycystic kidney disease, bilateral renal agenesis, pulmonary hypoplasia, obstructive uropathy and premature rupture of membrane, which compromises the life of the neonate sometime after birth. Within the evidence published so far, which is very little, no perforation of the gastrointestinal tract has been reported as a complication of this condition. CASE PRESENTATION: Male neonate born preterm with prenatal diagnosis of pulmonary hypoplasia, polycystic renal dysplasia and severe oligohydramnios (Potter sequence), presented acute respiratory distress syndrome 10 min after birth, requiring mechanical ventilation and admission to the intensive care unit. During her stay in intensive care, he developed abdominal distension and presence of biliary content in the nasogastric tube. An abdominal X-ray was performed and showed signs of pneumoperitoneum, evidencing gastric perforation on exploratory laparotomy. CLINICAL DISCUSSION: Gastric perforation in neonates is a condition that causes high health costs, morbidity, high risk of mortality and disability, regardless of the cause. The management of gastric perforation in Potter syndrome, as well as any other complication, represents a challenge due to the prognosis of these patients. Renal failure and acute respiratory distress syndrome are disorders that compromise the function of various structures and organs such as the heart and brain. CONCLUSION: Gastric perforation is a possible complication of the Potter sequence or syndrome. In addition, there is no literature describing the benefits or disadvantages of specific surgical techniques in the resolution of perforation.

Introducción: presentándose como una verdadera emergencia quirúrgica, la perforación gástrica neonatal constituye un reto para todos los galenos que laboran en este campo. Objetivo: exponer los resultados de nuestra experiencia en el tratamiento de recién nacidos con perforación gástrica, y revisar el estado actual del conocimiento sobre esta nosología. Métodos: se realizó una investigación descriptiva, retrospectiva, con la totalidad de los recién nacidos ingresados en la sala de neonatología quirúrgica del Hospital Pediátrico Universitario William Soler, diagnosticados con perforación gástrica, durante el período comprendido entre enero de 1990 y diciembre de 2010. Resultados: se trataron 8 pacientes, de los cuales 3 se consideraron como idiopáticos. El neumoperitoneo estuvo presente en la totalidad de los pacientes. El fundus gástrico y la cara anterior del óirgano constituyeron los sitios más frecuentemente afectados. Por su lado, la prematuridad no guardó una clara relación con la ocurrencia de la perforación. En la mayoría de los pacientes se realizó una reparación primaria de la perforación sin gastrostomía. La mortalidad general en el estudio fue de un 62 por ciento, y fue la peritonitis la complicación más frecuente. Conclusiones: la perforación gástrica neonatal constituye una entidad altamente letal, en la cual las distintas alternativas de tratamiento quirúrgico no son, en definitiva, por sí solas, la solución para lograr una adecuada sobrevida


Introduction: present like a real surgical emergence, the neonatal gastric perforation is a challenge for all physicians working in this field. Objective: to show the results of our experience with the treatment of newborns presenting with gastric perforation and also to review the current state of knowledge on this nosology. Methods: a retrospective and descriptive research was conducted in all the newborns admitted in the surgical neonatology ward of the William Soler University Children Hospital, diagnosed with a gastric perforation, from January, 1990 to December, 2010. Results: eight patients were treat concluded that three were idiopathic. The fundus of the stomach and the organ's anterior face were the more frequently involved sites. By his part, the prematurity hasn't an evident relationship with the occurrence of perforation. In most of patients a primary repair of the perforation without gastrotomy was carried out. The general morality in present study was of 62 percent and the peritonitis was the more frequent complication. Conclusions: the neonatal gastric perforation is a highly lethal entity in which the different alternatives of surgical treatment are not anyway by themselves, the solution to achieve an appropriate survival

RESUMO

The authors report a case of acute gastric volvulus in a 20-year-old male, complicated by perforation near the gastroesophageal junction and generalized peritonitis. This is an uncommon and potentially lethal conditon although our patient has been handled successfully with a partial gastrectomy.
