RESUMO

Achieving an accurate fit in a complete arch screw-retained implant prosthesis is paramount, regardless of whether analog or digital impressions are employed. In the context of complete arch implant prostheses, using intraoral scanners has sparked significant debate. However, recent advancements in scanner technology, specialized scan gauges meticulously calibrated for precise recording, and the implementation of appropriate scanning techniques have substantially improved the precision and accuracy of digital records. The Nexus iOS scan gauge system represents a pioneering approach, seamlessly integrating these optimization strategies into a purely digital workflow to fabricate complete arch screw-retained implant prostheses in edentulous patients. This case report elucidates the fabrication process of a complete arch screw-retained implant prosthesis that exhibited remarkable fitting accuracy and streamlined the treatment process to a mere 3 appointments. The patient reported high satisfaction with the efficient timeline, aesthetic appeal, and functional performance of the prosthesis. A distinctive advantage of this technique is the notable reduction in treatment steps compared to traditional procedures.

Background: Odontogenic maxillary sinusitis caused by a foreign body presents diagnostic and therapeutic challenges due to its infrequent occurrence and unique characteristics compared to sinusitis originating from other sources. CaseReport:Illustrating such fact, this report presents the clinical case of a 37-year-old woman referred complaining of pain in the same region where she had extracted her upper right first molar five days before. The intraoral examination revealed the presence of an orifice in the region, suggesting oroantral communication. Imaging exams revealed opacification of the right maxillary sinus and the unexpected presence of a highly radiodense object. With the diagnosis of maxillary sinusitis due to a foreign body established, the surgical approach initially consisted of administering preoperative medication, preceded by access to the maxillary antrum using the Caldwell-Luc technique. The object was found and removed, consisting of a surgical drill. At follow-up there was complete absence of symptoms and complete closure of communication. Conclusion: Cases of odontogenic maxillary sinusitis caused by drill detachment after tooth extraction are fairly uncommon. A thorough clinical evaluation proved to be essential and the Caldwell-Luc access was effective, safe and with good postoperative results, even with the absence of standardized diagnostic and management methods(AU)


Contexto: A sinusite maxilar odontogênica causada por corpo estranho apresenta desafios diagnósticos e terapêuticos devido à sua ocorrência infrequente e características únicas em comparação com sinusites originadas de outras fontes. Relato do Caso: Ilustrando tal fato, este relato apresenta o caso clínico de uma mulher de 37 anos de idade encaminhada com queixa de dor em mesma região que havia extraído o primeiro molar superior direito cinco dias antes. Ao exame intraoral verificou-se a presença de um orifício na região, sugerindo comunicação oroantral. Os exames de imagem revelaram opacificação do SM direito e a inesperada presença de um objeto altamente radiodenso. Com o diagnóstico de sinusite maxilar por corpo estranho estabelecido, a abordagem cirúrgica consistiu inicialmente na administração de medicação pré-operatória, precedida pelo acesso ao antro maxilar através da técnica de Caldwell-Luc. O objeto foi encontrado e removido, consistindo em uma broca cirúrgica. Ao acompanhamento houve ausência completa dos sintomas e total fechamento da comunicação. Conclusão: Casos de sinusite maxilar odontogênica causada por descolamento da broca após extração dentária são bastante incomuns. Uma avaliação clínica minuciosa mostrou-se primordial e o acesso de Caldwell-Luc eficaz, seguro e com bons resultados pós-operatórios, mesmo com as ausências de métodos de diagnóstico e manejo padronizados.(AU)

Introducción: El quiste dentígero es un quiste odontogénico, casi siempre asociado a la corona de un diente retenido. Sus ubicaciones más frecuentes son la zona de tercer molar inferior y canino superior. Objetivo: Caracterizar la presentación de un quiste dentígero en un paciente adulto. Presentación de caso: Se presentó el caso de una paciente de 27 años de edad que acude a consulta por presentar sensación de opresión, dolor e inflamación en región de tercer molar inferior izquierdo. Al examen oral se observa tejido dentario en correspondencia con molar 38 parcialmente brotado con ligero edema de la mucosa a su alrededor. En radiografía panorámica se observa imagen radiolúcida en relación con cara mesial del 38 con discreto halo radiopaco, redondeada, compatible con un quiste dentígero. Conclusiones: Un diagnóstico a tiempo de estas lesiones constituye la clave para un tratamiento exitoso y menos invasivo, lo cual evitaría complicaciones mayores como la malignización, el crecimiento exagerado que puede ocasionar deformidad facial y la disfunción masticatoria permanente(AU)


Introduction: Dentigerous cyst is an odontogenic cyst, almost always associated with the crown of a retained tooth. Its most frequent locations are the areas of the lower third molar and the upper canine. Objective: To characterize the presentation of a dentigerous cyst in an adult patient. Case presentation: The case is presented of a 27-year-old female patient who came to the clinic with a sensation of pressure, pain and inflammation in the region of the lower left third molar. Oral examination permits to observe some dental tissue corresponding with molar 38 partially erupted with slight edema of the surrounding mucosa. Panoramic radiograph shows a radiolucent image related to the mesial side of 38, with a discrete radiopaque halo, rounded, compatible with a dentigerous cyst. Conclusions: A timely diagnosis of these lesions is the key to a successful and less invasive treatment, which would avoid major complications, such as malignization, the exaggerated growth that can cause facial deformity and permanent masticatory dysfunction(AU)

OBJECTIVES: There is a scarcity of randomized clinical trials (RCT) that report medium- and long-term results and a lack of consensus in the literature on the predictability of immediately loaded unsplinted narrow diameter implants supporting mandibular overdentures. This RCT compared the performance of conventional (CL) and immediate loading (IL) of mandibular overdentures retained by two narrow-diameter implants for 3 years. MATERIALS AND METHODS: Patients from an RCT treated with CL or IL were invited to attend to 2- and 3-year follow-ups. Clinical, radiographic, functional, and oral health-related quality of life parameters were evaluated. Prosthetic maintenance events, biological complications, and success and survival rates were also recorded. The data were tested by multilevel mixed-effects linear regression analysis and chi-squared tests. RESULTS: The 1-year survival rates of 90% in the CL group and 85% in the IL group were maintained as no implants were lost between 1 and 3 years. The marginal bone loss (MBL) in the IL group was significantly lower after year 3 (-0.04; p < 0.01). Significant changes were found only for the intra-group comparisons in the third year of function: (i) CL and IL presented similar progression of implant stability, MBL, and posterior bone area resorption; (ii) while CL started deteriorating of masticatory function, IL still exhibited functional evolution and (iii) oral comfort domain in the CL and pain domain in the IL were improved. CONCLUSION: Although IL experienced the lowest MBL after 3 years, the outcomes showed that both loading protocols result in predictable medium-term rehabilitation when monitored annually. CLINICAL RELEVANCE: It can be expected that in the third year of function, patients with immediate loading may present more complaints related to general performance even with acceptable masticatory function and self-reported improvements in oral comfort.

BACKGROUND: This study aimed to evaluate the prevalence of retained primary teeth (RPT) associated with delayed permanent tooth eruption and the factors associated with this condition in German children. METHODS: This is a cross-sectional retrospective study that evaluated panoramic radiographs from orthodontic patients. The diagnosis of RPT was established according to Nolla developmental stage. The primary tooth was considered retained when its successor permanent tooth was in Nolla stage 8, 9, or 10. Statistical analysis was performed with an α of 5% (p < 0.05). RESULTS: A total of 102 children (48 girls and 54 boys), and 574 primary teeth and their respective permanent successor teeth were evaluated. We classified 192 teeth as RPT. Sixty-one (59.8%) children presented one or more RPT. Gender was not significantly different between RPT and control teeth (p = 0.838; odds ratio 0.95, confidence interval 95% 0.44-2.16). In the majority of the RPT cases (68.7%), no clear cause to explain the prolonged retention was identified. The pathological problems most commonly observed with RPT were dental fillings (19.3%), followed by dental caries (4.6%), and ectopic tooth eruption (2.1%). CONCLUSIONS: The incidence of RPT associated with delayed permanent tooth eruption in German children was high and the most common pathological condition associated with RPT was dental caries.

During any surgical procedure, complications may arise, some of which are fortuitous, whereas others, unfortunately, occur because of errors of the surgical team. Fortunately, most are minor and do not affect the patient's recovery, but others can cause severe morbidity and even mortality. A retained cotton or gauze surgical sponge inadvertently left in the body during an operation is known as a gossypiboma. This dreadful oversight is a marked complication that can cause serious postoperative complications, a severe economic burden on the healthcare system, and many medicolegal implications. We report the case of a 30-year-old male, who suffered a spinal fracture which was repaired through an anterior fixation approach 12 years ago in a local state hospital without complications. Suddenly, he presented with chest pain and cough, and sought medical attention. An 8 × 5 × 8 cm low-density heterogeneous mass was discovered on his chest; after successful surgery, a gossypiboma formed by several gauzes without radiopaque markers was discovered.

RESUMEN Los oblitos son cuerpos extraños olvidados dentro de la cavidad después de una intervención quirúrgica; su incidencia es desconocida, ya que no se informan todos los casos debido a su implicancia legal. Su forma de presentación más frecuente es el abdomen agudo obstructivo y las fístulas enterocutáneas. Se presenta el caso clínico de una paciente de sexo femenino con antecentes de cesárea abdominal hace 30 años, que consultó por una formación palpable a nivel de hipogastrio; en la tomografía se evidenció una tumoración alargada, heterogénea, calcificada y con burbujas en su interior. Se realizó una laparotomía exploradora, se constató una formación duro-pétrea con múltiples adherencias al intestino delgado circundante. Se realizó la resección y anastomosis del intestino comprometido. La anatomía patológica informó: cuerpo extraño en la luz intestinal, y presencia de adherencias interasas, con fístula enteroentérica, lo que constituye un caso poco frecuente.


ABSTRACT Retained foreign objects are any items left inside a patient after a surgery. Their incidence is unknown as they are often the cause of lawsuits and are not reported. They usual present as acute intestinal obstruction or enterocutaneous fistulas. We report the case of a 65-year-old female patient, with a history of a cesarean section 30 years before, who presented with a palpable tumor in the hypogastrium. The computed tomography scan revealed an elongated, heterogeneous, calcified mass with bubbles inside. On exploratory laparotomy a stony-hard formation was observed with multiple adhesions to the surrounding small bowel. The intestinal segment involved was resected and anastomosed. The pathology examination reported a retained foreign object in the intestinal lumen and presence of adhesions between loops with entero-enteric fistula, constituting a rare case.

Introducción: El hallazgo de un cuerpo extraño retenido, específicamente una hoja de cuchillo es infrecuente. En ocasiones, estos cuerpos extraños pasan inadvertidos y se diagnostican de forma diferida o retrasada. Objetivo: Describir el diagnóstico y tratamiento de un paciente con una hoja de cuchillo retenida en el hueco de la axila. Caso clínico: Varón de 25 años, agredido con un cuchillo, que sufrió una herida en la región posterior del hombro y fue suturado sin realizar ningún examen complementario. Dos semanas después acudió por molestias en el hombro lesionado, se indicó radiografía en la cual apareció una imagen radiopaca que correspondió con una hoja de cuchillo en proyección subescapular. Refería molestias del hombro e impotencia funcional. Se opera con anestesia general, se realiza incisión axilar, se identifica el objeto, en íntimo contacto con la vena axilar; la punta hacia el vértice axilar, cruzando bajo la vena axilar en su entrada al tórax. Se extrajo el cuerpo extraño bajo visión directa, sin complicaciones. El paciente evolucionó favorablemente. Conclusiones: Los cuerpos extraños retenidos, que pasan inadvertidos son raros, sin embargo, existen regiones como la axila en que por las características anatómicas del espacio se dificulta su diagnóstico. En este caso coincidió una combinación poco usual de fenómenos, la actitud defensiva del lesionado y la dirección de la herida, que propiciaron quedara la hoja en el hueco axilar.


Introduction: The finding of a retained foreign body, specifically a knife blade, is a rare event. Sometimes these foreign bodies go unnoticed and are diagnosed deferred or delayed. Objective: To describe the diagnosis and treatment of a patient with a knife blade retained on axillary hole. Clinical case: 25-year-old male, attacked with a knife, who suffered a wound in the posterior region of the shoulder and was sutured without performing any complementary examination. Two weeks later, he came due to discomfort from the injured shoulder. An X-ray of the shoulder was indicated, where a radiopaque image appeared that corresponds to the knife blade, in subscapular projection. He refers shoulder discomfort and functional impotence. It is operated under general anesthesia, axillary incision, the object is identified in intimate contact with the axillary vein, the tip towards the axillary vertex, crossing under the axillary vein at its entrance to the thorax, the foreign body is removed under direct vision and without complications. Evolved favorably. Conclusions: Retained foreign bodies that go unnoticed are rare. However, there are regions such as the armpit where, due to the anatomical characteristics of the space, their diagnosis is difficult. In this case, an unusual combination of phenomena coincided, the defensive attitude of the injured person and the direction of the lunge, which led to the blade remaining in the axillary hole.

La realización de un nuevo tratamiento luego de un fracaso con implantes dentarios representa un gran desafío para el profesional especialistas en implan-tología, así como para el paciente que debe someter-se a un nuevo procedimiento para rehabilitarse. En estos casos, el diagnóstico y la planificación exhaus-tiva previa nos permiten optimizar el tiempo y los re-cursos, arribando al resultado esperado de forma eficiente. En este reporte de caso se presenta la re-habilitación de una paciente que acude a la consulta con un fracaso implantario previo y con alta deman-da estética. Se diseñó una guía quirúrgica de preci-sión para la colocación de un implante Straumann®Bone Level Tapered 3,3 NC ­ Narrow CrossFit® ø 3,3 mm 8 mm junto a la regeneración ósea guiada para compensar la deficiencia de los tejidos circundantes. La rehabilitación protésica consistió en una corona cemento atornillada en disilicato de litio. En el control al año pudo valorarse la estabilidad de la rehabilita-ción protésica, así como de los tejidos circundantes y el contorno estético junto al perfil de emergencia mimético logrado al final del tratamiento (AU)


Carrying out a new treatment after a failure with dental implants represents a great challenge for the implantology specialist professional, as well as for the patient who must undergo a new procedure to rehabilitate. In these cases, the diagnosis and prior exhaustive planning allow us to optimize time and resources, arriving at the expected result efficiently. This case report presents the rehabilitation of a patient who comes to the clinic with a previous implant failure and with high aesthetic demand. A precision surgical guide was designed for the placement of a Straumann® Bone Level Tapered 3.3 NC ­ Narrow CrossFit® ø 3.3 mm 8 mm implant together with guided bone regeneration to compensate for the deficiency of the surrounding tissues. The prosthetic rehabilitation consisted of a screw-retained cement crown in lithium disilicate. In the one-year control, the stability of the prosthetic rehabilitation, as well as the surrounding tissues and the aesthetic contour, together with the mimetic emergence profile achieved at the end of the treatment, could be assessed (AU)

Resumen OBJETIVO: Plantear un procedimiento para pacientes con retención de restos corioplacentarios y riesgo de formación de fístulas arteriovenosas que sea efectivo, seguro y ambulatorio. MATERIALES Y MÉTODOS: Estudio de serie de casos, retrospectivo, de pacientes atendidas en el Hospital Gea González de enero a mayo de 2022. Se empleó un equipo de Bettocchi, de flujo continuo de 5 mm, lente cilíndrico de 2.9 mm y 30°. En casos específicos se programó un procedimiento quirúrgico con resectoscopio Richard Wolf Princess, con diámetro exterior de 7 mm, sistema óptico de 2.7 mm, dirección visual de 30°. RESULTADOS: Se revisaron 642 expedientes de los que se obtuvo una serie de 31 casos que se incluyeron conforme a la clasificación de Gutenberg, con biopsia por protocolo posaborto y resección con Grasper para los tipos 0 y 1; dos pacientes se enviaron a Urgencias por sangrado abundante para aspiración mecánica endouterina inmediata; dos pacientes se catalogaron con tipo 3; en la primera se optó por el tratamiento médico con metotrexato con lo que se consiguió la resolución completa y la segunda finalizó el embarazo con histerectomía laparoscópica. CONCLUSIONES: En pacientes con imágenes ecográficas sugerentes de retención de restos corioplacentarios o malformación arteriovenosa, los procedimientos con visión directa evitan las complicaciones inmediatas y futuras. El diagnóstico mediante la clasificación de Gutenberg permite definir el tipo de acceso con extracción en frío con pinza Grasper para los tipos 0 y 1, o la aplicación de una prueba farmacológica con metotrexato y resección histeroscópica para los tipos 2 y 3 en pacientes hemodinámicamente estables.


Abstract OBJECTIVE: To propose an effective, safe and outpatient procedure for patients with retained chorioplacental remnants and risk of arteriovenous fistula formation. MATERIALS AND METHODS: Retrospective case series study of patients attended at the Gea González Hospital from January to May 2022. A Bettocchi equipment was used, 5 mm continuous flow, 2.9 mm cylindrical lens and 30°. In specific cases a surgical procedure was scheduled with a Richard Wolf Princess resectoscope, 7 mm outer diameter, 2.7 mm optical system, 30° visual direction. RESULTS: We reviewed 642 files from which we obtained a series of 31 cases that were included according to the Gutenberg classification, with biopsy by postabortion protocol and resection with Grasper for types 0 and 1; two patients were sent to the ER for heavy bleeding for immediate MVA; two patients were categorized as type 3; in the first one we opted for medical treatment with methotrexate with which we achieved complete resolution and the second one ended the pregnancy with laparoscopic hysterectomy. CONCLUSIONS: In patients with ultrasound images suggestive of retained chorioplacental debris or arteriovenous malformation, direct vision procedures avoid immediate and future complications. Diagnosis by Gutenberg classification allows to define the type of access with cold extraction with Grasper forceps for types 0 and 1, or the application of a pharmacological test with methotrexate and hysteroscopic resection for types 2 and 3 in hemodynamically stable patients.
