RESUMO

BACKGROUND: Arsenic (As) is a well-recognized poison. Exposure may be of an acute nature, leading to high concentrations and acute arsenic poisoning. Chronic exposure may lead to benign skin changes, skin cancer, and internal malignancy. OBJECTIVE: Our purpose was to study the nature, incidence, and sequelae of skin disorders in a group of Argentinean patients suffering from chronic arsenicism due to the intake of contaminated well water. METHODS: All patients who presented with chronic arsenicism at the Dermatology Department of Hospital Posadas (Buenos Aires, Argentina) during a 10-year period (1988-1998) were included in this study. The patient group compromised 9 women and 14 men, the age range was 37-72 years. Diagnosis was based on the clinical triad (keratoderma, leucoderma and epiteliomatosis). We performed clinical, laboratory, and histopathologic studies to confirm diagnosis. We screened for possible internal diseases. RESULTS: All patients included in this study had cutaneous lesions associated with long-term arsenic exposure. The mean age of the patients was 58.2 years. The estimated mean time of the beginning of the lesions was of 3.7 years. All patients were Argentinean from endemic areas of our country where the arsenic levels are higher than those accepted by the World Health Organization. CONCLUSION: This study allows us to conclude that the relationship between arsenic and cancer is frequent and it describes the principal characteristics of this entity in our group of patients.

RESUMO

Chronic endemic hydroarsenicism in a 48-year-old man from Antofagasta, Chile, is reported. The literature on the global health problems of hydroarsenicism is reviewed, especially with regard to the carcinogenic action of arsenic.

RESUMO

El propósito de este estudio fué el de obtener información cuantitativa de los cambios del estroma subepitelial durante la carcinogénesis química experimental El carcinógeno DMBA fué aplicado localmente tres veces por semana durante cuatro semanas a la mucosa bucal (bolsa) y la palatina de veinte hamsters dorados machos, de cuatro a seis semanas de edad. A otros seis animales se les aplicó parafina líquida en sitios análogos, y diez animales no recibieron tratamiento. A las seis y doce semanas de iniciado el experimento, se tomaron muestras de tejido de paladar y mucosa bucal para microscopía de luz y electrónica, a cinco animales tratados con DMBA, a cinco no tratados y a tres tratados con parafina líquida. A las 21 semanas, la mucosa bucal de los animales tratados con dDMBA, mostró displasia epitelial o carcinoma escamocelular; la mucosa palatina no mostró cambios malignos o premalignos. Se tomaron fotomicrografías a un aumetno final de x14,210 de las secciones obtenidas de los bloques seleccionados al azar. Para calcular la densidad de volumen de colágeno, substancia intercelular, células del estroma y vasos sanguíneos, se aplicó la técnica de conteo de puntos. El analizador de imágenes proporcionó la densidad de volumen de las células del estroma y vasos sanguíneos. A las 6 y 12 semanas, la mucosa bucal tratada con DMBA mostró una disminución significativa de colágeno, un aumento significativo de substancia intercelular y un ligero aumento de vasos sanguíneos. A las 12 semanas hubo también un aumento significativo de las células del estroma. La mucosa palatina tratada con el carcinógeno no mostró cambios significativos en el estroma.

RESUMO

Two ulcerated basal cell carcinomas of the right lower leg developed almost 50 years after a psoriasis had been treated by oral arsenic therapy. While repeated biopsies proved negative, the lesions were thought to be chronic venous ulcers. During conservative treatment for 8 years, both ulcers showed slow progression. Multiple, aggressive biopsies revealed finally two basal cell carcinomas. Treatment consisted of radical excision of involved soft tissue and tibial bone. Closure of the resulting defect was accomplished by muscle-transposition and meshgraft. Normal function of the right leg could be preserved. Spontaneous fracture of the right lower leg occurred 3 years after surgical treatment and amputation was necessary because of extensive local recurrence.

RESUMO

Patients with chronic cutaneous arsenism presented with arsenical keratosis, guttate melanotic hyperpigmentation, Bowen´s disease, basal cell epithelioma and squamous cell carcinoma were treated with oral aromatic retinoid (Tigason) 1mg/kg/day. The patients were assessed by clinical and pathological studies. After three months of the treatment, only arsenical keratosis responded well to the therapy by showing desquamation of hyperkeratotic papules. Histophatological findings revealed reduction of hyperkeratosis and epidermal hyperplasia. Bowen`s disease and basal cell epithelioma responded partially to the treatment. On the other hand, this agent had no effect on malignant tumors e.g. squamous cell carcinoma and guttate malanosis induced by arsenic compound ingestion. The side effects were similar to other reports in the literature.
