BACKGROUND: The resolution of factors linked to the recurrence of cesarean section defects can be accomplished through a comprehensive technique that effectively addresses the dehiscent area, eliminates associated intraluminal fibrosis, and establishes a vascularized anterior wall by creating a sliding myometrial flap. OBJECTIVE: Propose a comprehensive surgical repair for recurrent and large low hysterotomy defects in women seeking pregnancy or recurrent spotting. STUDY DESIGN: A retrospective cohort analysis included 54 patients aged 25-41 with recurrent large cesarean scar defects treated at Otamendi, CEMIC, and Valle de Lili hospitals. Comprehensive surgical repair was performed by suprapubic laparotomy, involving a wide opening of the vesicouterine space, removal of the dehiscent cesarean scar and all intrauterine abnormal fibrous tissues, using a glide myometrial flap, and intramyometrial injection of autologous platelet-rich plasma. Qualitative variables were determined, and descriptive statistics were employed to analyze the data in absolute frequencies or percentages. The data obtained were processed using the InfostatTM statistic program. RESULTS: Following the repair, all women experienced normal menstrual cycles and demonstrated an adequate lower uterine segment thickness, with no evidence of healing defects. All patients experienced early ambulation and were discharged within 24 h. Uterine hemostasis was achieved at specific points, minimizing the use of electrocautery. The standard duration of the procedure was 60 min (skin-to-skin), and the average bleeding was 80-100 ml. No perioperative complications were recorded. A control T2-weighted MRI was performed six months after surgery. All patients displayed a clean, unobstructed endometrial cavity with a thick anterior wall (Median: 14.98 mm, IQR 13-17). Twelve patients became pregnant again, all delivered by cesarean between 36.1 and 38.0 weeks, with a mean of 37.17 weeks. The thickness of the uterine segment before cesarean ranged between 3 and 7 mm, with a mean of 3.91 mm. No cases of placenta previa, dehiscence, placenta accreta spectrum (PAS), or postpartum hemorrhage were reported. CONCLUSIONS: The comprehensive repair of recurrent low-large defects offers a holistic solution for addressing recurrent hysterotomy defects. Innovative repair concepts effectively address the wound defect and associated fibrosis, ensuring an appropriate myometrial thickness through a gliding myometrial flap.

La técnica anestésica de elección para operación cesárea es la anestesia subaracnoidea. La hipotensión el efecto adverso más frecuente de esta técnica anestésica y está relacionada con efectos deletéreos maternos y fetales que puede llegar a ocasionar graves complicaciones. Por esta razón es importante conocer las complicaciones y la aproximación terapéutica de la hipotensión materna, para disminuir la morbilidad y mortalidad del binomio materno-fetal. Las medidas para disminuir la hipotensión materna posterior al bloqueo subaracnoideo, tanto en el número de casos como en severidad de la misma son varias, pero la carga de fluidos se ha convertido en la piedra angular en la profilaxis de la hipotensión. Existen diversas alternativas para la prevención y manejo de la hipotensión arterial; una de ellas la fluidoterapia. El objetivo de esta revisión es evaluar la evidencia reciente disponible para las distintas alternativas de fluidoterapia y evaluar su efectividad en sus diferentes modalidades (precarga con coloides o cristaloides, cocarga con coloides o cristaloides), ninguna parece ser totalmente efectiva como monoterapia. El enfoque para la prevención y el tratamiento de la hipotensión debe ser multimodal. Por su disponibilidad y seguridad de uso, al día de hoy la cocarga con cristaloides asociada a terapia vasopresora alfa agonista parece ser la mejor alternativa...(AU)

OBJECTIVE: Placenta accreta spectrum (PAS) is a cause of massive obstetric hemorrhage and maternal mortality. The application of family-centered delivery techniques (FCDTs) during surgery to treat this disease is infrequent. We evaluate the implementation of FCDTs during PAS surgeries. METHODS: This was a prospective, descriptive study that included PAS patients undergoing surgical management over a 12-month period. The patients were divided according to whether FCDTs were applied (group 1) or not (group 2), and the clinical outcomes were measured. In addition, hospital anesthesiologists were surveyed to evaluate their opinions regarding the implementation of FCDTs during the surgical management of PAS. RESULTS: Thirteen patients with PAS were included. The implementation of FCDTs during birth was possible in 53.8% of the patients. The presence of a companion during surgery and skin-to-skin contact did not hinder interdisciplinary management in any case. CONCLUSION: Implementation of FCDTs during PAS care is possible in selected patients at centers with experience in managing this disease.


OBJETIVO: O espectro da placenta acreta (do inglês placenta accreta spectrum - PAS) é causa de hemorragia obstétrica maciça e mortalidade materna. A aplicação de técnicas de parto centrado na família (do inglês family-centered delivery techniques - FCDTs) durante a cirurgia para tratar esta doença é pouco frequente. Avaliamos a implementação das FCDTs durante as cirurgias do PAS. MéTODOS: Estudo prospectivo e descritivo que incluiu pacientes com PAS submetidos a tratamento cirúrgico durante um período de 12 meses. Os pacientes foram divididos de acordo com a aplicação de FCDTs (grupo 1) ou não (grupo 2), e os resultados clínicos foram medidos. Além disso, anestesiologistas hospitalares foram entrevistados para avaliar suas opiniões sobre a implementação das FCDTs durante o manejo cirúrgico do PAS. RESULTADOS: Foram incluídos 13 pacientes com PAS. A implementação de FCDTs durante o parto foi possível em 53,8% das pacientes. A presença do acompanhante durante a cirurgia e o contato pele a pele não prejudicou o manejo interdisciplinar em nenhum caso. CONCLUSãO: A implementação de FCDTs durante o atendimento do PAS é possível em pacientes selecionados em centros com experiência no manejo dessa doença.

Introducción: La cefalea pospunción dural es la complicación más habitual tras la anestesia neuroaxial, y es especialmente frecuente en obstetricia, un hallazgo común en el período posparto. Suele ser una complicación benigna y autolimitada, pero sin tratamiento puede conducir a otras complicaciones más graves. Objetivo: Describir la incidencia de cefalea pospunción dural en las pacientes obstétricas programadas para cesárea electiva con anestesia espinal y su relación con la deambulación precoz. Métodos: Se realizó un estudio observacional descriptivo en una serie de casos (50), todas las pacientes propuestas para cesárea electiva bajo el método anestésico espinal subaracnoideo con trocar calibre 25 en el período comprendido entre mayo a diciembre del 2018. Resultados: De un total de 50 pacientes estudiadas con edades entre 18 y 35 años de edad, al 96 por ciento se le realizó punción única de la duramadre, en todas se utilizó trócar 25, atraumático y ninguna presentó cefalea pospunción dural. Conclusiones: Se concluye que la incidencia de cefalea pospunción dural puede disminuir cuando se utilizan agujas espinales atraumáticas, de pequeño calibre; lo cual facilita también la deambulación temprana de la paciente(AU)


Introduction: Postdural puncture headache is the most common complication following neuraxial anesthesia, and is especially common in obstetrics, a common finding in the postpartum period. It is usually a benign and self-limited complication, but if not treated, it can lead to further serious complications. Objective: To describe the incidence of postdural puncture headache in obstetric patients scheduled for elective cesarean section with spinal anesthesia and its relationship with early ambulation. Methods: A descriptive observational study was carried out in a case series (50) of patients proposed for elective cesarean section under the subarachnoid spinal anesthesia method with 25-gauge trocar in the period from May to December 2018. Results: Out of a total of 50 patients aged 18-35 years who participated in the study, 96 percent underwent single dura mater puncture. In all cases, a 25-gauge trocar was used and none presented postdural puncture headache. Conclusions: The incidence of postdural puncture headache may be concluded to decrease when atraumatic spinal needles of small caliber are used, which also facilitates early ambulation of the patient(AU)

OBJECTIVES: The aim of this study was to perform neonatal clinical assessments at birth to identify newborn kittens at risk according to type of delivery, thus allowing immediate intervention and increasing their chances of survival. METHODS: This study compared Apgar scores, reflexes and clinical parameters (temperature, weight, blood glucose and peripheral oxygen saturation [SpO2]) between eutocic neonates and those delivered by emergency cesarean section. The animals were evaluated at birth and after 10 and 60 mins. RESULTS: Thirty-two neonates were evaluated, with 19 animals in the eutocic group (EG) and 13 animals in the cesarean group (CG). When comparing groups, CG neonates had significantly lower Apgar scores (P <0.0001), lower SpO2 (P = 0.0535), higher blood glucose (P = 0.0009), reduced reflexes (P <0.0001) and lower respiratory rates (P <0.0001) at birth and after 10 and 60 mins than EG neonates. Apgar scores positively correlated with parameters such as heart rate, reflex score, SpO2 and weight. The mortality rate in evaluated newborns was 15.6% (5/32). The early mortality rate (0-2 days old) was 80% (4/5) and the late mortality rate (3-30 days old) was 20% (1/5). CONCLUSIONS AND RELEVANCE: This study showed lower vitality in cats delivered by emergency cesarean section than in those delivered through eutocic birth. In general, neonates delivered by cesarean section have greater depression and low vitality at birth and may require advanced resuscitation procedures. The evaluations carried out in this study identified newborns with low vitality and those requiring advanced resuscitation, thus allowing immediate intervention. Apgar and reflex scores for feline neonates were suggested. Newborn-specific clinical assessment with these feline vitality scores allows the identification of at-risk neonates. Care immediately after birth increases the chance of survival among these patients.

OBJECTIVE: The training of groups responsible for managing patients with placenta accreta spectrum (PAS) is complex because of the lack of hospitals with a high flow of patients and absence of formal educational programs. We report here the results of a virtual training program (VTP) that implemented one-step conservative surgery (OSCS). METHODS: A prospective observation study of OSCS VTP between three expert groups and PAS reference hospitals without experience in OSCS was performed. Accessible or cost-efficient web meeting platforms were used to implement the VTP components: baseline observation of the participant's prior knowledge; instructions about essential PAS surgery topics; case selection and joint planning of surgery; expert group "telepresence" during surgery and postoperative debriefing. RESULTS: One-step conservative surgery was performed successfully at six hospitals. All patients had increta/percreta with a median intraoperative bleeding of 1300 ml (IQR 825-2325) and surgical time of 184 min (IQR 113-240). All groups considered the VTP very useful (n = 33, 97%) or useful (n = 1, 3%), they would use it again (definitely: n = 27, 81.8%; or probably: n = 6, 18.2%), and they would recommend it to other colleagues. CONCLUSION: Tele education and telepresence during PAS surgery facilitates the implementation of OSCS in selected cases.

A istmocele (ou "defeito cicatricial de cesariana") é uma alteração anatômica na parede uterina responsável por formar um "nicho", que é visualizado como uma área hipoecoica por histerografia ou por outros métodos de imagem, devido a uma cicatrização inapropriada de uma cesárea anterior. Essa modificação leva a complicações ginecológicas e obstétricas. Este estudo pretendeu analisar, entre os fatores de risco, a relação da istmocele com a técnica de fechamento uterino, comparando a sutura em camada simples com a dupla. Foi realizada uma revisão de literatura por meio de pesquisa bibliográfica em diferentes bases de dados eletrônicos. Foram encontrados 31 artigos, todavia apenas 13 estudos foram incluídos no presente estudo após avaliação criteriosa. A técnica de fechamento uterino é um dos fatores de risco possível de prevenir, sendo que a sutura em camada simples é associada a maior chance de se desenvolver a istmocele. A sutura de camada simples (ancorada) incluindo o endométrio está relacionada a menor espessura miometrial residual, associada a probabilidade de ruptura uterina e a istmocele, que a camada dupla não ancorada com a exclusão do endométrio.(AU)


The isthmocele is an anatomical alteration in the uterine wall responsible for forming a "niche", which is visualized as a hypoechoic area by hysterography or other imaging methods, due to inappropriate healing of a previous cesarean section. This modification leads to gynecological and obstetric complications. This study aimed to analyze among the risk factors the relation of isthmocele and the uterine closure technique comparing the single- and double-layer suture. It was conducted an electronic based search in different electronic databases. The research led to the retrieval of 31 articles; however only 13 studies were included in the present research after careful reading. The uterine closure technique is one of the possible risk factors to be modified; the single-layer suture is associated with a greater chance of developing isthmocele. The locked single-layer suture including the decidua is related to the decreased residual myometrial thickness, associated with the likelihood of uterine rupture and the isthmocele, than the double-layer unlocked excluding the decidua.(AU)

Existen múltiples técnicas y procedimientos quirúrgicos en la práctica clínica que todo médico general debería conocer y manejar ante una situación de urgencia que involucre la resolución inmediata. El no actuar en una situación emergente, ya sea por inexperiencia o desconocimiento puede poner en riesgo la vida del paciente. Entre estos procedimientos, podemos encontrar las suturas (y sus distintos tipos), la paracentesis, la apendicectomía (abierta y laparoscópica), la punción lumbar y la cesárea. Es por esto que se creó este artículo de educación médica utilizando un método explicativo de las principales técnicas quirúrgicas de forma resumida y concisa, basado en la literatura disponible, con el fin de servir como guía de bolsillo y revisión rápida para estudiantes, internos y médicos que deben enfrentarse a un procedimiento quirúrgico que no puede ser diferido.


There are multiple surgical techniques and procedures in the day to day clinical practice that every general practitioner should know how to do and handle in an emergency situation that requires immediate resolution. Failure to act in an emergent situation, either due to inexperience or ignorance, can put a patient's life at risk. Between these procedures there are sutures (and its variations), paracentesis, appendectomy (open and laparoscopic), lumbar puncture and cesarean delivery. That is why this medical education article was developed using an explicative method of the main surgical techniques in a summarized and concise way, based on the available literature, in order to serve as a pocket guide and quick review for students, interns and doctors who must face a surgical procedure that cannot be deferred.

BACKGROUND: Healthcare costs have substantially increased in recent years, threatening the population health. Obstetric care is a significant contributor to this scenario since it represents 20% of healthcare. The rate of cesarean sections (C-sections) has escalated worldwide. Evidence shows that cesarean delivery is not only more expensive, but it is also linked to poorer maternal and neonatal outcomes. This study assesses which type of delivery is associated with a higher healthcare value in low-risk pregnancies. RESULTS: A total of 9345 deliveries were analyzed. The C-section group had significantly worse rates of breastfeeding in the first hour after delivery (92.57% vs 88.43%, p < 0.001), a higher rate of intensive unit care (ICU) admission both for the mother and the newborn (0.8% vs 0.3%, p = 0.001; 6.7% vs 4.5%, p = 0.0078 respectively), and a higher average cost of hospitalization (BRL14,342.04 vs BRL12,230.03 considering mothers and babies). CONCLUSION: Cesarean deliveries in low-risk pregnancies were associated with a lower value delivery because in addition to being more expensive, they had worse perinatal outcomes.
