RESUMO

Coccidioidomycosis (CM) is an endemic fungal disease that is prevalent in the Southwestern United States, Mexico, and Central and South America. The majority of cases are asymptomatic. Symptomatic cases primarily present with pneumonic disease, varying in severity from mild to severe. In a minority, extrapulmonary disease is known to occur. These disseminated cases typically manifest as meningitis, osteomyelitis, synovitis, and integumentary. Coccidioidomycosis has been identified in nearly every part of the body, including the soft tissues and cartilaginous tissues of the oropharynx. Disseminated CM to the larynx specifically in the pediatric populations is rarely documented. This is a case of persistent stridor misdiagnosed as viral croup in a 9-month-old male with protracted disease. Herein, diagnosis and treatment of laryngeal mass found to be CM are defined with a salutatory outcome.

RESUMO

We conducted a retrospective review of medical records of patients with croup seen during the coronavirus disease 2019 pandemic. Approximately 50% underwent testing for severe acute respiratory syndrome coronavirus 2. During the Delta wave, 2.8% of those tested were positive for severe acute respiratory syndrome coronavirus 2; this increased to 48.2% during the Omicron wave, demonstrating a strong correlation between the Omicron variant and croup.

RESUMO

Croup is a frequent cause of outpatient and emergency consultation. It is frequently triggered by a viral respiratory infection and characterized by an abrupt onset. We describe current studies about the treatment of croup. The use of systemic corticosteroids stands out as choice treatment, and nebulized epinephrine as short term therapy while waiting for the anti-inflammatory effect of steroids.


El Croup (laringitis aguda) es motivo frecuente de consulta en servicios de urgencia y policlínicos pediátricos. Generalmente es gatillado por una infección respiratoria viral y se caracteriza por un inicio abrupto. En el presente texto se describen los estudios vigentes acerca de su tratamiento. Destacan los corticoides sistémicos como terapia de elección y la adrenalina nebulizada como terapia de acción corta en espera de la acción antinflamatoria esteroidal.

RESUMO

Las enfermedaes respiratorias infantiles constituyen la causa más frecuente de consulta al pediatra. Las características anatómicas de la vía aérea, la predisposición alérgica y las condiciones epidemiológicas hacen al niño más vulnerable al contagio con agentes virales. La laringotraqueobronquitis es una enfermedad de etiología viral y su curso normal dura aproximadamente una semana, descartando el tratamiento antibióticvo, en la mayoría de los casos. La evaluación médica es el método de disgnóstico más importante. Las medidas generales de tratamiento, como ambiente tranquilo, estímulos mínimos y reducción de estrés y dolor, son factores que favorecen la regresión espontánea de la obstrucción de la vía aérea

In a prospective investigation of 17 children with severe croup, we analyzed the effect of epinephrine inhalations and mild sedation with chloral hydrate on transcutaneous carbon dioxide pressure (tcPCO2), pulse oximetry measurements, and croup scores. There was a highly significant reduction (p less than 0.001) in the tcPCO2 values and croup scores after inhalation of epinephrine. The changes in the tcPCO2 values correlated with the clinical findings. Mild sedation also significantly improved the croup scores but failed to influence the tcPCO2 values. There was not statistically significant difference in pulse oximetry saturation, fraction of administered oxygen, heart rate, or respiratory rate before and after inhalation of epinephrine or chloral hydrate administration. Monitoring tcPCO2 appears to be a reliable and objective tool for managing patients with upper airway obstruction, whereas croup scores may be misleading.
