RESUMO

O coração é composto por duas importantes valvas átrio-ventriculares, a tricúspide e a bicúspide ou mitral. As válvulas cardíacas asseguram o sentido do fluxo sanguíneo dos átrios para os ventrículos e destes para as artérias. A endocardiose é um processo degenerativo crônico progressivo das válvulas, que acomete especialmente os cães, denominada endocardiose ou "doença do cão velho". Embora qualquer das válvulas possa ser afetada, é a válvula mitral que é habitualmente envolvida, isoladamente ou em combinação com a válvula tricúspide. A sua prevalência tem sido associada a parâmetros como idade, sexo e raça, afetando especialmente animais adultos de raças de pequeno porte. A etiologia da endocardiose é desconhecida, mas parece haver uma base hereditária. As lesões macroscópicas iniciais consistem em pequenos nódulos nas margens livres da valva, ficando distorcidas e com suas dimensões reduzidas, mas significativamente espessas e de coloração opaca, branca ou reluzente. Microscopicamente, a endocardiose é vista como um processo degenerativo do tecido valvular com discreta infiltração de tecido conjuntivo fibroso. Em estágios iniciais a doença apresenta, à auscultação, murmúrios cardíacos de baixa intensidade, sem sinais de descompensação e é geralmente achado incidental durante a rotina de exame clínico. As cúspides fundem-se em sua inserção, mas separam-se em direção ao centro da abertura, ajustando-se estreitamente ao mesmo tempo quando a valva se fecha e suas bordas livres são espessas e irregulares, especialmente durante uma fase tardia da vida. Com isso, o objetivo do presente trabalho foi averiguar as principais lesões, a epidemiologia e o grau de prevalência da endocardiose em cães na região noroeste do estado do Rio Grande do Sul, o que se verificou que não há casos frequentes na rotina clínica de pequenos animais de endocardiose. Não sendo possível verificar se há um sexo mais predisposto a apresentar a endocardiose. As lesões observadas na macroscopia eram referentes a nódulos brilhantes, e na microscopia presença de colágeno causando o espessamento da cúspide, além de achados inflamatórios.(AU)


The heart is composed of two important atrio-ventricular valves, the tricuspid and the bicuspid or mitral valve. The heart valves ensure the direction of blood flow from the atria to the ventricles and from the ventricles to the arteries. Endocardiosis is a chronic progressive degenerative process of the valves that affects dogs in particular, called endocardiosis or "old dog's disease". Although any of the valves can be affected, it is the mitral valve that is usually involved, either alone or in combination with the tricuspid valve. Its prevalence has been associated with parameters such as age, sex, and breed, affecting especially adult animals of small breeds. The etiology of endocardiosis is unknown, but there appears to be a hereditary basis. The initial macroscopic lesions consist of small nodules on the free margins of the valve, becoming distorted and reduced in size, but significantly thickened and opaque, white or shiny in color. Microscopically, endocardiosis is seen as a degenerative process of the valvular tissue with a discrete infiltration of fibrous connective tissue. In early stages the disease presents, on auscultation, with low-intensity cardiac murmurs without signs of decompensation and is usually an incidental finding during routine clinical examination. The leaflets fuse at their insertion, but separate towards the center of the opening, fitting closely together when the valve closes, and their free edges are thick and irregular, especially during a late phase of life. Thus, the aim of this study was to investigate the main lesions, the epidemiology and the degree of prevalence of endocardiosis in dogs in the northwest region of the state of Rio Grande do Sul, which was found that there are no frequent cases in the clinical routine of small animals of endocardiosis. It was not possible to verify whether there is a sex more predisposed to present endocardiosis. The lesions observed at macroscopy were referring to bright nodules, and at microscopy the presence of collagen causing thickening of the leaflet, besides inflammatory findings.(AU)


El corazón está compuesto por dos importantes válvulas aurículo- ventriculares, la tricúspide y la bicúspide o válvula mitral. Las válvulas cardíacas aseguran la dirección del flujo sanguíneo de las aurículas a los ventrículos y de los ventrículos a las arterias. La endocardiosis es un proceso degenerativo crónico y progresivo de las válvulas que afecta sobre todo a los perros, denominado endocardiosis o "enfermedad del perro viejo". Aunque cualquiera de las válvulas puede verse afectada, es la válvula mitral la que suele estar implicada, ya sea sola o en combinación con la válvula tricúspide. Su prevalencia se ha asociado a parámetros como la edad, el sexo y la raza, afectando especialmente a animales adultos de razas pequeñas. La etiología de la endocardiosis es desconocida, pero parece tener una base hereditaria. Las lesiones macroscópicas iniciales consisten en pequeños nódulos en los márgenes libres de la válvula, que se distorsionan y reducen de tamaño, pero se engrosan significativamente y adquieren un color opaco, blanco o brillante. Microscópicamente, la endocardiosis se observa como un proceso degenerativo del tejido valvular con una discreta infiltración de tejido conectivo fibroso. En fases tempranas, la enfermedad se presenta, en la auscultación, con soplos cardíacos de baja intensidad sin signos de descompensación y suele ser un hallazgo incidental durante el examen clínico rutinario. Las valvas se fusionan en su inserción, pero se separan hacia el centro de la apertura, encajando estrechamente cuando la válvula se cierra, y sus bordes libres son gruesos e irregulares, especialmente durante una fase tardía de la vida. Por lo tanto, el objetivo de este estudio fue investigar las principales lesiones, la epidemiología y el grado de prevalencia de endocardiosis en perros de la región noroeste del estado de Rio Grande do Sul, que se encontró que no hay casos frecuentes en la rutina clínica de pequeños animales de endocardiosis. No fue posible verificar si existe un sexo más predispuesto a presentar endocardiosis. Las lesiones observadas en la macroscopia fueron referentes a nódulos brillantes, y en la microscopia la presencia de colágeno causando engrosamiento de la valva, además de hallazgos inflamatorios.(AU)

Detection of proximal carious lesions involves the combination of clinical and radiographic methods, both with inherent difficulties. The present cross-sectional study is aimed at estimating the prevalence of cavitation in proximal carious lesions, based on a direct clinical assessment of previously detected radiographic lesions, in permanent molars and premolars. Proximal dental surfaces were radiographically evaluated using the ADA coding system and cavitation was determined through clinical visual examination of the surfaces after separation with elastomeric bands. One-hundred and twenty-six patients attending the dental clinics at the University of Talca were examined, comprising 508 proximal surfaces with radiographic codes ranging from E1 to D3. Two examiners were trained and calibrated for radiographic and clinical detection of proximal lesions. Most participants were females (61.9%). The age mean of participants was 28.7 (0.8) years old. A total of 22.2% of the examined surfaces were cavitated. Only few lesions coded as E1 (n = 4; 2.1%) and E2 (n = 9; 9.8%) were cavitated. Fifty D1 (35.5%) and 22 D2 (41.5%) lesions were cavitated after separation. Most lesions coded as D3 (n = 28; 84.8%) were cavitated. The multilevel binary regression model (p = 0.003) demonstrated that sex, age, jaw, tooth type, surface, and side were not associated with the likelihood of having proximal cavitation. Challenging conventional wisdom, most D1 and D2 lesions were not cavitated. Combining detection methods seems desirable to increase the accuracy in assessing approximal posterior lesions. The low proportion of cavitated lesions reinforces the idea of cautiously indicating invasive approaches for managing proximal carious lesions.

Orthodontics with low friction, low force, passive self-ligating brackets produces alveolar-dental remodeling, resulting in an increase in the transverse diameter of the dental arches, especially in the premolar sector. The aim of this study was to compare the modifications of the transverse diameter in the pre- and post-dental alignment cast models with orthodontics with passive self-ligating brackets in patients with moderate to severe dental discrepancy. The study included 28 patients of both sexes aged 16 to 48 years with dental discrepancies between -6 and -16 mm, treated with self-ligating Damon brackets and thermally activated Nickel- Titanium-Copper arches. With a digital pachymeter, Mitutoyo brand, five measurements were taken per dental arch: distance between canines (C), first premolars (1PM), second premolars (2 PM), first molars (1M) and second molars (2M), before and after orthodontic alignment. The variations were statistically evaluated by Student T Test for paired samples. Average distance between teeth varied with dental alignment in both jaws. The greatest increases in transverse diameter were recorded in the premolar areas. Minor though statistically significant variations also occurred in the 2M of the maxilla and in the C of the mandible In alignment with passive self-ligating brackets, there is an increase in the transverse diameter due to the development of the dental arches, mainly in the premolar sector of both jaws and also at the level of the canines in the mandible.


La ortodoncia con brackets autoligables pasivos, de baja fricción y baja fuerza, produce remodelación alvéolodentaria; con aumento del diámetro transversal de las arcadas, especialmente en el sector de los premolares. El objetivo de este trabajo fue comparar las modificaciones del diámetro transversal en los modelos de yeso pre y post alineación dentaria con ortodoncia con brackets autoligables pasivos en pacientes con discrepancia dentaria moderada a severa. El estudio incluyó 28 pacientes de ambos sexos de entre 16 y 48 años de edad con discrepancia dentaria entre -6 y -16mm, tratados con brackets autoligables sistema Damon y arcos termo activables de Níquel-Titanio-Cobre. Con un paquímetro digital marca Mitutoyo, se tomaron 5 medidas por arcada: distancia entre caninos (C), primeros y segundos premolares (1 PM y 2 PM) y primeros y segundos molares (1M y 2M); pre y post alineación ortodóncica. Las variaciones registradas fueron evaluadas estadísticamente mediante Student T Test para muestras apareadas. La distancia promedio entre dientes varió con la alineación dentaria en ambos maxilares. Los mayores aumentos del diámetro transversal se registraron en las zonas de los premolares. Se produjeron también variaciones menores, aunque estadísticamente significativas, en los 2M del maxilar superior y en los C del maxilar inferior. En la alineación con brackets autoligables pasivos, hay incremento del diámetro transversal por desarrollo de las arcadas, principalmente en el sector premolar de ambos maxilares y en el maxilar inferior también a nivel de los caninos.

Resumen Objetivo: Comparar in vitro la microfiltración apical en premolares unirradiculares obturados con cemento a base de resina epóxica y cemento biocerámico. Método: 40 piezas dentales unirradiculares fueron seleccionadas y divididas en dos grupos para ser obturadas esperando el tiempo de fraguado de acuerdo con las instrucciones del fabricante. El grupo A se obturó con cemento biocerámico Endosequence y, el grupo B, con cemento a base de resina epóxica AH-Plus: posteriormente los dos grupos fueron sellados con barniz de uñas hasta 3mm de la parte apical, luego se colocaron las muestras en la incubadora a 37ºC. Ambos fueron sometidos a 750 ciclos de termociclado, para reproducir el ambiente parecido a la cavidad oral. Para análisis de microfiltración se utilizó el método de difusión del colorante, se sumergieron en azul de metileno al 2 % por 6 días, posteriormente los especímenes fueron sometidos en una bomba de vacío. Los dientes fueron cortados longitudinalmente para ser evaluadas mediante el estereomicroscopio. El análisis estadístico fue hecho mediante T-Student y Chi-Cuadrado. Resultados: El grupo A corresponde a Endosequence obtuvo una medida de 0,55mm y el grupo B corresponde a AH-Plus 1,20mm con un estimado de significancia de p=0,013. Conclusión: El análisis de ambos cementos de obturación demostró que Endosequence posee menor microfiltración apical que AH-Plus.


Abstract Aim: To compare in vitro by the stereomiscroscope the lesser apical microfiltration in uniradicular premolars, between the cement based on Epoxy Resin and Bioceramic cement. Method: The study was performed on 40 uniradicular dental pieces divided into two groups, the samples were prepared using the protaper system and the obturation was performed using the lateral condensation technique, the first group was sealed withEndosequence bioceramic cement and the second group with Cement based on AH-Plus epoxy resin, then sealed with nail varnish up to 3mm from the apical part, then the samples were placed in the incubator at 37ºC, waiting for the set time corresponding to each group according to the manufacturer's instructions . The two groups of the present study were submitted to 750 cycles of thermocycling, to provide an environment similar to the oral cavity, then longitudinal cuts were made to the samples. The microfiltration was evaluated using the dye diffusion method, immersed in 2% methylene blue for 6 days, then the specimens were subjected to a vacuum pump. The teeth were cut longitudinally to be evaluated by the stereomicroscope. The statistical analysis was through T-Student and Chi-Square. Results: Group A corresponds to Endosequence obtained a mean of 0,55mm and Group B corresponds to AH-Plus 1,20mm with an estimated significance of p = 0,013. Conclusion: Both obturation cements have significant differences, therefore Endosequence has less apical microfiltration than AHPlus.

OBJECTIVE: The aim of this in vitro study, is to evaluate the penetration of a bioceramic root canal sealer into dentinal tubules at 3 mm and 5 mm from the apex after Nd:YAG laser irradiation. METHODS: Forty freshly extracted human mandibular premolars were prepared using Reciproc® and irrigated with 17% ethylenediaminetetraacetic acid (EDTA). Teeth were divided into 4 groups: group 1, obturated with control sealer (AH Plus®); group 2, obturated with bioceramic sealer (Endosequence BC Sealer®); group 3, Nd:YAG laser + control sealer (AH Plus®); and group 4, Nd:YAG laser + bioceramic sealer (Endosequence BC Sealer®). The samples were transversely sectioned 3 mm and 5 mm from the apex and examined using confocal laser scanning microscopy. Two parameters were measured: 1) sealer penetration into dentinal tubules of the root canal and 2) sealer penetration into the perimeter of the root canal walls. RESULTS: Penetration analysis showed that bioceramic sealer had a higher penetration at depths of 3 and 5 mm than that of the control sealer, regardless of laser use (p <0.05). Perimeter analysis showed that there was no difference between both sealers at a depth of 3 mm (p <0.05), regardless of laser use. At a depth of 5 mm, bioceramic sealer and laser showed a greater perimeter of penetration (p <0.05) than the control sealer. CONCLUSION: The use of Nd:YAG laser did not compromise the penetration of bioceramic sealer into dentinal tubules of root canals at 3 mm and 5 mm from the apex.

OBJECTIVES: This randomized controlled clinical trial evaluated the efficacy of sealing carious dentin in controlling the progression of lesions in primary molars for 2-year follow-up. MATERIALS AND METHODS: Children (6.79 ±â€¯1.81 years, n = 28) presenting primary molars with occlusal caries in the outer half of dentine were randomized and allocated into 2 groups: test (sealing caries with a flowable resin - SC) and control (partial removal of caries followed by restoration - PRC). The primary outcomes were: the clinical success of restorations evaluated by USPHS criteria and the radiographic analysis of caries progression. The children anxiety was evaluated by a Facial Image Scale; and the time required to perform the treatments was registered. RESULTS: In 21 patients evaluated after 2 years, 48 primary molars were analyzed. Clinically, there was no difference between the groups. There was no difference between treatments (p = 0.848) considering lesion progression. The anxiety level did not change after the two interventions (p = 0.650). The treatment time of SC (9.03 ±â€¯1.91 min) was lower (p = 0.002) than the PRC time (17.13 ±â€¯5.26 min). CONCLUSION: Sealing carious dentin may be used in dentistry since it did not alter the children anxiety, reduced the chair time and demonstrated clinical success rate and no radiographic difference in relation to the partial caries removal followed by restoration.

OBJECTIVES: With this in vitro study we aimed to assess the possibility of precise application of sealant on accessible artificial white spot lesions (WSL) on approximal surfaces next to a tooth surface under operative treatment. A secondary aim was to evaluate whether the use of magnifying glasses improved the application precision. MATERIAL AND METHODS: Fifty-six extracted premolars were selected, approximal WSL lesions were created with 15% HCl gel and standardized photographs were taken. The premolars were mounted in plaster-models in contact with a neighbouring molar with Class II/I-II restoration (Sample 1) or approximal, cavitated dentin lesion (Sample 2). The restorations or the lesion were removed, and Clinpro Sealant was placed over the WSL. Magnifying glasses were used when sealing half the study material. The sealed premolar was removed from the plaster-model and photographed. Adobe Photoshop was used to measure the size of WSL and sealed area. The degree of match between the areas was determined in Photoshop. RESULTS: Interclass agreement for WSL, sealed, and matched areas were found as excellent (κ = 0.98-0.99). The sealant covered 48-100% of the WSL-area (median = 93%) in Sample 1 and 68-100% of the WSL-area (median = 95%) in Sample 2. No statistical differences were observed concerning uncovered proportions of the WSL-area between groups with and without using magnifying glasses (p values ≥ .19). However, overextended sealed areas were more pronounced when magnification was used (p = .01). The precision did not differ between the samples (p = .31). CONCLUSIONS: It was possible to seal accessible approximal lesions with high precision. Use of magnifying glasses did not improve the precision.

RESUMO

The present study reports the treatment carried out in a patient with mandibular second premolar agenesis associated with early loss of a deciduous second molar, deep overbite, severe overjet and dentoalveolar bimaxillary protrusion, which led to lip incompetence and a convex facial profile. The main objectives of this treatment were: to eliminate the spaces in mandibular arch, correct overbite, as well as eliminate bimaxillary protrusion and lip incompetence, thus leading to a balanced profile. The case was presented to the Brazilian Board of Orthodontics and Dentofacial Orthopedics (BBO) as part of the requirements to obtain the title of BBO diplomate.

INTRODUCTION: The purpose of this study was to compare the shaping properties of TRUShape and Reciproc systems during preparation of oval-shaped canals from contralateral teeth by using micro-computed tomography scanning. METHODS: Thirteen pairs of freshly extracted contralateral mandibular premolars with a single oval-shaped canal were selected and divided into 2 groups according to the root canal instrumentation technique, TRUShape and Reciproc. The final apical size of instrumentation was 40/.06 for both systems. Data were statistically analyzed for the increase in canal volume and surface area and the amount of unprepared areas promoted by the 2 systems. RESULTS: Preparation with both systems significantly increased the volume and area of the canals (P < .001), with no significant differences between them (P > .05). When the full canal length was evaluated, the amount of uninstrumented areas was significantly larger for Reciproc (30%) than TRUShape (24%) (P < .05). However, analysis restricted to the apical 4-mm segment revealed no statistically significant difference in untouched areas (25% for Reciproc and 20% for TRUShape) (P > .05). CONCLUSIONS: Both instrument systems behaved similarly in promoting the increase of root canal volume and surface area. When the full canal length was evaluated, TRUShape instruments showed less unprepared areas than Reciproc. However, this difference did not occur for the apical part of the root canal. None of the techniques could completely prepare oval-shaped root canals.
