RESUMO
We report a case of transverse myelitis in an immunocompetent host with an atypical long onset of symptoms. A 56-year-old man was admitted to the hospital reporting 5 months of progressive ascending lower extremity weakness and numbness, inability to walk, bowel incontinence,urinary retention and several episodes of nausea and vomiting. MRI showed moderate spinal swelling and multiple hyperintense signal changes on cervical levels C2-C5 and thoracic levels T1-T3. Cerebrospinal fluid (CSF) showed pleocytosis and was positive for anti-cytomegalovirus (CMV) IgG intrathecal antibodies, but the CSF PCR for CMV was negative. The diagnosis of immune-mediated CMV-related transverse myelitis was established and the patient was treated with methylprednisolone and valgancyclovir. The patient had poor recovery and remained paraplegic at discharge.

Cytomegalovirus infection during pregnancy is associated with subtle and severe fetal disease including abortion or fetal death. Such complications depend on many factors including age of gestation at the moment of infection, and whether or not it is a primary maternal infection. A controversial point is if cytomegalovirus infection is capable of causing fetal growth restriction and fetal death due to placental insufficiency. In this article we report a case of fetal growth and umbilical artery Doppler velocimetry with a typical patron of intrauterine growth restriction due to placental insufficiency. We also made a review of medical literature about this topic.

Cytomegalovirus is the most important viral infection in kidney transplants, but rarely affects the allograft after the sixth month posttransplantation. We present a patient who developed renal failure eighteen months posttransplant; a kidney biopsy showed cytomegalovirus inclusions, acute tubular necrosis and mild interstitial nephritis. After intravenous ganciclovir, renal function transiently improved. Cytomegalovirus pp65 antigen was weekly reported as negative. One month later another biopsy was performed due to renal failure. The findings were consistent with tubular atrophy and severe interstitial nephritis. No cytomegalovirus cellular inclusions were found on histology, including immunohistochemical and polymerase chain reaction studies; pp65 antigen studies were persistently negative. Despite an attempt to recover renal function with steroid therapy, the patient restarted hemodialysis 20 months posttransplantation. This report suggests that cytomegalovirus should be considered as a late cause of kidney failure even in the absence of infection-related symptoms. The irreversible allograft damage can be caused despite the successful eradication of the virus with intravenous ganciclovir.

BACKGROUND: Cytomegalovirus (HCMV) has been described in abortion tissues, but seldom associated with inclusion bodies. However, the possible pathogenic role of this virus in abortion is under discussion. We attempted to verify the relationship between HCMV antigens in tissues from first trimester abortions presenting with inflammatory lesions. MATERIAL/METHODS: Sixteen cases of first trimester abortions with inflammatory lesions were selected from 340 cases studied at the Pathology Unit of the University Hospital in Vit ria, Esp rito Santo State, Brazil. Paraffin-embedded sections were subjected to indirect immunofluorescence (IFI) and immunoperoxidase (PAP), using monoclonal antibodies directed against immediate early (IEA) and late antigens (LA) of HCMV. RESULTS: Twelve out of sixteen cases (75%) presented at least one HCMV antigen. Eight cases presented both antigens, three cases only the IEA and one case only the LA. These antigens were present in decidual cells, in stromal cells of chorionic villi and in trophoblastic cells. CONCLUSIONS: The results showed high frequency of HCMV antigens in tissues from first trimester abortion. This frequency, higher than that previously reported, was probably due to the fact that necro-inflammatory lesions were always present in selected cases. The presence of LA in trophoblastic cells is evidence for cell permissiveness to viral replication in vivo. The results showing high presence of HCMV antigens in tissues from abortion with inflammatory lesions suggest a possible relationship of HCMV infection with inflammation and pregnancy loss.

Se evaluó la incidencia de alteraciones oculares en infantes nacidos de madres con titulación positiva contra el complejo TORCH y la infección por sífilis. Incluimos a 33 pacientes, captados en el periodo de septiembre de 1997 a junio de 1998. Se valoró la presencia de alteraciones oculares congénitas en infantes con antecedentes de infección intrauterina por el síndrome de TORCH. Se incluyeron 8 mujeres (24.2 por ciento) y 25 hombres (75.7 por ciento). Las lesione que más se encontró fue: catarata en 6 pacientes (18.1 por ciento), coloboma en 2 pacientes (6.0 por ciento), microftalmía, entropión, queratoconjuntivitis, masa retrobulbar, desprendimiento de retina, coriorretinitis y atrofia óptica en los restantes pacientes (3.0 por ciento). Las infecciones intrauterinas influyen en el dessarrollo del ojo. Estos datos sugieren que el diagnóstico temprano de tales infecciones por el complejo TORCH permite realizar el tratamiento adecuado

We report 4 unrelated patients with characteristic microscopic findings of microvillous inclusion disease (MID) with early-onset phenotype. All 4 patients came from the Navajo reservation in northern Arizona. A literature search revealed a fifth unrelated Navajo child with MID. The unusually high incidence in this population indicates that a founder effect might be responsible for an increased frequency of this rare genetic disorder in the Navajo. It is recommended that all Navajo infants presenting with severe diarrhea during early infancy undergo investigation for MID.

La infección por citomegalovirus humano es una de las principales causas de morbi-mortalidad en los hospederos inmunocomprometidos, entre ellos, los receptores de transplantes y los individuos con sida. También se han asociado con enfermedad congénita que causa principalmente sordera progresiva y retardo mental. Este virus tiene la capacidad de permenecer latente durante toda la vida del hospedero una vez lo ha infectado. Por lo tanto, no sólo se requiere el diagnóstico de infección primaria sino también el de reativación viral. En este módulo se describen la patogénesis, la epidemiología y las manifestaciones clínicas asociadas a la infección por este virus en los diferentes grupos de pacientes afectados (hospedero inmunocompetente, inmunocomprometido y recién nacido con infección congénita). Por último, se discuten las principales pruebas diagnósticas, su utilidad e interpretación.

Estudia las infecciones intestinales en pacientes con SIDA, las que les provocan diarrea con compromiso del intestino delgado y/o del colon. Se identifican los principales agentes patógenos y las manifestaciones clínicas de las infecciones que producen enfermedad primaria intestinal (Cryptosporidium, Enterocytozoon bieneusi, Isospora belli, Giardia lamblia, Salmonella, Shigella, Campylobacer, Clostridium difficile, Virus Herpes Simplex) y que producen compromiso intestinal secundario a infecciones sistémicas (Cytomegalovirus, Mycobacterium avium intracelular, Histoplasma capsulatum) y otros patógenos (Blastocystis hominis, Entamoeba hystolitica y otras). Se discuten las hallazgos relacionados con la enteropatía idiopática asociada al SIDA y a los neoplasmas intestinales (Sarcoma de Kaposi y linfomas). Se exponen algunos estudios realizados sobre tratamiento de estas infecciones (drogas empleadas, respuesta a las mismas y efectos adversos observados)
