RESUMO

Larva migrans cutánea es el resultado de una infección por nematodos y muestra una erupción serpiginosa característica. Con el aumento de los viajes a zonas tropicales, muchos pacientes pueden volver con ésta infección, que es a menudo, mal diagnosticada o tratada incorrectamente. Esta enfermedad cutánea parasitaria es causada por la migración de larvas provenientes de animales en la epidermis humana.


Cutaneous larva migrans is a result of a nematode infection and shows a characteristic creeping eruption. As travel to the tropics zone increases, many patients may be returning with this infection, which is often misdiagnosed or treated incorrectly. This parasitic skin disease is caused by the migration of animal hookworm larvae in the human epidermis.

BACKGROUND: Cutaneous larva migrans (CLM) is a common but neglected parasitic skin disease in impoverished communities of the developing world. METHODS: To describe the clinical features and the morbidity associated with CLM, active-case finding was performed during rainy and dry seasons in a rural community in the state of Alagoas, Northeast Brazil. RESULTS: A total of 62 individuals received a diagnosis for CLM, and among them there were a total of 75 larval tracks. The number of tracks per person ranged from 1 to 3. Children <9 years old were affected significantly more often than other age groups (P<.001). Children had their lesions mainly on the buttocks, genitals, and hands, whereas in older patients, the majority of lesions were located on the feet. Twenty-four percent of the 75 lesions were superinfected. Bacterial superinfection was more common among children. All but 1 patient complained about itching, and 84% of the 62 patients complained about sleep disturbances. Itching was classified as being severe by 61% of patients. Approximately 40% of the 62 patients had attempted to treat their creeping eruption previously. Larvae moved forward 2.7 mm per day. There was a significant correlation between the length of the track and the duration of infestation ( rho =.53; P<.0001). CONCLUSIONS: CLM is a common parasitic skin disease in this rural community, with different topographic patterns of lesions in children and adults. CLM is associated with considerable morbidity. The length of the track can be used to estimate the duration of infestation.

Two patients (a 35-year-old woman and a 33-year-old man, just returned from trips to Jamaica and Barbados, respectively) presented with cutaneous larva migrans with folliculitis, a rare and atypical clinical presentation of this infestation. Histopathological examination revealed a follicular and perifollicular infiltrate prevalently consisting of lymphocytes and neutrophils, with numerous eosinophils. Both patients were successfully treated with oral albendazole.

RESUMO

Cutaneous larva migrans (CLM) is a ubiquitous self-limited skin eruption, most frequently caused by the larvae of dog and cat hookworms. Although CLM is most frequent in tropical climates, the infection is becoming more common in urban areas. CLM has been frequently misdiagnosed and/or treated inappropriately, and mimics rheumatic, infectious, vascular, or dermatologic diseases. We here in report the clinical presentation and management of 18 cases of CLM.

Os autores apresentam 5 casos de pacientes com diagnóstico fundoscópico estabelecido como DUSN e avaliam nestes pacientes a presença de Larva Migrans Cutânea. Em apenas um paciente foram encontradas lesöes cicatriciais da Larva Migrans Cutânea. Estes achados sugerem ser a investigaçäo dermatológica de relativa importância em casos de DUSN

RESUMO

The case of a patient with thoracic abdominal and left arm, migratory erythema-edematous plates over a space of three years is described. The histopathologic features reveal a eosinophilic panniculitis due to probable larva migrans (gnathostomiasis). This appears to be the first report on the subject because we have not found any reference to the subject in Argentina bibliographic search.
