RESUMO

Abstract We report a case of Mycobacterium abscessus subsp. bolletii colonization in upper respiratory tract of an immunocompetent patient, who was misdiagnosed as tuberculosis by Acid Fast Bacilli (AFB) and cord factor formation observed directly from the sputa culture in liquid medium. This fact reflected a significant impact on the individual case's life and showed the importance to identify the mycobacteria isolated from clinical sample at species level, and to determine the true implication of nontuberculous mycobacteria (NTM) detected in clinical samples.

RESUMO

Bedaquiline (BDQ) has been proven to be effective in the treatment of multidrug-resistant tuberculosis. We hypothesized that BDQ could be a potential agent to treat nontuberculous mycobacterial (NTM) infection. The objective of this study was to evaluate the in vitro activity of BDQ against rapidly growing mycobacteria by assessing the minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) against 18 NTM strains. For MIC determination we performed the resazurin microtitre assay broth dilution, and for the MBC the c.f.u. was determined. BDQ exhibited a strong inhibitory effect against most NTM tested; however, for some NTM strains the MBC was significantly higher than the MIC. A new finding is that Mycobacterium flavescens has a mutation in the gene atpE associated with natural resistance to BDQ. These preliminary promising results demonstrate that BDQ could be potentially useful for the treatment of NTM.

RESUMO

The susceptibility of rapidly growing mycobacteria (RGM) to linezolid and ciprofloxacin was evaluated by using resazurin as a growth indicator. The assay with resazurin supplemented medium performed as well as its addition to the medium at reading time and was efficient for the determination for the susceptibility profile in RGM.

Structural analysis of mycolic acids from Mycobacterium simiae (including some 'habana' strains) was carried out using (1)H-NMR and MS. Results indicated that this species presents a general pattern of alpha-, alpha'- and keto-mycolates. alpha-Mycolates were composed of a complex mixture of 82 to 89 carbon atoms (C82-C89), with the predominant molecular species containing two di-substituted cyclopropane rings. Among keto-mycolates (C84-C89), those containing one trans di-substituted cyclopropane ring were the most abundant. The alpha'-mycolates were monounsaturated (C64, C66). According to MS and (1)H-NMR data, the strains studied differed in fine structural details of alpha-mycolates and keto-mycolates. Notably, strain 'habana' TMC 5135 (belonging to the 'habana' group, and considered as highly immunogenic in tuberculosis and leprosy) presented a particular composition of alpha-mycolates, with a major component (C87) containing one cis plus one trans di-substituted cyclopropane ring, unlike the type strain of M. simiae and other strains of the 'habana' group (IPK-220 and IPK-337R), in which the major component (C84) contained two cis di-substituted cyclopropane rings. In spite of this finding, the 'habana' strains were closely related to each other and mainly differed from the type strain of M. simiae in some details of the fine structure of keto-mycolates. The present work indicated that within an identical general pattern of mycolic acids, there is a complex composition in M. simiae and structural variation among different strains, as reported for pathogenic species of the genus. Noteworthy was the particular composition of alpha-mycolates in strain 'habana' TMC 5135.

Of 2,852 cultures of mycobacteria isolated in Lagos in the course of routine examinations for pulmonary tuberculosis, 170 (6%) were diagnosed as 'anonymous' mycobacteria. The majority were classified as members of Runyon's Group III or IV and many of the latter group wereprobably strains of Myco. fortuitum. No photochromogens were isolated. In 17 of 149 patients, 'anonymous' mycobacteria were isolated on two or more occasions. All had abnormal chest x-rays. Seven had a previous history of tuberculosis and Myco. tuberculosis was isolated from four. Myco. tuberculosis was also isolated from 5 patients without previous history of tuberculosis. It was not possible in this series of 149 patients to assess the clinical significance of the isolation of 'anonymous' mycobacteria

RESUMO

The limits of ph tolerance of 56 strains of mycobacteria have been studied. Saprophytic mycobacteria grow well through the widest range of ph and are able to multiply at the highest ph. Group III organisms (Runyon) grow well at a wider range of ph than do Groups I or II and all groups flourish at a wider range than does H37Rv. The increased range is the result of a better growth in the more acid levels
