RESUMO

Here, two papers are presented, which constitute the first reports of surgical procedures in Mexican children performed at the 19th century. The two publications refer to surgical operations for the extraction of bladder stones. At that time, there was no anesthesia, so part of the description alludes to the suffering of the patients and the operative difficulties. The first case, is referred to as a lithotomy in a 17-year-old girl, performed by surgeon José Victoriano Guerrero in Guadalajara in 1822. The publication is not an academic report, but a pamphlet written as a gift to Emperor Augustin I to celebrate his ascension to the throne. The second work, is a lateral lithotomy in a 5-year-old boy, published by Dr. Luis Jecker in the first issue of the Periódico de la Academia de Medicina de Mégico in 1836.


Se presentan dos trabajos que constituyen los primeros informes de procedimientos quirúrgicos en niños mexicanos en el siglo XIX. Las dos publicaciones se refieren a operaciones para la extracción de cálculos vesicales. En ese tiempo no existía anestesia, por lo que parte de la descripción incluye el sufrimiento de los pacientes y las dificultades operatorias. El primer caso está referido como una litotomía en una joven de 17 años, operada por el cirujano José Victoriano Guerrero en Guadalajara en 1822. La publicación no constituye un informe académico, sino un folleto escrito como un obsequio para el emperador Agustín I para celebrar su ascensión al trono. El segundo trabajo es una talla lateral en un niño de 5 años edad, publicado por el doctor Luis Jecker en el primer número del Periódico de la Academia de Medicina de Mégico en 1836.

With improved technologies and the development of new techniques, has emerged the concept of "scarless surgery" in an attempt to treat certain diseases obviating the need for incisions to access the peritoneal cavity, resulting in direct benefit to patients and assuming an improvement in quality of life and offering an advantage over conventional laparoscopy. Laparoendoscopic single-site surgery (LESS) and natural orifice transluminal endoscopic surgery (NOTES) are evolutions of laparoscopy, and they are complementary techniques that should be included in this new concept. NOTES is an emerging surgical modality that uses endoscopic instruments through hollow viscera to enter the peritoneal cavity and allow surgical procedures without incisions. The NOTES procedure may provide additional benefits when compared with current minimally invasive procedures. Potential advantages include no skin incisions, improved cosmetic result, reduced postoperative pain, diminished risk of postoperative hernias, and earlier recovery. NOTES surgery has still some inconvenient regarding instrumentals, viewing orientation, control of complications. Attempts to overcome the current limitations of the technique have given rise to the concept of hybrid NOTES, which is performed with the assistance of transabdominal ports for the use of conventional laparoscopy equipment. NOTES is a very promising technique although further investigation is necessary until implementing NOTES surgery in the common clinical practice.

CONTEXT: In tribute to the memory of Joaquín Albarrán in the centenary of his death, we review his biography, comment his work and highlight his urological activity as one of the most important persons of international Urology. The literature on his person and texts has been reviewed. EVIDENCE SYNTHESIS: Born in Cuba, he studied Medicine in Barcelona, graduating with an outstanding level 1877. He went to Paris to extend his studies where he settled and began an extraordinary professional career that led him to be a Professor of the Urinary Tract in the Parisian University and Chief of the Urology Clinic in the Hospital Necker, a pioneer center of specialization that was attended by many physicians from all nations world. He wrote five books on urology disease, of great importance in the literature on the specialty. These books are currently classics and still maintain their validity due to the profound anatomy-clinical and pathophysiological knowledge explained and the extraordinary description of the urinary tract operation techniques. CONCLUSIONS: An eminent urologist, not only because of the importance of his word but also due to design of the nail that carries his name as well as different urological eponyms, his human stature, his category as an investigator and his scientific excellence. Because of presence in Paris, the most important Spanish urologists were prepared at his side, this contributing to the rebirth and boom of the Urology in Spain.
