BACKGROUND: The neurological manifestations in COVID-19 adversely impact acute illness and post-disease quality of life. Limited data exist regarding the association of neurological symptoms and comorbid individuals. OBJECTIVE: To assess neurological symptoms in hospitalized patients with acute COVID-19 and multicomorbidities. METHODS: Between June 2020 and July 2020, inpatients aged 18 or older, with laboratory-confirmed COVID-19, admitted to the Hospital São Paulo (Federal University of São Paulo), a tertiary referral center for high complexity cases, were questioned about neurological symptoms. The Composite Autonomic Symptom Score 31 (COMPASS-31) questionnaire was used. The data were analyzed as a whole and whether subjective olfactory dysfunction was present or not. RESULTS: The mean age of the sample was 55 ± 15.12 years, and 58 patients were male. The neurological symptoms were mostly xerostomia (71%), ageusia/hypogeusia (50%), orthostatic intolerance (49%), anosmia/hyposmia (44%), myalgia (31%), dizziness (24%), xerophthalmia (20%), impaired consciousness (18%), and headache (16%). Furthermore, 91% of the patients had a premorbidity. The 44 patients with subjective olfactory dysfunction were more likely to have hypertension, diabetes, weakness, shortness of breath, ageusia/hypogeusia, dizziness, orthostatic intolerance, and xerophthalmia. The COMPASS-31 score was higher than that of previously published controls (14.85 ± 12.06 vs. 8.9 ± 8.7). The frequency of orthostatic intolerance was 49% in sample and 63.6% in those with subjective olfactory dysfunction (2.9-fold higher risk compared to those without). CONCLUSION: A total of 80% of inpatients with multimorbidity and acute COVID-19 had neurological symptoms. Chemical sense and autonomic symptoms stood out. Orthostatic intolerance occurred in around two-thirds of the patients with anosmia/hyposmia. Hypertension and diabetes were common, mainly in those with anosmia/hyposmia.


ANTECEDENTES: As manifestações neurológicas na COVID-19 impactam adversamente na enfermidade aguda e na qualidade de vida após a doença. Dados limitados existem em relação a associação de sintomas neurológicos e indivíduos com comorbidades. OBJETIVO: Avaliar os sintomas neurológicos em pacientes de hospitalizados com COVID-19 aguda e múltiplas comorbidades. MéTODOS: Entre junho e julho de 2020, pacientes de hospitais com idade 18 anos ou acima e COVID-19 laboratorialmente confirmada, admitidos no Hospital São Paulo (Universidade Federal de São Paulo), um centro de referência terciário para casos de alta complexidade, foram perguntados sobre sintomas neurológicos. O questionário Pontuação composta de sintoma autonômico (COMPASS-31) foi usado. Os dados foram analisados no geral e se a disfunção olfatória subjetiva estava presente ou não. RESULTADOS: A média de idade da amostra foi 55 ± 15.12 anos. 58 pacientes eram homens. Os sintomas neurológicos foram principalmente xerostomia (71%), ageusia/hipogeusia (50%), intolerância ortostática (49%), anosmia/hiposmia (44%), mialgia (31%), tontura (24%), xeroftalmia (20%), comprometimento na consciência (18%) e cefaleia (16%). Além disso, 91% dos pacientes tinham uma pré-morbidade. Os 44 pacientes com disfunção olfatória tinham maior chance de ter hipertensão, diabetes, fraqueza, falta de ar, ageusia/hipogeusia, tontura, intolerância ortostática e xeroftalmia. A pontuação do COMPASS-31 foi maior do que a de controles previamente publicados (14,85 ± 12,06 vs. 8,9 ± 8,7). A frequência de intolerância ortostática foi 49% na amostra e 63,6% naqueles com disfunção olfatória subjetiva (risco 2.9 vezes maior comparado com os sem). CONCLUSãO: Um total de 80% dos pacientes hospitalizados com múltiplas morbidades e COVID-19 aguda tinham sintomas neurológicos. Os sintomas do sentido químico e autonômicos se destacaram. A intolerância ortostática ocorreu em cerca de dois terços dos pacientes com anosmia/hiposmia. A hipertensão e o diabetes foram comuns, principalmente naqueles com anosmia/hiposmia.

Considering the small number of studies aimed at evaluating outcomes related to dizziness and falls in the older adults who live in long-term care facilities, especially in north-eastern Brazil, and considering the negative impact of these events on the quality of life of these individuals, this study purpose was to evaluate dizziness as a predictor of risk of falls in institutionalised older adults. This cross-sectional analysis of data on falls and dizziness of 187 institutionalised older adults carried out between the years 2014 and 2016. The data collection instruments used were a socioeconomic and demographic questionnaire and questionnaires for assessing variables related to falls. The different diagnostic methods were compared using the Spearman correlation, and the Dizziness variable was evaluated by means of logistic regression. Inferential statistical analysis showed a significant association between dizziness and number of morbidities (p = .03) and dizziness and a history of falls (p = .04). Spearman's correlation analysis revealed statistically significant results only for FES-I (Falls Efficacy Scale-International) (p < .01). Dizziness was a predictor of risk of falls and that it was associated with a greater number of morbidities and previous history of falls. Implementing rehabilitation practices to reduce dizziness symptoms can significantly reduce the risk of falls for institutionalised older adults.

Introdução: A tontura é vista como um dos sintomas mais comuns na população em geral, ocorrendo com maior incidência na população idosa que frequentemente apresenta distúrbios do equilíbrio e comprometimento do sistema vestibular. A caracterização de um grupo de idosos da região centro sul do estado do Paraná com relação à ocorrência de tontura tem sua importância a fim de viabilizar a promoção de ações que visem um cuidado ampliado a esta população por parte da Atenção Básica. Objetivo: caracterizar as relações da queixa de tontura, sua autopercepção dos efeitos incapacitantes, o auto relato de uso de medicamentos e ocorrência de quedas em idosos atendidos por uma equipe de Estratégia Saúde da Família. Método: Setenta e cinco idosos de ambos os sexos foram questionados sobre presença de tontura; os que a referiram, foram submetidos ao Dizziness Handicap Inventory (DHI) na versão brasileira e a um questionário para caracterização do sintoma. Resultados: Dos 75 idosos questionados, 42 (56,00%) relataram queixa de tontura, a média etária foi de 67,61±5,68 anos (média ± desvio padrão), sendo 25 (59,52%) do sexo feminino e 17 (40,48%) do masculino. O escore total médio do DHI foi de 12,52±6,80 pontos no domínio físico, de 11,90±7,93 pontos no domínio funcional, 9,33±8,25 pontos no domínio emocional e 33,67±21,05 pontos no domínio total. Conclusão: A tontura, caracterizada por impacto moderado, de tipo não rotatória, de persistência superior a 12 meses e associada com uso de polifármacos, foi um sintoma frequente na população idosa deste estudo, sobretudo no sexo feminino. A auto percepção de qualidade de vida indicou diminuição no domínio físico impactando negativamente nas atividades cotidianas.


Introduction: Dizziness is seen as one of the most common symptoms in the general population, occurring with a greater incidence in the elderly population, which frequently presents balance disorders and impaired vestibular systems. The characterization of a group of elderly people in the south central region of the Brazilian state of Paraná in relation to the occurrence of dizziness has its importance in order to enable a promotion of actions that aim an expanded care for this population by Primary Care. Objective: to characterize the relation between the dizziness complaints, their self-perception of the disabling effects, the self-report of medication use and the occurrence of falls in the elderly assisted by a Family Health Strategy team .Method: Seventy-five elderly people of both genders were asked about the presence of dizziness; those who reported it were submitted to the Dizziness Handicap Inventory (DHI) in the Brazilian version and a questionnaire to characterize the symptom. Results: In 75 elderly people questioned, 42 (56.00%) reported complaints of dizziness, the average age was 67.61 ± 5.68 years (mean ± standard deviation), 25 (59.52%) being female and 17 (40.48%) were male. The mean total DHI score was 12.52 ± 6.80 points in the physical domain, 11.90 ± 7.93 points in the functional domain, 9.33 ± 8.25 points in the emotional domain and 33.67 ± 21, 05 points in the total domain. Conclusion: Dizziness, characterized by moderate impact, of a non-rotating type, persisting for more than 12 months and associated with the use of polypharmaceuticals, was a frequent symptom in the elderly population of this study, especially in females. The self-perception of quality of life indicated a reduction in the physical domain, negatively impacting daily activities.


Introducción: Se considera que los mareos son uno de los síntomas más comunes en la población en general, y se producen con mayor incidencia en la población de edad avanzada, que con frecuencia presenta trastornos del equilibrio y deterioro del sistema vestibular. La caracterización de un grupo de ancianos de la región centro-sur del estado de Paraná en relación con la aparición de mareos, tiene su importancia para permitir la promoción de acciones encaminadas a una atención prolongada de esta población por parte de la Atención Básica. Objetivo: Caracterizar las relaciones de la queja de mareo, su autopercepción de los efectos incapacitantes, la auto información sobre el uso de medicamentos y la ocurrencia de caídas en ancianos atendidos por un equipo de Estrategia de Salud Familiar. Método: Se interrogó a 75 hombres y mujeres de edad avanzada sobre la presencia de mareo; los que lo mencionaron fueron sometidos al Inventario de Discapacidades por Mareos (DHI) en la versión brasileña y a un cuestionario para caracterizar el síntoma. Resultados: De los 75 ancianos interrogados, 42 (56,00%) informaron de mareos, la edad media fue de 67,61 ± 5,68 años (media ± desviación estándar), siendo 25 (59,52%) mujeres y 17 (40,48%) hombres. La media total de la puntuación del DHI fue de 12,52±6,80 puntos en el dominio físico, 11,90±7,93 puntos en el dominio funcional, 9,33±8,25 puntos en el dominio emocional y 33,67±21,05 puntos en el dominio total. Conclusión: El mareo, caracterizado por un impacto moderado, de tipo no rotacional, con persistencia superior a 12 meses y asociado al uso de poli farmacéuticos, fue un síntoma frecuente en la población de edad avanzada de este estudio, especialmente en las mujeres. La autopercepción de la calidad de vida indicaba una disminución del dominio físico, lo que afectaba negativamente a las actividades cotidianas.

INTRODUCCIÓN: Los trastornos del equilibrio constituyen un grupo heterogéneo de patologías. Son patologías frecuentes en medicina, representando una carga asistencial significativa en los distintos niveles de atención. En ese sentido, los estudios de frecuencia son una herramienta para entender patrones de enfermedad, como también la evaluación y planificación de una red asistencial. Material y método: Estudio descriptivo transversal sobre pacientes que asistieron al policlínico de otoneurología del Hospital Clínico Herminda Martin con motivo de consulta síntomas vestibulares durante el año 2018. Resultados: 183 pacientes cumplieron con los criterios de inclusión y exclusión, 78% (n=143) del sexo femenino y 22% (n=40) del masculino. Con una edad promedio de 58 años. El diagnóstico otoneurológico más frecuente fue vértigo posicional paroxístico benigno 34,97% (n=64), seguido de migraña vestibular 7,1% (n=13), enfermedad de Ménière 5,46% (n=10) y neuritis vestibular 4,37% (n=8). En 26,78% (n=49) no se logró un diagnóstico definitivo. Los trastornos de origen central representaron un 3,83% (n=7) y aquellos de origen no otoneurológico un 17,49% (n=32). Discusión: Los datos de frecuencia de los distintos trastornos del equilibrio registrados en el policlínico de otoneurología del Hospital Clínico Herminda Martin, son similares a la literatura internacional. Los trastornos vestibulares periféricos son los más frecuentes, de los cuales destaca el vértigo posicional paroxístico benigno representando el 34,97% del total de casos. Dentro de las causas otoneurológicas, la migraña vestibular fue la segunda causa en nuestro medio con un 7,1% del total de casos.


INTRODUCTION: Balance disorders are a heterogeneous group of diseases. They are a frequent medical condition, representing a significant healthcare burden at the different levels of care. Thus, frequency studies are a tool to understand disease patterns, as well as the evaluation and planning of a healthcare network. Material and method: Cross-sectional study of patients who attended the otoneurology out-patient clinic of the Hospital Clinico Herminda Martin with chief complaint of vestibular symptoms during 2018. Results: 183 patients met the inclusion and exclusion criteria, 143 (78%) were female and 40 (22%) male. With an average age of 58 years. The most frequent otoneurological diagnosis was benign paroxysmal positional vertigo 34.97% (n=64), followed by vestibular migraine 7.1% (n=13), Ménière's disease 5.46% (n=10) and vestibular neuritis 4.37% (n=8). In 26.78% (n=49) a definitive diagnosis was not achieved. Disorders of central origin represented 3.83% (n=7) and non-otoneurological origin 17.49% (n=32). Discussion: The frequency data of the balance disorders registered in the otoneurology out-patient clinic are similar to the international literature, peripheral vestibular disorders are the most frequent, of which benign paroxysmal positional vertigo stands out, representing 34.97% of the total cases. Among the otoneurological causes, vestibular migraine was the second cause in our study with 7.1% of the total cases

Objective To describe and compare the vestibular findings most evident among the hereditary ataxias, as well as correlate their clinical features with the nervous structures affected in this disease. Methods Seventy-five patients were evaluated and underwent a case history, otorhinolaryngological and vestibular assessments. Results Clinically, the patients commonly had symptoms of gait disturbances (67.1%), dizziness (47.3%), dysarthria (46%) and dysphagia (36.8%). In vestibular testing, alterations were predominantly evident in caloric testing (79%), testing for saccadic dysmetria (51%) and rotational chair testing (47%). The presence of alterations occurred in 87% of these patients. A majority of the alterations were from central vestibular dysfunction (69.3%). Conclusion This underscores the importance of the contribution of topodiagnostic labyrinthine evaluations for neurodegenerative diseases as, in most cases, the initial symptoms are otoneurological; and these evaluations should also be included in the selection of procedures to be performed in clinical and therapeutic monitoring.

RESUMO Objetivo Descrever e comparar os achados vestibulares mais evidentes entre a ataxia hereditária, bem como correlacionar seus aspectos clínicos com o estudo das estruturas nervosas afetadas nesta doença. Métodos 75 pacientes foram avaliados e submetidos aos seguintes procedimentos: anamnese, avaliação otorrinolaringológica e vestibular. Resultados Clinicamente, os pacientes apresentaram sintomas de distúrbios da marcha (67,1%), tonturas (47,3%), disartria (46%) e disfagia (36,8%). No teste vestibular, as alterações foram predominantemente evidentes no teste calórico (79%), dismetria sacádicas (51%) e no teste rotatório (47%). A presença de alterações ocorreu em 87% dos pacientes. A maioria das alterações observadas foram da disfunção vestibular central (69,3%). Conclusão O estudo ressalta a importância da contribuição da avaliação labiríntica no topodiagnóstico para doenças neurodegenerativas, uma vez que, na maioria dos casos, os sintomas iniciais são otoneurológicos, e essas avaliações também devem ser incluídas na seleção de procedimentos a serem realizados no monitoramento clínico e terapêutico.

Abstract Introduction The state of Minas Gerais, Brazil has no data on the prevalence of dizziness in the population and this information can be fundamental as the basis of public health policies, promotion, prevention and rehabilitation campaigns. Objective Investigate the prevalence of the symptom of dizziness in the population of Minas Gerais according to Sample Survey of Households, as well as describe the profile of interviewed individuals and the association between dizziness and socioeconomic, demographic features and health status. Methods This was a cross-sectional observational study that analyzed individuals with dizziness symptom reported in the previous month. The data entered in the Sample Survey of Households of 2011 were analyzed. An independent statistical association was determined between the selected variables and dizziness through multivariate analysis. Results Dizziness was the third major complaint among individuals who mentioned any health problems in the previous month, with an estimated population of 209,025 individuals and reported by 6.7% of symptomatic ones, with higher prevalence values only reported for the symptoms of fever and headache. Among individuals who reported dizziness, 94% were adults or elderly (p ≤ 0.001) and 63% were females (p = 0.003). A statistically significant association (p < 0.001) was observed between the response variable and the variables: self-perceived health, hypertension, heart disease, diabetes, depression, seeking or requiring medical or health care in the previous month and private health care plan or insurance. Among individuals with dizziness, 84.2% sought or required medical or health care and 80.1% did not have a private health plan or insurance in the assessed period. Conclusion The dizziness symptom was highly prevalent in the population of Minas Gerais during the assessed month of the investigation. Dizziness was prevalent in adults and the elderly and showed a statistical association with socioeconomic and demographic characteristics, as well as the assessed health status.


Resumo Introdução O Estado de Minas Gerais, Brasil, não tem dados sobre a prevalência de tontura na população e essas informações podem ser fundamentais para basear políticas de saúde pública, campanhas de promoção e prevenção e a reabilitação. Objetivo Investigar a prevalência do sintoma de tontura na população do Estado de Minas Gerais segundo a Pesquisa por Amostra de Domicílio (PAD-MG), assim como descrever o perfil dos indivíduos entrevistados e as relações entre tontura e características socioeconômicas, demográfica e condições de saúde. Método Estudo de caráter observacional transversal com análise dos indivíduos com relato de sintoma de tontura no último mês. Foram analisados os dados inseridos na PAD-MG de 2011. Determinou-se associação estatística independente entre as variáveis selecionadas e a tontura por intermédio de análise multivariada. Resultados A tontura foi a terceira queixa principal entre os indivíduos que mencionaram algum problema de saúde no último mês, com estimativa populacional de 209.025 indivíduos e relatada por 6,7% dos sintomáticos, com valores inferiores somente aos sintomas de febre e dores de cabeça, respectivamente. Dentre indivíduos com relato de tontura, 94% são adultos ou idosos (p ≤ 0,001) e 63% do sexo feminino (p = 0,003). Foi encontrada associação estatisticamente significante (p < 0,001) entre a variável resposta e as variáveis autopercepção de saúde, hipertensão, doenças cardíacas, diabetes, depressão, procura ou necessidade de atendimento médico ou de saúde no último mês e presença de cobertura de plano ou seguro-saúde. Dentre os indivíduos com tontura, 84,2% procuraram ou precisaram de atendimento médico ou de saúde e 80,1% não tinham cobertura de plano ou seguro-saúde no período pesquisado. Conclusão O sintoma de tontura se mostrou altamente prevalente na população de Minas Gerais no mês de referência da pesquisa. A tontura foi prevalente nos indivíduos adultos e idosos e apresentou associação estatística com as características socioeconômicas, demográficas e condições de saúde estudadas.

INTRODUCTION: The state of Minas Gerais, Brazil has no data on the prevalence of dizziness in the population and this information can be fundamental as the basis of public health policies, promotion, prevention and rehabilitation campaigns. OBJECTIVE: Investigate the prevalence of the symptom of dizziness in the population of Minas Gerais according to Sample Survey of Households, as well as describe the profile of interviewed individuals and the association between dizziness and socioeconomic, demographic features and health status. METHODS: This was a cross-sectional observational study that analyzed individuals with dizziness symptom reported in the previous month. The data entered in the Sample Survey of Households of 2011 were analyzed. An independent statistical association was determined between the selected variables and dizziness through multivariate analysis. RESULTS: Dizziness was the third major complaint among individuals who mentioned any health problems in the previous month, with an estimated population of 209,025 individuals and reported by 6.7% of symptomatic ones, with higher prevalence values only reported for the symptoms of fever and headache. Among individuals who reported dizziness, 94% were adults or elderly (p≤0.001) and 63% were females (p=0.003). A statistically significant association (p<0.001) was observed between the response variable and the variables: self-perceived health, hypertension, heart disease, diabetes, depression, seeking or requiring medical or health care in the previous month and private health care plan or insurance. Among individuals with dizziness, 84.2% sought or required medical or health care and 80.1% did not have a private health plan or insurance in the assessed period. CONCLUSION: The dizziness symptom was highly prevalent in the population of Minas Gerais during the assessed month of the investigation. Dizziness was prevalent in adults and the elderly and showed a statistical association with socioeconomic and demographic characteristics, as well as the assessed health status.

OBJECTIVE: To study national prevalence of dizziness and balance problems in US children and explore associated risk factors and patterns of healthcare utilization. STUDY DESIGN: A multistage, nationally representative, probability sample of children (n = 10,954; aged 3-17 years) was examined based on the 2012 National Health Interview Survey Child Balance Supplement. Parents were asked if during the past year their child was bothered by symptoms of dizziness and balance problems: vertigo (motion sensation), light-headedness/fainting, clumsiness/poor coordination, poor balance/unsteadiness when standing-up or walking, frequent falls, or other dizziness and balance problems. Logistic regression was used to examine associations with sociodemographic information, birth weight, developmental delays, and significant health conditions. RESULTS: Prevalence of dizziness and balance problems was 5.3% (3.3 million US children); females, 5.7%, males, 5.0%. Non-Hispanic white (6.1%) had increased prevalence compared with Hispanic (4.6%) and non-Hispanic black (4.3%) children, P = .01. Prevalence increased with age, from 4.1% for children aged 3-5 years to 7.5% for children aged 15-17 years, P < .001. Even though the majority had symptoms rated as "no problem" or "a small problem," 18.6% (600,000 US children) had symptoms rated as "moderate," "big," or "very big" problems. Overall, 36.0% of children with dizziness and balance problems were seen by healthcare professionals during the past year and 29.9% received treatment. Among children with dizziness and balance problems rated as moderate/big/very big problems, 71.6% had seen healthcare professionals and 62.4% received treatment for dizziness and balance problems. CONCLUSIONS: The risk factors identified provide useful epidemiologic information about dizziness and balance problems in children and will be used in tracking the Healthy People 2020 goal to increase utilization of healthcare services for these children.

Vertigo and dizziness are among the most common medical complaints in the emergency room, and are associated with a considerable personal and health care burden. Scarce and conflicting reports indicate those symptoms may present a seasonal distribution. This study aimed at investigating the existence of a seasonal distribution of vertigo/dizziness in a tropical region, and the correlations of these findings with climatic variables. The charts of all patients consecutively admitted between 2009 and 2012 in the emergency room of a Brazilian general hospital were reviewed. A total of 4920 cases containing these terms were sorted from a sample of 276,076 emergency records. Seasonality was assessed using Cosinor Analysis. Pearson's correlations were performed between the incidence of consultations, considering separately dizziness and vertigo and each of the predictor climatic variables of that index month. Significant seasonal patterns were observed for dizziness and vertigo in the emergency room. Vertigo was more frequent in late winter-spring, negatively correlating to humidity (r = -0.374; p = 0.013) and rainfall (r = -0.334; p = 0.020). Dizziness peaked on summer months, and positively correlated to average temperatures (r = 0.520; p < 0.001) and rainfall (r = 0.297; p = 0.040), but negatively to atmospheric pressure (r = -0.424; p = 0.003). The different seasonal patterns evidenced for dizziness and vertigo indicate possible distinct underlying mechanisms of how seasons may influence the occurrence of those symptoms.
