RESUMO

The author proposes a symbolic reading of panic disorder as a psychopathological emergency that currently overwhelms our society, relating to an explicit process of transformation. She understands this phenomenon as resulting from the lack of the creative feminine in people's lives, which contributes to the emergence of symptoms that are understood as alerts from the Self, which decry the danger under which the person lives. She amplifies the proposition with a symbolic reading of two great mythical moments: Oedipus' encounter with the Sphinx, and the institution of the first jury court, dramatized in the tragic play The Eumenides, by Aeschylus (Ésquilo).


L'autrice propose une lecture symbolique du trouble panique comme une urgence psychopathologique qui submerge notre société actuelle, dans un franc processus de transformation. Elle comprend ce phénomène comme une réalité qui résulte du manque de féminin créatif dans la vie des personnes. Ceci contribue à l'émergence de symptômes qui sont compris comme des alertes venant du Soi parce qu'ils dénoncent le danger dans lequel la personne vit. L'autrice amplifie la proposition avec une lecture symbolique de deux grands moments mythiques: Œdipe et sa rencontre avec le Sphinx et l'institution du premier tribunal avec jury, exprimé dans la tragédie d'Eschyle, Les Euménides.


La autora propone una lectura simbólica del trastorno de pánico como una emergencia psicopatológica que abruma a nuestra sociedad actual, en franco proceso de transformación. Ella comprende este fenómeno como una realidad que es resultado de la falta de lo femenino creativo en la vida de las personas, lo cual contribuye a la emergencia de síntomas que son comprendidos como alertas del Self, debido a que denuncian el peligro en el cual la persona vive. La autora amplifica esta propuesta a partir de la lectura simbólica de dos importantes momentos míticos: Edipo: su encuentro con la Esfinge, y la institución de la primera corte de justicia, expresada en la obra trágica, Las Euménides de Esquilo.


O autor propõe a leitura simbólica do transtorno de pânico como uma emergência psicopatológica que sobrecarrega nossa sociedade atual, em um processo franco de transformação. Ela entende esse fenômeno como uma realidade resultante da falta do criativo feminino na vida das pessoas, o que contribui para o surgimento de sintomas que são entendidos como alertas do Si Mesmo, porque denunciam o perigo sob o qual a pessoa vive. Ela amplifica a proposição com uma leitura simbólica de dois grandes momentos míticos: Édipo: seu encontro com a Esfinge e a instituição da primeira corte do júri, expressa na trágica peça: Os Eumênides de Ésquilo.

INTRODUCTION: In the last few decades, exercise has been explored as a potential tool to reduce symptoms experienced by patients with panic disorder (PD). This systematic review aims to assess the effects of regular exercise interventions on panic severity, global anxiety, and depression symptoms of these patients. AREAS COVERED: A search was conducted on PubMed, ISI Web of Science, and Cochrane Central Register of Controlled Trials using search terms related to PD and exercise. Eight trials were included, Furthermore, regular exercise programs presented different methodological characteristics. There is o clear evidence indicating that regular exercise programs (at least two 20-minute sessions per week for at least 6 weeks) reduce panic-related symptoms. Regular exercise is effective in improving global anxiety measures and depression. EXPERT OPINION: Continuous aerobic exercise is the main type of intervention in the literature, generally providing a limited prescription. Currently, it is recommended the interval training, with intense and shorter stimuli, and long-term duration trials. However, despite the use of self-selected intensities and control based on the internal load be interesting as recommendation to increase adherence, careful is needed regarding training prescription due to scarce evidence.

Anxiety sensitivity (AS) is a multidimensional construct associated with the etiology and maintenance of panic disorder (PD) symptoms. However, only a few studies have evaluated whether cognitive-behavioral group therapy (CBGT) can modify the condition. The objective of this study was to evaluate the impact of CBGT on AS in patients with PD and to analyze AS and its dimensions as predictors of response to CBGT. In the present clinical trial, an intervention group (n = 37) attended 12 CBGT sessions, while a control group (n = 52) did not receive any intervention. The severity of symptoms and of AS were evaluated before and after CBGT in the intervention group and once in the control group. Significant improvement occurred in all specific PD symptoms and in general anxiety and depressive symptoms. Furthermore, AS scores reduced significantly after intervention. This study confirmed that AS is higher in patients with more severe PD. The effectiveness of CBGT for reducing the physical, cognitive, and social dimensions of AS was also observed, supporting the hypothesis of a positive impact of therapy.

BACKGROUND: Although resilience and coping are important factors associated with mental health, they are rarely investigated in the treatment of patients with panic disorder (PD). OBJECTIVE: To evaluate the response to four resilience and coping strategy sessions added to the standard cognitive behavioral group therapy (CBGT) protocol for PD. DESIGN: Controlled clinical trial. METHODS: The control group (n = 50) attended 12 CBGT sessions, while the intervention group (n = 50) received four additional resilience and coping strategy sessions, i.e., 16 in total. Symptom severity, resilience, coping strategies, and quality of life were assessed at baseline and post-CBGT. RESULTS: Symptom severity and maladaptive coping strategies decreased significantly in both groups. However, the intervention group had increased resilience and improvement in the environment domain of quality of life. CONCLUSIONS: Additional sessions have potential benefits for coping skills and resilience in PD patients, but these benefits should be evaluated in further long-term studies.

OBJECTIVE: In this study, we compared duration of untreated illness (DUI) in obsessive-compulsive disorder (OCD), panic disorder (PD), and social anxiety disorder (SAD) patients and investigated its correlates, both within specific diagnoses and across the whole sample. METHODS: Eighty-eight patients (33 OCD, 24 SAD, and 31 PD) had their diagnosis confirmed by the Mini International Neuropsychiatric Interview, were assessed for treatment-seeking variables, and were evaluated with instruments aimed at quantifying transdiagnostic features (i.e., the Cause subscale of the Illness Perception Questionnaire-Mental Health and the Anxiety Sensitivity Index-Revised) and severity of illness (i.e., Beck Depression and Anxiety Inventories, the Dimensional Obsessive-Compulsive Scale, the Panic and Agoraphobia Scale, and the Social Phobia Inventory). RESULTS: The only differences between groups with short (<2 years) versus long (>2 years) DUI were greater fear of public display of anxiety in the first group and greater social avoidance in the second group. The DUI was significantly different between groups that sought treatment after the onset of illness, with OCD patients having longer DUI than PD patients and shorter DUI than SAD patients. Further, DUI correlated negatively with the perception of OCD being caused by stress and positively with severity of panic-related disability in SAD patients, but not in PD or OCD patients. CONCLUSION: There was substantial delay in treatment seeking among the anxiety and obsessive-compulsive disorder patients, particularly those with OCD or SAD. Perception of stress as a cause of OCD prompted treatment seeking, while severity of panic symptoms delayed treatment seeking.

RESUMO

Trabalho realizado a partir do relato de caso clínico de uma jovem com diagnóstico de Transtorno de Ansiedade (Transtorno de Pânico em co-morbidade a Transtorno de Ansiedade Generalizada), os quais implicavam sofrimento emocional e significativas limitações para esta paciente. O autor propôs o tratamento e condução do caso de acordo com os fundamentos do tratamento homeopático clássico, com resultado bastante favorável. São comentados detalhes do tratamento homeopático realizado, o qual mostrou-se como alternativa eficaz ao tratamento psiquiátrico alopático convencional. Conclui citando que a abordagem homeopática se mostrou relevante ao reduzir os sintomas ansiosos gerais da paciente e trazer importante qualidade de vida e retomada da funcionalidade para a mesma. Estima-se que cerca de 5% da população americana sofra de Transtorno do Pânico e 23 % preencha todos os critérios, de ao menos, um ataque de pânico isolado durante a vida. Tanto fatores genéticos quanto fatores ambientais interferem na gravidade do transtorno e o tratamento com maior índice de sucesso atualmente é a terapia combinada de psicofármacos e psicoterapia. A Homeopatia surge como uma forma de terapia alternativa. Isolada ou associada a terapia combinada já conhecida. Deve ser considerada pelo seu princípio de terapia individualizada, no intuito de diminuir os riscos de toxicidade e dependência química a que esses indivíduos estão expostos no curso da patologia, além de promoção de saúde. Relata-se aqui um caso clínico de um jovem de 14 anos, com quadro típico de Transtorno do Pânico, controlado com medicação homeopática.(AU)

The association between panic disorder (PD) and cardiovascular diseases (CVD) has been extensively studied in recent years and, although some studies have shown anxiety disorders co-existing or increasing the risk of heart disease, no causal hypothesis has been well established. Thus, a critical review was performed of the studies that evaluated the association between PD and cardiovascular diseases; synthesizing the evidence on the mechanisms mediators that theoretically would be the responsible for the causal pathway between PD and CVD, specifically. This overview shows epidemiological studies, and discusses biological mechanisms that could link PD to CVD, such as pleiotropy, heart rate variability, unhealthy lifestyle, atherosclerosis, mental stress, and myocardial perfusion defects. This study tried to provide a comprehensive narrative synthesis of previously published information regarding PD and CVD and open new possibilities of clinical management and pathophysiological understanding. Some epidemiological studies have indicated that PD could be a risk factor for CVD, raising morbidity and mortality in PD, suggesting an association between them. These studies argue that PD pathophysiology could cause or potentiate CVD. However, there is no evidence in favour of a causal relationship between PD and CVD. Therefore, PD patients with suspicions of cardiovascular symptoms need redoubled attention.

BACKGROUND: Investigating the contribution of therapeutic factors arising from the collective nature or group therapy to treat mental disorders may help therapists maximize the outcome of therapy. Studies about the role of therapeutic factors in cognitive-behavioral group therapy (CBGT) for panic disorder (PD) patients are still scarce. OBJECTIVES: To identify the therapeutic factors rated as the most useful by patients during CBGT. Also, we aimed to investigate the relationship between patient rating of therapeutic factors and specific stages of CBGT. DESIGN: Non-controlled clinical trial. METHODS: A 12-session CBGT protocol was set up, covering psychoeducation, techniques for anxiety coping, cognitive restructuring, interoceptive and naturalistic exposure, and live exposure to avoidant behavior. PD symptom severity was assessed before and after the CBGT protocol. Yalom's Curative Factors Questionnaire was self-administered at the end of each session to evaluate the 12 therapeutic factors. RESULTS: The sample consisted of 16 patients, who produced 192 assessments of therapeutic factors. Severity of symptoms improved at the end of CBGT, with a large effect size (>1.0). Different ratings were attributed to therapeutic factors at different phases of CBGT. Seven factors were rated as significantly helpful: altruism, interpersonal learning/input, guidance, identification, family reenactment, self-understanding, and existential factors. CONCLUSIONS: Therapeutic factors are dynamic and interdependent. Therefore, recognizing the impact of these factors during CBGT may potentially contribute to a better understanding of the therapeutic process.

O objetivo é caracterizar o Transtorno do Pânico (TP) com ênfase em seu diagnóstico e tratamento. O TP é um dos transtornos de ansiedade, caracterizado por ataques de pânico recorrentes acompanhados por uma persistente preocupação com ataques adicionais e alterações mal adaptativas do comportamento (Associação Americana de Psiquiatria - DSM-V). Sua etiologia ainda não é conhecida, mas deve envolver uma interação de fatores genéticos, de desenvolvimento e ambientais que resultam em altera- ções no funcionamento de algumas áreas cerebrais. O tratamento farmacológico de primeira escolha é com o uso de antidepressivos inibidores seletivos da recaptação de serotonina, os quais apresentam uma latência de 20 a 30 dias para o início do efeito. The aim of this paper is to characterize the Panic Disorder (PD) with an emphasis on diagnosis and treatment. PD is one of the anxiety disorders, characterized by recurrent panic attacks accompanied by a persistent preoccupation with additional attacks and maladaptive behavioral changes (American Psychiatric Association ­ DSM-V). Its etiology is not known, but should involve an interaction of genetic, developmental and environmental factors that result in changes in the functioning of some brain areas. The pharmacological treatment of choice is with the use of selective serotonin reuptake inhibitors, which has a latency of 20 for 30 days for the beginning of the therapeutic effect. (AU)
