PURPOSE: to consolidate a Trauma Register (TR) trough REDCap data acquisition platform and to validate, in this context, local Quality Indicators (QI) as improvement opportunities in trauma management. METHODS: continuous data acquisition of all patients admitted in Irmandade da Santa Casa de Misericórdia de São Paulo adult Trauma bay and it's validation in REDCap platform; 6 months retrospective cohort of QI impact in length of hospitalar stay, complications and mortality. Fisher, Chi-squared, Wilcoxon and Kruskal-Wallis tests were used to correlate QIs fails with the endpoints, considering p<0.05 and CI <95% as statically significant. RESULTS: 465 were admitted in Trauma bay, with 137 patients hospitalized (29.5%); the number of QIs compromised were related with more complications (p=0.075) and increased length of stay (p=0.028), especially the delay in open fracture's surgical management, which increased the severe complications' incidence (p=0.005). CONCLUSION: the REDCap data acquisition platform is useful as a tool for multi center TR implementation, from ethical and logistical point of view; nevertheless, the proposed QIs are validated as attention points in trauma management, allowing improvements in traumatized patients treatment.

BACKGROUND: Major surgery triggers trauma-like stress responses linked to age, surgery duration, and blood loss, resembling polytrauma. This similarity suggests elective surgery as a surrogate model for studying polytrauma immune responses. We investigated stress responses across age groups and compared them with those of polytrauma patients. STUDY DESIGN: Patients undergoing major spinal reconstruction surgery were divided into older (age >65 years, n = 5) and young (age 18 to 39 years, n = 6) groups. A comparison group consisted of matched trauma patients (n = 8). Blood samples were collected before, during, and after surgery. Bone marrow mononuclear cells and peripheral blood mononuclear cells were analyzed using cellular indexing of transcriptomes and epitopes sequencing or single-cell RNA sequencing. Plasma was subjected to dual-platform proteomic analysis (SomaLogic and O-link). RESULTS: Response to polytrauma was highest within 4 hours. By comparison, the response to surgery was highest at 24 hours. Both insults triggered significant changes in cluster of differentiation 14 monocytes, with increased inflammation and lower major histocompatibility complex-class 2 expression. Older patient's cluster of differentiation 14 monocytes displayed higher inflammation and less major histocompatibility complex-class 2 suppression; a trend was also seen in bone marrow mononuclear cells. Although natural killer cells were markedly activated after polytrauma, they were suppressed after surgery, especially in older patients. In plasma, innate immunity proteins dominated at 24 hours, shifting to adaptive immunity proteins by 6 weeks with heightened inflammation in older patients. Senescence-associated secretory phenotype proteins were higher in older patients at baseline and further elevated during and after surgery. CONCLUSIONS: Although both major surgery and polytrauma initiate immune and stress responses, substantial differences exist in timing and cellular profiles, suggesting major elective surgery is not a suitable surrogate for the polytrauma response. Nonetheless, distinct responses in young vs older patients highlight the utility of elective spinal in studying patient-specific factors affecting outcomes after major elective surgery.

OBJECTIVE: Treating traumatic hemorrhage is time sensitive. Prehospital care and transport modes (eg, helicopter and ground) may influence in-hospital events. We hypothesized that prehospital time (on-scene time [OST] and total prehospital time [TPT]) and transport mode are associated with same-day transfusion and mortality. Furthermore, we sought to identify regions of anatomic injury that modify the relationship between prehospital time and outcomes in strata corresponding to transport types. METHODS: We obtained prehospital, in-hospital, and trauma registry data from an 8-center cohort of adult nonburn trauma patients from 2017 to 2022 directly transported from the scene to the hospital and having an Injury Severity Score (ISS) > 9 for the Task Order 1 project of the Linking Investigators in Trauma and Emergency Services research network. We excluded patients missing prehospital times, patients < 18 years of age, patients from interfacility transfers, and recipients of prehospital blood. Our same-day outcomes were in-hospital transfusions within 4 hours and 24-hour mortality. Each outcome was adjusted using multivariable logistic regression for covariates of prehospital phases (OST and TPT), mode of transport (helicopter and ground), age, sex, ISS, Glasgow Coma Scale motor subscale score < 6, and field hypotension (systolic blood pressure < 90 mm Hg). We evaluated the association of prehospital time on outcomes for scene missions by transport mode across severe injury patterns defined by Abbreviated Injury Scale > 2 body regions. RESULTS: Of 78,198 subjects, 34,504 were eligible for the study with a mean age of 47.6 ± 20.3 years, ISS of 18 ± 11, OST of 15.9 ± 9.5 minutes, and TPT of 48.7 ± 20.3 minutes. Adjusted for injury severity and demographic factors, transport type significantly modified the relationship between prehospital time and outcomes. The association of OST and TPT with the odds of 4-hour transfusion was absent for the ground emergency medical services (GEMS) cohort and present for the helicopter emergency medical services (HEMS) ambulance cohort, whereas these times were associated with decreased 24-hour mortality for both transport types. When stratifying by injury to most anatomic regions, OST and TPT were associated with a decreased need for 4-hour transfusions in the GEMS cohort. However, OST was associated with increased early transfusion only among patients with severe injuries of the thorax, and this association persisted after adjusting additionally for injury type (odds ratio [OR] = 1.03; 95% confidence interval [CI], 1.00-1.05; P = .02). The presence of polytrauma supported an association between prehospital time and decreased 24-hour mortality for the GEMS cohort (OST: OR = 0.97; 95% CI, 0.95-0.99; P < .01; TPT: OR = 0.99; 95% CI, 0.98-0.99; P = .02), whereas no injuries showed significant association of helicopter prehospital time on mortality after adjustment. CONCLUSION: We determined that transport type affects the relationship between prehospital time and hospital outcomes (4-hour transfusion: positive relationship for HEMS and negative for GEMS, 24-hour mortality: negative for both transport types). Furthermore, we identified regions of anatomic injury that modify the relationship between prehospital time and outcomes in strata corresponding to transport types. Of these regions, most notable were severe isolated injuries to the thorax that supported a positive relationship between HEMS OST and 4-hour transfusions and polytrauma that showed a negative relationship between GEMS OST or TPT and 24-hour mortality after adjustment.

Objetivo: Sistematizar a assistência de enfermagem à luz da teoria de Callista-Roy no cuidado ao paciente politraumatizado. Método:Trata-se de um estudo do tipo qualitativo de abordagem descritiva e exploratória na modalidade de caso clínico, realizado por estudantes de Enfermagem da Universidade Estadual de Feirade Santana no Hospital Geral Clériston Andrade na Clínica Ortopédica. A prática ocorreu em setembro de 2023, onde foram coletados dados e prestados cuidados à paciente. Resultados:Foi aplicada a Sistematização de Enfermagem, sob perspectiva da teoria deCallista-Roy, onde a coleta de dados foi realizada por meio do prontuário e contato direto com a paciente. Segundo a teoria, foram identificados os modos de adaptação físico-fisiológico, identidade de autoconceito, interdependência e desempenho de papel. Por fim, com base nas informações colhidas, foram realizados o planejamento, implementação e avaliação das condutas adotadas. Considerações finais:. Portanto, através da aplicação da sistematização de Enfermagem e do Modelo de adaptação é possível observara importância do exercício profissional da Enfermagem no processo de cuidado holístico, a fim de garantir as melhores condições de recuperação que perpassam o físico, como também na fundamentação teórico-prática do trabalho de enfermagem.


Objective: Systematize nursing assistance based on Callista-Roy's theory in the care of polytraumatized patients. Method:This is a qualitative study with a descriptive and exploratory approach in the form of a clinical case, carried out by nursing students from the State University of Feira de Santana at Clériston Andrade General Hospital in the Orthopedic Clinic. The practice took place in September 2023, where data were collected, and care was provided to the patient. Results:The Nursing Process was applied from the perspective of Callista-Roy's theory, with data collection performed through medical records and direct contact with the patient. According to the theory, modes of physical-physiological adaptation, self-concept identity, interdependence, and role performance were identified. Finally, based on the gathered information, planning, implementation, and evaluation of the adopted measures were carried out. Final considerations: Therefore, through the application of the nursing process and the Adaptation Model, it is possible to observe the importance of the nursing profession in the holistic care process, aiming to ensure the best conditions for recovery that go beyond the physical, as well as in the theoretical-practical foundation of nursing work.

Finalmente, con base en la información recopilada, se llevaron a cabo la planificación, implementación y evaluación de las medidas adoptadas. Consideraciones finales: Por lo tanto, mediante la aplicación del proceso de enfermería Y el Modelo de Adaptación, es posible observar la importancia de la profesión de enfermería en el proceso de atención integral, con el objetivo de garantizar las mejores condiciones de recuperación que van más allá de lo físico, así como en la fundamentación teórico-práctica del trabajo de enfermería.

Background. Numerous health conditions in the older adult population can be attributed to falls, including traumatic brain injury (TBI), which can lead to devastating short and long-term sequelae. Older adults are also more likely to experience frailty, which encompasses physical, psychological, and social deficits that may lead to adverse health outcomes. Our literature review synthesizes current evidence for understanding frailty in the context of TBI among older adults using the Integral Model of Frailty as a framework. Content synthesis. A total of 32 articles were identified, and 9 articles were included. The results of this review indicate that outcomes resulting from TBI are closely linked to the physical, psychological, and social domains of frailty. Conclusions. A small amount of literature currently examines frailty in the context of TBI among older adults. Antecedentes. Numerosos trastornos de salud en la población de adultos mayores pueden atribuirse a las caídas, incluida la lesión cerebral traumática (LCT), que puede provocar secuelas devastadoras a corto y largo plazo. Los adultos mayores también son más propensos a experimentar fragilidad, que abarca déficits físicos, psicológicos y sociales que pueden conducir a resultados adversos para la salud. Nuestra revisión de la literatura sintetiza la evidencia actual para la comprensión de la fragilidad en el contexto de la LCT entre los adultos mayores utilizando el Modelo Integral de Fragilidad como marco. Síntesis del contenido. Un total de 32 artículos fueron identificados, y 9 artículos fueron incluidos. Los hallazgos de esta revisión indican que los resultados de la LCT están estrechamente relacionados con los dominios físico, psicológico y social de la fragilidad. Conclusión. Una pequeña cantidad de literatura examina actualmente la fragilidad en el contexto de la LCT entre los adultos mayores. Usar el Modelo Integral de Fragilidad para entender la fragilidad en el contexto de la LCT puede ayudar a los clínicos a anticipar los resultados de los pacientes y mejorar los planes de cuidados. Enfatizamos la necesidad de una mayor comprensión de la LCT en relación con la fragilidad para mejorar los resultados de salud entre los pacientes adultos mayores.

Os idosos também são mais propensos a experimentar fragilidade, que engloba déficits físicos, psicológicos e sociais que podem levar a resultados adversos à saúde. Nossa revisão da literatura sintetiza as evidências atuais para entender a fragilidade no contexto do TCE entre idosos usando o Modelo Abrangente de Fragilidade como estrutura. Síntese de conteúdo. Um total de 32 artigos foram identificados e 9 artigos foram incluídos. As descobertas desta revisão indicam que os resultados do TCE estão intimamente relacionados aos domínios físico, psicológico e social da fragilidade. Conclusão.Um pequeno corpo de literatura atualmente examina a fragilidade no contexto do TCE entre adultos mais velhos. Usar o Modelo Abrangente de Fragilidade para entender a fragilidade no contexto do TCE pode ajudar os médicos a antecipar os resultados do paciente e melhorar os planos de tratamento. Objetivo: Descrever o perfil dos pacientes atendidos pela onda vermelha em 2018 e 2019. Métodos: Trata-se de uma pesquisa quantitativa, de caráter transversal, retrospectiva, descritiva e documental realizada no Hospital de Pronto-Socorro João XXIII, Belo Horizonte, Brasil. O universo deste estudo foram 83 prontuários eletrônicos de pacientes que entraram no protocolo da Onda Vermelha. Foi realizada distribuições de frequência, medidas de tendência central (média e mediana) e de variabilidade (desvio padrão). Resultados: A maioria dos pacientes foi do sexo masculino, com idade entre 1 e 95 anos e média de 33,4 anos. O mecanismo do trauma mais frequente foi contuso, o meio transporte foi ambulância do Serviço de Atendimento Móvel de Urgência, a causa mais frequente dos traumas foi acidente automobilístico seguido de trauma por projétil de arma de fogo. As cirurgias mais frequentes foram laparotomia, toracotomia e craniectomia. Óbito foi o desfecho mais comum. Conclusão: Os pacientes chegam muito graves e o óbito foi o principal desfecho. Sugere-se estudos que possibilitem análise comparativa de dados e padronização do cálculo da probabilidade de sobrevivência. Recomenda-se atualização do protocolo da onda vermelha, incluindo outras cirurgias que já são realizadas e novos critérios de inclusão de pacientes. (AU)


Objective: To describe the profile of patients treated in the "Red Wave", in the period of 2018-2019. Methods: It is a quantitative, cross-sectional, retrospective, descriptive and documentary research. Held at the first-aid post João XXIII Hospital, in Belo Horizonte. The universe of this study was 83 electronic medical records of patients who entered the "Red Wave" protocol. Simple frequency distributions, measures of central tendency (mean and median) and variability (standard deviation) have been performed. Results: Male, aged between 1 and 95 years old, being an average at 33,4 years old. The most frequent trauma mechanism was blunt, the means of transport was an ambulance from the Mobile Emergency Service, the causes of the trauma were an automobile accident, followed by trauma by a firearm projectile. From the performed surgeries, the most frequent ones were laparotomy, thoracotomy and craniectomy. Among the outcome, death was the most common one among patients. Conclusion: Patients arrive very seriously and death was the main outcome. Studies that allow comparative data analysis and standardization of the calculation of survival probability are suggested. It is recommended to update the red wave protocol, including other surgeries that are already performed and new inclusion criteria for patients. (AU)

Métodos: se trata de un estudio cuantitativo, transversal, retrospectivo, descriptivo y documental realizado en el Hospital de Pronto-Socorro João XXIII, Belo Horizonte, Brasil. El universo de este estudio fue de 83 historias clínicas electrónicas de pacientes que ingresaron al protocolo Red Wave. Se realizaron distribuciones de frecuencia, medidas de tendencia central (media y mediana) y variabilidad (desviación estándar). Resultados: La mayoría de los pacientes eran varones, con edades comprendidas entre 1 y 95 años y media de 33,4 años. El mecanismo de traumatismo más frecuente fue contundente, el medio de transporte fue una ambulancia del Servicio Móvil de Emergencias, la causa más frecuente de traumatismo fue un accidente automovilístico seguido del traumatismo por proyectil de arma de fuego. Las cirugías más frecuentes fueron laparotomía, toracotomía y craniectomía. La muerte fue el resultado más común. Conclusión: Los pacientes llegan muy en serio y la muerte fue el resultado principal. Se sugieren estudios que permitan el análisis de datos comparativos y la estandarización del cálculo de la probabilidad de supervivencia. Se recomienda actualizar el protocolo de onda roja, incluyendo otras cirugías que ya se realizan y nuevos criterios de inclusión de pacientes. (AU)

Objetivo: Apresentar as características clínicas dos pacientes vítimas de trauma que necessitaram de transfusão emergencial para uma abordagem do Diagnóstico de Enfermagem "Risco de Choque"; descrever a atuação da equipe de Enfermeiros do Trauma nesse contexto. Métodos: Trata-se de um estudo descritivo, retrospectivo, de abordagem quantitativa. Os dados retratam o período de outubro de 2018 a dezembro de 2019, perfazendo um total de 447 pacientes com choque hemorrágico por trauma inseridos no Protocolo de Transfusão Maciça de um hospital público. Resultados: O "Risco de Choque" foi o diagnóstico de enfermagem prioritário nos pacientes com hemorragia grave por trauma. Houve predominância do sexo masculino, vítimas de politraumatismo em sua maioria, com idade entre 18 e 29 anos. 197 pacientes apresentaram líquido livre que foi detectado através do exame de ultrassom de emergência nos pacientes com trauma torácico ou abdominal. O Serviço de Atendimento Móvel de Urgência foi o meio utilizado por 378 pacientes para atendimento pré-hospitalar. Conclusão: A caracterização mostra a gravidade desses casos, com necessidade de transfusão maciça. O conhecimento desses fatores pela equipe multiprofissional de pacientes críticos com hemorragia grave associada ao trauma é fundamental, tornando-se necessária a abordagem do diagnóstico de enfermagem "risco de choque" pelo profissional Enfermeiro. Objective: To present the clinical characteristics of the trauma patients who required emergency transfusion for an approach to the Nursing Diagnosis "Risk of Shock"; describe the role of the Trauma Nurses team in this context. Methods: This is a descriptive, retrospective study with a quantitative approach. The data portrays the period from October 2018 to December 2019, making up a total of 447 patients with traumatic hemorrhagic shock inserted in the Mass Transfusion Protocol of a public hospital. Results: There was a predominance of male subjects, victims polytrauma in that majority, aged between 18 and 29 years old. 197 patients had free fluid that was detected by emergency ultrasound examination in patients with chest or abdominal trauma. The Mobile Emergency Care Service was the resource used by 378 patients to the pre-hospital care. Conclusion: The characterization shows the severity of these cases, requiring massive transfusion. The knowledge of these factors by the multidisciplinary team of critically ill patients with severe hemorrhage associated with trauma is essential, making it necessary for the nursing professional to approach the nursing diagnosis "risk of shock". Objetivo: Presentar las características clínicas de los pacientes traumatizados que requirieron transfusión urgente para el abordaje del Diagnóstico de Enfermería "Riesgo de Choque"; describir el papel del equipo de enfermeras de trauma en este contexto. Métodos: Se trata de un estudio descriptivo, retrospectivo con enfoque cuantitativo. Los datos retratan el período de octubre de 2018 a diciembre de 2019, lo que hace un total de 447 pacientes con choque hemorrágico traumático insertado en el Protocolo de Transfusión Masiva de un hospital público. Resultados: El "Riesgo de Choque" fue el diagnóstico de enfermería prioritario en los pacientes con hemorragia por trauma severo. Predominó el sexo masculino, mayoritariamente víctimas de politraumatismos, con edades comprendidas entre los 18 y 29 años. 197 pacientes tenían líquido libre que fue detectado por el examen de ultrasonido de emergencia con traumatismo torácico o abdominal. El Servicio Móvil de Atención de Emergencias fue el medio utilizado por 378 pacientes para la atención prehospitalaria. Conclusión: La caracterización muestra la gravedad de estos casos, requiriendo transfusión masiva. El conocimiento de estos factores por parte del equipo multidisciplinario de pacientes críticos con hemorragia severa asociada a trauma es fundamental, por lo que es necesario que el profesional de enfermería aborde el diagnóstico de enfermería "riesgo de shock". BACKGROUND Tracheocele are rarely encountered air cysts formed due to tracheal wall outpouching through a weak vantage point. The majority are acquired in the adult population and are associated with conditions that weaken the tracheal wall. Most tracheoceles are diagnosed incidentally since many are asymptomatic or present with nonspecific symptoms. Multidetector computed tomography (MDCT) scans are the most common imaging modality for diagnosis of silent tracheal cysts. Tracheocele have been very rarely documented in the trauma setting; therefore, in the setting of multiple body trauma diagnosis can be challenging. CASE REPORT We report a case of an acquired tracheocele after a high-impact blunt thoracic trauma with an admission diagnosis of suspected tracheal perforation. MDCT of the neck and chest demonstrated an irregularly multicystic-shaped air collection at the right posterolateral trachea upon evaluation. Flexible laryngoscopy and bronchoscopy results were unremarkable. CONCLUSIONS Tracheocele are rare and asymptomatic pseudo-diverticulum of the tracheal wall. Many are diagnosed incidentally with imaging studies for other conditions. Nevertheless, in the trauma setting its diagnosis can be challenging and misleading. Consideration of conditions such as tracheocele is important to prevent any unwarranted treatment modalities.

INTRODUCTION: Traumatic brain injury (TBI) entails chronic neurological symptoms and deficits, such as smell and taste impairment. In the general population, a prevalence of 13.5% for smell impairment, 17% for taste impairment, and 2.2% for both have been reported. Studies establishing prevalence of sensorial dysfunction in the U.S. general population showed that prevalence increased with age and was higher in ethnic minorities and men. To understand the mechanisms that lead to these deficits, the prevalence of sensory dysfunction was studied in the Veteran TBI population of the VA Caribbean Healthcare System (VACHS). The aims were to find the prevalence of sensorial dysfunction in smell and/or taste in TBI patients at the VACHS Polytrauma Clinic and its association with demographic characteristics and medical comorbidities. The hypothesis was that the prevalence of sensory dysfunction in smell and/or taste of VACHS Veterans (mostly Hispanics minority) with TBI will be higher than the one historically reported in the literature for the U.S. general population. MATERIALS AND METHODS: A retrospective record review was held at the VACHS Polytrauma Clinic from January 2018 to January 2020 (before coronavirus disease 2019 pandemic) to evaluate the prevalence of sensory dysfunction. Data on demographics and comorbidities in the electronic medical records, and the TBI Second-Level Evaluation note, that was previously completed by a physician from the Polytrauma Clinic at the VACHS to diagnose and characterized the TBI event, were reviewed. Data were summarized using descriptive statistics. To establish the relation among demographic characteristics and comorbidities with the prevalence of smell and/or taste sensory dysfunction, chi-square and Fisher's exact tests were used. RESULTS: A total of 81 records were reviewed. This corresponded to all the patients diagnosed with TBI in the VACHS Polytrauma Clinic from January 2018 to January 2020. The prevalence of sensory dysfunction in the studied population was 38.3%. Men tend to present a higher prevalence of smell and/or taste dysfunction (40.0%) in comparison with women (16.7%); however, the difference did not achieve statistical significance (P = .399). Hispanics had a relatively higher prevalence of sensory dysfunction than non-Hispanics, but this difference did not reach statistical significance (P = .210). Forty-nine subjects were combat Veterans (60.5%). There was a significant correlation regarding the combat status of the subjects (P = .014), where 24 of the 49 combat Veterans presented smell and/or taste dysfunction (49.0%). A marginal significance was observed for obesity; obese participants were less likely to have a significant smell and/or taste dysfunction (P = .053). CONCLUSION: The investigators found that the prevalence of sensory dysfunction in smell and/or taste in VACHS Veterans with TBI was 38.3% (n = 31). A significant association was found between smell and/or taste dysfunction and being a combat veteran (P = .018). A marginally significant association to obesity was also observed (P = .053). To the scientific community, the results will serve as a base for sensorial dysfunction and TBI research given that this prevalence, and the correlation to demographics and comorbidities, has not been fully established in the Veteran population.
