OBJECTIVES: The new corona virus infection, has a wide range of clinical manifestations. Fever and cough are the most common symptoms. The olfactory function may be also affected with COVID-19. In this randomized clinical trial, we wanted to evaluate the therapeutic effect of olfactory training with and without oral vitamin A for COVID-19-related olfactory dysfunction. METHODS: Patients answered to the standard Persian version of anosmia reporting tool and performed the quick smell test before and after 12 weeks and at the end of the 12 months follow up. The patients were randomly allocated to three groups; Group A treatment with olfactory training, Group B treatment with oral vitamin A and olfactory training, and Group C as control group which only underwent nasal irrigation twice a day. Patients were treated for 3 months and followed up for 12 months. RESULTS: Totally 90 patients were included in three groups. After interventions, 76.9% of patients in Group A, 86.7% of patients in Group B, and 26.7% of patients in Group C completely improved. The average intervention time was statistically significant in relationship with the final olfactory status of the patients in the 12 months follow-up. The olfactory training has significantly improved the smell alteration at the end of 3- and 12- months follow-up in A and B groups. CONCLUSION: A three-months olfactory training is effective for improvement of COVID-19-related olfactory dysfunction. Adding daily oral vitamin A to olfactory training did not lead to better results in improving olfactory dysfunction. LEVEL OF EVIDENCE: Step 2 (Level 2*): Randomized trial.

OBJECTIVES: Olfactory loss is a recognized long-term dysfunction after Coronavirus Disease 2019 (COVID-19) infection. This investigation aimed to assess the effect of alpha-lipoic acid as an adjuvant treatment of olfactory training on the improvement of smell loss in post-COVID-19 patients. METHODS: This randomized controlled trial included 128 adult outpatients who had persistent smell loss for more than 3-months after COVID-19 infection. The participants were randomly allocated into two groups: the intervention treatment group, which received alpha-lipoic acid associated to olfactory training, and comparison treatment group, which received placebo pills associated to olfactory training. The participants were followed-up for 12-weeks. Olfactory dysfunction was assessed in terms of Visual Analog Scale (VAS), and the Connecticut Chemosensory Clinical Research Center (CCCRC) test for the Brazilian population. RESULTS: A total of 100 participants completed the follow-up period and were analyzed in this study. Both groups have improved CCCRC score (p = 0.000), olfactory threshold (p = 0.000), identification score (p = 0.000) and VAS score (p = 0.000) after 12-weeks follow-up. No significant differences were determined between the intervention and comparison treatment groups in CCCRC score (p = 0.63), olfactory threshold (p = 0.50), identification score (p = 0.96) and VAS score (p = 0.97). In all these criteria, comparison treatment group went slightly worse. At the endpoint of the study, the frequency of anosmia reduced to 2% in the intervention treatment group and to 7.8% in the comparison treatment group. Also, 16.8% of the intervention group' subjects, and 15.7% of comparison treatment group's patients reached normosmia. CONCLUSIONS: Overall, there was a strongly significant difference in olfactory function between baseline and endpoint for both groups. However, based on the lack of significant difference between the intervention treatment and the comparison treatment groups in terms of olfactory changes, our study appoints that the alpha-lipoic acid is not better than olfactory training alone to treat olfactory loss after COVID-19. El olfato desempeña una función importante en el desarrollo diario de la vida de las personas. Sus alteraciones pueden ser cualitativas (disosmias, parosmias o fantosmias) y cuantitativas (hiposmia o anosmia). Estas alteraciones pueden deberse por enfermedades rinosinusales, neurodegenerativas o infecciones respiratorias como el resfriado común, la gripe y también el SARS-CoV-2, el agente causal del COVID-19. En relación a esta etiología, su fisiopatología no es del todo clara y la anosmia es poco distinguible clínicamente del resto de las infecciones virales, con una duración variable desde ocho días hasta más de cuatro semanas, en cuyo caso se denomina síndrome pos-COVID-19 o COVID prolongado. Este cuadro se caracteriza por la presencia de síntomas que se desarrollan durante la enfermedad aguda por COVID-19,y que persisten durante más de cuatro semanas, sin explicación por un diagnóstico alternativo. El autor de este artículo realiza una revisión de la literatura médica a partir de un caso clínico con el fin de identificar y resumir la evidencia disponible sobre la terapia de rehabilitación olfatoria. Sense of smell plays an important role in the daily development of people's lives. Its alterations can be both qualitative (dysosmias, parosmias, or phantosmias) and quantitative (hyposmia or anosmia). These alterations may be due to rhinosi-nusal diseases, neurodegenerative diseases, and respiratory infections such as common cold, influenza, and SARS-CoV-2,causative agent of COVID-19. In relation to this etiology, its pathophysiology is not entirely clear, and anosmia is clinically indistinguishable from the rest of the viral infections, with a variable duration ranging from eight days to more than four weeks, in which case it would be called post COVID- 19 syndrome or prolonged COVID. This condition is characterized by the presence of symptoms that develop during acute COVID-19 illness and persist for more than four weeks without an explanation by an alternative diagnosis.The author of this article carries out a review of the medical literature based on a clinical case in order to identify and summarize the available evidence on olfactory rehabilitation therapy. Resumen Introducción: La laringectomía total (LT) tiene como secuela la perdida de la voz, pero otra consecuencia no estudiada es la pérdida del olfato. Objetivo: Demostrar que la "maniobra de inducción del flujo aéreo nasal" (MIFAN) rehabilita el olfato en pacientes con LT. Material y Método: Estudio cuasiexperimental antes-después en pacientes laringectomizados por cáncer de laringe del Servicio de Otorrinolaringología del Hospital Barros Luco Trudeau (HBLT) de Santiago de Chile. Evaluación a través de encuesta, examen físico, nasofibroscopía y test olfatométrico. Pacientes con alteración del olfato por transmisión serán enrolados y se enseñará la MIFAN. Resultados: Se estudiaron 12 pacientes: 10 hombres, 2 mujeres. Edad promedio 66,3 años, todos autovalentes. 66,6% presentó anosmia y 33,3% hiposmia. Todos lograron realizar la maniobra. Posrehabilitación el 100% presentó presencia de olfato valorada por olfatometría. Población intervenida similar a otras series en cuanto a sexo y edad. La erigmofonación facilita la rehabilitación con MIFAN. La rehabilitación del olfato se logró en todos y paralelamente mejoró el sentido del gusto. Conclusión: La MIFAN es una técnica sencilla, barata y asequible para lograr rehabilitar el sentido del olfato en pacientes laringectomizados.


Abstract Introduction: Total laryngectomy (TL) has as a consequence the loss of voice, but another not studied consequence is the loss of smell. Aim: To demonstrate that the "nasal airflow inducing maneuver" (NAIM) rehabilitates smell in patients with TL. Material and Method: A quasi-experimental before-after study in laryngectomized patients for laryngeal cancer from the Otorhinolaryngology Service (ENT) of the Barros Luco Trudeau Hospital (BLTH) at Santiago, Chile. Evaluation through survey, physical examination, nasofibroscopy and olfactory test. Patients with transmission impairment of smell were enrolled and NAIM was performed. Results: 12 patients were studied: 10 men, 2 women. Average age 66.3 years. All self-supporting. 66.6% presented anosmia and 33.3% hyposmia. They all managed to perform the maneuver. Post-rehabilitation, 100% presented the presence of smell assessed by olfactometry. Intervened population similar to other series in terms of sex and age. Esophageal speech facilitates NAIM rehabilitation. Rehabilitation of smell was achieved in all of them and in parallel, the sense of taste improved. Conclusion: NAIM is a simple, cheap and affordable technique to rehabilitate the sense of smell in laryngectomized patients.
