Cases of cranial superficial epigastric vein (CSEV) phlebitis with subsequent septicemia were observed in dairy farms in Minas Gerais, Brazil. Autopsy on 4 affected cows, from 2 farms, revealed CSEV thrombophlebitis with perivascular abscesses, pulmonary abscesses, valvular endocarditis, arthritis, thromboembolic nephritis, and renal infarcts. Microscopic examination revealed fibrosing and lymphoplasmacytic phlebitis with occasional endothelial loss, subendothelial areas of necrosis, and abundant fibrin deposition. Trueperella pyogenes, Escherichia coli, and Staphylococcus aureus were isolated from lesions of 3 different cows. Thrombophlebitis of the CSEV is a rare condition in dairy cows; however, it has become more frequent in dairy farms in southeastern Brazil after repeated venipuncture of this vein, likely with contaminated needles used for administration of oxytocin.

BACKGROUND: Pylephlebitis or septic thrombophlebitis of the portal venous system is a rare but serious complication of intra-abdominal infections which drain into the portal venous system. Its diagnosis is based on clinical suspicion and imaging tests, mainly a computed tomography scan, given the lack of specificity of the signs and symptoms. Spread of septic emboli is the major cause of morbidity and mortality. The aim of the study was to analyse patients diagnosed in our hospital. MATERIAL AND METHODS: Retrospective descriptive study of patients diagnosed with pylephlebitis in our hospital. CLINICAL CASES: Four patients were included, 3 men and one woman. In 3 cases it was acute cholecystitis that led to the diagnosis of pylephlebitis at the same time as the intra-abdominal infection. Emergency surgery was performed in one case, whilst the other 2 were treated conservatively. Blood cultures were performed in all cases, and empirical antibiotic treatment was used. In the only case of acute appendicitis, diagnosis of pylephlebitis was achieved during the study of postoperative fever, with empirical antibiotic treatment also being started. The haematologist was requested to start the required anticoagulation therapy in all cases. CONCLUSIONS: Pylephlebitis is a rare complication of intra-abdominal infections that may make lead to a worse outcome. A high level of suspicion is required as well as imaging tests to make an early diagnosis and appropriate treatment.

Septic thrombophlebitis of the portal vein is an unusual and serious complication of abdominal infection. We present a patient with thrombophlebitis of the portal vein of unknown origin, suffering from fever, abdominal pain, jaundice, abnormal liver test function and bacteremia related to Bacteroides fragilis. Ultrasonography, with doppler of the portal vein, was performed which showed thrombosis of the portal vein together with signs of portal hypertension. The patient underwent six weeks of antibiotic treatment. The evolution was favourable, the infection was overcome and the portal vein was de-obstructed as a consequence of which the signs of portal hypertension disappeared.

We report two patients, an 82 years old female and a 71 years old male, who had a severe sepsis with positive blood cultures for Staphylococcus aureus and a superficial phlebitis as the only probable focus. In both the diagnosis of septic phlebitis was reached and an emergency phlebotomy was performed under local anesthesia. The clinical response was satisfactory and the pathological examination of excised veins showed an acute exudative leukocytic thrombophlebitis.

El propósito de este estudio fue determinar la incidencia de la Tromboflebitis Superficial (TFS), las posibles causas, extención y prevención en pacientes admitidos en el Instituto de Gastroenterología Boliviano Japones de Sucre (IGBJ). Primer estudio, de 314 pacientes admitidos que recibieron líquidos intra venosos (IV) se detectó que el 14 porciento hizo TFS. En una segunda etapa bajo un protocolo más estricto se observó que el 52 porciento hizo TFS. El mismo dia de la cirugia llegaron con TFS de quirófano a su sala 18 porciento de los pacientes. El Segundo dia postquirurgico el 25 porciento de pacientes hiza esta complicación; el tercer dia, el 8 porciento y el cuarto, dia el 1 porciento . La mayoria estuvo en relación a la administración endovenosa de fármacos, siendo los principales : anestésicos pentotal, alloferin; analgésico (Klosidol,Dioxadol), antibioticos (Ampicilina, Gentamicina,Metronidazol). Es una complicación prevenible en un buen porcentaje de casos. se establece la necesidad de "salvar" las venas para necisidades del futuro.

The authors describe two cases of cutaneous nose infection that quickly spread and extended to the orbital venous complex. At first glance, the clinical presentation could be mistaken for a complicated sinusal infection; therefore, the evaluation of the sinuses, by means of physical examination and radiological investigation, was of great concern, showing that there was no important pathology in the sinuses. The CT scan and the color Doppler imaging (orbital ultrasound with Doppler) demonstrated, throughout the development of the disease, that the superior ophthalmic vein was affected in both patients and the cavernous sinus in one of them. On physical examination, chemosis of the conjunctiva, proptosis, and edema of the eyelids were prominent. Patients improved only after appropriate intravenous antibiotic therapy against staphylococcus (clindamycin) and corticosteroids, making one conclude that treatment of this disease should be initiated as soon as possible in order to decrease morbidity and mortality.

Os autores descrevem cinco casos de tromboflebite desencadeados pelo bacilo de Koch, cujos focos primários näo foram detectados pelos métodos de investigaçöes rotineiramente utilizados para este mister. Trata-se de uma afecçäo vascular periférica de apresentaçäo extremamente rara. As manifestaçöes clínicas que seguem o seu aparecimento têm na dor seu sintoma mais significativo. A ressecçäo cirúrgica dos vasos lesionados foi a conduta mais condizente com a realidade patológica apresentada, já que as medidas clínicas foram incapazes de proporcionar resultados satisfatórios
