RESUMO

Blastocystis is a human common enteric protist that may colonize a large variety of non-human hosts linked to symptoms and diseases such as abdominal pain, constipation, diarrhea, urticaria, flatulence and irritable bowel syndrome (IBS). Blastocystis exhibits remarkable genetic diversity and multiple subtypes (STs) within the genus with no absolute associations with clinical symptomatology. Here we analyzed fecal samples from Argentinean patients (n=270) belonging to symptomatic (urticaria and non-specific gastrointestinal symptoms, n=39) and asymptomatic control (n=28). Those patients infected with Blastocystis (n=67) were submitted for morphological analysis, DNA extraction, 18S PCR, sequencing and STs identification according to DNA barcoding. Blastocystis vacuolar forms were the predominant morphotype (75%), ameboid-like forms were evidenced in 1.5% of samples. Blastocystis ST3 was detected in 71.6% (n=48), of which 71.4%, (n=35) and 28.6% (n=14) belonged to symptomatic and asymptomatic respectively. Other subtypes identified were ST1 (14.9%), ST6 (7.5%) and ST2 (5.9%). Blastocystis 18S barcoding evidenced in non-urticaria symptomatic patients and asymptomatic control group the presence of allele 134 (ST3) (p<0.0001), while allele 34 (ST3) was detected in 85.7% (18/21) of symptomatic uricaria as compared with control group (1/21) (p<0.0001). The presence of a particular allele (a34) significantly associated with urticaria patients was detected and the clinical implications of these findings are herein discussed.

An attempt has been made to contribute to the understanding of the symptoms and factors associated with the Blastocystis Hominis infection, as seen in persons seeking outside consultation from the Dermatological and Transmissible Diseases Department (DTDD) at the C.H.N.H. This is a case-control study carried out in people between the ages of 5 and 80 in a period from January to March 1999. The cases tested positive in parasitological tests for Blastocystis Hominis and were absent of other enteropathogens. The controls tested negative in parasitological tests for Blastocystis Hominis and were absent of other enteropatoghens. A clinical chart was used to register details of symptomatology and factors associated with the Blastocystis Hominis infection. 74 cases and 70 controls were studied, matched by sex and age. A statistical correlation was obtained (p<0.05) among symptomatic persons and presence of Blastocystis Hominis (91,9%). The symptomatology associated with the Blastocystis Hominis infection by order of statistical significance (p<0.05) was: Abdominal pain (OR=3) 1.47

The zoonoses are those diseases transmitted from animals to humans. Although commonly thought to be infectious in origin such as scabies or dermatophytosis, inflammatory processes may also be zoonotic. Contact dermatitis may result from allergens being carried from animals to humans. Although the scabies mite is somewhat species specific, various mites sometimes have the opportunity of producing skin disease on man (ie, canine scabies, porcine scabies, and feline scabies [notoedric mange]); cheyletiellosis is caused by a similar mite, often found in rabbits. The index case of superficial fungal infections may be the household pet, whereas the cause of flea bite dermatitis (papular urticaria) may be the lack of an animal, resulting in the flea choosing a human for feeding. Understanding zoonotic skin diseases will improve the clinician's diagnostic skills. Both the human and animal patient will then receive more effective treatment in a quicker fashion.

RESUMO

Se estudiaron 100 pacientes diagnosticados de urticaria crónica de causa parasitaria a Giardia lambia. Se reealizaron investigaciones en heces fecales seriadas , drenaje biliar , frotis duodenal, dosificasión de inmunoglobulinas séricas G, M,a y E; además de pruebas de lectura inmediata con un extracto de Giardia lambia al 1, 10, y 100 UNP. Se encontró 93% de mejoria clínica con la utilización de la inmunoterapia específica.

RESUMO

The main goal of this paper is to call the attention of general practitioners and internists on the different affections which may involve urticaria as a clinical manifestations. A differential diagnosis of the causes of urticaria with an allergic and a nonallergic pathogenesis is made. Intestinal parasitism is among the first causes of urticaria. Six cases studied at the allergy service which had been referred from health areas or other hospital services are reviewed and reported. In these patients, a total regression of the clinical picture was achieved once intestinal parasitism was eradicated.

Relationship betwen parasitism and allergic diseases are briefly commented. The parasites which most frequently are involved in allergic conditions in our environment are pointed out. Some cases are reported and conclusions from the material reviewed are made.
